Background Interactions between polymorphisms of the melatonin receptor 1B (MTNR1B) gene and lifestyle intervention for gestational diabetes have been described. Whether these are specific for physical activity or the healthy eating intervention is unknown. Objectives The aim was to assess the interaction between MTNR1B rs10830962 and rs10830963 polymorphisms and lifestyle interventions during pregnancy. Methods Women with a BMI (in kg/m(2)) of >= 29 (n = 436) received counseling on healthy eating (HE), physical activity (PA), or both. The control group received usual care. This secondary analysis had a factorial design with comparison of HE compared with no HE and PA compared with no PA. Maternal outcomes at 24-28 wk were gestational weight gain (GWG), maternal fasting glucose, insulin, insulin resistance (HOMA-IR), disposition index, and development of GDM. Neonatal outcomes were cord blood leptin and C-peptide and estimated neonatal fat percentage. The interaction between receiving either the HE or PA intervention and genotypes of both rs10830962 and rs10830963 was assessed using multilevel regression analysis. Results GDM risk was increased in women homozygous for the G allele of rs10830962 (OR: 2.60; 95% CI: 1.34, 5.06) or rs10830963 (OR: 2.83; 95% CI: 1.24, 6.47). Significant interactions between rs10830962 and interventions were found: in women homozygous for the G allele but not in the other genotypes, the PA intervention reduced maternal fasting insulin (beta: -0.16; 95% CI: -0.33, 0.02; P = 0.08) and HOMA-IR (beta: -0.17; 95% CI: -0.35, 0.01; P = 0.06), and reduced cord blood leptin (beta: -0.84; 95% CI: -1.42, -0.25; P = 0.01) and C-peptide (beta: -0.62; 95% CI: -1.07, -0.17; P = 0.01). In heterozygous women, the HE intervention had no effect, whereas in women homozygous for the C allele, HE intervention reduced GWG (beta: -1.6 kg; 95% CI: -2.4, -0.8 kg). No interactions were found. Conclusions In women homozygous for the risk allele of MTNR1B rs10830962, GDM risk was increased and PA intervention might be more beneficial than HE intervention for reducing maternal insulin resistance, cord blood C-peptide, and cord blood leptin.

Background: It is clear that breastfeeding is the gold standard of infant feeding because of the many advantages it offers to both the child and the mother. Objective: to identity the main reasons for cessation breastfeeding declares by the mother themselves during the first year. Design: A prospective cohort study was conducted, recruiting 969 newborns in a third level hospital in Spain. The main maternal variables studied were: maternal age, parity, educational level, work occupation, smoking habit, gestational age, birth, weigh, feeding type, and duration of breastfeeding. All the participants were followed for a year to determinate the duration of breastfeeding and to know the reason of the abandonment. Results: At 6 months, the percentage of maternal lactation was cut in half and only 24.6% of these mothers maintain. Mainly 15.80% of the mothers decide to give up the exclusive maternal lactation of their own free desire, and 15.41% because they suspect hypogalactia. The work cause is the third reason of abandonment in both cases. Conclusions: Our results show the need to improve the health policies of promotion, protection and support the initiation of breastfeeding. In particular, our results show the importance of the work factor with particular emphasis on improving conciliation measures.

Human papillomavirus genotype 16 (HPV16) is by far the genotype most strongly associated with cervical cancer; viral variant and/or viral load of HPV16 could modulate this association. The objective was to determine the association between the viral variant and viral load of HPV16 and the presence of cervical high-grade lesions. This cross-sectional study included all women in whom HPV infection was found by cervical smear during routine gynecologic health checks. Women with single or multiple HPV16 infections (n = 176) were selected for viral variant and viral load analysis. Smear results were classified using the Bethesda system. HPV types were classified according to the International Agency for Research on Cancer. Odds ratios (OR) with their 95% confidence intervals (CI) were estimated by logistic regression, adjusted for age, immigrant status, and coinfection with other high-risk genotypes. No statistically significant associations were found regarding the detected viral variants. A viral load above the median (>1,367.79 copies/cell) was associated with a significant risk of high-grade epithelial lesion or carcinoma, after adjusting for age, immigrant status, coinfections, and viral variant: (adjusted OR 7.89; 95% CI: 2.75-22.68). This relationship showed a statistically significant dose-response pattern after categorizing by viral load tertiles: adjusted OR for a viral load greater than the third textile was 17.23 (95% CI: 4.20-70.65), with adjusted linear P-trend = 0.001. In patients infected with HPV16, viral load is associated with high-grade intraepithelial lesions or cervical carcinoma. This could be useful as prognostic biomarker of neoplastic progression and as screening for cervical cancer.

A methodology based on coauthorship analysis is shown for the identifícation of research teams in a given discipline or research centre. The method includes using in-house programs to analyse a collection of documents downloaded from the Science Citation Index database. Three basic stages can be distinguished in the process: retrieval, downloading and normalisation of bibliographic data, construction of the author productivity file, and delimitation of research teams through coauthorship analysis. As a final step, teams are characterised by means of bibliometric indicators: team size, production, productivity, collaboration rate and subject specialisation of groups. The Spanish production in both a discipline and a research centre, over the years 1990-1993, is analysed through the methodology to test its performance and main results. Se presenta un método para la identificación de los grupos de investigación activos en una determinada área o centro de investigación, a través de un análisis de co-autoría en las publicaciones científicas del área o centro objeto de estudio. La metodología expuesta se basa en programas de elaboración propia aplicados a una descarga de documentos del Science Citation Index. El proceso incluye tres etapas básicas: recuperación, descarga y normalización de los datos bibliográficos a estudiar; construcción del fichero de productividad de los autores; y delimitación de grupos a través de las frecuencias de co-autoría. La última etapa del proceso consiste en la caracterización de los grupos mediante indicadores bibliométricos: tamaño de grupo, producción, productividad, tasas de colaboración y especialización temática. Se ilustra el funcionamiento de la metodología mediante su aplicación a la producción española de una disciplina y de un centro de investigación en el período 1990-93.
