Abstract

In the present paper we reconsider the integrable case of the Hamiltonian $N$-species Volterra system, as it has been introduced by Vito Volterra in 1937 and significantly enrich the results already published in the ArXiv in 2019 by two of the present authors (M. Scalia and O. Ragnisco). In fact, we present a new approach to the construction of conserved quantities and comment about the solutions of the equations of motion; we display mostly new analytical and numerical results, starting from the classical predator-prey model and arriving at the general $N$-species model, Comment: 24 pages, 9 figures. This paper is a natural prosecution of the work arXiv:1903.03595 and presents different enrichments and enhancements of the results there appeared. Abstract

In recent work, we presented the construction of a family of difference equations associated with the Stieltjes continued fraction expansion of a certain function on a hyperelliptic curve of genus $g$. As well as proving that each such discrete system is an integrable map in the Liouville sense, we also showed it to be an algebraic completely integrable system. In the discrete setting, the latter means that the generic level set of the invariants is an affine part of an abelian variety, in this case the Jacobian of the hyperelliptic curve, and each iteration of the map corresponds to a translation by a fixed vector on the Jacobian. In addition, we demonstrated that, by combining the discrete integrable dynamics with the flow of one of the commuting Hamiltonian vector fields, these maps provide genus $g$ algebro-geometric solutions of the infinite Volterra lattice, which justified naming them Volterra maps, denoted ${\cal V}_g$. The original motivation behind our work was the fact that, in the particular case $g=2$, we could recover an example of an integrable symplectic map in four dimensions found by Gubbiotti, Joshi, Tran and Viallet, who classified birational maps in 4D admitting two invariants (first integrals) with a particular degree structure, by considering recurrences of fourth order with a certain symmetry. Hence, in this particular case, the map ${\cal V}_2$ yields genus two solutions of the Volterra lattice. The purpose of this note is to point out how two of the other 4D integrable maps obtained in the classification of Gubbiotti et al. correspond to genus two solutions of two different forms of the modified Volterra lattice, being related via a Miura-type transformation to the $g=2$ Volterra map ${\cal V}_2$. We present some observations on the tau-function for the fourth Painlev\'e equation. By considering a Hirota bilinear equation of order four for this tau-function, we describe the general form of the Taylor expansion around an arbitrary movable zero. Abstract

Urban greening is increasingly advocated as a strategy to counteract the loss of green spaces and associated ecosystem services due to urban densification. However, how to combine greater population density with more green spaces is still a topic of debate. Recent studies revealed cases of cities that became overall denser and greener during the last decades, but the underlying types of vegetation trends and their spatial distribution in relation to population growth have not been investigated yet.We focus on one of the mentioned successful cases, Berlin, and apply an own-developed algorithm to examine urban vegetation dynamics using NDVI temporal series. The algorithm distinguishes between abrupt changes linked to variations in the extent of vegetation cover and gradual changes associated to vegetation growth or decline. We analyze the two dynamics between 2004 and 2017 in a 500-m circular neighborhood around the more than 332,000 residential address points in the city of Berlin, and quantify population change within the same areas.An increase in both population density and NDVI characterized the surroundings of most of the analyzed residential address points. However, the observed NDVI increase was most frequently an effect of vegetation growth, which sometimes compensates for the loss of vegetation cover. The results question the relevance of simple NDVI-based indicators to monitor greening trends. Furthermore, they raise doubts about the greening strategies associated to densification interventions and their effectiveness in providing the ecosystem services demanded by a growing population.

We propose an extension of the concept of algebraic entropy, as introduced by Bellon and Viallet for rational maps, to algebraic maps (or correspondences) of a certain kind. The corresponding entropy is an index of the complexity of the map. The definition inherits the basic properties from the definition of entropy for rational maps. Abstract

We present some observations on the asymptotic behaviour of the coefficients in the Laurent series expansion of solutions of the first Painleve equation. For the general solution, explicit recursive formulae for the Taylor expansion of the tau-function around a zero are given, which are natural extensions of analogous formulae for the elliptic sigma function, as given by Weierstrass. 18. B\'acklund Transformations as exact integrable time-discretizations for the trigonometric Gaudin model

Abstract

We construct a two-parameter family of B\"acklund transformations for the trigonometric classical Gaudin magnet. The approach follows closely the one introduced by E.Sklyanin and V.Kuznetsov (1998,1999) in a number of seminal papers, and takes advantage of the intimate relation between the trigonometric and the rational case. As in the paper by A.Hone, V.Kuznetsov and one of the authors (O.R.) (2001) the B\"acklund transformations are presented as explicit symplectic maps, starting from their Lax representation. The (expected) connection with the XXZ Heisenberg chain is established and the rational case is recovered in a suitable limit. It is shown how to obtain a "physical" transformation mapping real variables into real variables. The interpolating Hamiltonian flow is derived and some numerical iterations of the map are presented., Comment: 17 pages, 2 figures

Abstract

Background: To evaluate the impact of the Laparoscopic Approach to Cervical Cancer (LACC) Trial on patterns of care and surgery-related morbidity in early-stage cervical cancer. Methods: This is a retrospective, a multi-institutional study evaluating 90-day surgery-related outcomes of patients undergoing treatment for early-stage cervical cancer before (period I: 01/01/2016–06/01/2018) and after (period II: 01/01/2019–06/01/2021) the publication of the results of the LACC trial. Results: Charts of 1295 patients were evaluated: 581 (44.9%) and 714 (55.1%) before and after the publication of the LACC trial, respectively. After the publication of the LACC trial, the number of patients treated with minimally invasive radical hysterectomy decreased from 64.9% to 30.4% (p < 0.001). Overall, 90-day complications occurred in 110 (18.9%) and 119 (16.6%) patients in the period I and period II, respectively (p = 0.795). Similarly, the number of severe (grade 3 or worse) complications did not differ between the two periods (38 (6.5%) vs. 37 (5.1%); p = 0.297). Overall and severe 90-day complications were consistent between periods even evaluating stage IA (p = 0.471), IB1 (p = 0.929), and IB2 (p = 0.074), separately. Conclusions: The present investigation highlighted that in referral centers the shift from minimally invasive to open radical hysterectomy does not influence 90-day surgery-related morbidity.

Abstract

Previous evidence seems to support the more common presence of certain pigmentation types in women with endometriosis. The aim of this study was to assess the association of certain somatic phenotypes with specific localizations of the disease. The genetic makeup of those somatic traits may will help in better define the disease pathogenesis.Multicentric, retrospective study of women aged 18 to 45 with histologically confirmed endometriosis. 575 patients were recruited at eleven different Italian endometriosis clinics from March 2015 to January 2021. Data regarding clinical and surgical features were recorded following the self-administered endometriosis patient questionnaire and the surgical standard of reports approved by the World Endometriosis Research Foundation (WERF). Pigmentation types/somatic phenotypes frequencies among endometriosis localizations were reported. A logistic regression analysis was performed to determine somatic types independently associated with disease' localizations.Having green eyes increased by ∼4 folds (OR 3.7; 95% CI: 1.42-9.61; p = 0.007) the risk of having a ureteral nodule, whereas brown/black eyes decreased this risk (OR 0.34; 95% CI: 0.13-0.87; p = 0.025). Consistently, the combination of green eyes and blonde/light brown hairs increased the odds of ureteral endometriosis by more than 5 folds (OR 5.40; 95%CI: 2.02-14.49; p = 0.001), even after correction for anthropometric confounders (aOR 5.85; 95% CI: 2.13-16.09; p 0.001).The association between endometriosis and pigmentary traits has been herein confirmed, with the novel finding of the possible predisposition of ureteral endometriosis in patients with green eyes and blonde/light brown hairs. Further investigation on the genetic makeup of somatic traits may provide new inroads also into the molecular aspects of endometriosis leading to a better understanding of this complex disease.

Abstract

The objective of this work is to assess the 5-year outcomes of patients undergoing conization for high-grade cervical lesions that simultaneously present as risk factors in the persistence of HPV infection and the positivity of surgical resection margins. This is a retrospective study evaluating patients undergoing conization for high-grade cervical lesions. All patients included had both positive surgical margins and experienced HPV persistence at 6 months. Associations were evaluated with Cox proportional hazard regression and summarized using hazard ratio (HR). The charts of 2966 patients undergoing conization were reviewed. Among the whole population, 163 (5.5%) patients met the inclusion criteria, being at high risk due to the presence of positive surgical margins and experiencing HPV persistence. Of 163 patients included, 17 (10.4%) patients developed a CIN2+ recurrence during the 5-year follow-up. Via univariate analyses, diagnosis of CIN3 instead of CIN2 (HR: 4.88 (95%CI: 1.10, 12.41); p = 0.035) and positive endocervical instead of ectocervical margins (HR: 6.44 (95%CI: 2.80, 9.65); p < 0.001) were associated with increased risk of persistence/recurrence. Via multivariate analyses, only positive endocervical instead of ectocervical margins (HR: 4.56 (95%CI: 1.23, 7.95); p = 0.021) were associated with worse outcomes. In this high-risk group, positive endocervical margins is the main risk factor predicting 5-year recurrence.

Abstract

The tensor rank of some Gabidulin codes of small dimension is investigated. In particular, we determine the tensor rank of any rank metric code equivalent to an $8$-dimensional $\mathbb{F}_q$-linear generalized Gabidulin code in $\mathbb{F}_{q}^{4\times4}$. This shows that such a code is never minimum tensor rank. In this way, we detect the first infinite family of Gabidulin codes which are not minimum tensor rank.

Abstract

Background: The 2020 ESGO/ESTRO/ESP guidelines stratify the prognosis of endometrial carcinoma (EC) patients combining The Cancer Genome ATLAS (TCGA) molecular signature and pathological factors, including lymphovascular space invasion (LVSI). However, little is known about the prognostic independence of LVSI from molecular signature. Aim: To assess whether the prognostic value of LVSI is independent from the TCGA signature. Material and methods: A systematic review and meta-analysis was performed by searching 5 electronic databases from their inception to March 2021. All peer-reviewed studies reporting assessing LVSI as a prognostic factor independent from the TCGA groups in EC were included. Multivariate HRs with 95% confidence interval (CI) were pooled separately for overall survival (OS), disease-specific survival (DSS) and disease-free survival (DFS). The absence of LVSI was considered as a reference. In DFS analyses, locoregional and distant recurrence were separately considered for one study. Results: Six studies with 3331 patients were included in the systematic review and three studies with 2276 patients in the meta-analysis. LVSI showed a pooled multivariate HR of 1.818 (CI 95%, 1.378–2.399) for OS, 1.849 (CI 95%, 1.194–2.863) for DSS, 1.377 (CI 95%, 1.008–1.880) for DFS excluding one study, 1.651 (CI 95%, 1.044–2.611) for DFS additionally considering locoregional recurrence from one study, and 1.684 (CI 95%, 1.05–2.701) for DFS additionally considering distant recurrence from the same study. Conclusion: LVSI has a prognostic value independent of TCGA signature, as well as age and adjuvant treatment, increasing the risk of death of any cause, death due to EC and recurrent or progressive disease by 1.5–2 times.

Abstract

Objective: Coronavirus disease 2019 (COVID-19) outbreak has correlated with the disruption of screening activities and diagnostic assessments. Endometrial cancer (EC) is one of the most common gynecological malignancies and it is often detected at an early stage, because it frequently produces symptoms. Here, we aim to investigate the impact of COVID-19 outbreak on patterns of presentation and treatment of EC patients. Methods: This is a retrospective study involving 54 centers in Italy. We evaluated patterns of presentation and treatment of EC patients before (period 1: March 1, 2019 to February 29, 2020) and during (period 2: April 1, 2020 to March 31, 2021) the COVID-19 outbreak. Results: Medical records of 5,164 EC patients have been retrieved: 2,718 and 2,446 women treated in period 1 and period 2, respectively. Surgery was the mainstay of treatment in both periods (p=0.356). Nodal assessment was omitted in 689 (27.3%) and 484 (21.2%) patients treated in period 1 and 2, respectively (p<0.001). While, the prevalence of patients undergoing sentinel node mapping (with or without backup lymphadenectomy) has increased during the COVID-19 pandemic (46.7% in period 1 vs. 52.8% in period 2; p<0.001). Overall, 1,280 (50.4%) and 1,021 (44.7%) patients had no adjuvant therapy in period 1 and 2, respectively (p<0.001). Adjuvant therapy use has increased during COVID-19 pandemic (p<0.001). Conclusion: Our data suggest that the COVID-19 pandemic had a significant impact on the characteristics and patterns of care of EC patients. These findings highlight the need to implement healthcare services during the pandemic.

Abstract

Objective: Coronavirus disease 2019 (COVID-19) outbreak has correlated with the disruption of screening activities and diagnostic assessments. Endometrial cancer (EC) is one of the most common gynecological malignancies and it is often detected at an early stage, because it frequently produces symptoms. Here, we aim to investigate the impact of COVID-19 outbreak on patterns of presentation and treatment of EC patients. Methods: This is a retrospective study involving 54 centers in Italy. We evaluated patterns of presentation and treatment of EC patients before (period 1: March 1, 2019 to February 29, 2020) and during (period 2: April 1, 2020 to March 31, 2021) the COVID-19 outbreak. Results: Medical records of 5,164 EC patients have been retrieved: 2,718 and 2,446 women treated in period 1 and period 2, respectively. Surgery was the mainstay of treatment in both periods (p=0.356). Nodal assessment was omitted in 689 (27.3%) and 484 (21.2%) patients treated in period 1 and 2, respectively (p<0.001). While, the prevalence of patients undergoing sentinel node mapping (with or without backup lymphadenectomy) has increased during the COVID-19 pandemic (46.7% in period 1 vs. 52.8% in period 2; p<0.001). Overall, 1,280 (50.4%) and 1,021 (44.7%) patients had no adjuvant therapy in period 1 and 2, respectively (p<0.001). Adjuvant therapy use has increased during COVID-19 pandemic (p<0.001). Conclusion: Our data suggest that the COVID-19 pandemic had a significant impact on the characteristics and patterns of care of EC patients. These findings highlight the need to implement healthcare services during the pandemic.

Abstract

Objective: To evaluate outcomes of dichorionic twin pregnancies undergoing early vs late selective termination of pregnancy (ST). Methods: MEDLINE, EMBASE, CINAHL and the Web of Science databases were searched electronically up to March 2022. The primary outcome of this study was pregnancy loss prior to 24 weeks' gestation. The secondary outcomes included preterm birth (PTB) before 37, 34, and 32 weeks, preterm prelabor rupture of membranes (PPROM), gestational age (GA) at delivery, Cesarean delivery, mean birth weight, 5‐min Apgar score < 7, overall neonatal morbidity and neonatal survival. Only prospective or retrospective studies reporting data on the outcome of early (before 18 weeks) vs late (at or after 18 weeks) ST in dichorionic twin pregnancies were considered suitable for inclusion. Quality assessment of the included studies was performed using the Newcastle–Ottawa scale for cohort studies. Random‐effects head‐to‐head meta‐analysis was used to analyze the data. Results: Seven studies reporting on 649 dichorionic twin pregnancies were included in this systematic review. The risk of pregnancy loss prior to 24 weeks was significantly lower in dichorionic twin pregnancies undergoing early compared with late ST (1% vs 8%; odds ratio (OR), 0.25 (95% CI, 0.10–0.65); P = 0.004). The risk of PTB was significantly lower in dichorionic twin pregnancies undergoing early compared with late ST when considering PTB before 37 weeks (19% vs 45%; OR, 0.36 (95% CI, 0.23–0.57); P < 0.00001), before 34 weeks (4% vs 19%; OR, 0.24 (95% CI, 0.11–0.54); P = 0.0005) and before 32 weeks (4% vs 20%; OR, 0.21 (95% CI, 0.05–0.85); P = 0.03). The mean birth weight was significantly greater in the early‐ST group (mean difference (MD), 392.2 g (95% CI, 59.1–726.7 g); P = 0.02), as was the mean GA at delivery (MD, 2.47 weeks (95% CI, 0.04–4.91 weeks); P = 0.049). There was no significant difference between dichorionic twin pregnancies undergoing early compared with late ST in terms of PPROM (P = 0.27), Cesarean delivery (P = 0.38), 5‐min Apgar score < 7 (P = 0.35) and neonatal survival of the non‐reduced twin (P = 0.54). Conclusions: The risk of pregnancy loss prior to 24 weeks and the rate of PTB before 37, 34 and 32 weeks were significantly higher in dichorionic twin pregnancies undergoing late vs early ST, thus highlighting the importance of early diagnosis of fetal anomalies in twin pregnancies. © 2022 International Society of Ultrasound in Obstetrics and Gynecology.
