Abstract Background Mining occupies a prominent place in Brazil, which, if observed, means that one must work with the contingencies that arise from its activity. Mining disasters, such as those in Mariana and Brumadinho, exemplify the impact on the health system and are models for similar situations, so the study sought to investigate the impact of these disasters on hospital admissions in the Brazilian public health system. Methods Through segmented regression, we sought to assess possible changes in the variables HAA (authorized hospital admissions), total admission value, and mortality rate in Mariana and Brumadinho. This measurement method allows the researcher to identify changes during the study period. Results The​​​​​​​ study observed significant changes in the variable mortality rate in the city of Brumadinho. Although the other variables, both in Mariana and Brumadinho, do not present a level of significance compatible with possible effects, we can still say that they present a trend that can be inferred as an effect of the disaster. Conclusions The mining disaster significantly changed the mortality profile in the city of Brumadinho, with implications for the health system. In Mariana, there have been no objective changes, but there is evidence of potential impacts.

Amyotrophic lateral sclerosis (ALS) is a neuromuscular condition resulting from the progressive degeneration of motor neurons in the cortex, brainstem, and spinal cord. While the typical clinical phenotype of ALS involves both upper and lower motor neurons, human and animal studies over the years have highlighted the potential spread to other motor and non-motor regions, expanding the phenotype of ALS. Although superoxide dismutase 1 (SOD1) mutations represent a minority of ALS cases, the SOD1 gene remains a milestone in ALS research as it represents the first genetic target for personalized therapies. Despite numerous single case reports or case series exhibiting extramotor symptoms in patients with ALS mutations in SOD1 (SOD1-ALS), no studies have comprehensively explored the full spectrum of extramotor neurological manifestations in this subpopulation. In this narrative review, we analyze and discuss the available literature on extrapyramidal and non-motor features during SOD1-ALS. The multifaceted expression of SOD1 could deepen our understanding of the pathogenic mechanisms, pointing towards a multidisciplinary approach for affected patients in light of new therapeutic strategies for SOD1-ALS.

Abstract Extracellular vesicles (EVs) are nanosized heat-stable vesicles released by virtually all cells in the body, including tumor cells and tumor-infiltrating dendritic cells (DCs). By carrying molecules from originating cells, EVs work as cell-to-cell communicators in both homeostasis and cancer but may also represent valuable therapeutic and diagnostic tools. This review focuses on the role of tumor-derived EVs (TEVs) in the modulation of DC functions and on the therapeutic potential of both tumor- and DC-derived EVs in the context of immunotherapy and DC-based vaccine design. TEVs were originally characterized for their capability to transfer tumor antigens to DCs but are currently regarded as mainly immunosuppressive because of the expression of DC-inhibiting molecules such as PD-L1, HLA-G, PGE2 and others. However, TEVs may still represent a privileged system to deliver antigenic material to DCs upon appropriate engineering to reduce their immunosuppressive cargo or increase immunogenicity. DC-derived EVs are more promising than tumor-derived EVs since they expose antigen-loaded MHC, costimulatory molecules and NK cell-activating ligands in the absence of an immunosuppressive cargo. Moreover, DC-derived EVs possess several advantages as compared to cell-based drugs such as a higher antigen/MHC concentration and ease of manipulation and a lower sensitivity to immunosuppressive microenvironments. Preclinical models showed that DC-derived EVs efficiently activate tumor-specific NK and T cell responses either directly or indirectly by transferring antigens to tumor-infiltrating DCs. By contrast, however, phase I and II trials showed a limited clinical efficacy of EV-based anticancer vaccines. We discuss that the future of EV-based therapy depends on our capability to overcome major challenges such as a still incomplete understanding of their biology and pharmacokinetic and the lack of standardized methods for high-throughput isolation and purification. Despite this, EVs remain in the limelight as candidates for cancer immunotherapy which may outmatch cell-based strategies in the fullness of their time.

In this study, we systematically examined the importance of human rights standards and principles for rights-based pre-exposure prophylaxis (PrEP) provision for marginalized adolescents. Nested within a demonstration study of PrEP provision to adolescent men who have sex with men, travestis, and transgender women, we carried out interviews in São Paulo, Brazil with 25 adolescents, eight health providers, and six workers involved in community-based demand creation. Analysis focused on participants’ narratives about aspects of human rights within service delivery, including the availability, accessibility, acceptability, and quality of services; informed decision-making; nondiscrimination; and privacy and confidentiality. Clients and service providers highlighted the importance of availing a range of services beyond PrEP and described how community outreach and social media helped promote accessibility. Acceptability centered around clients feeling heard and respected. Health workers appreciated having time to build trusting relationships with clients to ensure quality of care and support informed decision-making. Nondiscrimination was valued by all, including using clients’ chosen pronouns. Privacy and confidentiality were primary concerns for clients who were not “out” about their sexuality or PrEP use; to mitigate this, health workers sought to accommodate clients’ preferred channels of communication. Rights-based PrEP services can help promote engagement and retention in PrEP services, particularly for marginalized populations.

Abstract Background Every year in Italy, influenza affects about 4 million people. Almost 5% of them are hospitalised. During peak illness, enormous pressure is placed on healthcare and economic systems. This study aims to quantify the clinical and economic burden of severe influenza during 5 epidemic seasons (2014–2019) from administrative claims data. Methods Patients hospitalized with a diagnosis of influenza between October 2014, and April 2019, were analyzed. Clinical characteristics and administrative information were retrieved from health-related Administrative Databases (ADs) of 4 Italian Local Health Units (LHUs). The date of first admission was set as the Index Date (ID). A follow-up period of six months after ID was considered to account for complications and re-hospitalizations, while a lookback period (2 years before ID) was set to assess the prevalence of underlying comorbidities. Results Out of 2,333 patients with severe influenza, 44.1% were adults ≥ 65, and 25.6% young individuals aged 0–17. 46.8% had comorbidities (i.e., were at risk), mainly cardiovascular and metabolic diseases (45.3%), and chronic conditions (24.7%). The highest hospitalization rates were among the elderly (≥ 75) and the young individuals (0–17), and were 37.6 and 19.5/100,000 inhabitants/year, respectively. BackgroundCurrent diving physiology postulates that SCUBA divers’ arterial blood gas (ABG) levels vary proportionally to environmental pressure, but, to date, ABGs have only been obtained during simulated dives. Also, recent evidence supports the use of the arterial/alveolar (a:A) partial pressures of oxygen (PO2) ratio to predict the arterial PO2 (PaO2) under hyperbaric conditions from measurements obtained at 1 atmosphere absolute (ATA). This work summarizes ABGs obtained in SCUBA divers in real underwater conditions and aims to validate the a:A ratio in predicting PaO2 in this subset of individuals at depth.MethodsThe study was approved by the local ethics committee. After cannulating the radial artery of the non-dominant limb, ABGs were sampled at the surface before the dive (A), at depth (15 meters of freshwater (mfw) or 42 mfw) before (B) and after (C) pedaling on a submersed bicycle for 10 minutes, and back at surface (D). After calculating the surface alveolar PO2 for each subject, the a:A ratio was obtained and used to predict PaO2 at depth. A linear regression between measured and predicted PaO2 was reported, along with the goodness-of-fit F test.ResultsSix subjects performed the dive at 15 mfw, and four others at 42 mfw. The PaO2 proportionally increased at both depths, remaining stable before and after pedaling. RESUMO OBJETIVO: Compreender as percepções e práticas de profissionais de saúde no contexto da oferta de profilaxia pré-exposição ao HIV (PrEP) entre adolescentes e jovens gays e trans. MÉTODOS: Trata-se de pesquisa qualitativa desenvolvida como parte do estudo PrEP1519, realizada entre 2018 e 2021, com o objetivo de analisar a efetividade de PrEP entre adolescentes e jovens. Os dados foram produzidos de julho de 2020 a fevereiro de 2021 no sítio de São Paulo, combinando técnicas de observação-participante e entrevistas semiestruturadas. O processo analítico envolveu imersão no material empírico, codificação e categorização, com apoio do software NVivo®. A interpretação seguiu o princípio hermenêutico-dialético e teve como horizonte o conceito de cuidado inserido nas práticas de saúde. RESULTADOS: A construção do vínculo de confiança foi informada por práticas que reconheciam a singularidade dos/as adolescentes/jovens e suas demandas e buscavam impulsionar sua autonomia. A escuta sensível e solidária foi apontada como uma prática de acolhimento propulsora de ações de cuidado. Atitudes acolhedoras e suporte frente a situações de estigma e violências, relacionadas ou não ao uso de PrEP, convergiram para o reconhecimento da necessidade de apoio no desenvolvimento de autonomia para a prevenção entre adolescentes e jovens. O uso de linguagem próxima do cotidiano favoreceu a construção de relações de confiança, influenciando positivamente o desenvolvimento da autonomia e a adesão a PrEP. A tensão entre êxito técnico e sucesso prático foi observada na busca idealizada pela normatividade adultocêntrica em contraposição à intersubjetividade. CONCLUSÃO: As percepções e práticas dos/as profissionais de saúde se mostram coerentes com o conceito de cuidado, pois compreendem ações além dos saberes técnicos e reconhecem os contextos que aumentam a vulnerabilidade dos/as adolescentes e jovens ao HIV.

RESUMO OBJETIVO: Avaliar se adolescentes de minorias sexuais que iniciaram a profilaxia pré-exposição sexual (PrEP) em organizações comunitárias (OC) apresentam maior vulnerabilidade social e ao HIV em comparação com adolescentes em PrEP de um serviço de saúde convencional. Além disso, avaliar se esses adolescentes tiveram um acesso mais oportuno à profilaxia. MÉTODOS: Estudo demonstrativo da efetividade de PrEP, realizado na cidade de São Paulo, em duas OC, localizadas no centro (OC-centro) e na periferia (OC-periferia), e em um serviço convencional de testagem para o HIV (CTA-centro). Foram elegíveis para PrEP, entre 2020 e 2022, adolescentes homens cisgêneros que fazem sexo com homens (aHSH), travestis, mulheres transexuais e pessoas transfemininas (aTTrans), de 15 a 19 anos, HIV-negativos e com práticas de maior risco para o HIV. Indicadores de acesso oportuno e de vulnerabilidades dos adolescentes iniciando PrEP nas OC foram analisados, tendo por referência o CTA-centro e empregando regressão logística multinomial. RESULTADOS: 608 adolescentes iniciaram PrEP nas OC e CTA-Centro. Adolescentes das OC estiveram associados a um menor tempo de início de PrEP (1–7 dias; OC-periferia: ORa = 2,91; IC95% 1,22–6,92; OC-centro: ORa = 1,91; IC95% 1,10–3,31); e a um menor IDH de moradia (OC-centro: ORa = 0,97; IC95% 0,94–1,00; OC-periferia: ORa = 0,82; IC95% 0,78–0,86). Na OC-periferia houve aumento na chance de os adolescentes serem mais jovens (ORa = 3,06; IC95% 1,63–5,75) e morarem mais próximos ao serviço (ORa = 0,82; IC95% 0,78–0,86, média 7,8 km). Enquanto adolescentes da OC-centro estiveram associados ao maior conhecimento prévio de PrEP (ORa = 2,01; IC95% 1,10–3,91) e a alta percepção de risco (ORa = 2,02; IC95% 1,18–3,44). Não estiveram associadas aos adolescentes das OC as práticas sexuais de maior risco e as situações de vulnerabilidade ao HIV. CONCLUSÕES: A oferta de PrEP nas OC facilitou o acesso de adolescentes vulnerabilizados e pode contribuir para reduzir inequidades.

ABSTRACT OBJECTIVE: To identify the factors associated with HIV self-testing (HIVST) uptake among adolescent men who have sex with men (AMSM) and adolescent transgender women (ATGW) before and during the COVID-19 pandemic. METHODS: A cross-sectional HIVST uptake study was conducted among AMSM and ATGW. Peer educators and health professionals began providing HIVST in February 2019. The outcome was the HIVST uptake before and during the COVID-19 pandemic. The association between each predictor and outcome in each period was analyzed using simple and multiple logistic regressions, estimating odds ratios, and their respective 95% confidence intervals. RESULTS: The uptake was 229/510 (44.9%) and 382/1,075 (35.5%) before and during the pandemic. During the pre-pandemic period, HIVST uptake was higher in participants who reported receptive anal sex. During the pandemic, uptake was lower in participants with a steady sexual partner and higher in those with frequent oral sex with a steady partner in the previous three months. Before and during the pandemic, HIVST uptake was lower in ATGW and higher in those aged 18–19 years and in participants who lived alone. CONCLUSIONS: Uptake decreased during the pandemic. Sexual behavioral factors associated with HIVST uptake changed during the COVID-19 pandemic, showing the fluid dynamics of sexuality in AMSM and ATGW during this period. HIV programs can optimize the implementation of HIVST among adolescents and young people by incorporating effective and differentiated service delivery models to increase HIV testing uptake and to reach undiagnosed individuals effectively.

RESUMO OBJETIVO: O objetivo deste estudo é analisar o conhecimento sobre o esquema de Profilaxia Pré-Exposição ao HIV sob demanda (PrEP-SD) e a percepção do seu uso potencial entre jovens homens que fazem sexo com homens (HSH), travestis e mulheres trans (TrMT) em acompanhamento na coorte. MÉTODOS: O estudo qualitativo incluiu 50 entrevistas com participantes das cidades de Salvador e São Paulo, entre 15 e 19 anos, que estavam usando PrEP diária ou outros métodos preventivos. Eles foram abordados por meio de diferentes estratégias de criação de demanda. As entrevistas em profundidade abordaram temas como práticas sexuais, conhecimento sobre PrEP-SD, aceitabilidade e motivações para o uso. Foi realizada análise temática em duas fases usando o software Nvivo versão 12. RESULTADOS: A maioria dos participantes desconhecia a PrEP-SD e muitos questionaram a sua efetividade e segurança ao receberem informações sobre ela. No entanto, ao conhecer o esquema, muitos jovens perceberam vantagens, como a não obrigatoriedade diária de medicamentos e a possibilidade de uso apenas em momentos de maior risco. Barreiras para o uso da PrEP-SD também foram identificadas pelos participantes, como a imprevisibilidade das relações sexuais e a dificuldade em administrar as dosagens nessa modalidade. CONCLUSÕES: O conhecimento limitado e as experiências com a PrEP oral diária influenciaram o interesse por ela, o que aponta a necessidade de estratégias informativas que permitam o letramento dos jovens HSH e TrMT na PrEP-SD. Os jovens valorizaram a autonomia e a gestão da prevenção mais alinhada à dinâmica de sua vida sexual propiciada pela nova modalidade, mas enfrentam desafios na gestão da PrEP-SD.

RESUMO OBJETIVO: Analisar os impactos dos sistemas de opressão intersectados no continuum do cuidado de profilaxia pré-exposição sexual (CCPrEP) entre gays, bissexuais e outros homens que fazem sexo com homens (aGBHSH) e como estes são identificados e tratados por profissionais de saúde (PS), visando o cuidado à saúde sexual e prevenção do HIV. MÉTODOS: Estudo de investigação qualitativa integrado a uma pesquisa de coorte com aGBHSH e mulheres transexuais e travestis (aMTrT) em uso de PrEP. Foram analisados dados produzidos na cidade de São Paulo, integrando 16 entrevistas com aGBHSH e oito com PS. O referencial teórico metodológico se orientou pela categorização interativa de análise temática com enquadramento interseccional. RESULTADOS: Os conhecimentos sobre PrEP dos aGBHSH são afetados pelos efeitos negativos dos sistemas de opressão, sobretudo entre adolescentes negros que adquirem conhecimento de forma menos técnica quando comparados aos brancos. A maioria dos profissionais enxerga as opressões e as suas consequências, e confere protagonismo ao racismo, mas poucos articulam os diferentes marcadores sociais que potencializam barreiras para o sucesso do CCPrEP. CONCLUSÕES: As opressões sociais impactam o sucesso do CCPrEP de diferentes maneiras. Os PS são centrais para mitigação e não propagação dessas experiências negativas dentro do serviço de saúde e no acesso, uso e adesão à PrEP.

RESUMO OBJETIVO: Compreender as percepções e experiências de mulheres transexuais, travestis e pessoas não binárias ou de gênero fluido (MTrT+) em serviços de saúde que fazem segmento de profilaxia pré-exposição sexual (PrEP), antes e durante o contexto de pandemia, quanto às ressignificações na relação com os serviços e à continuidade da prevenção ao HIV por meio da PrEP. MÉTODOS: Investigação qualitativa e análise de material empírico produzido no âmbito de estudos mais amplos. Realizaram-se 45 entrevistas semiestruturadas com MTrT+ usuárias da PrEP da cidade de São Paulo, aplicando-se posteriormente a análise de conteúdo temática iterativa. RESULTADOS: As MTrT+ entrevistadas, com relação à pandemia de covid-19; protagonizam e ressignificam suas próprias lutas, seus cotidianos e suas formas de cuidar da saúde. Com suas particularidades, a pandemia se torna um acontecimento a mais dentre todos os demais que as MTrT+ enfrentam quotidianamente. Paralelamente, transitam pela epidemia de HIV, mas com a possibilidade de se prevenir por meio da PrEP. CONCLUSÃO: Por uma perspectiva situada e consciente, mais explícita no contexto da crise sanitária, as MTrT+ entrelaçam ações conscientes e práticas de cuidado em diálogo com os serviços de saúde. Aprofundar as investigações sobre esses grupos, a partir de uma dinâmica espaço-temporal situada em contextos específicos de produção de sentido, possibilita avançar em estratégias de prevenção e cuidado, especialmente em momentos de crise sanitárias.

RESUMO OBJETIVO: Compreender as representações sociais do HIV e suas repercussões no cuidado de jovens recentemente diagnosticados. MÉTODOS: Foi realizada pesquisa qualitativa no contexto do estudo PrEP15-19, que analisou a efetividade da profilaxia pré-exposição sexual ao HIV entre adolescentes de 15 a 19 anos. Foram realizadas entrevistas semiestruturadas com nove jovens homens que fazem sexo com homens (HSH) e uma travesti. Todos receberam diagnóstico de HIV durante a inclusão no estudo realizado em São Paulo e Salvador entre 2019 e 2020. O roteiro de entrevista abordou aspectos da prevenção ao HIV e repercussões no cuidado. Utilizou-se análise temática dos dados, interpretados a partir da teoria das representações sociais. RESULTADOS: Os jovens relataram experiências de estigma e discriminação relacionadas à orientação sexual, identidade e expressão de gênero. Essas vivências trouxeram sofrimentos que foram atualizados na comunicação do diagnóstico, pois remetiam às representações sociais do HIV e aids disseminadas pelo senso comum, baseadas no conhecimento precário sobre prevenção, tratamento e transmissão do HIV. Na análise dos facilitadores e barreiras ao cuidado, a vinculação ao tratamento esteve fortemente relacionada ao acolhimento nos serviços de saúde, com informações e apoio, além de escuta qualificada. As barreiras estiveram relacionadas às fragilidades nos espaços de saúde que obstaculizam o cuidado, como falta de privacidade, hostilidade de profissionais e insegurança quanto à confidencialidade do diagnóstico. CONCLUSÕES: As representações sociais do HIV constituem uma importante dimensão da experiência diagnóstica de jovens, sobretudo por atualizar histórias de violência, homofobia, transfobia, estigma e discriminação. Sua compreensão, a partir dos discursos de jovens, constitui importante ferramenta para formulação de políticas públicas voltadas às demandas dessa faixa etária. Portanto, a construção de novas representações sociais que visem à mitigação do estigma torna-se um dos elementos mais importantes para o enfrentamento da epidemia de HIV entre adolescentes e jovens.

Orphan perennial native species are gaining importance as sustainability in agriculture becomes crucial to mitigate climate change. Nevertheless, issues related to the undomesticated status and lack of improved germplasm impede the evolution of formal agricultural initiatives. Acrocomia aculeata - a neotropical palm with potential for oil production - is an example. Breeding efforts can aid the species to reach its full potential and increase market competitiveness. Here, we present genomic information and training set optimization as alternatives to boost orphan perennial native species breeding using Acrocomia aculeata as an example. Furthermore, we compared three SNP calling methods and, for the first time, presented the prediction accuracies of three yield-related traits. We collected data for two years from 201 wild individuals. These trees were genotyped, and three references were used for SNP calling: the oil palm genome, de novo sequencing, and the A. aculeata transcriptome. The traits analyzed were fruit dry mass (FDM), pulp dry mass (PDM), and pulp oil content (OC). We compared the predictive ability of GBLUP and BayesB models in cross- and real validation procedures. Afterwards, we tested several optimization criteria regarding consistency and the ability to provide the optimized training set that yielded less risk in both targeted and untargeted scenarios. Using the oil palm genome as a reference and GBLUP models had better results for the genomic prediction of FDM, OC, and PDM (prediction accuracies of 0.46, 0.45, and 0.39, respectively). Using the criteria PEV, r-score and core collection methodology provides risk-averse decisions. Training set optimization is an alternative to improve decision-making while leveraging genomic information as a cost-saving tool to accelerate plant domestication and breeding. The optimized training set can be used as a reference for the characterization of native species populations, aiding in decisions involving germplasm collection and construction of breeding populations

ABSTRACT Objective To characterize laparoscopy teaching in Medical Residency Programs in Gynecology and Obstetrics in Brazil, and to evaluate preceptors’ characteristics in laparoscopy programs and map laparoscopic training practice scenarios. Methods This descriptive cross-sectional study evaluated questionnaire responses from coordinators of the Medical Residency Programs in Gynecology and Obstetrics from 2019 to 2021. Results The questionnaire was sent to 175 programs, and 90 responses were received (51.4%). From the 85 valid responses, it was noted that 67 programs had laparoscopic training. Of the 64 responses received regarding location, 32 replies (50%) indicated the Southeast of Brazil, particularly some country’s capitals. In 37.3% (n=25) of the cases, the program coordinator performed laparoscopy. The chief of the laparoscopy sector has advanced experience in most 52.5% (n=10) medical residency services; the preceptors also had advanced experience in 89.4% (n=59) of the services. Residents received laboratory training in 39.4% (n=26) of the services. In most cases, training was performed using a physical simulator. Of the 26 medical residency services with laparoscopy training outside the operating room, 80.8% (n=21) performed them as part of the curriculum, 61.5% (n=16) had a schedule for the same, and only 3.9% (n=1) were objectively evaluated. Conclusion Laparoscopy teaching in Brazil is heterogeneous, with only a few programs offering any training in laparoscopy. The preceptors had advanced experience and participated in laboratory and operating room training. Only a few programs have their own laboratories or training centers, and most teaching programs do not plan to set up training centers.

Abstract Background Disasters are events that bring with them effects that contribute to the disruption of the normality of a population and thus highlight the vulnerabilities of the health system. In Mariana and Brumadinho, the collapse of the dam of ore tailings brought with it several impacts that were felt in the short term and will be felt in the medium and long term. And that by being intensely intertwined with issues of economic and productive nature, has as its meaning an uninterrupted result of its activities. Methods Through the DATASUS database, two specific variables were chosen to perform the analysis: the approved amount and the approved value. For this research, a methodological device, the segmented regression line, was used to observe the influences that the disasters that occurred in Mariana and Brumadinho had on the ambulatory health systems. Results The results of the segmented regression line show that, with Mariana, the amount approved continued to grow throughout the period, which shows that there was no change because of the disaster. There was a reduction in spending. In Brumadinho, regarding the amount approved, there was an upward trend in the disaster’s month, which did not change immediately afterwards, and regarding expenditure, the growth pattern was maintained in all three periods. Corroborating this data, the relative and absolute base elements show an increase in the amount approved and in the number of services provided at various posts compared with Minas Gerais. Conclusions Based on the findings, it was possible to understand that although disasters exert an influence that may have some effect on the health system, the lack of significance sometimes cannot be interpreted as a lack of impact on the disaster. The segmented regression line outlines some effects that are not conclusive but indicative of a numerical interpretation and a trend interpretation.

Questo elaborato racchiude alcuni contributi scientifici in materia di sviluppo dell’identità sessuale in adolescenti e giovani adulti. In ambito psicologico, indagare lo sviluppo dell’identità sessuale risulta particolarmente importante per comprendere, pianificare e implementare interventi capaci di supportare ragazzi queer nel processo di coming out interno ed esterno. Malgrado il panorama scientifico internazionale attesti una riduzione dei fenomeni di discriminazione e controllo sociale nel contesto sportivo, accademico e scolastico, l’Italia nel 2021 si è posizionata all’ultimo posto della Rainbow Europe Map and Index. Secondo la teoria del Minority stress, molti giovani che si identificano nella comunità queer sperimentano fattori di stress distali e prossimali che possono determinare lo sviluppo di: preoccupazioni, difficoltà relazionali e/o stati d’ansia e depressivi. Tra le principali cause di questo malessere vi è l’interiorizzazione del pregiudizio sessuale, diffuso e perpetrato dalla cultura omofoba tutt’ora presente in Occidente. La presente indagine identifica i principali fattori individuali, sociali e culturali incidenti sul benessere di queste minoranze sessuali al fine di progettare efficaci strategie di sostegno nell’ambiente familiare. La famiglia, in effetti, rappresenta la rete sociale primaria attraverso la quale gli individui formano le proprie cognizioni e i propri valori, dove si tessono i primi rapporti e si sperimentano emozioni e sentimenti, apprendendo competenze emotive e relazionali, ma anche il primo potenziale contesto di esposizione e/o educazione al pregiudizio sessuale. Questo lavoro, pertanto, si è focalizzato sulla relazione esistente tra: i) il sistema valoriale familiare e il coming out esterno; ii) il coming out familiare e il livello di coesione della famiglia; iii) il supporto familiare alle autonomie individuali e la pubblica dichiarazione della propria identità sessuale ai familiari; iv) il possesso di una relazione sentimentale stabile e il comig out familiare; v) la disposizione di amicizie appartenenti alla comunità queer e il pubblico svelamento della propria identità sessuale; vi) il pregiudizio sessuale interiorizzato, dall’adolescente stesso e/o dai propri cari, e il coming out. Questa revisione vuole inoltre ispirare una ricerca esplorativa di quei nuclei familiari resistenti all’accettazione e all’accoglienza di figli LGBTQICAPF2K+ che abitano il nostro Paese, denominabili come famiglie italiane eteronormative.

Objective To assess the impact of different disease activity patterns—long quiescent (LQ), chronically active (CA) and relapsing-remitting (RR)—on health-related quality of life (HRQoL) in a cohort of patients with systemic lupus erythematosus (SLE).Methods A retrospective, monocentric analysis of prospectively collected data. Adult SLE outpatients were enrolled between 2017 and 2021.For each year of follow-up, three disease activity patterns were defined: LQ if at each visit clinical Safety of Estrogens in Lupus Erythematosus National Assessment-Systemic Lupus Activity Index (SELENA-SLEDAI)=0, Physician Global Assessment (PGA)=0; CA if at each visit clinical SELENA-SLEDAI >0, PGA >0; RR if patients presented active disease in at least one visit during the observation period, interspersed with periods of remission. These patterns were applied to the year and the 3 years before enrolment.At enrolment, each patient completed: Short Form 36 (SF-36), Lupus Impact Tracker, Functional Assessment of Chronic Illness Therapy (FACIT), Hospital Anxiety and Depression Scale (HADS). The correlation between disease patterns and Patient-Reported Outcomes was analysed.Results 241 SLE patients were enrolled, of which 222 had complete clinical data for the 3-year period before enrolment. Both in the year and during the 3 years before enrolment, the most frequent disease pattern was the LQ (154/241 and 122/222 patients, respectively), followed by RR (53/241 and 92/222 patients, respectively) and CA (34/241 and 8/222 patients, respectively).At baseline, fibromyalgia, organ damage, age and daily glucocorticoid dose were associated with worse HRQoL.At the multivariable analysis, after adjusting for confounding factors, patients with LQ disease during the 3 years before enrolment presented a better physical HRQoL (SF-36 physical component summary, regression coefficient=3.2, 95% CI 0.51–5.89, p=0.02) and minor depressive symptoms (HADS-D, regression coefficient=−1.17, 95% CI −2.38 to 0.0.27, p=0.055), compared with patients with CA/RR disease.Conclusion A persistently quiescent disease may have a positive impact on patients’ physical HRQoL and on depressive symptoms. However, this condition appears insufficient to obtain a significant improvement in mental health, fatigue and disease burden among patients with SLE.

Objectives To analyse the response to immunosuppressants (IS) in extrarenal flares of SLE to determine the most appropriate timing during follow-up for a change in therapeutic strategy.Methods Observational cohort study including a total of 81 patients with SLE with extrarenal flares requiring a change in IS over the period 2015–2022. Baseline clinical variables were described, and follow-up data at 1, 3, 6 and 12 months time-points were collected.Results Among patients flaring that achieved lupus low disease activity state (LLDAS5) at 12 months of follow-up, we identified two subgroups (‘late responders’ and ‘early responders’), which showed no significant differences in demographic characteristics, baseline clinical data, cumulative dosage of glucocorticoids or type of IS. Cox model analysis revealed a significant association of a change in IS (p=0.019) and achieving LLDAS5. Contingency table analysis indicated a significant relationship (p=0.004) between IS change at 6 months and individuals achieving LLDAS5 and remission at 12 months.Conclusions Our findings suggest that clinical improvement of extrarenal flares typically occurs within 6 months of initiating IS. This timeframe could represent an appropriate timing to evaluate the response in a treat-to-target approach in SLE.
