1. Primordial Black Holes and Second-order Gravitational Waves in Axion-like Hybrid Inflation

Abstract: We investigate the possibility that primordial black holes (PBHs) can be formed from large curvature perturbations generated during the waterfall phase transition in a hybrid inflation model driven by an axion-like particle (ALP) $\phi$. The model predicts a spectral index $n_s \simeq 0.964$ and a tensor-to-scalar ratio $r \simeq 0.003$, in agreement with Planck data and potentially testable by next generation cosmic microwave background (CMB) experiments such as CMB-S4 and LiteBIRD. We find that the PBH mass and the peak of the associated scalar-induced gravitational wave (SIGW) spectrum are correlated with the ALP mass. In particular, PBHs in the mass range $10^{-13}\, M_\odot$ can constitute either the entire dark matter (DM) content of the universe or a significant fraction of it. The predicted second-order GWs from this mechanism are within the sensitivity reach of future observatories like LISA and ET. 2. Inflation, Proton Decay and Gravitational Waves from Metastable Strings in $SU(4)_C \times SU(2)_L \times U(1)_R$ Model

Abstract: We present a realistic supersymmetric $\mu$-hybrid inflation model within the framework of $SU(4)_C \times SU(2)_L \times U(1)_R$ gauge symmetry, wherein the symmetry breaking $SU(4)_C \times SU(2)_L \times U(1)_R\rightarrow SU(3)_C\times SU(2)_L \times U(1)_{B-L}\times U(1)_R$ occurs before observable inflation, effectively eliminating topologically stable primordial monopoles. Subsequent breaking of $U(1)_{B-L} \times U(1)_R \rightarrow U(1)_Y$ after inflation leads to the formation of superheavy metastable cosmic strings (CSs), capable of producing a stochastic gravitational wave background (SGWB) consistent with the recent PTA data. Moreover, the scalar spectral index $n_s$ and the tensor-to-scalar ratio $r$ align with Planck 2018 observations. A consistent scenario for reheating and non-thermal leptogenesis is employed to explain the observed matter content of the universe. Finally, the embedding of $G_{421}$ into the Pati-Salam gauge symmetry $G_{422}$ is briefly discussed, predicting potentially observable proton decay rates detectable at facilities such as Hyper Kamiokande and DUNE., Comment: 10 pages, 7 figures

3. Smooth $\mu$-Hybrid and Non-Minimal Higgs Inflation in $SU(4)_{C}\times SU(2)_{L}\times SU(2)_{R}$ With Observable Gravitational Waves

Abstract: We propose to study a smooth variant of the $\mu$-hybrid inflation model and a non-minimal Higgs model of inflation with quartic non-minimal coupling between the Higgs field and gravity within the context of a realistic GUT gauge group based on supersymmetric $SU(4)_{C}\times SU(2)_{L}\times SU(2)_{R}$. These models are incorporated with a realistic scenario of reheating and non-thermal leptogenesis, compatible with the constraints from the baryon asymmetry of the universe. Notably, both models successfully address the MSSM $\mu$-problem and avoid the issue of primordial magnetic monopoles. Our analysis reveals that both models predict a scalar spectral index $n_s$ that closely aligns with the central observationally favored value of Planck2018 + BICEP2/Keck Array (BK15) data and yield a large tensor-to-scalar ratio ($r > 10^{-3}$), potentially detectable in forthcoming CMB experiments., Comment: 35 pages, 11 figures

4. Probing Stochastic Gravitational Wave Background from $SU(5) \times U(1)_{\chi}$ Strings in Light of NANOGrav 15-Year Data

Abstract: A realistic model of $SU(5) \times U(1)_{\chi}$, embedded in $SO(10)$ supersymmetric grand unified theory, is investigated for the emergence of a metastable cosmic string network. This network eventually decays via the Schwinger production of monopole-antimonopole pairs, subsequently generating a stochastic gravitational wave background that is compatible with the NANOGrav 15-year data. In order to avoid the monopole problem in the breaking of both $SO(10)$ and $SU(5)$, a non-minimal Higgs inflation scenario is incorporated. The radiative breaking of the $U(1)_{\chi}$ symmetry at a slightly lower scale plays a pivotal role in aligning the string tension parameter with the observable range. The resultant gravitational wave spectrum not only accounts for the signal observed in the most recent pulsar timing array (PTA) experiments but is also accessible to both current and future ground-based and space-based experiments., Comment: 22 pages and5 figures

5. Evolution of pharmacist roles in antimicrobial stewardship: A 20-year systematic review

Abstract: Introduction: Whilst pharmacists are recognized as key contributors to antimicrobial stewardship (AMS) programs, the extent of their participation varies in different countries. We report a systematic review of pharmacist roles in AMS over 20 years. Methods: A systematic review of the literature describing pharmacist-led or driven AMS and its outcomes published in English between January 01, 2000, and June 30, 2020, was conducted across the main databases for research publication. Results: The analysis included 111 papers, of which 77.5% (86/111) were from high-income countries (HIC) predominantly from the USA. While pharmacist-led or driven AMS interventions were reported only from HICs in the early 2000s, the review found a progressive rise in such studies from all income settings with audit and feedback being the most frequent intervention reported. Between 2016 and 2020, studies on pharmacists-led or driven outpatient AMS and interventions related to beta-lactam or penicillin allergy were reported from HICs. Key outcomes reported include improved appropriateness of antimicrobial therapy and decreased consumption of antimicrobials. Conclusion: Existing evidence demonstrates the positive impact of pharmacists’ involvement in AMS. There needs to be a concerted effort in facilitating pharmacist roles in AMS across all countries, irrespective of income setting.

6. LHC Run-3, $b-\tau$ Yukawa Unification and Dark Matter Implications in SUSY 4-2-2 model

Abstract: We revisit the bottom and $\tau$ Yukawa coupling unification in supersymmetric $4$-$2$-$2$ model and present for the first time the sbottom-neutralino co-annihilation scenario consistent with the bottom and $\tau$ Yukawa coupling unification. In addition, we show gluino-neutralino, stop-neutralino, stau-neutralino, chargino-neutralino, and A-resonance scenario and show that all such solutions are consistent with existing experimental collider constraints, Planck2018 dark matter relic density bounds as well as direct and indirect bounds on neutralino-nucleons scattering cross sections. We show that in sbottom-neutralino co-annihilation scenario, the sbottom mass is about 2 TeV whereas in the case of gluino-neutralino, stop-neutralino, the gluino mass can be between 1 TeV to 3 TeV and stop mass in the range of 1 TeV to 3.5 TeV. {Moreover, in the case of co-annihilation scenario, the stau and chargino masses can be as heavy as 3.5 TeV,} while the A-resonance solutions are in the range of 0.5 TeV to 3.5 TeV. 7. Radiative Symmetry breaking, Cosmic Strings and Observable Gravity Waves in $U(1)_R$ symmetric $SU(5) \times U(1)_{\chi}$

Abstract: We implement shifted hybrid inflation in the framework of supersymmetric $SU(5) \times U(1)_{\chi}$ GUT model which provides a natural solution to the monopole problem appearing in the spontaneous symmetry breaking of $SU(5)$. The $U(1)_{\chi}$ symmetry is radiatevely broken after the end of inflation at an intermediate scale, yielding topologically stable cosmic strings. The Planck's bound on the gravitational interaction strength of these strings, characterized by $G_N \mu_s$ are easily satisfied with the $U(1)_{\chi}$ symmetry breaking scale which depends on the initial boundary conditions at the GUT scale. The dimension-5 proton lifetime for the decay $p \rightarrow K^+ \bar{\nu}$, mediated by color-triplet Higgsinos is found to satisfy current Super-Kamiokande bounds for SUSY breaking scale $M_{\text{SUSY}} \gtrsim 12.5$ TeV. We show that with minimal K\"ahler potential, the soft supersymmetry breaking terms play a vital role in bringing the scalar spectral index $n_s$ within the Planck's latest bounds, although with small tensor modes $r \lesssim 2.5 \times 10^{-6}$ and $SU(5)$ gauge symmetry breaking scale in the range ($2 \times 10^{15} \lesssim M_{\alpha} \lesssim 2 \times 10^{16}$) GeV. By employing non-minimal terms in the K\"ahler potential, the tensor-to-scalar ratio approaches observable values ($r \lesssim 10^{-3}$) with the $SU(5)$ symmetry breaking scale $M_{\alpha} \simeq 2 \times 10^{16}$ GeV., Comment: 30 pages, 9 figures

Abstract: We analyse the shifted hybrid inflation in a no-scale SU(5) model with supersymmetry, which naturally circumvents the monopole problem. The no-scale framework is derivable as the effective field theory of the supersymmetric (SUSY) compactifications of string theory, and it yields a flat potential with no anti-de Sitter vacua, resolving the $\eta$ problem. The model predicts a scalar spectral tilt $n_s$ compatible with the most recent measurements by the Planck satellite, while also accommodating observable values of the tensor-to-scalar ratio $r$ ($\sim 0.0015$), potentially measurable by the near-future experiments. Moreover, the proton decay lifetime in the presence of the dimension-5 operators is found to lie above the current limit imposed by the Super-Kamiokande experiment. 9. Constraining the Cosmic Strings Gravitational Wave Spectra in No Scale Inflation with Viable Gravitino Dark matter and Non Thermal Leptogenesis

Abstract: We revisit the hybrid inflation model gauged by $U(1)_{B-L}$ extension of the Minimal Supersymmetric Standard Model (MSSM) in a no-scale background. Considering a single predictive framework, we study inflation, leptogenesis, gavitino cosmology, and the stochastic background of gravitational waves produced by metastable cosmic strings. The spontaneous breaking of $U(1)_{B-L}$ at the end of inflation produces a network of metastable cosmic strings while, the interaction between $U(1)_{B-L}$ Higgs field and the neutrinos generate heavy Majorana masses for the right-handed neutrinos. The heavy Majorana masses explain the tiny neutrino masses via the seesaw mechanism, a realistic scenario for reheating and non-thermal leptogenesis. We show that a successful non-thermal leptogenesis and a stable gravitino as a dark matter candidate can be achieved for a wide range of reheating temperatures and $U(1)_{B-L}$ symmetry breaking scales. The possibility of realizing metastable cosmic strings in a grand unified theory (GUT) setup is briefly discussed. We find that a successful reheating with non-thermal leptogenesis and gravitino dark matter restrict the allowed values of string tension to a narrow range $10^{-9} \lesssim G\mu_{CS} \lesssim 8 \times 10^{-6}$, predicting a stochastic gravitational-wave background that lies within the 1-$\sigma$ bounds of the recent NANOGrav 12.5-yr data, as well as within the sensitivity bounds of future GW experiments., Comment: 26 pages, 5 figures

The $d = 5$ proton lifetime for the decay $p \rightarrow K^+ \bar{\nu}$, mediated by color-triplet Higgsinos is found to satisfy current experimental bounds if split-high scale SUSY scenario is employed. We show that with minimal K\"ahler potential, the soft supersymmetry breaking terms play a vital r\^ole in bringing the scalar spectral index $n_s$ within the Planck's latest bounds. In a minimal K\"ahler potential setup, small values of tensor-to-scalar ratio $r \lesssim 3.5 \times 10^{-7}$ are obtained, whereas the gravitino mass turns out to be in the range that favors PeV scale SUSY but is not sufficiently high to avoid the $d = 5$ proton decay. A non-minimal K\"ahler potential including higher-order corrections is required to realize successful inflation with central value of scalar spectral index $n_s = 0.9655$, large tensor modes $r \lesssim 0.056$ and a low reheat temperature $(3\times 10^{6} \lesssim T_r \lesssim 6.5 \times 10^7 )$ GeV consistent with leptogenesis and baryogenesis., Comment: 23 pages, 6 figures

11. Primordial Black Holes and Gravitational Waves in Hybrid Inflation with Chaotic Potentials

Abstract: We study the formation of primordial black hole (PBH) dark matter and the generation of scalar induced secondary gravitational waves (SIGWs) in a non-supersymmetric model of hybrid inflation with chaotic (polynomial-like) potential, including one-loop radiative corrections. A radiatively corrected version of these models is entirely consistent with Planck's data. By adding non-canonical kinetic energy term in the lagrangian, the inflaton experiences a period of ultra-slow-roll, and the amplitude of primordial power spectrum is enhanced to $O(10^{-2})$. The enhanced power spectra of primordial curvature perturbations can have both sharp and broad peaks. %In the enhancement mechanism we explore two possible extensions by employing a Guassian and a step size kinetic function. A wide mass range of PBHs is realized in our model with the frequencies of scalar induced gravitational waves ranged from nHz to kHz. We present several benchmark points where the PBH mass generated during inflation is around $(1 - 100) \, M_{\odot}$, $(10^{-9} - 10^{-7}) \, M_{\odot}$ and $(10^{-16} - 10^{-11}) \, M_{\odot}$. The PBHs can make up most of the dark matter with masses around $(10^{-16} - 10^{-11}) \, M_{\odot}$ and $(1 - 100) \, M_{\odot}$, and their associated SIGWs can be probed by the upcoming ground and space-based gravitational wave (GW) observatories. The evidence of stochastic process recently reported by NANOGrav may be interpreted as SIGWs associated with the formation of PBHs. These SIGWs may also be tested by future interferometer-type GW observations of SKA, DECIGO, LISA, BBO, TaiJi, TianQin, CE and ET.

13. Gravity Waves and Proton Decay in Flipped SU(5) Hybrid Inflation Model

Abstract: We revisit supersymmetric hybrid inflation in the context of flipped $SU(5)$ model. With minimal superpotential and minimal K\"ahler potential, and soft SUSY masses of order $(1 - 100)$ TeV, compatibility with the Planck data yields a symmetry breaking scale $M$ of flipped $SU(5)$ close to $(2 - 4) \times 10^{15}$ GeV. This disagrees with the lower limit $M \gtrsim 7 \times 10^{15}$ GeV set from proton decay searches by the Super-Kamiokande collaboration. We show how $M$ close to the unification scale $2\times 10^{16}$ GeV can be reconciled with SUSY hybrid inflation by employing a non-minimal K\"ahler potential. Proton decays into $e^+ \pi^0$ with an estimated lifetime of order $10^{36}$ years. 15. Simplified Smooth Hybrid Inflation in Supersymmetric SU(5)

Abstract: A scheme of simplified smooth hybrid inflation is realized in the framework of supersymmetric $SU(5)$. The smooth model of hybrid inflation provides a natural solution to the monopole problem that appears in the breaking of $SU(5)$ gauge symmetry. The supergravity corrections with nonminimal K\"ahler potential are shown to play important role in realizing inflation with a red-tilted scalar spectral index $n_s <1$, within Planck's latest bounds. Abstract: Abstract. Management of candidemia in developing countries like India encounters laxity in appropriate clinical management and challenges in terms of healthcare capacity, despite its association with high morbidity and mortality. Our study aims to evaluate the impact of a comprehensive candidemia care bundle implementation on appropriateness of therapy and major clinical outcomes. The single-center, quasi-experimental study conducted at a south Indian tertiary care center included adult patients diagnosed with candidemia. Following a retrospective review of candidemia patients of the pre-implementation period (January 2013–December 2015), the hospital antifungal stewardship team instituted a clinical pharmacist driven comprehensive candidemia care bundle for candidemia patients during the post-implementation period (October 2017–2019) and its impact on appropriateness of antifungal prescriptions and inpatient mortality was evaluated. The study included 175 patients with candidemia, comprising of 103 patients in the pre-implementation period and 72 patients in the post-implementation period. Appropriateness of antifungal prescriptions rose to 65% during post-implementation period from 30% observed in pre-implementation phase (P = .0005). The inhospital mortality rate reduced from 40% in the pre-implementation phase to 36% in the post-implementation phase, recording a 10% reduction over 2 years post-implementation (P = .26). No significant difference was observed in terms length of stay (P = .17). Our study demonstrates the successful implementation of an antifungal stewardship led comprehensive care bundle in a low middle income countries setting. The results of our study will have profound implications in improving the appropriateness of management of candidemia and feasibility of scaling up to wider settings could be explored.

22. LHC run 3, <math><mrow><mi>b</mi><mtext>−</mtext><mi>τ</mi></mrow></math> Yukawa unification, and dark matter implications in a SUSY 4-2-2 model

Abstract: We revisit the bottom and τ Yukawa coupling unification in the supersymmetric 4-2-2 model and present for the first time the sbottom-neutralino coannihilation scenario consistent with the bottom and τ Yukawa coupling unification. In addition, we present gluino-neutralino, stop-neutralino, stau-neutralino, chargino-neutralino, and A-resonance scenario and show that all such solutions are consistent with existing experimental collider constraints, Planck 2018 dark matter relic density bounds as well as direct and indirect bounds on neutralino-nucleons scattering cross sections. It is shown that in the sbottom-neutralino coannihilation scenario, the sbottom mass is between 1.2 to 3.5 TeV, whereas in the case of gluino-neutralino, stop-neutralino, the gluino mass can be between 1 to 3 TeV and the stop mass in the range of 1 to 3.5 TeV. Moreover, in the case of a coannihilation scenario, the stau and chargino masses can be as heavy as 3.5 TeV, while the A-resonance solutions are in the range of 0.5 to 3.5 TeV. We anticipate that some part of the parameter space will be accessible in the supersymmetry searches at LHC run 3 and beyond.

Abstract: Polymyxins being last resort drugs to treat infections triggered by multidrug-resistant pathogens necessitates the implementation of antimicrobial stewardship program (ASP) initiatives to support its rational prescription across healthcare settings. Our study aims to describe the change in the epidemiology of polymyxins and patient outcomes following the implementation of ASP at our institution. The antimicrobial stewardship program initiated in February 2016 at our 1300 bed tertiary care center involved post-prescriptive audits tracking polymyxin consumption and evaluating prescription appropriateness in terms of the right indication, right frequency, right drug, right duration of therapy and administration of the right loading dose (LD) and maintenance dose (MD). Among the 2442 polymyxin prescriptions tracked over the entire study period ranging from February 2016 to January 2020, the number of prescriptions dropped from 772 prescriptions in the pre-implementation period to an average of 417 per year during the post-implementation period, recording a 45% reduction. The quarterly patient survival rates had a significant positive correlation with the quarterly prescription appropriateness rates (r = 0.4774, p = 0.02), right loading dose (r = 0.5228, p = 0.015) and right duration (r = 0.4361, p = 0.04). Our study on the epidemiology of polymyxin use demonstrated favorable effects on the appropriateness of prescriptions and mortality benefits after successful implementation of antimicrobial stewardship in a real-world setting.

Abstract: Objective: Sonography is widely used by clinicians to provide imaging assistance in guiding invasive procedures. Many sonography users tend to prefer their dominant hand to operate the needle and their nondominant hand on the ultrasound transducer. The aim was this study was to determine whether the dominant hand guiding the needle achieves a faster time to target during ultrasound-guided procedures compared with the nondominant hand. Materials and Methods: Forty novice sonography users, medical students, were randomized to participate in the study. Twenty medical students used a Sonosite® ultrasound equipment system and the other 20 used a Mindray® ultrasound equipment system. Following a video education training session, an informed consent was obtained from each participant. In each equipment group, participants were randomly allocated to either a right-handed needle and a left-handed transducer preference or a left-handed needle and right-handed transducer preference group. A total of ten participants were in each group. A timer was started when the ultrasound transducer came in contact with the phantom model and stopped when the needle reached the target. This simulated task was repeated ten times by the participants. The hand arrangement for the needle and transducer was then switched and the task repeated another ten times by each participant. Results: Handedness was observed over multiple attempts and “time to target” was recorded for groups with both hand preferences and varied ultrasound equipment systems. The most significant finding was that the difference in time was statistically significant between the groups, when comparing the first and fifth procedural attempts. Conclusion: In this novice group of users, despite training for ultrasound-guided procedures using phantom models, irrespective of the ultrasound equipment system, or hand preference, there was no difference in the time to target.

28. A focused survey of immediate postoperative practices in liver transplantation in India

Abstract: Aim: Over the last decade, the number of liver transplantations and centers that provide this service in India have grown exponentially. However, not all practices relevant to liver transplantation are uniform across the country. We decided to get the opinion from living donor liver transplant (LDLT) centers across the country on four specific aspects relating to LDLT. Methods: We formulated four specific questions that were carefully worded to incorporate aspects of “routine practice” and also answerable in the negative or affirmative, so as to make comparison possible. We collected the data of LDLT centers in India from popular resources such as MOHAN Foundation, from our institutional memory, and also by inquiring with respondents. The following questions were asked: (1) Do you routinely use N-acetyl cysteine for LDLT donors? (2) Do you use routinely use prostaglandin E1 (alprostadil) to facilitate hepatic artery flow in recipients? (3) Do you routinely use antiplatelets to prevent hepatic artery thrombosis in adult LDLT recipients? (4) Do you routinely use low molecular weight heparin for deep vein thrombosis prophylaxis after donor hepatectomy? Results: We received a total of 38 responses, of which 34 were complete. Conclusions: There is no consensus among practitioners regarding the studied aspects of the practice pertinent to immediate postoperative care in liver transplantation. This is primarily because there is neither evidence nor clear guidance from learned societies regarding these issues.

Abstract: Background: Safe and effective use of colistin requires robust pharmacokinetic (PK) and pharmacodynamic (PD) data to guide dosing. Aim: To evaluate the pharmacokinetics of colistimethate sodium and colistin in critically ill patients and correlate with clinical efficacy and renal function. Materials and Methods: Twenty critically ill adult patients with colistin-susceptible multidrug-resistant (MDR) infections and normal renal function treated with intravenous colistimethate sodium – at a 9 million units (270 mg CBA) loading dose followed by maintenance (MD) of 3 million units t.i.d, 24 hours later – were evaluated for clinical cure (CC) at the end of therapy. Patient characteristics and plasma colistin levels at 0, 0.5, 1, 2, 4, 8 and 12 hours after the loading dose and at 1, 2 and 8 hours after the eighth and ninth infusion of MD were evaluated. Colistimethate sodium and colistin levels were measured by high-performance liquid chromatography and tandem mass spectrometry (HPLC-MS/MS). Results: Among the 20 patients who were evaluated, 60% had pneumonia. Predominant pathogens were Klebsiella pneumoniae and Acinetobacter spp. Clinical cure was 50% (10/20). Mean peak loading dose concentrations were 3 ± 1.1 mg/L (1.75–5.14) and 2.37 ± 1.2 mg/L (1.52–5.54) for ‘cure’ and ‘failure’ groups, respectively (p = 0.13), while mean steady-state (Cssavg) concentrations were 2.25 ± 1.3 mg/L and 1.78 ± 1.1 mg/L in ‘cure’ and ‘failure’ groups, respectively (p = 0.19). Nephrotoxicity was 5% on day 7 of therapy. However, bacteriological cure could not be correlated with PK/PD. Conclusions: Subtherapeutic Cssavg with clinical failure and lower efficacy without significant nephrotoxicity highlights the need for therapeutic drug monitoring to guide colistin dosing.

Abstract: Background Sepsis remains a leading cause of death worldwide despite advances in management strategies. Preclinical and observational studies have found mortality benefit with high-dose vitamin C in sepsis. Our study aims to prospectively evaluate the effect of intravenous hydrocortisone, vitamin C [ascorbic acid (AA)], and thiamine (HAT) administration in reducing inpatient all-cause mortality among patients with septic shock. Materials and methods Our single-center, prospective, open-label, randomized controlled trial recruited patients with admitting diagnosis of septic shock and assigned eligible patients (1:1) into either intervention (HAT) or control group (routine). The HAT group received intravenous combination of vitamin C (1.5 g every 6 hours), thiamine (200 mg every 12 hours), and hydrocortisone (50 mg every 6 hours) within 6 hours of onset of septic shock admission. The treatment was continued for at least 4 days, in addition to the routine standard of care provided to the control group. Thiamine and hydrocortisone use in control arm was not restricted. Vitamin C levels were estimated at baseline and at the end of the 4 days of treatment for both groups. The primary outcome evaluated was mortality during inpatient stay. Results Among 90 patients enrolled, 88 patients completed the study protocol. The baseline characteristics between the HAT (n = 45) and the routine (n = 43) groups were comparable. The all-cause mortality in the HAT cohort was 57% (26/45) compared to 53% (23/43) in the routine care group (p = 0.4, OR 1.19, 95% CI 0.51-2.76). The time to reversal of septic shock was significantly lower in the HAT (34.58 ± 22.63 hours) in comparison to the routine care (45.42 ± 24.4 hours) (p = 0.03, mean difference -10.84, 95% CI -20.8 to -0.87). No significant difference was observed between the HAT and the routine care with respect to changes in sequential organ failure assessment (SOFA) scores at 72 hours (2.23 ± 2.4 vs 1.38 ± 3.1), the use of mechanical ventilation (48% vs 46%), and mean Vasoactive Inotropic Score (7.77 ± 12.12 vs 8.86 ± 12.5). Conclusion Intravenous administration of vitamin C, thiamine, and hydrocortisone did not significantly improve the inpatient all-cause mortality among patients with septic shock. Clinical significance HAT protocol does not reduce hospital mortality but decreases time to shock reversal in septic shock. Abstract: Introduction Tracheostomy is among the common procedures performed in the intensive care unit (ICU), with percutaneous dilatational tracheostomy (PDT) being the preferred technique. We sought to understand the current practice of tracheostomy in Indian ICUs. Materials and methods A pan-India multicenter prospective observational study, endorsed and peer-reviewed by the Indian Society of Critical Care Medicine (ISCCM), on various aspects of tracheostomy performed in critically ill patients was conducted between September 1, 2019 and December 31, 2019. The SPSS software was used for the statistical analysis. Cross tables were generated and the chi-square test was used for testing of association. The p value < 0.05 was considered statistically significant. Results Out of 67 ICUs that participated, 88.1% were from private sector hospitals. A total of 923 tracheostomies were performed during the study period; out of which, 666 were PDT and 257 were surgical tracheostomy (ST). Coagulopathic patients received more platelet transfusion [p = 0.037 with platelet count (PC) < 50 × 109, p = 0.021 with PC 50–100 × 109] and fresh frozen plasma transfusion in the ST group (p = 0.0001). The performance of PDT vs ST by day 7 of admission was 28.4% vs 21% (p = 0.023). The single dilator technique (60.4%) was the preferred technique for PDT followed by the Grigg's forceps and then the multiple dilator technique. Fiberoptic bronchoscope (FOB) and ultrasonography (USG) were used in 29.3% and 16.8%, respectively, for guidance during tracheostomy. Most of the PDTs were performed by a trained intensivist (74.2%), whereas ST was mostly done by an ENT surgeon (56.8%). Percutaneous dilatational tracheostomy resulted in less hemorrhagic (2.6% vs 7%, p = 0.002) and desaturation complications (2.3% vs 6.6%, p = 0.001) as compared to ST. The duration of procedure was shorter in the PDT group (average shortening by 9.2 minutes) and the ventilator-free days (VFD) were higher in the PDT group. The cost was less in PDT by approximately Rs. 13,104. Conclusion Percutaneous dilatational tracheostomy, especially the single dilator technique, is preferred by clinicians in Indian ICUs. The incidence of minor complications like hemorrhagic episodes is lower with PDT. Percutaneous dilatational tracheostomy was found to be cheaper on cost per patient basis as compared to ST (with or without complications). Abstract: Management of candidemia in developing countries like India encounters laxity in appropriate clinical management and challenges in terms of healthcare capacity, despite its association with high morbidity and mortality. Our study aims to evaluate the impact of a comprehensive candidemia care bundle implementation on appropriateness of therapy and major clinical outcomes.The single-center, quasi-experimental study conducted at a south Indian tertiary care center included adult patients diagnosed with candidemia. Following a retrospective review of candidemia patients of the pre-implementation period (January 2013-December 2015), the hospital antifungal stewardship team instituted a clinical pharmacist driven comprehensive candidemia care bundle for candidemia patients during the post-implementation period (October 2017-2019) and its impact on appropriateness of antifungal prescriptions and inpatient mortality was evaluated.The study included 175 patients with candidemia, comprising of 103 patients in the pre-implementation period and 72 patients in the post-implementation period. Appropriateness of antifungal prescriptions rose to 65% during post-implementation period from 30% observed in pre-implementation phase (P = .0005). The inhospital mortality rate reduced from 40% in the pre-implementation phase to 36% in the post-implementation phase, recording a 10% reduction over 2 years post-implementation (P = .26). No significant difference was observed in terms length of stay (P = .17).Our study demonstrates the successful implementation of an antifungal stewardship led comprehensive care bundle in a low middle income countries setting. The results of our study will have profound implications in improving the appropriateness of management of candidemia and feasibility of scaling up to wider settings could be explored.

Abstract: Polymyxins being last resort drugs to treat infections triggered by multidrug-resistant pathogens necessitates the implementation of antimicrobial stewardship program (ASP) initiatives to support its rational prescription across healthcare settings. Our study aims to describe the change in the epidemiology of polymyxins and patient outcomes following the implementation of ASP at our institution. The antimicrobial stewardship program initiated in February 2016 at our 1300 bed tertiary care center involved post-prescriptive audits tracking polymyxin consumption and evaluating prescription appropriateness in terms of the right indication, right frequency, right drug, right duration of therapy and administration of the right loading dose (LD) and maintenance dose (MD). Among the 2442 polymyxin prescriptions tracked over the entire study period ranging from February 2016 to January 2020, the number of prescriptions dropped from 772 prescriptions in the pre-implementation period to an average of 417 per year during the post-implementation period, recording a 45% reduction. The quarterly patient survival rates had a significant positive correlation with the quarterly prescription appropriateness rates (r = 0.4774, p = 0.02), right loading dose (r = 0.5228, p = 0.015) and right duration (r = 0.4361, p = 0.04). Our study on the epidemiology of polymyxin use demonstrated favorable effects on the appropriateness of prescriptions and mortality benefits after successful implementation of antimicrobial stewardship in a real-world setting.

A previously healthy, 48-year-old woman, admitted with a working diagnosis of acute-on-chronic liver failure (Grade III) secondary to an autoimmune etiology, was found to be SARS COV-2 RTPCR positive on routine admission screening. She was initially managed with standard medical care for COVID, including steroids. She required invasive ventilation for worsening encephalopathy and when her antigen test was negative 10 days later, she underwent an urgent liver transplantation. Her preoperative infection screen (culture of blood, bronchoalveolar lavage, urine) was negative and computerized tomography (CT) of the chest was normal (Figure 1). She was extubated on day 3 after liver transplantation. Her recovery was uneventful until the 10th postoperative day when she developed cough and desaturation. A repeat CT chest showed multiple multilobular consolidatory nodules with central breakdown involving both lung (Figure 2). Abstract: In many parts of the world, including in India, pharmacist roles in antimicrobial stewardship (AMS) programmes remain unexplored. We describe the evolution and effect of the role of adding clinical pharmacists to a multidisciplinary AMS at a tertiary care teaching hospital in Kerala, India. Through effective leadership, multidisciplinary AMS (February 2016) and antitubercular therapy (ATT) stewardship programmes (June 2017) were established. Clinical pharmacists were introduced as core members of the programmes, responsible for the operational delivery of key stewardship interventions. Pharmacy-led audit and feedback monitored the appropriateness of antimicrobial prescriptions and compliance to AMS/ATT recommendations. Between February 2016 and January 2017, 56% (742/1326) of antimicrobial prescriptions were appropriate, and 54% (318/584) of recommendations showed compliance. By the third year of the AMS, appropriateness increased to 80% (1752/2190), and compliance to the AMS recommendations to 70% (227/325). The appropriateness of ATT prescriptions increased from a baseline of 61% (95/157) in the first year, to 72% (62/86, June 2018–February 2019). The compliance to ATT recommendations increased from 42% (25/60) to 58% (14/24). Such a model can be effective in implementing sustainable change in low- and middle-income countries (LMICs) such as India, where the shortage of infectious disease physicians is a major impediment to the implementation and sustainability of AMS programmes.

Abstract: OBJECTIVE: to determine the role of thyroid stimulating immunoglobulin (TSI) in early diagnosis of patients with euthyroid grave’s ophthalmopathy visiting Oculoplastic service of a tertiary care centre Study design: cross sectional descriptive study Place and Duration of Study: Oculoplastic service, out patient department of ophthalmology, -------from August 2015 to August 2016. Methodology: ninety patients of both the genders with euthyroid Greave`s Ophthalmopathy at any stage of severity of the disease by non-probability purposive sampling technique. One-way ANOVA test was also done for TSI with Age groups and Gender. RESULTS: Among 90 patients, Females were more than males, 68.9% and 31.1% respectively. 4 age groups were made in which age group of 41 – 55 years patients were more (37.8%) and after that was patients of more than 56 years of age (32.2%). 84% had their thyroid stimulating immunoglobulin (TSI) test positive. Cross tabulation was performed for TSI with Gender. 84.4% of the males and females had their TSI positive with females having more tests positive than males, 55.6% and 28.9% respectively with a p value of 0.139. CONCLUSION: TSI is positive in majority of female Euthroid Graves ophthalmopathy patients visiting Oculoplastics clinics in age having 41 – 55 years. The association of TSI with gender showed higher significance than age. Early diagnosis with the help of TSI without any expensive investigations even when other thyroid function tests are normal can prevent complications especially ophthalmopathy by timely referral to endocrinologist.

Abstract: Background: Synthetic colloids, both starches and gelatins, are commonly used as intravascular fluid replacements on account of increased vascular persistence. The safety on renal outcomes during perioperative use is poorly understood. Aims: We evaluated renal outcomes of hydroxyethyl starch 6% (HES) and gelatins 4% (G) in patients undergoing elective abdominal surgery. The primary outcome was serum creatinine measurements at baseline, 12 h, 36 h, and 1 week postoperatively (T0, T12, T36, and D7). The secondary outcomes were measurements of prothrombin time (PT), international normalized ratio (INR), fibrinogen, and activated partial thromboplastin time (aPTT) at baseline, 12 h, and 36 h postoperatively. Setting and Design: A prospective randomized study was conducted at a tertiary care institute. Materials and Methods: Seven-five adult patients received either HES (Group H) or gelatin (Group G) at 20-ml/kg body weight or only crystalloids (Group C) during surgery. Statistical tests used were one-way ANOVA, Student's t-test, Pearson correlation method, and Chi-square test. Results: Serum creatinine assessed at T0, T12, T36, and D7 was comparable between the three groups. PT/INR and aPTT showed no significant increase in values of T12 and T36 in comparison to T0. Fibrinogen level was significantly higher in Group C at T12 and T36. Intraoperative vasopressor use, need for product transfusion, length of intensive care unit stay, and return of bowel function were similar between the three groups. Conclusions: Intraoperative use of HES (130/0.4) or gelatin (4%) at 20-ml/kg body weight was not associated with renal dysfunction or altered PT and aPTT in adult patients undergoing elective abdominal major surgeries.

Abstract: Background Antimicrobial resistance is a major public health threat internationally but, particularly in India. A primary contributing factor to this rise in resistance includes unregulated access to antimicrobials. Implementing antimicrobial stewardship programs (ASPs) in the acute hospital setting will help curb inappropriate antibiotic use in India. Currently, ASPs are rare in India but are gaining momentum. This study describes ASP implementation in a large, academic, private, tertiary care center in India. Methods An ASP was established in February 2016 consisting of an administrative champion, hospitalist, microbiologist, intensivist, and pharmacists. Antimicrobial stewardship program interventions included postprescriptive audit and establishment of institutional guidelines. The ASP tracked appropriate drug selection including loading dose, maintenance dose, frequency, route, duration of therapy, de-escalation, and compliance with ASP recommendations. Defined daily dose (DDD) of drugs and cost of antimicrobials were compared between the pre-implementation phase (February 2015–January 2016) and post-implementation phase (February 2016–January 2017). Results Of 48 555 patients admitted during the post-implementation phase, 1020 received 1326 prescriptions for restricted antibiotics. Antibiotic therapy was appropriate in 56% (742) of the total patient prescriptions. A total of 2776 instances of “inappropriate” antimicrobial prescriptions were intervened upon by the ASP. Duration (806, 29%) was the most common reason for inappropriate therapy. Compliance with ASP recommendations was 54% (318). For all major restricted drugs, the DDD/1000 patient days declined, and there was a significant reduction in mean monthly cost by 14.4% in the post-implementation phase. Conclusions Implementation of a multidisciplinary antibiotic stewardship program in this academic, large, Indian hospital demonstrated feasibility and economic benefits.

Abstract: Background and Aims: The practice of intensive care includes withholding and withdrawal of care, when appropriate, and the goals of care change around this time to comfort and palliation. We decided to survey the attitudes, training, and skills of intensive care residents in relation to end-of-life (EoL) care. All residents at our institute who has worked for at least a month in an adult Intensive Care Unit were invited to participate. Materials and Methods: After Institutional Ethics Committee approval, a Likert-scale questionnaire, divided into five composite measures of EoL skills including training and attitude, was handed over to individual residents and completed data were anonymized. Frequency and descriptive analysis was performed for the demographic variables. Central tendency, variability, and reliability were examined for the five composite measures. Scale internal consistency was checked by Cronbach's coefficient alpha. Multivariate forward conditional regression analysis was conducted to examine the association of demographic data or EoL experience to composite measures. Results: Of the 170 eligible residents, we received 120 (70.5%) responses. Conclusions: Internal medicine residents have more experience in caring for dying patients and conducting EoL discussions. Even though majority of participants reported that they are comfortable with the concept of EoL care, this does not always reflect the actual practice in the hospital. There is a need for further training in skills around EoL care. As this is a self-assessment survey, the specific measures of attitudes and skills in EoL are poorly reflected, indicating a need for further research.

50. A focused survey of immediate postoperative practices in liver transplantation in India

Abstract: Aim: Over the last decade, the number of liver transplantations and centers that provide this service in India have grown exponentially. However, not all practices relevant to liver transplantation are uniform across the country. We decided to get the opinion from living donor liver transplant (LDLT) centers across the country on four specific aspects relating to LDLT. Methods: We formulated four specific questions that were carefully worded to incorporate aspects of “routine practice” and also answerable in the negative or affirmative, so as to make comparison possible. We collected the data of LDLT centers in India from popular resources such as MOHAN Foundation, from our institutional memory, and also by inquiring with respondents. The following questions were asked: (1) Do you routinely use N-acetyl cysteine for LDLT donors? (2) Do you use routinely use prostaglandin E1 (alprostadil) to facilitate hepatic artery flow in recipients? (3) Do you routinely use antiplatelets to prevent hepatic artery thrombosis in adult LDLT recipients? (4) Do you routinely use low molecular weight heparin for deep vein thrombosis prophylaxis after donor hepatectomy? Results: We received a total of 38 responses, of which 34 were complete. Conclusions: There is no consensus among practitioners regarding the studied aspects of the practice pertinent to immediate postoperative care in liver transplantation. This is primarily because there is neither evidence nor clear guidance from learned societies regarding these issues.
