L’incidence de la maladie de Kaposi à l’ère de la multithérapie aux antirétroviraux est en croissance. Elle se manifeste couramment par des lésions papulo-nodulaires angiomateuses se localisant fréquemment au niveau des membres inférieurs. Sa gravité réside dans ses possibilités de dissémination locorégionale. A partir de cinq cas, les auteurs rappellent la diversité clinique de la maladie, la difficulté de confirmation du diagnostic dans notre contexte, et la gravité de certaines formes cliniques. La maladie concernait trois hommes et deux femmes dont l’âge variait entre 30 et 43 ans. Il s’agissait de trois cas de Kaposi cutané, un cas de kaposi œsophagien et un cas de Kaposi ulcéro bourgeonnante de jambe avec atteinte pleurale. La confirmation anatomopathologique a été faite chez un seul patient et deux cas étaient décédés en cours d’hospitalisation. La maladie de Kaposi était la circonstance de dépistage de l’infection à VIH dans deux cas, elle est survenue au cours du traitement antirétroviral dans trois cas. La maladie de Kaposi est une pathologie classant Sida. Son diagnostic chez le sujet vivant avec le VIH justifie l’instauration rapide d’un traitement antirétroviral efficace quel que soit la valeur des lymphocytes TCD4. Sa prise en charge requiert une collaboration pluridisciplinaire.

Abstract Introduction Hyperreactive malarial splenomegaly (HMS) is one of the main causes of massive splenomegaly in malaria-endemic zones. Diagnosis is often challenging in Bobo-Dioulasso. This study aimed to describe the clinical and socio-demographic profile, and the reasons for delay in the diagnosis of HMS cases recorded in the Medicine and Medical Specialties wards of Souro Sanou Teaching hospital. Methods A retrospective descriptive study was conducted from August 2022 by focusing on HMS cases diagnosed in the Infectious Diseases and Clinical Hematology wards of Souro Sanou Teaching Hospital. Results Overall, 65 patients met our inclusion criteria over the 12-year period. Burkinabe nationals and have been residing in Burkina Faso since their birth. 79% (79%) of the patients were seen for medical consultation with the reason for consultation being a voluminous mass in the left hypochondrium. Indigence, self-medication, and lack of information were essential elements in late diagnosis of HMS in Bobo-Dioulasso. All patients were treated with a single tablet of Artemether (80 mg) and Lumefantrine (480 mg) in the morning and evening for 3 days, followed by sulfadoxine–pyrimethamine per week. Nine months later, patients were clinically asymptomatic. Conclusion This study provides a database on hyperreactive malarial splenomegaly (HMS) in the south-west region of Burkina Faso. Rapid and accurate diagnosis of the disease and appropriate use of effective antimalarial drugs would significantly reduce the burden of HMS in Sub-Saharan African countries.

Background: The emergence of antimicrobial resistance (AMR) within bacterial pathogens necessitates a local comprehension of the epidemiological context. This information is indispensable for both clinical therapeutic determinations and the reevaluation of prevailing care protocols. This study aimed to highlight the antibiotic susceptibility profile of uropathogenic bacteria isolated within a university hospital in Burkina Faso, with a focus on enhancing probabilistic antibiotic therapy for both community and hospital-based urinary tract infections (UTIs). Methodology: Data from cytological urine analysis and antimicrobial susceptibility testing spanning 29 months (January 2017 to May 2019) was retrospectively collected and systematically analyzed. Results: In both hospital and community based UTIs, Enterobacterales dominated, constituting 79.86% (81.27% vs. 79.11%) of isolates. This was followed by non-fermentative Gram-negative bacteria at 6.60% (6.35% vs. 6.88%) and Gram-positive cocci at 6.41% (7.98% vs. 5.57%). Escherichia coli (61.37%), Klebsiella pneumoniae (10.66%), and Enterobacter spp. emerged as the predominant pathogens in the same rank regardless of the origin of the ITUs. Imipenem (97.19%), amikacin (69.26%), ceftriaxone (58.44%), and ciprofloxacin (47.60%) displayed superior susceptibility against all uropathogens. Subtle but significant variations emerged between hospital and community strains' susceptibility to various antibiotics, including amoxicillin + clavulanic acid (26.05% vs. 32.26%), Imipenem (96.43% vs. 98.59%), and ciprofloxacin (45.51% vs. 51.41%). Conclusion: Penicillins showcased diminished efficacy against uropathogens, while resistance to fluoroquinolones escalated. The combined use of aminoglycosides and third-generation cephalosporins holds promise as an optimal probabilistic therapy for UTIs. Notably, the profiles of hospital and community UTIs showed substantial similarities in terms of implicated uropathogens, yet hospital strains demonstrated higher resistance levels.

Background and Aim: Houseflies (Musca domestica) are synanthropic insects that are vectors of a wide range of multidrug-resistant pathogens responsible for infectious diseases. The aim of this study was to detect and characterize multidrug-resistant Enterobacteriaceae carried by houseflies in Bobo-Dioulasso. Materials and Methods: A total of 500 houseflies were captured in hospital and non-hospital environments in the city of Bobo-Dioulasso. For bacteriological analysis, they were divided into 125 batches of five flies each. Multidrug-resistant bacteria isolated on MacConkey agar supplemented with 4 μg/mL cefotaxime were identified on the basis of biochemical characteristics. Antibiotic susceptibility profiles were determined using the agar diffusion method. blaCTX-M resistance genes and quinolone resistance genes (plasmid-mediated quinolone resistance) were detected by conventional polymerase chain reaction. Results: Among 115 bacterial strains obtained, 26 were extended-spectrum beta-lactamase (ESBL)-producing enterobacteria: Escherichia coli (15), Klebsiella pneumoniae (6), Enterobacter cloacae (4), and Morganella morganii (1). Carriers were statistically more important in hospitals (12/26, p = 0.03). No carbapenem-resistance strains were observed. We identified ESBL resistance genes (Cefotaximase Munich; CTX-M group 1) (25/26) and quinolone resistance genes (QnrS) (6/26). Conclusion: Houseflies in the city of Bobo-Dioulasso are vectors for the transmission of multidrug-resistant enterobacteria. There is a need to monitor the associated risks for public health.

Alors que le climat politique et économique semble favorable à la mise en place de projets de forages géothermiques en Alsace, une controverse éclate en 2014. Les conséquences sont importantes : sur les six projets envisagés, seuls deux débouchent sur des travaux de forage. Comment, de technologie adulée, la géothermie profonde s’est-elle transformée en problème public auquel la préfecture du Bas-Rhin et l’Eurométropole de Strasbourg doivent apporter des solutions ? Cet article revient sur les moments de la controverse en portant la focale sur les différentes arènes de débat (associatives, politiques, médias et enquêtes publiques). Il se propose notamment de suivre la trajectoire de deux groupes d’arguments : concernant les impacts environnementaux des projets, d’une part, et dénonçant le manque d’engagement de l’Eurométropole de Strasbourg dans la gestion du dossier, d’autre part. Il examine enfin les mécanismes d’appropriation et de transformation des arguments des riverains par une nouvelle gouvernance des projets qui se met en place en 2016-2017.

Background: Due to the low number of individuals with HIV-2, no randomised trials of HIV-2 treatment have ever been done. We hypothesised that a non-comparative study describing the outcomes of several antiretroviral therapy (ART) regimens in parallel groups would improve understanding of how differences between HIV-1 and HIV-2 might lead to different therapeutic approaches., Methods: This pilot, phase 2, non-comparative, open-label, randomised controlled trial was done in Burkina Faso, Côte d'Ivoire, Senegal, and Togo. Adults with HIV-2 who were ART naive with CD4 counts of 200 cells per μL or greater were randomly assigned 1:1:1 to one of three treatment groups. A computer-generated sequentially numbered block randomisation list stratified by country was used for online allocation to the next available treatment group. In all groups, tenofovir disoproxil fumarate (henceforth tenofovir) was dosed at 245 mg once daily with either emtricitabine at 200 mg once daily or lamivudine at 300 mg once daily. The triple nucleoside reverse transcriptase inhibitor (NRTI) group received zidovudine at 250 mg twice daily. The ritonavir-boosted lopinavir group received lopinavir at 400 mg twice daily boosted with ritonavir at 100 mg twice daily. The raltegravir group received raltegravir at 400 mg twice daily. The primary outcome was the rate of treatment success at week 96, defined as an absence of serious morbidity event during follow-up, plasma HIV-2 RNA less than 50 copies per mL at week 96, and a substantial increase in CD4 cells between baseline and week 96. This trial is registered at ClinicalTrials.gov, NCT02150993, and is closed to new participants., Findings: Between Jan 26, 2016, and June 29, 2017, 210 participants were randomly assigned to treatment groups. Five participants died during the 96 weeks of follow-up (triple NRTI group, n=2; ritonavir-boosted lopinavir group, n=2; and raltegravir group, n=1), eight had a serious morbidity event (triple NRTI group, n=4; ritonavir-boosted lopinavir group, n=3; and raltegravir group, n=1), 17 had plasma HIV-2 RNA of 50 copies per mL or greater at least once (triple NRTI group, n=11; ritonavir-boosted lopinavir group, n=4; and raltegravir group, n=2), 32 (all in the triple NRTI group) switched to another ART regimen, and 18 permanently discontinued ART (triple NRTI group, n=5; ritonavir-boosted lopinavir group, n=7; and raltegravir group, n=6). The Data Safety Monitoring Board recommended premature termination of the triple NRTI regimen for safety reasons. The overall treatment success rate was 57% (95% CI 47-66) in the ritonavir-boosted lopinavir group and 59% (49-68) in the raltegravir group., Interpretation: The raltegravir and ritonavir-boosted lopinavir regimens were efficient and safe in adults with HIV-2. Both regimens could be compared in future phase 3 trials. Objective: This study aimed to evaluate the seroprevalence of anti-SARS-CoV-2 IgG and factors associated with the infection among PLWHIV over the first 12 months following the outbreak of COVID-19 in Burkina Faso., Design: A retrospective cross-sectional study of plasma samples collected from March 9, 2020, and March 8, 2021, at the outpatient HIV referral center, before the introduction of the SARS-CoV-2 vaccine in Burkina Faso., Methods: Anti-SARS-CoV-2 IgG were detected in plasma using DS-ЕIA-ANTI-SARS-CoV-2-G (S) kit. Logistic regressions were used to compare SARS-CoV-2 specific immune responses between groups and within subgroups., Results and Discussion: A total of 419 plasma were subjected to serological diagnosis. None of the participants was vaccinated against COVID-19 during the period of sample collection, and 130 samples were positive for anti-SARS-CoV-2 IgG, giving a prevalence of 31.0% (95% CI 26.6-35.7). The median CD4 cell count was 661 cells/μL (IQR,422-928). Retailers had half the risk of being infected compared to housemaids with an OR of 0.49 (p = 0.028, 95% CI 0.26-0.91). Likewise, the risk of infection was 1.69 times higher in patients on integrase inhibitors compared to that of patients on non-nucleoside reverse transcriptase inhibitors (p = 0.020, 95% CI 1.09-2.63)., Conclusion: Our study reveals a high seroprevalence among PLWHIV to SARS-CoV-2 during the first year of the pandemic. Background: An issue of particular concern is the impact of the 2019 novel coronavirus (2019 nCOV) on the people coinfected with the Human Immuno-deficiency Virus (HIV) and/or tuberculosis (TB). Unfortunately, this interaction has not been well explored in African despite the large proportion of these risk populations living with HIV and/or patients and/or tuberculosis (TB) in the African region. This study aims to design a research protocol for assessment of the impact of coronavirus disease 2019 (COVID-19) on these risk populations in response to COVID-19 strategic plans in Burkina Faso by generating serological, epidemiological, virological, clinical and socio-anthropological evidence-based data., Methods: A multidisciplinary research will be conducted in the city of Bobo-Dioulasso, Burkina Faso using mixed methods. Data will be collected from a cohort of people living with HIV and/or TB patients in the city (i) to determine the proportion of people with specific antibodies against the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) using retrospective data ; (ii) to determine the proportion of people infected with Covid-19 and the dynamics of viral loads and antibodies in these people based on prospective data; (iii) to identify circulating SARS-COV-2 variants and novel biomarkers using prospective data ; (iv) to analyze perceptions, community experiences and response strategies during the public health emergencies imposed by COVID-19 through a qualitative study., Discussion: This study will generate factual and comprehensive data that will contribute in improving response strategies to COVID-19 and the other possible emerging diseases with keen interest on the risk populations living with HIV and/or TB infected patients., (© 2023. Objectives: Our study aimed to assess the statistical relationship between the use of chloroquine phosphate or hydroxychloroquine plus azithromycin (CQ/HCQ + AZ) and virological recovery, disease worsening, and death among out- and inpatients with COVID-19 in Burkina Faso., Methods and Designs: This was a retrospective observational study that compared outcomes in terms of time to recovery, worsening, and death in patients who received CQ/HCQ + AZ and those who did not using a multivariable Cox or Poisson model before and after propensity matching., Results: Of the 863 patients included in the study, about 50% (432/863) were home-based follow-up patients and 50% were inpatients. Of these, 83.3% (746/863) received at least 1 dose of CQ/HCQ + AZ and 13.7% (118/863) did not. There were no significant differences in associated time to recovery for patients receiving any CQ/HCQ + AZ (adjusted HR 1.44; 95% CI 0.76-2.71). Similarly, there was no significant association between CQ/HCQ + AZ use and worsening (adjusted IRR 0.80; 95% CI 0.50-1.50). However, compared with the untreated group, the treated group had a lower risk of death (adjusted HR 0.20; 95% CI 0.10-0.44)., Conclusions: The study provided valuable additional information on the use of CQ/HCQ in patients with COVID-19 and did not show any harmful outcomes of CQ/HCQ + AZ treatment., (Copyright © 2022 The Author(s). Introduction: HIV2 is endemic in West Africa. In Burkina Faso, its prevalence was estimated at 2%. The aim of this work was to evaluate the follow-up of patients and also to contribute to the availability of data. Methods: We involved 18 years or older. Infection was screened according to the national algorithm. A cross- sectional study from first June 2017 to 31 December 2017 was performed. For each patient, sociodemographic, clinical, biological, therapeutic and evolution data were collected and analyzed. Results: The proportion of patients infected with HIV2 (n  =  48; 1.7%) and HIV2  +  1 (n  =  67; 2.4%) was 4.3%. The sex rat mean age was 50.3  ±  8.5 years. The combination of 2INTI  +  LPV/r was the most prescribed (n  =  73; 63.5%). The average gain of LTCD4 has evolved from + 236 cells/mm 3 in 2011 to + 364 cells/mm 3 in 2015. The retention rate at grade 5 was about 70%. Conclusion: The immunological and clinic response of the patients was satisfactory. More than half of the patients remained in the continuum of care after five years of follow-up.

Introduction: APOL1 G1 and G2 alleles have been associated with kidney-related outcomes in people living with HIV (PLHIV) of Black African origin. No APOL1-related kidney risk data have yet been reported in PLHIV in West Africa, where high APOL1 allele frequencies have been observed., Methods: We collected clinical data from PLHIV followed in Burkina Faso ( N  = 413) and in the ANRS-12169/2LADY trial (Cameroon, Senegal, Burkina Faso, N  = 369). APOL1 G1 and G2 risk variants were genotyped using TaqMan assays, and APOL1 high-risk (HR) genotype was defined by the carriage of 2 risk alleles., Results: In West Africa (Burkina Faso and Senegal), the G1 and G2 allele frequencies were 13.3% and 10.7%, respectively. In Cameroon (Central Africa), G1 and G2 frequencies were 8.7% and 8.9%, respectively. APOL1 HR prevalence was 4.9% in West Africa and 3.4% in Cameroon. We found no direct association between APOL1 HR and estimated glomerular filtration rate (eGFR) change over time. Nevertheless, among the 2LADY cohort participants, those with both APOL1 HR and high baseline viral load had a faster eGFR progression (β = -3.9[-7.7 to -0.1] ml/min per 1.73 m 2 per year, P  < 0.05) than those with low-risk (LR) genotype and low viral load., Conclusion: Overall, the APOL1 risk allele frequencies in PLHIV were higher in the West African countries than in Cameroon, but much lower than previously reported in some Nigeria ethnic groups, which strongly advocates for further investigation in the African continent. Introduction: Although dengue is the most common arbovirus infection worldwide, studies of severe dengue in Africa are lacking, and risk factors for severe dengue have been insufficiently described. This study was conducted in the context of the 2016 dengue epidemic in Burkina Faso to determine the prevalence of severe dengue, identify factors associated with severe dengue, and perform mapping of dengue cases in the country's capital, Ouagadougou., Methods: This cross-sectional study was conducted from November 2015 to January 2017. Data were collected in 15 public and private health centres, and included sociodemographic, clinical and patient outcome variables. Dengue was diagnosed using SD Bioline Dengue Duo rapid diagnostic tests. Data were analysed using Epi-Info Version 7. Logistic regression was used to identify predictors of severe dengue. P <0.05 was considered significant. Dengue case mapping was performed using Geographic Information System software (ArcGIS)., Results: Of the 811 patients who tested positive for dengue, 609 (75%) had early dengue (AgNS1 positive) and 272 (33.5%) had severe dengue. Patient age ranged from 1 to 83 years (median 30.5 years) and 393 (48.3%) were female. Renal failure (13.1%) and severe bleeding (10.6%) were the most common signs of severe dengue. Risk factors for severe dengue included age, male sex, haemoglobin S, diabetes, hypertension, and primary dengue. Dengue cases were more concentrated  in sectors located in the centre of the city and close to the health centres., Conclusion: Dengue is increasingly common in Africa and factors associated with severity should be sought systematically as soon as a patient tests positive. Background: The presence of the human leukocyte antigen HLA-B*57:01 is associated with the development of a hypersensitivity reaction to abacavir (ABC). Limited data exist on HLA-B*57:01 prevalence in individuals with HIV-1 in Africa. This study aimed to estimate HLA-B*57:01 prevalence in individuals with HIV-1 in West and Central Africa., Methods: A cross-sectional study was conducted in four countries in West and central Africa (Burkina-Faso, Côte d'Ivoire, Gabon, and Togo) from January 2016 to February 2020 to determine the status of HLA-B*57:01 in adults with HIV-1. The presence of HLA-B*57:01 was determined by using Single Specific Primer-Polymerase Chain Reaction (SSP-PCR) in blood samples. Prevalence rates were stratified based on country., Results: A total of 4016 (69.8% women) individuals with HIV were enrolled. Their median age was 45, and the interquartile range was 38-52. We included 500 (12.4%) patients in Burkina-Faso, 1453 (36.2%) in Côte d'Ivoire, 951 (23.7%) in Gabon, and 1112 (27.7%) in Togo. The overall HLA-B*57:01 prevalence was 0.1% [95% CI: 0.0-0.2%]. The prevalence of HLA-B*57:01 was similar according to the four countries. Only one case was reported in each country except Togo, with no cases., Conclusions: HLA-B*57:01 prevalence is low in individuals with HIV in West and central Africa, and there is no difference among countries.
