This study aimed to determine the characteristics of cerebral vasoreactivity for ischemia and demyelination. Materials and methods: This case–control study included: 28 patients with lacunar infarctions verified by history, physical examination, and MRI; 28 age- and sex-matched healthy controls; 28 patients with relapsing–remitting multiple sclerosis (MS), based on McDonald criteria; and 28 age- and sex-matched healthy controls for the MS group. Transcranial Doppler sonography was undertaken in all subjects to calculate the mean flow velocity (MFV) of the right middle cerebral artery (MCA) and, after a breath-holding (BH) maneuver, the breath-holding index (BHI) was determined. Results: There was no significant difference of BHI and changes of MFV of the MCA in MS patients compared to controls (1.02 ± 0.4 vs 1.02 ± 0.3, p = 0.993; and 16.8 ± 8.1 vs 11.3 ± 10.8, p = 0.057). BHI in patients with lacunar infarctions was significantly lower (0.8 ± 0.4 vs 1.2 ± 0.3, p < 0.001) compared to controls. The BHI (p = 0.040) and variations of MFV of MCA (p = 0.007) in MS patients were significantly higher than in patients with lacunar infarctions. The vasoreactivity of demyelinating lesions was higher than that of ischemic ones. Conclusion: Therefore, cerebral vasoreactivity determined by transcranial Doppler could be utilized for differentiating demyelinating from ischemic lesions. Accurate modeling of the diverse and dynamic interests of users remains a significant challenge in the design of personalized recommender systems. Existing user modeling methods, like single-point and multi-point representations, have limitations w.r.t.\ accuracy, diversity, and adaptability. To overcome these deficiencies, we introduce density-based user representations (DURs), a novel method that leverages Gaussian process regression (GPR) for effective multi-interest recommendation and retrieval. Our approach, GPR4DUR, exploits DURs to capture user interest variability without manual tuning, incorporates uncertainty-awareness, and scales well to large numbers of users. Experiments using real-world offline datasets confirm the adaptability and efficiency of GPR4DUR, while online experiments with simulated users demonstrate its ability to address the exploration-exploitation trade-off by effectively utilizing model uncertainty., Comment: 22 pages

5. Ever Evolving Evaluator (EV3): Towards Flexible and Reliable Meta-Optimization for Knowledge Distillation

We introduce EV3, a novel meta-optimization framework designed to efficiently train scalable machine learning models through an intuitive explore-assess-adapt protocol. In each iteration of EV3, we explore various model parameter updates, assess them using pertinent evaluation methods, and then adapt the model based on the optimal updates and previous progress history. EV3 offers substantial flexibility without imposing stringent constraints like differentiability on the key objectives relevant to the tasks of interest, allowing for exploratory updates with intentionally-biased gradients and through a diversity of losses and optimizers. Additionally, the assessment phase provides reliable safety controls to ensure robust generalization, and can dynamically prioritize tasks in scenarios with multiple objectives. With inspiration drawn from evolutionary algorithms, meta-learning, and neural architecture search, we investigate an application of EV3 to knowledge distillation. Our experimental results illustrate EV3's capability to safely explore the modeling landscape, while hinting at its potential applicability across numerous domains due to its inherent flexibility and adaptability. Abstract Background This case series presents five cases of adult atlantoaxial rotatory subluxation (AARS) following traumatic events, focusing on treatment strategies and long-term outcomes. Case presentation This paper includes four Iranian male patients and one Iranian female patient with ages ranging from 25 to 46 years old. Each case involved unique presentations and management approaches. Initial conservative treatments, including skull traction and orthoses, were attempted, but surgical intervention became necessary due to inadequate response. Surgical fixation significantly improved functional status and alleviated symptoms in all patients, leading to long-term relief. Radiological assessments demonstrated successful outcomes post-surgery. Conclusions This study underscores the importance of considering both conservative and surgical options in managing adult atlantoaxial rotatory subluxation to achieve optimal outcomes.

It had a terrible effect on human physical and mental health resulting in massive death, hospitalization, etc. All societies and industries were extremely involved during the COVID-19 pandemic as well including the food industry. In other words, the COVID-19 pandemic has impacted the functioning of food systems globally. All over the world, food businesses have been disturbed by the isolation, strict lockdowns, suspension of nearly all activities, and other protectionist practices have been taken to restrain coronavirus spread. The issue of COVID-19 is significant in various research fields as it presents both opportunities and important challenges. This review article deals with the limitations that the pandemic causes for food systems, food insecurity, food waste and loss, consumers’ different demands, and food quality and safety. Also, this paper brings up some opportunities created during the pandemic time like creative food packaging, the adaptation of food technology, enhancing functional food production, and enhancing food resilience.

Abstract Despite substantial progress in understanding the complex pathophysiology, hypertension remains a serious public health challenge affecting over 1.2 billion adults aged 30–79 years worldwide. Appropriate knowledge of the different pharmaceutical classes of antihypertensive agents and an understanding of the characteristics of individual molecules are essential to optimize clinical outcomes in patients with hypertension. We conducted a computer‐assisted web interviewing (CAWI) quantitative survey in Italy, Poland, and Turkey to investigate physicians’ prescriptions, knowledge, and perceptions of antihypertensive drugs with a focus on β‐blockers, to assess antihypertensive usage patterns and the reasons underlying prescription choices. The survey findings show that β‐blockers retain a pivotal role in the management of hypertension and are prescribed more often for patients with cardiovascular comorbidities than for patients with diabetic comorbidities. In all three countries, nebivolol is the only β‐blocker among the ones analyzed which is consistently prescribed to 20% or more of patients and is overall the most prescribed one for the population with comorbid diabetes. In terms of specific β‐blockers’ features, this study revealed knowledge gaps that underline the need for educational activities focused on the differences among β‐blockers, which are important in choosing the most suitable agent for individualized antihypertensive therapy.

The recent literature on online learning to rank (LTR) has established the utility of prior knowledge to Bayesian ranking bandit algorithms. However, a major limitation of existing work is the requirement for the prior used by the algorithm to match the true prior. In this paper, we propose and analyze adaptive algorithms that address this issue and additionally extend these results to the linear and generalized linear models. We also consider scalar relevance feedback on top of click feedback. Moreover, we demonstrate the efficacy of our algorithms using both synthetic and real-world experiments.

Tissue engineering is a multidisciplinary field that uses biomaterials to restore tissue function and assist with drug development. Over the last decade, the fabrication of three-dimensional (3D) multifunctional scaffolds has become commonplace in tissue engineering and regenerative medicine. Thanks to the development of 3D bioprinting technologies, these scaffolds more accurately recapitulate in vivo conditions and provide the support structure necessary for microenvironments conducive to cell growth and function. The purpose of this review is to provide a background on the leading 3D bioprinting methods and bioink selections for tissue engineering applications, with a specific focus on the growing field of developing multifunctional bioinks and possible future applications.

Abstract Background The current convention for treatment of children with myelomeningocele (MMC) is timely surgical intervention combined with long-term follow-up by a multidisciplinary specialized team. This study aims to investigate the outcomes of MMC patients treated at Namazi Hospital. Methods All children presenting to Namazi Hospital with myelomeningocele between May 2001 and August 2020 were eligible for this study. For those with a documented telephone number, follow-up phone surveys with the patient’s caregivers, on top of the review of the medical documents were carried out to assess mortality, morbidities, and the functional outcome of the care provided to them. Results A total of 125 patients were studied (62 females). All of the patients were followed up for a mean duration of 6.28 years (range 1–23 years). The majority were located in the lumbosacral area. All of the patients underwent postnatal surgical intervention for MMC in Namazi Hospital. Mean age at surgery was 9.51 days. There were statistically significant differences between urinary and bowel incontinence and presence of scoliosis, MMT grading of the lower limbs, school attendance, number of readmissions, and requirement of laminectomy at the initial surgical intervention. Conclusions This study is the first to characterize the long-term outcomes of MMC patients in Iran. This study illustrates that there is a great need for improved access to and coordination of care in antenatal, perioperative, and long-term stages to improve morbidity and mortality.

Abstract The purpose of this research is to investigate the impact of environmental stimuli on the implementation of the agility policy in logistics villages of Tehran province. The current research is applicable-developmental in terms of purpose. The statistical population of the research includes managing directors and members of the board of directors, deputies, managers and heads and supervisors of different departments of logistics villages in Tehran province, whose number is 99 and the sampling method is total number. The information collecting tool was the questionnaire of environmental stimuli made by the researcher, taken from the qualitative section. The reliability of the research was checked and confirmed using Cronbach's alpha criterion in SPSS software. SPSS software was also used for data analysis, and PLS software was used for structural equation modeling. The findings of the research showed that the acceptance percentage of the language term "very high pressure and impact" is confirmed for the category of the impact of environmental stimuli on the implementation of the agility policy of the administrative system of logistics villages in Tehran province, and also the model has a good fit. Extended Abstract Introduction One of the most important policies included in the policies of the administrative system announced by the Supreme Leader is the issue of making administrative structures agile and flexible (paragraphs 10 and 11 of the mentioned document) (Office of Preservation and Publishing of Her Holiness's Works, 2009). And the reason for this importance and emphasis on it is the scientific and research propositions of great scientists and researchers such as Goldman (1995) and SE Lootah et al, (2020) that tie the issue of agility of administrative systems to their survival and organizational life. Evaluation is part of the most important requirements for the perfection of public policies, including the agility of administrative systems, which guarantees their thought-oriented, accurate implementation, and continuous improvement (Vaezi & MOhammadi, 2017). According to many experts, evaluation of implementation is a legal requirement, but in principle, evaluation is a way for effectiveness, policy improvement, policy development, service delivery, ensuring the fulfillment of needs, and using existing potentials. The main core of the new policy is the evaluation that can lead to the effectiveness and efficiency of the service (Equality and Human Rights Commission (EHRC)); this proposition is obviously imaginable and generalized for evaluation of agility policy in logistic villages. Determining suitable areas for the construction of logistics villages and determining its functions in the "Document for the preparation of logistics centers of the country" as an upstream document in 2017 was approved by the Ministry of Roads and Urban Development of IRI., and "Public Warehouses and Customs Services Company of Iran" to as the only example of logistics centers in the country, whose nationalization was approved by the Revolutionary Council of IRI. in the meeting dated 2/20/1359, will be studied as the target community. Logistics centers in different countries not only act as connecting points of different types of transportation, but have appeared as links connecting trade and transportation (Uyanik et al, 2018), which shows the importance of these institutions in the formation and development of logistics in the country. Therefore, the researcher is trying to answer the question: what is the effect of environmental stimuli on the implementation of the agility policy in logistics villages of Tehran province? Theoretical Framework Policy The measures necessary for the effectiveness or execution of the selected policies are concentrated, which is called policy implementation. Enforcement of public policies refers to the enforcement of a specific policy decision in accordance with the law, the court, or the directives of any administrator. In the process of managing policies, they are often developed or added in their details. One of the main stages of policy making is its implementation stage. Establishing a logical and appropriate communication between policy makers and implementers is considered one way to develop countries and it will cause the opinions of employees at the level of organizations and the views of people at the level of society to be somehow involved in policy making and drafting of laws. Paying attention to implementation in the policy-making process accelerates the realization of the goals of the system and leads to the realization of bureaucracy in society (Parsanasab et al, 2022). Organizational Agility Organizational agility can be considered an organizational competency and ability to effectively engage with the ecosystem and help organizations to keep up with technological, economic, social and cultural challenges in a changing environment (Dupont, 2019). Research background Rahimi et al, (2023) discussed the effect of using knowledge management on organizational agility with the mediating role of innovation in defense project-oriented organizations. The findings of this research showed that the application of knowledge management had a direct, positive and significant effect on the agility of defense project-oriented organizations with a coefficient of 0.498, and it can also increase organizational agility through organizational innovation with an indirect effect and a coefficient of 0.363. Therefore, defense project-oriented organizations should focus more on the effective implementation of knowledge management, so that through strengthening innovation and improving the level of organizational agility, they can respond to the rapid changes in the defense environment and create a suitable level of defense deterrence. Parsanasab et al, (2022) examined the presentation of a model for the acceptance of public policies in the field of personnel by the target community. The relevant results showed that the value of the GOF index for the numerical model is equal to 0.495, which is a strong index and shows the overall high quality of the model. Based on this index and other indicators of the model, it was proved that the 22 identified criteria can, in general and in the form of a model, well measure the acceptance of the public policy, and the effects of each of the criteria present a suitable predictability. The impact of the identified variables in the model showed that all the identified factors have a significant effect in explaining the acceptance of the policies. The factor of interference and policy dependence, the methods of implementing the interference policy and the culture of the target society, and the environment of the target society have the greatest effect in explaining the acceptance of policies, and organization has the least effect. The results of this research are consistent with the results of Rahimi et al, (2023), Jamaluddin Ahmad (2020), Jae Moon (2020), Jamaluddin, Ahmad (2020), and Tavakoli Rudi et al, (2019). Jamaluddin Ahmad (2020) showed in a research that the use of management information systems based on information and communication technology had a significant impact on the quality of local government financial reports and accountability and transparency of local government. According to him, these results are strengthened by using the principles of government agility in the form of government agencies that use skill, accountability, flexibility agility, and competence agility. According to the research results, it is suggested: - Involving stakeholders: make maximum use of the network of stakeholders in their work agenda. For this reason, there will be no discrimination, bias and personal interests in the conclusions, and it makes the model objective. - Comprehensive rooting in the ruling paradigms in policy effectiveness: Unlike many public policy evaluation models, categories such as the implementation process, environmental drivers, organizational framework makers, outputs and effects of policy implementation, and implementation have been considered. - Making decisions and defining actions as a result of the evaluation - Use of uncertainty: in the real world, programs and policies are implemented in an environment where uncertainty is one of the main characteristics of it, and decision-making and management, which is an integral part of the evaluation process, also takes place in the light of culture, perceptions, beliefs and values, attitudes, personality, knowledge and human insight, whose main essence is uncertainty. - Emphasis on learning and dissemination of knowledge: learning is the ultimate goal of policy evaluation, which can be facilitated by knowing the activation mechanisms and possible obstacles. - The comprehensiveness of evaluation values: by considering the environmental conditions, organizational frameworks, policy implementation outputs, policy implementation consequences, implementation process and procedures, learning, etc., it has covered different angles of evaluation with a metaphor from the systems theory approach.

22. Ensemble Classifier Design Tuned to Dataset Characteristics for Network Intrusion Detection

Machine Learning-based supervised approaches require highly customized and fine-tuned methodologies to deliver outstanding performance. This paper presents a dataset-driven design and performance evaluation of a machine learning classifier for the network intrusion dataset UNSW-NB15. Analysis of the dataset suggests that it suffers from class representation imbalance and class overlap in the feature space. We employed ensemble methods using Balanced Bagging (BB), eXtreme Gradient Boosting (XGBoost), and Random Forest empowered by Hellinger Distance Decision Tree (RF-HDDT). BB and XGBoost are tuned to handle the imbalanced data, and Random Forest (RF) classifier is supplemented by the Hellinger metric to address the imbalance issue. Two new algorithms are proposed to address the class overlap issue in the dataset. These two algorithms are leveraged to help improve the performance of the testing dataset by modifying the final classification decision made by three base classifiers as part of the ensemble classifier which employs a majority vote combiner. The proposed design is evaluated for both binary and multi-category classification. Comparing the proposed model to those reported on the same dataset in the literature demonstrate that the proposed model outperforms others by a significant margin for both binary and multi-category classification cases.

Free tissue flap transfer can be utilized for reconstruction following tumor resection. While flap failure occurs primarily within 72 h post-operation, late failure after day 7 is rare. We present the case of a 14-year-old with a late lower extremity free flap vascular compromise, along with the successful flap salvage.

24. The Contribution of Teachers' Interpersonal Behaviors to Learners' Autonomous and Controlled Motivation

- Abstract
English as a foreign language (EFL) teachers differ in their autonomy-supportive and controlling interpersonal behavioral styles when motivating learners. Drawing upon the self-determination theory (SDT) and the model of interpersonal teacher behavior (MITB), this study aimed to investigate the impact of teachers' interpersonal behaviors, according to eight behavioral dimensions: leadership, helpful/friendly, understanding, student responsibility/freedom, uncertain, dissatisfied, admonishing, and strict. 1,209 Iranian EFL learners were requested to assess their teachers' interpersonal behavior via the Questionnaire on Teacher Interaction (QTI) as well as their degree and type of motivation (autonomous and controlled) through the Academic Self-Regulation Questionnaire (SDQ-A). The results of the partial least squares structural equation modelling revealed that teachers' controlling behaviours of leadership and strictness significantly enhanced EFL learners' controlled motivation, while their autonomy-supportive behaviours of helpfulness/friendliness and understanding positively influenced learners' autonomous motivation. Teachers' autonomy-suppressive behaviour of uncertainty negatively influenced learners' autonomous motivation. This study's findings encourage stakeholders in the educational sector to consider EFL teachers' interpersonal behaviours and the influence of these behaviors on learning motivation.

Abstract Childhood malnutrition is a metabolic condition that affects the physical and mental well‐being of children and leads to resultant disorders in maturity. The development of childhood malnutrition is influenced by a number of physiological and environmental factors including metabolic stress, infections, diet, genetic variables, and gut microbiota. The imbalanced gut microbiota is one of the main environmental risk factors that significantly influence host physiology and childhood malnutrition progression. In this review, we have evaluated the gut microbiota association with undernutrition and overnutrition in children, and then the quantitative and qualitative significance of gut dysbiosis in order to reveal the impact of gut microbiota modification using probiotics, prebiotics, synbiotics, postbiotics, fecal microbiota transplantation, and engineering biology methods as new therapeutic challenges in the management of disturbed energy homeostasis. Understanding the host–microbiota interaction and the remote regulation of other organs and pathways by gut microbiota can improve the effectiveness of new therapeutic approaches and mitigate the negative consequences of childhood malnutrition.

Approaching posterior fossa pathologies is fairly challenging. Poor exposure, Cerebrospinal Fluid (CSF) leak following surgery, post-operative suboccipital and neck pain, and wound healing are common challenges following traditional suboccipital midline incision. Herein, we present a new incision for approaching posterior fossa pathologies. The incision is shaped like a question mark and makes a musculofascial flap supplied by occipital artery on top of providing a wide area for craniotomy. In our technique, the dura is also incised in a question mark shaped manner. Three patients with masses in posterior fossa were operated with the new incision. Following surgeries, there were no adverse events including CSF leak, wound complications, severe suboccipital pain and neck instability in any of the patients. This new incision not only facilitates approaching to pathologies in posterior fossa with providing wider exposure, but also enables us for watertight Dural closure which decreases CSF leak. Also, as the muscular incision provides a sufficient area for craniotomy, muscular retraction can be minimized to avoid post-operative pain. Moreover, as opposed to the midline avascular incision, the flap is well supplied by occipital artery which facilitates the healing procedure.

Aqueous hydrolysis is used to chemically recycle polyethylene terephthalate (PET) due to the production of high-quality terephthalic acid (TPA), the PET monomer. PET hydrolysis depends on various reaction conditions including PET size, catalyst concentration, reaction temperature, etc. So, modeling PET hydrolysis by considering the effective factors can provide useful information for material scientists to specify how to design and run these reactions. It will save time, energy, and materials by optimizing the hydrolysis conditions. Machine learning algorithms enable to design models to predict output results. For the first time, 381 experimental data were gathered to model the aqueous hydrolysis of PET. Effective reaction conditions on PET hydrolysis were connected to TPA yield. The logistic regression was applied to rank the reaction conditions. Two algorithms were proposed, artificial neural network multilayer perceptron (ANN-MLP) and adaptive network-based fuzzy inference system (ANFIS). The dataset was divided into training and testing sets to train and test the models, respectively. The models predicted TPA yield sufficiently where the ANFIS model outperformed. R-squared (R2) and Root Mean Square Error (RMSE) loss functions were employed to measure the efficiency of the models and evaluate their performance., Comment: 36 pages, 11 figures, 3 tables

As for the buy-out clause, if the player pays the buy-out fee, he/she can terminate the contract prematurely without specifying any cause. The current research principally aims to analyze the legal nature of the buy-out clause and what are its legality conditions in FIFA regulations and Iranian law. Regarding the nature of the buy-out clause, different perspectives have been raised. From a perspective, the buy-out clause can be classified as a liquidated damage clause and is subject to prohibition of exorbitant and unfair liquidation damage provision. According to another perspective, the nature of the buy-out clause is a contractual right. The buy-out fee is in consideration for the exercise of contractual rights. Currently, the latter opinion is preferred by FIFA. Under Iranian law, basically, the nature of buy-out is valid and acceptable, but according to the Iranian Football League’s new charter of registration of employment contracts for the season 2023-2024, the use buy-out clause in the contract of Iranian players is prohibited. The findings of the present study with the analytical-descriptive approach indicate that the nature of the buy-out clause is a conditional opt-out right. Termination by this method should be deemed to be based on the parties’ (prior) consent. The player can trigger the buy-out clause and terminate his/her contractual relationship unilaterally and prematurely upon the unconditional and complete payment of the buy-out fee.

34. Seroprevalence of anti-Helicobacter pylori antibodies in population of Azerbaijan, Iran

Helicobacter pylori (H. pylori) is a common infection which can lead to gastritis, peptic ulcer disease and gastric-originated malignancies. In this study prevalence of seropositivity of each immunoglobulins against H. pylori and also, their association with sex and age were evaluated in a sample of the ordinary population from Tabriz, Iran.In this study, 3733 individuals referred to the laboratory for Para clinical tests between 2019 and 2022, participated. Enzyme-linked immunosorbent assay (ELISA) was utilized to detect the quantity of anti-H. pylori Immunoglobulin G (IgG), Immunoglobulin M (IgM), and Immunoglobulin A (IgA). The statistical analysis was conducted using the 20th version of SPSS software.Out of 3733 participants, 1235 (33.1%) were male and 2498 (66.9%) were female. 57.9% of the participants have positive IgG serology. Also, this index was 0.3% and 11.6% for IgM and IgA, respectively. The mean (SD) age was 40.72 (16.91). There was no significant relationship between gender and IgG and IgA positiveness (p-values = 0.11 and 0.08 respectively). For IgM, serum positiveness was higher in females (0.4% for females and 0.2% for males; p-value: 0.009). The prevalence of positive IgG was increasing significantly (p-value < 0.001). For IgM and IgA there was a significant increase in the number of seropositive individuals with an increase in age (p-value = 0.005; and < 0.001 respectively).This study reveals that the prevalence of H. pylori in Tabriz is approximately 57% which is in the range estimated to be in Iran, but in comparison to developed countries, it was higher. The prevalence of anti-H. pylori immunoglobulins increases significantly with age.
