El traumatismo craneoencefálico (TCE) puede conllevar impactantes cambios en la vida cotidiana que incluyen alteraciones a nivel social - profesional - comunicativo y cognitivo (dificultades atencionales. mnemónicas y ejecutivas). Este estudio tuvo por objeto caracterizar la ocurrencia de déficits neuropsicológicos post-TCE y constatar el impacto ocasionado por el nivel de severidad del trauma en el desempeño cognitivo de los pacientes. Participaron 96 adultos en la muestra total que fue dividida en dos grupos para evaluar el nivel de severidad del trauma: TCE leve (n=39) y TCE grave (n=77). La gravedad de la lesión se clasificó por medio de la Escala de Coma de Glasgow por la duración de la pérdida de consciencia o por la amnesia postraumática. No había diferencias entre la edad y la escolaridad de los participantes. Para la comparación entre los grupos en cuanto a la distribución de ocurrencia de déficits neuropsicológicos se utilizó el Chi-cuadrado. Se empleó una batería de evaluación neuropsicológica flexible conformada por tareas verbales y viso espacial de habilidades lingüísticas mnemónicas y ejecutiva. Los grupos no se diferenciaron en cuanto a las variables sociodemográficas. Los pacientes con TCE leve tuvieron mejores puntajes comparados con los de TCE grave (número de errores y categorías completadas del Test de clasificación de tarjetas de Wisconsin [WCST. por sus siglas en inglés]; errores en la parte B del Test de Hayling; y en la interferencia retro y proactiva del Test de aprendizaje auditivo verbal de Rey [RAVLT. por sus siglas en inglés]. El nivel de severidad del trauma parece mostrar diferencias en los individuos en cuanto al desempeño en memoria episódica de información nueva y en el control de interferencia entre los recuerdos; lo mismo se aplica a las funciones de flexibilidad e inhibición. Estos resultados sugieren que es necesaria una mayor inversión en acciones de políticas públicas priorizando intervenciones neurocognitivas reeducativas y métodos de prevención de accidentes relacionados con lesiones traumáticas que tengan alta incidencia de secuelas. Traumatic brain injury (TBI) can lead to significant changes in daily life as well as in social labor communicative and cognitive domains (attention. memory and executive functions). This study aimed to characterize the occurrence of post-TBI neuropsychological deficits as well as to determine whether there is an impact related to the level of severity of the trauma on the patient’s performance. Ninety-six adults participated in the study who were divided in two groups to assess the trauma’s level of severity: mild TBI (n=39) and severe TBI (n=57). This severity was classified by the Glasgow Coma Scale by the duration of consciousness loss or by post-traumatic amnesia. There were no differences between the groups regarding variables of age and years of schooling. A Chi- square test was used to do a comparison between the two groups in terms of occurrence of neuropsychological deficits. Verbal. Visuospatial – mnemonic - linguistic and executive tests composed a flexible neuropsychological battery. Patients with mild TBI had better scores compared to those with severe TBI (number of errors and in completed categories of the Modified Wisconsin Card Sorting Test (MWCST); errors in Part B of The Hayling Test; and proactive and retroactive interference in the Rey Auditory Verbal Learning Test (RAVLT). The severity of the trauma seems to differentiate individual’s performance on episodic memory of new information and in the control of interference between memories; the same is applied to flexibility and inhibition functions. These results suggest the need for more investments in public health policy actions prioritizing neurocognitive remedial intervention and prevention methods for such condition with high incidence of sequels.

Over recent years, neuropsychological research has been increasingly concerned with the need to develop more ecologically valid instruments for the assessment of executive functions. The Hotel Task is one of the most widely used ecological measures of executive functioning, and provides an assessment of planning, organization, self-monitoring and cognitive flexibility. OBJECTIVE: The goal of this study was to adapt the Hotel Task for use in the Brazilian population. METHODS: The sample comprised 27 participants (three translators, six expert judges, seven healthy adults, ten patients with traumatic brain injuries and one hotel manager). The adaptation process consisted of five steps, which were repeated until a satisfactory version of the task was produced. The steps were as follows: (1) Translation; (2) Development of new stimuli and brainstorming among the authors; (3) Analysis by expert judges; (4) Pilot studies; (5) Assessment by an expert in business administration and hotel management. RESULTS: The adapted version proved adequate and valid for the assessment of executive functions. However, further research must be conducted to obtain evidence of the reliability, as well as the construct and criterion validity, sensitivity and specificity, of the Hotel Task. CONCLUSION: Many neurological and/or psychiatric populations may benefit from the adapted task, since it may make significant contributions to the assessment of dysexecutive syndromes and their impact on patient functioning. A busca por validade ecológica dos instrumentos neuropsicológicos vem recebendo uma atenção especial nos últimos anos, especialmente daquelas ferramentas que avaliam funções executivas. A Tarefa do Hotel é uma das tarefas ecológicas de exame executivo mais reconhecida internacionalmente, por examinar planejamento, organização, automonitoramento e flexibilidade cognitiva. OBJETIVO: O objetivo deste estudo é apresentar o processo de adaptação da Tarefa do Hotel para uma versão brasileira. MÉTODO: A amostra constitui-se de 27 indivíduos (três tradutores, seis juízes especialistas, sete adultos saudáveis, dez pacientes pós traumatismocranioencefálico e um gerente de hotel). Cinco etapas foram conduzidas, replicadas ao longo do processo: (1) tradução, (2) desenvolvimento de novos estímulos e brainstorming entre os autores, (3) análise de juízes especialistas em neuropsicologia, (4) estudos pilotos e (5) julgamento de expert em administração e hotelaria. RESULTADO: A versão adaptada mostrou-se adequada e válida para avaliar componentes executivos, sendo necessários estudos futuros em busca de evidências de fidedignidade, validades de construto e de critério, sensibilidade e especificidade, considerando-se as particularidades de uma ferramenta ecológica. CONCLUSÃO: Muitas populações clínicas neurológicas e/ou psiquiátricas poderão se beneficiar do exame com a Tarefa do Hotel, por esta auxiliar no diagnóstico de síndrome disexecutiva relacionada à demanda de componentes executivos no cotidiano.

OBJECTIVE: To present performance norms and discrepancy score of three one-minute verbal fluency tasks (VFTs); to investigate age and education effects; to analyze the differences between time intervals; and to investigate whether these differences varied according to age and education. METHOD: Three hundred adults divided into three age groups (19-39; 40-59; 60-75) and two groups of educational level (2 to 7 years; 8 years or more) performed unconstrained, semantic, and phonemic VFTs. We compared the performance of the groups using two-way ANOVA with post-hoc Bonferroni test. The depression scale score was covariate. The time interval of verbal fluency was the variable used for subjects' comparison (repeated measures ANOVA). RESULTS AND CONCLUSIONS: Our results suggest that there are age and education effects on phonemic and unconstrained VFTs. We also found an interaction between those variables in the semantic VFT (time intervals and total time) and in the differences between semantic and phonemic tasks. The repeated measures analysis revealed age effects on semantic VFTs and education effects on the phonemic and semantic VFTs. Such findings are relevant for clinical neuropsychology, contributing to avoid false-positive or false-negative interpretation. OBJETIVO: Esse estudo teve como objetivo apresentar normas de desempenho e discrepância em três tarefas de fluência verbal de um minuto e investigar os efeitos de idade e escolaridade. Além disso, investigamos as diferenças entre os intervalos de tempo e se essas diferenças variaram em função da idade e escolaridade. MÉTODO: Participaram 300 adultos divididos em três grupos de idade (19-39; 40-59; 60-75) e anos de escolaridade (2 a 7; 8 ou mais), avaliados com tarefas de fluência verbal livre, fonêmica ou semântica. O desempenho entre grupos foi comparado por uma two-way ANOVA com post-hoc Bonferroni, com o escore da escala de depressão como co-variável. A variável de comparação intragrupos foi o tempo da fluência verbal (ANOVA medidas repetidas). RESULTADOS E CONCUSÕES: Os resultados indicaram efeitos principais de idade e escolaridade nas tarefas ortográfica e livre, e uma interação entre essas variáveis na tarefa semântica (intervalos e total), e na discrepância entre as tarefas semântica e ortográfica. A análise de medidas repetidas demonstrou efeitos principais de idade na tarefa semântica e de educação nas tarefas semântica e ortográfica. Tais achados são relevantes para futuras interpretações de dados clínicos, evitando assim falsos positivos ou negativos.

Os indiv?duos que sofrem TCE podem ter grande incapacidade funcional. A maior parte dessas disfun??es tem rela??o com manifesta??es cl?nicas de preju?zos cognitivos, o que cria diversas perdas em diferentes ?reas, tais como, trabalho, autonomia e como conseq??ncia uma qualidade de vida n?o apenas o indiv?duo, mas tamb?m todos aqueles que est?o envolvidos com ele. Neste contexto, os estudos t?m discutido a heterogeneidade das manifesta??es cl?nicas do TCE, ainda um grande desafio em ensaios cl?nicos e estudos de caracteriza??o como um todo. A maior lacuna na ?rea de neuropsicologia cl?nica e cognitiva ? a compreens?o das fun??es executivas (FE) p?s-TCE e de seus correlatos neurais. ? de grande interesse integrar a avalia??o neuropsicol?gica, os m?todos cl?ssicos cl?nicos da neuropsicologia, e t?cnicas avan?adas de neuroimagem. A presente disserta??o visou ainvestigar FE no contexto cl?nico do TCE. Dois estudos foram realizados nesta disserta??o. O primeiro estudo avaliou o perfil de funcionamento executivo de uma amostra de TCE. Este panorama executivo foi baseado em uma extensa avalia??o neuropsicol?gica com foco principal em FE. Os resultados apontaram tr?s perfis diferentes de FE: o Cluster 1 foi caracterizado por dificuldades em velocidade de processamento, flu?ncia verbal fon?mica e inibi??o; o Cluster 2 foi formado por m?ltiplos d?ficits em FE, tais como, de velocidade de processamento, mem?ria de trabalho, planejamento, flexibilidade cognitiva e flu?ncia verbal; finalmente o Cluster 3 n?o apresentou dificuldades executivas objetivamente examinadas. O segundo estudo investigou o desempenho em tarefas de FE e ?ndices de volumetria e de espessura cortical em regi?es de interesse por seu correlato com FE em dois casos de adultos com TCE leve com diferentes escolaridades. O paciente com alta escolaridade superou o paciente de baixa escolaridade em quatro vari?veis das FE e em diferentes estruturas de volume cerebral e espessura cortical. Os resultados sugerem que a educa??o parece ser uma caracter?stica de reserva cognitiva no TCE leve. Juntos, esses estudos contribuem com respostas a uma pergunta importante sobre poss?veis solu??es para a heterogeneidade neuropsicol?gica do TCE. Nossos achados refor?am a relev?ncia de interven??es em grupo constitu?do de acordo com vari?veis socioculturais e perfis cognitivos. Para a forma??o de subgrupos cl?nicos de TCE, seu funcionamento executivo parece ser sido a principal vari?vel, na medida em que n?o houve diferen?as quanto a fatores socioculturais,m individuais nem cl?nicos. No entanto, quando se considera an?lise de casos, vari?veis socioculturais parecem importantes para o desempenho cognitivo e para a reorganiza??o cerebral no TCE leve. TBI individuals may present great functional disability. Most of these dysfunctions is related to clinical manifestations of cognitive impairment, which creates several losses in different areas, such as work, autonomy and, as a consequence, on the quality of life of individuals and their relatives. In this context, studies have discussed the heterogeneity of clinical manifestations in TBI, which challenges research of clinical trials and characterization studies as a whole. One of the main issues in the field of clinical and cognitive neuropsychology is the understanding of executive functions (EF) post-TBI and their neural correlates. For this reason, it is of great interest to integrate neuropsychological evaluation, classical methods of clinical neuropsychology and neuroimaging. This dissertation aimed to investigate EF in TBI individuals by means of two studies. The first study investigated the profile of executive functioning in a sample of TBI individuals. This study included an extensive neuropsychological evaluation with a primary focus in FE. Results showed three different profiles of FE: Cluster 1 was characterized by difficulties in processing speed, phonemic verbal fluency and inhibition; Cluster 2 was formed by multiple deficits in FE, such as processing speed, working memory, planning, cognitive flexibility and verbal fluency; finally Cluster 3 had no or very mild difficulties on the EF examined. The second study investigated the two cases of adults with mild TBI with different education levels on EF performance and brain structures volumetry and cortical thickness. The patient with high education surpassed the patient with low education in four variables of FE and different structures of brain volume and cortical thickness. The results suggest that education seems to be a feature of cognitive reserve in mild TBI. Together, these studies contribute for answers to an important question about heterogeneity of TBI and clinical studies. Our findings reinforce the importance of group interventions constituted according to sociocultural variables and cognitive profiles, rather than sociocultural, individual and clinical variables. However, when considering case analysis, sociocultural variables seem to be important for cognitive performance and brain reorganization in mild TBI.

Este estudo objetivou examinar os efeitos de um Treinamento da Memória de Trabalho (TMT) em idosos saudáveis. Vinte participantes compuseram a amostra final, onze do grupo experimental (TMT) e nove do controle (socialização). Todos foram submetidos a uma avaliação neuropsicológica pré e pós-intervenção. Os encontros foram realizados uma vez por semana, durante três meses. Houve melhora significativa, no grupo experimental, em atenção concentrada, aprendizagem, memória de curto prazo e episódica, e no grupo controle, em um menor número de variáveis, na atenção concentrada e memória episódica. O TMT parece ter promovido efeito de transferência, principalmente na memória episódica, que é relacionada diretamente aos subsistemas da memória de trabalho (MT), sugerindo que o TMT pode ser útil no contexto da neuropsicologia do envelhecimento.
