In recent years, with the gradual and extensive application of digital medical treatment in the field of medica anesthesia, its role in the postoperative pain management of children has attracted increasing attention. It provides new ideas for the preoperative prediction, postoperative monitoring and evaluation, and treatment management of postoperative pain in children, as well as a new scheme for anesthesiologists to formulate the best postoperative pain management strategy for children. This article reviews digital medical treatment and its application in clinical anesthesia, and reviews the application prospects of this therapy in postoperative pain in children, in order to deepen clinicians' knowledge.

Background: To explore the correlation between left atrial appendage morphology, blood flow velocity and plasma galectin-3 and thrombosis in patients with atrial fibrillation. Methods: Patients with atrial fibrillation who received treatment and completed ultrasound examination in hospital from 2022 to December 2023 were enrolled. According to whether there was left atrial appendage thrombosis, the patients were divided into a control group (no left atrial appendage thrombosis was found) and a study group (left atrial appendage thrombosis was found). The morphology and structure of the left atrial appendage, blood flow velocity and plasma galectin-3 level were recorded exploring its correlation with left atrium thrombosis. Results: A total of 330 patients with atrial fibrillation were enrolled, including 278 in the control group and 52 in the study group. Left group and the control group of morphological structure differences (P < 0.05). The main lobe length, ostial area, longest diameter, shortest diameter, left atrial appendage volume and left atrial volume in the study group were higher than those in the control group (P < 0.05). The left atrial appendage emptying velocity, filling velocity and left ventricular ejection fraction of the study group were lower than those of the control group, and the left ventricular end-diastolic diameter was higher than that of the control group (P < 0.05). Group of white blood cell count, neutrophils/lymphocyte ratio, plasma galactose lectin-3 levels were higher than control group (P < 0.05). ROC curve analysis of left atrial appendage emptying velocity, left atrial appendage filling velocity, left atrial enddiastolic diameter and left atrial ejection fraction had higher diagnostic value (P < 0.05). Conclusions: Left atrial appendage morphology, blood flow velocity and plasma galectin-3 level are important factors to evaluate the risk of left atrial appendage thrombosis in patients with atrial fibrillation. This study improves the understanding of thrombosis, further elucidates the risk factors for thrombosis, and improves patient prognosis.

This paper aims to improve college students’ entrepreneurial abilities as its main goal. A platform for college students’ entrepreneurship development is constructed using cloud technology, and the entrepreneurship data on the platform is classified and divided by combining it with the KNN algorithm. The algorithm is optimized for distance calculation and distance sorting stages to reduce inter-program dependencies and shorten the program execution time. The distance between matrices is calculated by calling the functions in the cublas function library. The experimental data are used to analyze the feasibility of entrepreneurship training for college students. The results show that the KNN algorithm can be applied to data processing in cloud computing. When the data dimension is 128 and the number of sample points is 215, the algorithm can get a maximum speedup of 1.52 times. The server configuration of the platform needs to be in a 4-way dual-core or 4-core to ensure the platform’s normal operation. This study is beneficial in promoting the improvement and development of entrepreneurial skills for college students.

Autism spectrum disorder (ASD) is becoming one of the fastest growing neurodevelopmen-tal disorders around the world, yet its clinical treatment still faces challenge due to the heterogeneity in etiology and symptom phenotypes. It is believed that excitatory-inhibitory (E-I) imbalance in the central nervous system may play an important role in the pathogenic mechanisms of ASD. E-I imbalances in synaptic transmission and neural circuits are frequently observed in different animal models of ASD, and the corresponding reversion normalizes the autism-like behaviors in these animals. Some E-I modulators have been tested for their therapeutic potential on ASD patients with encouraging results. This article expounds the mechanism of E-I imbalance in ASD and E-I imbalance regulators treatment progress, to provide new insights on the therapeutic targets for ASD.

Objective To investigate the role of tumor draining lymph nodes (TDLN) in the generation of memory CD8+ T cells under the context of neoadjuvant anti-PD-L1 therapy. Methods A mouse transplanted model of CT26/MC38-OVA colon cancer was established to recapitulate clinic neoadjuvant anti-PD-L1 therapy, and then the tumor masses and TDLN were removed in 15 d after transplantation. On day 30 after surgery, the effect of TDLN on the formation of immune memory was assessed under the condition of neoadjuvant PD-L1 antibody therapy by tumor re-challenge. The effect of neoadjuvant PD-L1 on tumor-specific CD8+ T cells was investigated by adoptive transfer CD45.1+ OT-Ⅰ cells. The distribution and phenotype of CD45+ CD8+T cells in various tissues were determined by flow cytometry at the time of surgery (effector phase) and 30 d after surgery (memory phase). Results The tumor mass was reduced significantly after receiving neoadjuvant anti-PD-L1 therapy (P < 0.05). After tumor re-challenge, tumor growth was significantly slowed in the mice treated with neoadjuvant PD-L antibody, whereas TDLN resection accelerated the tumor growth (P < 0.05). However, long-term survival of tumor re-challenge showed that all tumors were eventually subsided, whether or not the mice treated with neoadjuvant PD-L antibody. The results of adoptive reinjection indicated that the number of CD45.1+ CD8+ T cells being stored in TDLN and no-draining lymph nodes (NLN) was the highest (P < 0.05). Tcm (T central memory) cells and Trm (tissue-resident memory) cells were mainly distributed in TDLN and NLN in memory phase (P < 0.05). The distribution and memory phenotype of CD45.1+ CD8+ T cells in TDLN and NLN had no obvous changes after neoadjuvant anti-PD-L1 therapy. Conclusion TDLN is the primary site for the storage of memory CD8+ T cells, but under the condition of neoadjuvant PD-L1 antibody treatment, TDLN resection does not affect their long-term anti-tumor immunity.

Turbulence intensity in test section is an important index to evaluate the flow-field performance of wind tunnel, and provide guarantee for model's dynamics and accurate test. The prediction model should be established to evaluate the effect of rectification design to ensure the turbulence-intensity index. Two methods are introduced and used to predict the turbulence-intensity of a continuous and transonic wind tunnel comparing with test results. It was found that the turbulence-intensity variation trend obtained from the two methods is basically same, turbulence-intensity attenuation is mainly concentrated in the first three screens, about 65%, the result calculated by model-one is 21% larger than the measured value and the model-two result is much closer. These two methods can be used to optimize the design of rectifying device effectively although the results obtained by two methods are slightly different form the measured value.

High-end CNC machine tools' primary transmission mechanism is a high-speed electric spindle. Thermal displacement of the spindle occurs as a result of heat created inside the spindle during transmission, which has an impact on the machining precision of high-end CNC machine tools. Establishing a high-precision motorized spindle simulation model is crucial because it serves as a foundation for optimizing and testing the motorized spindle's construction and material, adjusting for thermal errors, and estimating its life. The thermal-solid coupling model of the motorized spindle is created by Ansys based on the experimental data of temperature and thermal displacement of the A02 motorized spindle and in accordance with the boundary conditions. The accuracy of front and rear bearings is 93.42% and 90.52%, respectively, when compared to experimental data, and the accuracy of axial thermal displacement is 95.16%. Finally, the motorized spindle is optimized to extend its service life after the thermal displacement, stress, and strain of the bearing are model.

Objective o optimize the system conditions and improve the patients' perioperative surgical safety through feedback learning for adverse events. Methods We retrospectively analyzed the adverse events (AEs) and risks of patients' safety reported by the Department of Anesthesiology, Peking Union Medical College Hospital from January 2010 to December 2014. Fish bone diagram was used to analyze and summarize the root causes, timing, avoidability and injury from adverse events. Results A total of 370 cases of AEs were collected. The root causes of AEs were mainly human factors (43.08%, 224/520), followed by systemic factors (30.96%, 161/520), and patient factors were the lowest (25.96%, 135/520). Forty-four cases (11.89%, 44/370) were emergent operations; 37 cases (10.00%, 37/370) occurred at night; 243 cases (65.68%, 243/370) could be avoided. According to the different occurrence time, the top four incidence rates were intraoperative (28.11%, 104/370), before anesthesia induction (17.30%, 64/370), during anesthesia induction (13.78%, 51/370), and at the end of surgery (12.97%, 48/370). Eighty AEs (21.62%, 80/370) did not cause injury; 262 cases (70.81%, 262/370) caused temporary minor injury; 15 cases (4.05%, 15/370) caused permanent injury; 13 cases (3.51%, 13/270) caused non-anesthesia related death intraoperatively or within 24 hours after the surgery. Compared with avoidable AEs, the proportion of patient factors (60.26% vs. 11.26%, P < 0.001) and that of non-anesthesia related death intraoperatively or within 24 hours after the surgery (8.66% vs. 0.82%, P < 0.001) in the unavoidable group were higher. Conclusions The root causes of perioperative AEs are mainly human factors, most of which can be avoided and cause temporary mild injury to patients. Strengthening feedback learning of AE, improving personnel training and optimizing system conditions might reduce the incidence of AE and promote perioperative safety and quality.

Pediatric ambulatory surgery can minimize the separation of pediatric patients from their parents, relieve anxiety, and offer care at a reduced cost. However, children are prone to experiencing pain, nausea, vomiting, delirium, and other complications after anesthesia and ambulatory surgery due to their anatomical and physiological characteristics. So, standardized postoperative management is critical to ensure the safety of children and improve the medical quality of pediatric ambulatory surgery. This article reviews new progress in research on common postoperative complications and discharge criteria of pediatric ambulatory surgery.

Long-term infection of the stomach with Helicobacter pylori can cause gastric cancer. Here, Kinoshita-Daitoku et al. show that a small non-coding RNA of H. pylori regulates bacterial adaptation to the stomach environment and bacterial oncoprotein production.

The fatigue behavior of cotton warps was studied by a newly weaving load simulator (WLS) developed in our laboratory. Reborn hairiness, strength retention, and elongation retention of sized warps were adopted to evaluate the physical and mechanical properties of sized warps under stretch-abrasion cyclic loading. The influences of different fatigue cycles on the above three indicators were also discussed. The results indicated that the fatigue behavior of the cotton warps accompanied by abrasion yields a three-parameter Weibull distribution. All the fitting plots show acceptable linearity. Moreover, there is a strong relationship of quadratic polynomial between the tensile properties and the fatigue cycle of the sized warps according to the scatter fitting (R2 > 91.08%). Similarly, there is also a good relationship of quadratic polynomial between the reborn hairiness index and the fatigue cycle of the sized warps (R2 > 94.51%). Finally, regardless of the strength retention, elongation retention, and reborn hairiness, the physical and mechanical properties of the cotton warps still change with the continuous increase of the fatigue cycle after 40% of the fatigue cycle, but it is not significant. The research was helpful to estimate the capacity of the warps to sustain failure.

Abstract Enterobacter cloacae complex (Ecc) species are widely distributed opportunistic pathogens mainly associated with humans and plants. In this study, the genomes of clinical isolates including E. hormaechei, E. kobei, and E. ludwigii and non-clinical isolate including E. nimipressuralis were analysed in combination with the genome of E. asburiae by using the reference strain E. cloacae subsp. cloacae ATCC 13047; the Ecc strains were tested on artificial sputum media (ASM), which mimics the host, to evaluate T6SS genes as a case study. All five Ecc strains were sequenced in our lab. Comparative genome analysis of the Ecc strains revealed that genes associated with the survival of Ecc strains, including genes of metal-requiring proteins, defence-associated genes and genes associated with general physiology, were highly conserved in the genomes. However, the genes involved in virulence and drug resistance, specifically those involved in bacterial secretion, host determination and colonization of different strains, were present in different genomic regions. For example, T6SS accessory and core components, T4SS, and multidrug resistance genes/efflux system genes seemed vital for the survival of Ecc strains in various environmental niches, such as humans and plants. Moreover, the ASM host-mimicking growth medium revealed significantly high expression of T6SS genes, including PrpC, which is a regulatory gene of the T6SS, in all tested Ecc strains compared to the control medium. The variations in T6SS gene expression in ASM vs. control showed that the ASM system represents a simple, reproducible and economical alternative to animal models for studies such as those aimed at understanding the divergence of Ecc populations. In summary, genome sequencing of clinical and environmental Ecc genomes will assist in understanding the epidemiology of Ecc strains, including the isolation, virulence characteristics, prevention and treatment of infectious disease caused by these broad-host-range niche-associated species.

Temperature field and thermal-structure coupling stress field calculation were conducted to a main steam safety valve prototype for nuclear power stations using finite element software Ansys,and the stress of the valve subjected to operating pressure load and thermal stress was in compliance with the stress evaluation criteria. In addition,alternating stress of the pressure components for the specified cyclic operation application of loads and thermal stresses were calculated,and fatigue analysis was evaluated according to RCC-M code. Results indicate that the valve prototype is able to meet the requirements of fatigue failure criteria for the specified cycles and would have good prospects in engineering applications.

A new type of frictionless synchronizer is designed，and its three-dimensional model is built by using the software CATIA. The three-dimensional model is imported into Adams to establish a virtual prototype model and a two-gear pure electric vehicle model is built in Matlab/Simulink which is used to simulate the speed synchronization and backing process of the new synchronizer. The simulation results show that the jerk produced by the new type of synchronizer in the speed synchronization and backing process is reduced and the required time of speed synchronization and backing is also greatly reduced，which reduce the power interruption and shift frustration in the process of automobile shift，comparing with the traditional synchronizer which relies on friction to achieve speed synchronization. Finally，the influence of vehicle speed in synchronization process, helical spring stiffness and shift force for the synchronization process of frictionless synchronizer is analyzed.

This study designs one kind of hydraulic auto-brake-valve calculates its driving force and velocity, simulates its braking force and stability, in order to accomplish auto brake in the landing condition and keep braking in cruising condition of the unmanned aerial vehicle. The auto-brake-valve uses a servo motor to drive lead screw and nut. The output press of the auto-brake-valve is changed by the variation of rotate speed and direction of servo motor, that can be completed the control of the brake device.

Unlike the normal sizing method, the foam sizing had been proven to be a low-add-on technology. To investigate the effect of foam sizing, film thickness, sized-yarn evenness, and size penetration rate were necessary to evaluate the performances of foam-sized yarns. However, the conventional image analysis of sized-yarn cross sections primarily relied on artificial testing with a low efficiency. This paper proposed a novel dynamic method to measure the sized-yarn properties including film thickness, sized-yarn evenness, and size penetration rate based on yarn sequence images captured from a moving yarn. A method of dynamic threshold module was adopted to obtain threshold for segmenting yarns in the sequence images. K-means clustering algorithm was applied to segment pixels of the images into yarn and background. To further remove burrs and noise in the images, two judgment templates were carried out to extract the information of yarn core. The film thickness, sized-yarn evenness, and size penetration rate were measured based on the yarn core of each frame in sequence images. In order to compare with the experimental results of the dynamic method, the yarn properties of the same samples were tested by static and artificial testing. Results revealed that the proposed method could efficiently and accurately detect the film thickness, sized-yarn evenness, and size penetration rate.

This study was aimed at investigating the process optimization of foam sizing for cotton yarns. In this work, effects of major foam-sizing process factors including size concentration, blowing ratio, stirring speed, pre-wetting temperature, pre-drying temperature, squeezing pressure and drying temperature were studied on the hairiness (more than 3 mm) and abrasion resistance of foam-sized yarns. The combination of Plackett-Burman, steepest ascent path analysis and Box-Behnken design were adopted to optimize the foam-sizing process of cotton yarns. Results revealed that size concentration, blowing ratio and squeezing pressure were significant factors that affected the hairiness and abrasion resistance. Optimum hairiness and abrasion resistance were obtained when the cotton yarns were sized at size concentration of 19.33%, blowing ratio of 4.27 and squeezing pressure of 0.78kN. The theoretical values and the observed values were in reasonably good agreement and the deviation was less than 1%. Verifcation and repeated trial results showed that it has good reproducibility and imparts the foam sizing process of cotton yarns.

To achieve a better energy-efficient and comfortable vehicle driving, electric vehicle energy predictive optimal control was discussed in this article. It used Vehicle to Infrastructure (V2I) communication of traffic light distance and time gap to make velocity decision in order to improve whole driving performance. Based on the data of standard New European Driving Cycle, this article put forward a real-time cycle including traffic light position and time information at first. Then, a preview and optimal decision controller was built. Four evaluating indexes were set for optimal decision making to decide the optimal expected speed, according to the pass time that the forward traffic lights allowed and the distance or the forward vehicle speed and distance. After that AMESim and Simulink co-simulation platform was used to build the vehicle dynamics model and to exploit the control algorithm. The simulation results indicated that vehicle with V2I can effectively improve the operating condition of the whole vehicle and reduce the frequency of vehicle starting and stopping, which makes the vehicle driving smoother and steady. Moreover, it can reduce vehicle energy consumption and improve endurance mileage. Meanwhile, the vehicle acceleration and jerk have decreased dramatically with better comfortable performance.

The continuous rigid frame bridge is one of the main bridge types used to span rivers in mountainous areas due to its high rigidity. However, due to the poor geological conditions in mountainous areas, the probability of instability and other safety accidents increases. Therefore, strict construction scheme design must be carried out. According to the cast-in-place section of the side span of a continuous rigid frame bridge in a mountainous area, this paper puts forward three construction schemes that may be applicable. The finite element software is used to establish the model, and the girder stress and bridge alignment with different construction schemes are analyzed. Combined with the grading index evaluation method, the optimal construction scheme is finally obtained.
