• Purpose: Surgical site infection (SSI) is a rare and serious complication of total knee arthroplasty (TKA), which causes a poor prognosis for patients. The purpose of this study was to explore the effect of intraosseous (IO) antibiotics in preventing infection and complications after TKA compared with intravenous (IV) antibiotics and to provide a certain theoretical basis for clinical treatment. • Methods: The review process was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We searched the PubMed, Embase, Ovid, Web of Science, and the Cochrane Central Register of Controlled Trials databases about trials on IO antibiotics (into the proximal tibia before skin incision) to prevent infections in TKA from the respective inception dates to September 30, 2022. The infection occurred within 3 months after surgery. Both researchers individually screened the studies in accordance with the inclusion and exclusion criteria, performed the literature quality evaluation and data extraction, and used Stata 17 software for data analysis. • Results: Five studies that enrolled 3801 patients were included in this meta-analysis. The results showed that IO antibiotics were effective in reducing the incidence of SSI (OR: 0.25, P = 0.001) and periprosthetic joint infections (OR: 0.16, P = 0.004) relative to IV. Moreover, the percentage of infection due to Gram-positive bacteria (OR: 0.18, P = 0.025) was reduced in the IO group compared with that in IV group, but Gram-negative bacteria levels were not significantly reduced (P = 0.14). There was no difference between the two groups for other systemic adverse effects of the drug. • Conclusions: IO antibiotics in TKA are safe and effective alternatives to IV antibiotics. Large randomized clinical studies comparing infection rates and related complications with IO and IV antibiotics are required.

Objectives: To develop an evaluation model for, and identify key factors contributing to, burnout in orthopedic surgeons, providing a reference for the management of burnout among orthopedic surgeons in hospitals.Methods: We developed an analytic hierarchy process (AHP) model with 3 dimensions and 10 sub-criteria based on an extensive literature review and expert assessment. We used expert and purposive sampling and 17 orthopedic surgeons were selected as research subjects. The AHP process was then used to obtain the weights and to prioritize the dimensions and criteria for burnout in orthopedic surgeons.Results: The dimension of C1 (personal/family) was the key factor affecting burnout in orthopedic surgeons, and in the sub-criteria, the top four sub-criteria were C11 (little time for family), C31 (anxiety about clinical competence), C12 (work-family conflict), and C22 (heavy work load).Conclusion: This model was effective in analyzing the key factors contributing to job burnout risk, and the results can inform improved management of the levels of burnout affecting orthopedic surgeons in hospitals.

IntroductionThe study aimed to evaluate the efficacy of pronator quadratus (PQ) repair versus no repair following volar plate fixation of distal radius fractures.MethodsA comprehensive search was performed in PubMed, CNKI, EMBASE, Web of Science, Ovid, and Cochrane Library databases. All randomized controlled trials comparing PQ repair with no repair in distal radius fractures before January 2023 were included. Two investigators independently screened eligible articles, assessed the study quality, and extracted data from included studies. Continuous variables used standardized mean difference and 95% confidence interval as efficacy statistics. The meta-analysis was performed using the Revman 5.4 software.ResultsA total of 430 patients in 7 RCT studies were included in this meta-analysis, of which 218 underwent PQ repair, while 212 patients underwent no repair. The results of the meta-analysis displayed statistically significant differences in grip strength (short-term), pronation angle (short-term), and pronation strength (short- and long-term) between the two groups. No significant difference in other outcomes was found between the two treatment arms.DiscussionThe repair of PQ may further increase grip strength and pronation function in the short-term and enhance long-term pronator muscle strength compared to no repair. However, due to the small number of articles included in the study, the above conclusions need to be verified by a larger sample and multi-center clinical study.

ObjectiveTo evaluate the safety and effectiveness of using a new sternoclavicular hook plate combined with a shoulder locking hook plate for the treatment of bipolar clavicle injuries.MethodsRetrospective analysis of 7 patients with bipolar clavicle injury, all-male, with a mean age of 51.1 years, who underwent a new sternoclavicular hook plate combined with a shoulder clavicular hook plate implantation.ResultsAll 7 patients were fixed with a sternoclavicular hook plate combined with a repositioned shoulder locking hook plate and received 13–24 months of follow-up. There were no postoperative complications, no wound infections, and no plate or screw fractures. The mean ASES score was 94.3 ± 2.8.ConclusionThe safety and effectiveness of a new sternoclavicular hook plate combined with a shoulder locking hook plate in the treatment of bipolar clavicle injuries.

Clostridial myonecrosis is most often seen in settings of trauma, surgery, malignancy, and other underlying immunocompromised conditions. Since 1953 cases of gas gangrene have been reported in orthopaedic patients including open fractures, closed fractures, and orthopaedic surgeries. We present a case of 55-year-old obese woman who developed rapidly progressive gas gangrene in her right leg accompanied by tibial plateau fracture without skin lacerations. She was diagnosed with clostridial myonecrosis and above-the-knee amputation was carried out. This patient made full recovery within three weeks of the initial episode. We identified a total of 50 cases of gas gangrene in orthopaedic patients. Several factors, if available, were analyzed for each case: age, cause of injury, fracture location, pathogen, and outcome. Based on our case report and the literature review, emergency clinicians should be aware of this severe and potentially fatal infectious disease and should not delay treatment or prompt orthopedic surgery consultation.

Hydrogel, a polymer material with a three-dimensional structure, has considerably expanded in research across multiple fields lately. However, the lack of a comprehensive review integrating the research status of hydrogel across diverse fields has hindered the development of hydrogel. This bibliometric analysis reviewed the hydrogel-related research over the past decades, emphasizing the evolution, status, and future directions within a multitude of fields, such as materials science, chemistry, polymer science, engineering, physics, biochemistry molecular biology, pharmacology pharmacy, cell biology, biotechnology applied microbiology, etc. We encapsulated applications and the potential of hydrogel in wound healing, drug delivery, cell encapsulation, bioprinting, tissue engineering, electronic products, environment applications, and disease treatment. This study integrated the current matrix system and characteristics of hydrogels, aiming to offer a cross-field reference for hydrogel researchers and promote the advancement of hydrogel research. Furthermore, we proposed a novel and reproducible bibliometric research paradigm, which can provide a more comprehensive analysis of the trends and trajectory of a research field. An efficient composite adsorbent was prepared based on acryloyl starch/carboxymethyl cellulose grafting copolymerization for the adsorptive removal of methylene blue. The developed composite hydrogels were characterized by means of FTIR, SEM (EDS-SEM), and XRD. Various parameters affecting the removal of methylene blue such as the type of adsorbents, pHs, the amount of adsorbents dosage, initial methylene blue concentration, and contact time were optimized using a series of batch adsorption experiments. The experimental data of the adsorption process were more fitted to Langmuir isotherm (R2 = 0.9898) with a maximum adsorption capacity of 483.5 mg/g and batch kinetic experiments revealed that the adsorption process followed quasi-second-order kinetic model. The adsorption process was rapid and equilibrium was reached within 30 min. When the adsorption dosage is 0.2 g L−1, the adsorption yield for 100 mg L−1 methylene blue solution can reach 96.7 %. Therefore, acryloyl starch/carboxymethyl cellulose grafting copolymerization composite hydrogel present excellent adsorption efficiency, shows the potential application in future treatment of dyes wastewater. Macrophorins H (4) and L (5), two rare HMG-conjugate macrophorins along with three known macrophorins (1-3), three DMOA-derived meroterpenoids (6-8) and two ergosterol derivates (9-10) were isolated from sterilized rice medium cultured Penicillium sp. NX-05-G-3. Their structures were elucidated by 1D and 2D NMR. The cytotoxicities of all compounds were evaluated, and compounds 1 and 2 showed extensive cytotoxicity against human cancer cell lines Hela, SCC15, MDA-MB-453 and A549, with IC50 values ranging from 17.6 to 32.8 µM.
