Objective To explore the effect of miR-182 in non small cell lung cancer (NSCLC) and its mechanism. The aim of the present study was to investigate the optimal strategy and dosimetric measurement of thoracic radiotherapy based on three-dimensional (3D) modeling of mediastinal lymph nodes (MLNs). A 3D model of MLNs was constructed from a Chinese Visible Human female dataset. Image registration and fusion between reconstructed MLNs and original chest computed tomography (CT) images was conducted in the Eclipse™ treatment planning system (TPS). There were three plans, including 3D conformal radiotherapy (3D-CRT), intensity-modulated radiotherapy (IMRT) and volumetric-modulated arc therapy (VMAT), which were designed based on 10 cases of simulated lung lesions (SLLs) and MLNs. The quality of these plans was evaluated via examining indexes, including conformity index (CI), homogeneity index and clinical target volume (CTV) coverage. Dose-volume histogram analysis was performed on SLL, MLNs and organs at risk (OARs). A Chengdu Dosimetric Phantom (CDP) was then drilled at specific MLNs according to 20 patients with thoracic tumors and of a medium-build. These plans were repeated on fused MLNs and CDP CT images in the Eclipse™ TPS. Radiation doses at the SLLs and MLNs of the CDP were measured and compared with calculated doses. The established 3D MLN model demonstrated the spatial location of MLNs and adjacent structures. Objective To establish a modified method, mutant-enriched PCR combined with pyrosequencing, for the detection of K-ras mutations in plasma and cancer tissue of patients with NSCLC. Methods The cancer tissue and matched blood samples were collected and DNA was extracted from 43 NSCLC patients. The mutations of codon 12 and 13 in expressed region 2 of K-ras gene were detected by the established mutant-enriched PCR-pyrosequencing method. Cell line possessing mutant type/wild type of K-ras gene (A549/HCC827) was used to evaluate the sensitivity of the newly developed method. Results Four K-ras mutations were detected from cancer tissue samples (4/43, 9.3%), and 3 mutations were detected from plasma samples (3/43, 7.0%), all the mutations were 12 codon mutation. The consistency of K-ras mutation observed in both plasma and its matched tissue was 75%, and the sensitivity of the newly developed method was 0.1%. Conclusion The newly developed mutant-enriched PCR-pyrosequencing method has such advantages as rapid, accurate and high sensitivity. Objective To assess the expression of Rab25 protein in non-small cell lung cancer (NSCLC), and explore the correlation of its expression with tumor proliferation and metastasis. Methods Sixty-one cases of NSCLC specimens (31 cases of squamous cell carcinoma, 26 cases of adenocarcinoma, and 4 cases of adenosquamous carcinoma) undergone surgical treatment, and 40 specimens of adjacent normal lung tissues were obtained from Jan. 2009 to Jun. 2010 at Xingqiao Hospital of Third Military Medical University. Immunochemistry method of MaxVision was used to detect the expression of Rab25 in the specimens, and then the correlation of the expression with the clinicopathological parameters (patients' sex, age, smoking history, tumor type, differentiation, volume, TNM stage, lymph metastasis, etc.) was analyzed using statistical software SPSS 21.0. Results Rab25 protein was mainly expressed in cytoplasm and cell membrane. The positive rate of Rab25 in NSCLC was 93.4%, which was significantly higher than that in adjacent normal tissues (27.5%, P＜0.01). The expression of Rab25 protein was significantly associated with the TNM stage and tumor size (P＜0.05). Conclusions The expression of Rab25 is obviously higher in NSCLC than in the adjacent normal tissues, and the expression is associated with TNM stage and tumor size. Moreover, the later of the NSCLC stage, the larger of tumor size, and the higher of Rab25 expression will be in the NSCLC tissue. DOI: 10.11855/j.issn.0577-7402.2014.02.16

Background: Phase II-III trials in patients with untreated and previously treated locally advanced or non-small cell lung cancer (NSCLC) suggested that Endostar was able to enhance the effect of platinum-based chemotherapy (NP regimen) with tolerable adverse effects. Methods Four hundred and eighty six patients were randomized into two arms: study arm A: NP plus Endostar (n = 322; vinorelbine, cisplatin, Endostar), and study arm B: NP plus placebo (n = 164; vinorelbine, cisplatin, 0.9% sodium chloride). Patients were treated every third week for two to six cycles. Results: Overall response rates were 35.4% in arm A and 19.5% in arm B (P = 0.0003). The median time to progression was 6.3 months for arm A and 3.6 months for B, respectively (P 0.05). There were two treatment related deaths in arm A and one in arm B (P > 0.05). The median overall survival was longer in arm A than in arm B (P < 0.0001). Conclusion: Long-term follow-up revealed that the addition of Endostar to an NP regimen can result in a significant clinical and survival benefit in advanced NSCLC patients, compared with NP alone.

Through data conversion technology, the model is imported into ADAMS, and follow-up constraints and loads are added, with which the kinematics analysis on shoulder, upper arm and wrist joint are completed. It lays a solid foundation for the in-depth study on robot arm of massage working mechanism and the practical application. It can realize the prediction of the massage arm performance on the design phase, simplify the design process and shorten the cycle of the design as well as improve the design quality.

Objective To summarize the progress and development tendency of oncology all over the world,prospect the future development of military oncology during the coming "12th Five-Year Plan" period based on the current level.Methods Bibliographical methods were employed to review the articles and reviews published during last five years in the field of oncology,analyze the dynamics and tendency on the development of oncology,and summarize the new achievements of the oncology gained recent years.Results A mighty advance has been made in the studies on cancer stem cells and miRNA regulation,while molecular targeted therapy and high precise radiation therapy contributed a lot for lengthening the survival of cancer patients.Moreover,more scientific research items,papers and achievements have been issued,published and got than ever before.Conclusion It is suggested that the basic and clinical researches on oncology should be further enhanced,a great effort should be made to develop new therapeutic techniques to improve the life quality and lengthen the survival of cancer patients.

Pulmonary intravascular macrophages (PIMs) are often responsible for the clearance of blood-borne pathogens, including endotoxin, lipopolysaccharide of Gram-negative bacteria. It is well accepted that PIMs play a pivotal role in the pathogenesis of endotoxin-induced acute lung injury. However, the mechanisms by which PIMs are involved in the lipopolysaccharide-induced inflammatory responses remain unclear. Through the present study the following results were found: (1) When challenged with lipopolysaccharide (10 micrograms/ml), PIMs underwent marked cellular enlargement, intercellular adhesion plaques became longer, and some particulates were enwrapped in the pseudopods. (2) Lipopolysaccharide could up-regulate the expression of some inflammatory mediators in PIMs, including TNF-alpha, IL-1beta, IL-6, IL-8, and COX-2, and these up-regulated expression of inflammatory mediators correlated with NF-kappaB activation. (3) Dexamethasone as well as acetylsalicylic acid reduced the expression of TNF-alpha in lipopolysaccharide-challenged PIMs, and the decreased expression of TNF-alpha was also consistent with decreased NF-kappaB activation. Our results suggest that NF-kappaB activation in PIMs followed by phagocytizing lipopolysaccharide resulted in the up-regulation of TNF-alpha, IL-1beta, IL-6, IL-8, and COX-2, which could be alleviated by dexamethasone.

To investigate the role of pulmonary intravascular macrophages (PIMs) in the pathogenesis of acute lung injury (ALI) due to infection.Porcine pulmonary blood vessels were flushed by modified Morton method, and PIMs were isolated and cultured. The adhered PIMs were collected with adhesion method and incubated in RPMI 1640 medium. They were challenged with lipopolysaccharide (LPS, 10 mg/L). The activity of interleukin-1β (IL-1β), and contents of IL 6 and IL 8 in the culture supernatant were measured by method of thymocyte proliferation and enzyme linked immunoadsorbent assay (ELISA).The released contents of IL-1β, IL-6 and IL-8 from PIMs were increased significantly compared with those before LPS challenge , and they peaked at 2 hours [IL-1β activity: (10 400 ± 2 389) scintillant count/min], 4 hours [IL-6 content: (0.80 ± 0.36) μg/L], and 6 hours [IL-8 content: (4.94 ± 1.19) μg/L ] after LPS challenge , and the differences were significant compared with hose before LPS challenge [IL-1β activity: (213 ± 85) scintillant count/min, IL-6 content: (0.27 ± 0.12) μg/L, IL-8 content: (1.84 ± 0.53) μg/L, all P0.01].Among the cytokines released from PIMs after LPS challenge , the increase in IL-1β occurred earlier in comparison with that of IL-6 and IL-8, suggesting that the former might play an important role at the early stage of ALI; on the other hand, though the increase in IL-6 and IL-8 contents occurred later than that of IL- 1β but it lasted for a longer duration, suggesting that they might be associated with the advancement of ALI. The Results also suggested that interaction of these cytokines played a more important role in the pathogenesis of ALI.

The aim of this study was to investigate the clinical and prognostic significance of lymphangiogenesis in non-small cell lung carcinoma (NSCLC).The expression of lymphatic vessel endothelium markers podoplanin, VEGFR-3 and vascular endothelium marker CD31 was detected in paraffin sections from 68 cases of NSCLC and 8 cases of pulmonary inflammatory myofibroblastic tumor (PIMT) by immunohistochemistry (SP). Microvessel density (MVD) and lymphatic vessel density (LVD) were counted. Specificity of lymphatic vessel endothelium markers was compared between VEGFR-3 and podoplanin. Lymphangiogenesis, quantified by evaluating LVD, was correlated with various clinical pathological parameters and prognostic relevance.No significant association was found between the number of podoplanin positive vessels and the number of CD31 positive or VEGFR-3 positive vessels (r = -0.171, P = 0.124; r = 0.003, P = 0.979), but the counts of VEGFR-3 positive vessels were associated with CD31 positive vessel counts (r = 0.331, P = 0.002). LVD in PIMT group was significantly lower than that in NSCLC group (P = 0.004). Compared with that without the lymph node metastasis group, LVD in the positive group increased significantly (P = 0.033); LVD in pathological stage III and IV was higher than that in pathological stage I and II (P = 0.001). There was no difference of LVD in different cell differentiation, age and sex groups. The 5-year survival rate for high LVD was significantly lower than that of low LVD. Multivariate analysis showed that LVD was a significant and independent prognostic factor.Lymphangiogenesis may be a significant prognostic factor for NSCLC.

The differences were significant (all P0.01).Among the cytokines released by PIMs after LPS challenge, the increase in TNF-alpha content occurs earlier in comparison with that of IL-6 and IL-8, suggesting that the former may play an important role at the early stage of ALI. On the other hand, the increase in IL-6 and IL-8 contents is later than that of TNF-alpha and lasts for a longer time, suggesting that they may be associated with the development of ALI. The results also suggest that interaction of these cytokines is more important in the pathogenesis of ALI.

To construct the DNA vaccine containing ovalbumin (OVA) and Fc fusion gene targeting dentritic cells (DCs) and evaluate its anti-tumor efficiency in an E.G7-OVA-bearing tumor model.Constructed OVA-Fc-pcDNA3.1 plasmids were transfected into CHO cells with lipofectamine and the in vitro expression of OVA-Fc was determined by flow cytometry and ELISA. (51)Cr-relaese assay was used to determine the anti-tumor activity of cytotoxic T lymphocytes (CTLs) from splenocytes of immunized mice. According to tumor volume and survival time of the mice, the therapeutic effect of this vaccine was evaluated in E.G7-OVA-bearing tumor model.DNA sequencing and restriction endonuclease digestion analysis indicated that the recombinant expression vector OVA-Fc-pcDNA3.1 had been constructed successfully. OVA-Fc expression could be detected in CHO cells transfected with OVA-Fc-pcDNA3.1 by ELISA and flow cytometry. The DNA vaccine containing OVA-Fc fusion gene inhibited tumor's growth and prolonged the time of tumor-bearing mice due to elicit the CTL-mediated anti-tumor immunity.OVA-Fc-pcDNA3.1 DNA vaccine targeting dendritic cells can elicit the CTL-mediated anti-tumor immunity, which lays the foundation for further clinical experiments.

To express Livin alphabeta in A549 cells by using gene transfection, and to observe its effect on cell growth and cell sensitivity to chemotherapy drugs and radiation.Eukaryotic expression vectors of Livin alphabeta were transfected into A549 cells and cell clones with stable expression were obtained. Livin alphabeta expression levels in the transfected A549 cells were assessed at mRNA level and protein level, respectively. Cell growth status was assessed by biological features. MTT was performed to test effects of Livin on sensitivity of the A549 cells to chemotherapy drugs and radiation, and cell cycle analysis was performed to evaluate cell apoptosis.After transfection, positive cells, especially A549 cells expressing Livin, showed an increase of about 20% in colony-forming ability, a shorter doubling time (P0.05) and lower sensitivity to chemotherapy drugs and radiation (P0.01). Only 0.2% of the cells committed apoptosis with 10 Gy radiation.Livin isoforms, especially Livin alpha, are implicated in genesis and development of lung cancer, thus may be an important mechanism for drug resistance of lung cancer cells.

To explore the in vitro immune response to taxol resistance associated antigen 3 (TRAG-3)-derived cytotoxic T lymphocyte (CTL) epitope-pulsed dendritic cell (DC).The HLA-A2.1 restricted CTL epitope of TRAG-3 was previously identified to be a nonameric peptide sequence from 58 to 66 amino acid residues (TRAG-3(58-66)). The peptide was synthesized according to Fmos procedure, purified and molecular weight determined. Peripheral blood mononuclear cells (PBMC) of normal HLA-A2.1(+) individuals were used to isolate DC and DCs' phenotype identified by flow cytometry. Induction of CTL was achieved by in vitro stimulation of HLA-A2.1(+) PBMC with peptide-pulsed DC. CTL activity against HLA-A2.1(+)/TRAG-3(+) melanoma cell line LB373-MEL was assessed by (51)Cr release assay and IFN-gamma released by ELISA.The synthetic nonameric peptide was above 90% pure and the determined values of molecular weight were conformed to their theoretical values. The phenotype of the isolate DCs was characteristic for mature ones. PBMC stimulated in vitro by TRAG-3-derived epitope-pulsed DCs for three times could specifically kill the target cells and secreted high concentration of IFN-gamma.TRAG-3-derived epitope-pulsed DC can elicit specific anti-tumor immune response in vitro. Clinical trials using it as tumor vaccine may be feasible as specific immunotherapy for patients with TRAG-3 expressing cancers.

Purpose The purpose of this study was to determine whether the distribution of radiosensitivities in normal tissues of head and neck cancer patients, measured using a DNA damage assay on lymphocytes, is likely to provide sufficient discrimination to enable reliable identification of patients with abnormal sensitivities. Material and methods Radiosensitivity was assessed in 307 lymphocyte samples from unselected head and neck cancer patients and was quantified as the initial number of DNA double-strand breaks (dsb) induced per Gray and per DNA unit (200 Mbp). Results The existence of an inter-individual variation in the radiosensitivity parameter is described by the range (0.41--9.38 dsb/Gy/DNA unit) of the values found. We detected 37 patients who developed severe skin reactions during radiotherapy treatment and we compared their radiosensitivity values with the remaining patients treated. Radiosensitivity values of >7.20 dsb/Gy/DNA unit should theoretically correspond to highly radiosensitive patients. Conclusions Our results suggest that initial DNA damage measured on lymphocytes offers an approach to predict the acute response of human normal tissues prior to radiotherapy. .

To study the role of pro-inflammatory cytokines including tumor necrosis factor-alpha(TNF-alpha), interleukin-1beta(IL-1beta), IL-6, and anti-inflammatory cytokines including IL-4, IL-10, and IL-13 in acute lung injury(ALI).Reverse transcription-polymerase chain reaction(RT-PCR) were used to assess the expressions of mRNA of TNF-alpha, IL-1beta, IL-6, IL-4, IL-10 and IL-13 in the lung tissue after lipopolysaccharide challenge.The expression of TNF-alpha mRNA was seen to peak at 1 hour and IL-6 mRNA at 4 hours in lung tissue after the challenge. The highest expression of mRNA of other cytokines, such as IL-1beta, IL-4, IL-10 and IL-13 in lung tissue appeared 2 hours after LPS-induced lung injury.TNF-alpha is an early presenting cytokine in ALI, and IL-6 may play an important role in delayed development of ALI. The over-expression of anti-inflammatory cytokines IL-4, IL-10 and IL-13 may act as promoters rather than protectors in amplification of inflammatory process.

Radio-genetic therapy is a novel strategy for cancer treatment; however, a limited success was shown due to lower sensitivity of tumor cells to radiation under hypoxia, which is a unique feature for solid tumors. In order to improve the efficacy of radiogenetic therapy for lung cancer, a hypoxia/radiation dual-sensitive promoter was constructed to enhance the expression of HSV-TK in transfected cells exposed to radiation under hypoxia.The chimeric promoter HRE-Egr was generated by insertion of hypoxia response elements (HREs) upstream of the Egr-1 (early growth response gene-1) promoter. HSV-TK expression vector was constructed by cloning HRE-Egr promoter upstream of HSV-TK gene, which was transfected into A549 cells via liposome. The expression of HSV-TK in transfected cells exposed to irradiation (6 Gy) and/or hypoxia (1% O2) were analyzed by Northern blot, and the relative survival rate of cells in presence of prodrug ganciclovir (GCV) was tested using MTT method. To examine the efficacy of this HRE-Egr-TK gene therapy in vivo, the A549 adenocarcinoma xenografts were planted in BALB/c nude mice. The tumor volumes and the suppression rates were assayed in nude mice bearing xenografts infected with plasmids and exposed to radiation.HSV-TK gene expression in transfected cells was markedly increased in both radiation (227 U) and hypoxia (94 U) groups compared with control group (21 U). The HSV-TK expression (769 U) in transfected cells exposed to radiation under hypoxia is much more higher than the former groups. The survival rate of transfected cells exposed to radiation under hypoxia in the presence of GCV was obviously decreased with comparison of cells under normoxia (7.2%+/-1.8 % vs 32.7%+/-4.6 %). HRE-Egr promoter transfected tumors regressed significantly after a combination therapy of irradiation and GCV in all mice (n=10), the tumor suppression rates was 91.2%.The hypoxia/radiation dual-sensitive promoter HRE-Egr can enhance the HSV-TK gene expression in solid tumors under hypoxia. Enhanced tumor suppression effect was observed in A549 xenografts infected with HRE-Egr promoter exposed to radiation.

OSM expression vector was constructed by cloning HRE-Egr promoter upstream of OSM gene, which was transfected into A549 cells. The expression of OSM in transfected cells exposed to irradiation and(or) hypoxia was analyzed, and the relative survival rate of transfected cells exposed to the above conditions was tested. To examine the efficacy of this HRE-Egr-OSM gene therapy in vivo, the tumor suppression effects were investigated in 40 nude mice bearing lung adenocarcinoma xenograft.Expression of OSM gene in transfected cells exposed 6 Gy irradiation was markedly increased under hypoxia. A gene therapy experiment in vitro showed that the survival rate of transfected cells exposed to radiation under hypoxia was obviously decreased with comparison of cells under normoxia. HRE-Egr promoter transfected tumors regressed significantly after a combination therapy of irradiation and HRE-Egr transfection in all mice (n = 10), and six tumors disappeared in 3 weeks without any side effects.The data indicate that tumor targeted expression of OSM gene under the control of a hypoxia/radiation dual-sensitive promoter represents a novel strategy for safe and effective gene therapy of lung carcinoma and might have clinical application in the future.

In recent years,great development have been made in cancer therapeutics with replication-competent viruses (oncolytic viruses), E1B deleted adenovirus is one of promising viruses. The current study was designed to evaluate the efficacy and toxicity of intratumoral H101, a E1B-deleted adenovirus, in combination with chemotherapy on patients with cancer.A total of 50 patients with malignant tumors in multiple centers clinical trial were treated with H101, 0.5ml 5x10(11) viral particle per day for 5 consecutive days every three weeks. Routine chemotherapy was performed at the same time. And the efficacy and toxicity were recorded.Among 46 valuable cases, the overall response rate was 30.4%. The response rate was 28.0% (14/50) among ITT population, including 3 complete response (CR) and 11 partial response (PR). The overall response rate of control lesion was 13.0%, including 1 case of CR and 5 cases of PR. Thus, the response rate in injected lesion is clearly higher than that in control lesion (P0.001). Main side effects were injection site pain (26.9%) and fever (30.2%). Grade 1 hepatic dysfunction was found in 4 patients, grade 2 in 1 patients, grade 4 in 1 patients. Grade 4 hematological toxicities were found in 4 patients.The study showed that the combination of genetically modified adenovirus (H101) and chemotherapy possessed some effect for treating the patients with refractory malignant tumors, and the toxicities were lower, well tolerated.

To improve the efficacy and selectivity of gene therapy for lung carcinoma, a strategy was designed for suicide gene therapy in conjunction with irradiation therapy, by constructing a plasmid tgEgr-HyTK in which HSV-TK gene was driven by the early growth responsive gene-1 (Egr-1) promoter.The radio-inducible suicide gene was constructed by insertion of Egr-1 promoter upstream of the HSV-TK gene. The expressions of HSV-TK in lung carcinoma cell lines A549 which were infected with tgEgr-HyTK and exposed to different doses of gamma-ray irradiation were analyzed, and the relative survival rates of cells in presence of prodrug ganciclovir (GCV) were tested. The tumor suppression effects were investigated in 40 nude mice bearing lung tumors to examine the efficacy of this Egr-TK gene therapy in vivo.Expression of HSV-TK gene in lung carcinoma cells infected with tgEgr-HyTK plasmids was markedly increased in a radiation dose-dependent manner. A gene therapy experiment in vitro showed that tgEgr-HyTK transduced lung carcinoma cells became highly sensitive to GCV after irradiation, but not without irradiation. tgEgr-HyTK transfected tumors regressed significantly after a combination therapy of irradiation and GCV in all mice (n = 10), and five tumors disappeared in 3 weeks without any side effect.The data indicate that tumor targeted expression of HSV-TK gene under the control of a radio-inducible promoter represents a novel strategy for safe and effective gene therapy of lung carcinoma, which may have clinical application in the future.

There were a few reports about the role of PAX3-FKHR fusion gene in the oncogenesis of alveolar rhabdomyosarcoma (ARMS); however, its effect on muscle-producing factor (MPF) is not clear. This study was designed to observe the effects of fusion gene PAX3-FKHR transfection on expression of MRF4 and differentiation of cultured human rhabdomyosarcoma RD cells.To construct an eukaryotic expression vector of PAX3-FKHR gene; the plasmids of MRF4 cDNA and PAX3-FKHR cDNA were transfected into cultured RD cells with lipofectin methods. The changes of cells growth, morphologic and the expression of MHC and alpha-actin were observed.There was no significant changes in RD cells after transfection with PAX3-FKHR gene in growth rate, growth manner and the expression of MHC and alpha-actin. In MRF4 expressing RD cells after transfected with PAX3-FKHR, although the expression of MRF4 was not changed, morphologic appearance showed a low differentiation; expression of MHC and alpha-actin significantly decreased.There is no obvious influence on biological character and differentiation in RD cells with transfection of PAX3-FKHR gene; PAX3-FKHR does not inhibit the expression of MRF4, but can reverse the effect of MRF4 on promoting the differentiation of RD cell.
