Objective: This study aimed to analyze the associations between depressive and anxiety symptoms and risk of incident kidney failure in patients with biopsy-proven diabetic nephropathy (DN). Methods: This retrospective study enrolled 241 type 2 diabetic patients with biopsy-proven DN. Huaxi Emotional-Distress Index (HEI) was used to evaluate the depression and anxiety status of patients on admission. According to the HEI score, DN patients were divided into HEI score ≤ 8 group (without depression and anxiety) and HEI score > 8 group (with depression and anxiety). The study endpoint was defined as progression to kidney failure. The cox proportional hazard analysis was performed to investigate the risk factors for progression to kidney failure in DN patients. Results: Twenty-three patients had HEI score > 8, accounting for about 9.5% of all patients. Compared with HEI score ≤ 8 group, those with HEI score > 8 had more severe proteinuria, higher systolic blood pressure, and lower baseline eGFR and serum albumin levels. During a median follow-up of 28 months, the outcome event occurred in 89 (36.9%) of all the patients. After multivariable adjustment, HEI score > 8 (HR 1.825, 95% CI 1.050–3.172) was associated with an increased risk of progression to kidney failure. Conclusion: Depressive and anxiety symptoms might be associated with an increased risk of progression to kidney failure in patients with DN, which implied psychosocial issues should be early screened, assessed and intervened to delay the progression of DN. The purpose of this study was to evaluate the stability of small-incision lenticule extraction (SMILE) and laser in situ keratomileusis (LASIK) when ascending from near sea level to an altitude of 3874 m. The visual acuity (VA), intraocular pressure (IOP), spherical equivalent refraction (SER) and biometric parameters of 20 normal subjects (40 eyes, controls) and 36 subjects who underwent corneal refractive surgery (35 eyes with SMILE and 36 eyes with LASIK) were examined in Chongqing (C, 500 m above sea level) and 7–10 days after a collective travel to Shigatse (S, 3874 m above sea level). SER and corneal thickness (CT) were choosed as main indicators of the stability of corneal refractive surgery at high altitude. The IOP decreased and CT increased from C to S, while VA and SER did not significantly change in the control subjects. The VA and IOP of both the SMILE and LASIK subjects did not significantly change at an altitude of 3874 m. At high altitudes, no significant difference in the SER was found among the SMILE subjects, while a significant myopic shift was found among the LASIK subjects. With an increase in the altitude of 3874 m in S, an obvious increase in CT was observed in subjects who underwent corneal refraction surgery. The increase in CT in the SMILE group was significantly less than the increase in CT in the LASIK group. In addition, the biometric parameters of the SMILE and LASIK subjects showed the same minor changes as those of the control subjects when exposed to high altitude. The VA, IOP and biometric parameters of the subjects who underwent SMILE and LASIK remained stable at an altitude of 3874 m. SMILE shows better stability than LASIK in the refractive state and CT when exposed to high altitude. This study aimed to investigate the correlations between estimated small dense low-density lipoprotein-cholesterol (esd-LDL-c) and the development of end-stage kidney disease (ESKD), cardiovascular mortality, and all-cause mortality in individuals with diabetic kidney disease (DKD) or diabetes mellitus (DM) concomitant chronic kidney disease (CKD). We analyzed the data from a biopsy-proven DKD cohort conducted at West China Hospital of Sichuan University between 2009 and 2021 (the DKD cohort) and participants with DM and CKD in the National Health and Nutrition Examination Survey (NHANES) 2011–2014 (the NHANES DM-CKD cohort). Cox regression analysis was also used to estimate associations between esd-LDL-c and the incidence of ESKD, cardiovascular mortality, and all-cause mortality. There were 175 ESKD events among 338 participants in the DKD cohort. Patients were divided into three groups based on esd-LDL-c tertiles (T1 < 33.7 mg/dL, T2 ≥ 33.7 mg/dL to <45.9 mg/dL, T3 ≥ 45.9 mg/dL). The highest tertile of esd-LDL-c was associated with ESKD (adjusted HR 2.016, 95% CI 1.144–3.554, p =.015). Furthermore, there were 99 deaths (39 cardiovascular) among 293 participants in the NHANES DM-CKD cohort. Participants were classified into three groups in line with the tertile values of esd-LDL-c in the DKD cohort. The highest tertile of esd-LDL-c was associated with cardiovascular mortality (adjusted HR 3.95, 95% CI 1.3–12, p =.016) and all-cause mortality (adjusted HR 2.37, 95% CI 1.06–5.32, p =.036). Higher esd-LDL-c was associated with increased risk of ESKD in people with biopsy-proven DKD, and higher cardiovascular and all-cause mortality risk among those with DM-CKD. [ABSTRACT FROM AUTHOR]

Introduction: This study aimed to investigate the correlation between fundus blood flow parameters and the severity of pathological biopsy in patients with diabetic kidney disease (DKD). Methods: Data of patients with type 2 diabetes mellitus who completed renal pathology biopsies and optical coherence tomography angiography (OCTA) examinations, including renal function, 24-h urine protein quantification, and macular flow imaging, were collected. DKD pathology biopsies were graded as stages 1–4, and differences and correlations of the parameters were compared between groups. The grading was transformed into early (stage 1) and late (stages 2–4), and regression analyses were conducted to develop a model, draw a nomogram, and test efficacy. Results: This study included 157 eyes from 157 individuals in total. Urinary microalbumin and to urinary creatinine ratio (mALB/NCR) increased with pathological grading, whereas while glomerular filtration rate was decreased (p < 0.01). Corresponding retinal blood flow in superficial, deep, and full paracentral rings was decreased, which correlated with pathological grading (p < 0.01), with the highest blood flow density in the whole layer (r2 = −0.707). Meaningfully, in the early DKD model (area under the curve = 0.929 [0.889–0.970], p < 0.01), whole-layer blood flow density, mALB/NCR, and diabetes duration were statistically significant. Conclusions: The decrease in macular retinal blood flow density detected by OCTA is closely associated with the increase in pathological grading of DKD and can be used as a noninvasive parameter for monitoring early changes in DKD. With the development of DC distribution system within the isolated power system of a ship or an aircraft, more constant frequency loads will be supplied by inverters connected to DC main bus. In the operating mode conversion process of an isolated power system, inverters will inevitably suffer from serious disturbance and affect the stability of the system. Therefore, it is important to establish a model of the inverter that reflects its dynamic characteristics and based on which to conduct the stability analysis. This paper proposes a 12-pulse inverter model based on the generalized state space averaging (GSSA) method. This model can overcome the limitations of 12-pulse inverter state space averaging (SSA) model in transient analysis with good accuracy and fast analysis ability effectively. Three kinds of models for a 12-pulse aircraft inverter are built in MATLAB, namely GSSA model, SSA model and detail device model. The simulation results show the high accuracy of GSSA model in stability analysis. This study provides an effective analytical tool for stability analysis of 12-pulse inverter and also provides a reference for inverter modeling research of isolated power system such as in aircraft or ship.
