24. A Qualitative Study on Community General Practitioners' Perception of Dementia Screening

Abstract

Background China is seeing an increasing number of people suffering from dementia as aging advances and life expectancy prolongs. Early diagnosis is extremely important for dementia. Objective To understand the attitudes and views of community general practitioners (GPs) regarding dementia screening, providing suggestions for the development of dementia screening in the community. Methods In July 2021, by use of purposive sampling, GPs were recruited from community health centers (stations) in Lanzhou, Gansu, and invited to attend a semi-structured, in-depth, face-to-face individual interview for understanding their attitudes and views toward dementia screening. The interview results were analyzed using phenomenological analysis and thematic analysis. Results Ten GPs from five community health centers and five community health stations were finally enrolled, including five males and five females, with an average age of (46.6±6.5) years〔range (35, 57) 〕, an average years of (14.90±8.46) working as a GP〔range (5, 26) 〕; seven with a bachelor degree; four with a title of attending physician. Three themes were extracted: insufficient basic conditions for carrying out community-based dementia screening, difficulties in carrying out community-based dementia screening, and improvement of community GPs' abilities to participate in dementia screening. Nine subthemes were also extracted. Conclusion Community GPs supported community-based dementia screening, but had insufficient capacities to carry out the screening. The following may be effective measures for promoting early screening and intervening dementia: improving the ability of community GPs to screen dementia, strengthening the publicity and popularization of dementia-related knowledge to reduce social discrimination against dementia, and deepening the development of contracted family doctor services.

25. Exploring the lived experience of people with dementia and family caregivers in China : an interpretative phenomenological analysis

Abstract

Family supported care has dominated Chinese society for thousands of years. Filial piety is an important value of Chinese traditional culture. However, the capacity and availability of family-based care has changed dramatically because of cultural, social, economic and demographic changes. Therefore, in the context of these transitions, more needs to be understood about the meaning of living with and caring for people with dementia in their own homes. Previous studies involving Chinese people with dementia and their family caregivers which focus on their lived experiences are scarce. This study gives a voice to Chinese people with dementia and their family caregivers as active participants in dementia care research. The study aims to gain insight into the lived experiences of people with dementia and their family caregivers, and how they cope with their new roles in China. It specifically explores the meaning of family responsibilities, social relationships and cultural influences from the perspective of different family members and people with dementia. A qualitative approach has been adopted, using interpretative phenomenological analysis (IPA) in this study. The participants were recruited from in-patient and out-patient lists of one mental health centre in Shandong province, China. The inclusion criteria are people with dementia who have a recorded diagnosis of dementia with the severity of dementia ranging from mild to moderate and have communication capacity to take part in an interview. Family caregivers had to have over 6 months period caring experience to ensure they had sufficient experience to reflect on and comment on. Semi-structured interviews with people with dementia (n=10) and family caregivers (n=14) were conducted in both urban and rural areas of China. The interviews were transcribed and have been analysed using an adaptation of Smith's (2009) 6 steps of interpretative phenomenological analysis (IPA). There are four superordinate themes or key themes that have been found from people with dementia and family caregivers' accounts: (a) 'negotiating the roles'; (b) 'the meaning of living with dementia in the social context'; (c) 'the meaning of responsibility in the cultural context'; (d) 'the settled life'. These themes have been discussed in relation to family responsibilities and wider social relationships, which have reflected the challenges and dilemmas that the participants faced when caring for people with dementia or living with dementia within a specific social and cultural context. The study draws on the phenomenology of care-giving and care-receiving to show that both people with dementia and family caregivers struggle with their family responsibility and their roles; they face a series of challenges in coping with the 'altered' life of living with dementia at a time when the traditional family support care system is also changing. It highlights the participants' stress and burden in carrying out family caring responsibility or living with dementia under the current social situation. It indicates that the traditional value of family responsibility has been influenced by sociocultural transitions, which indirectly affect the quality and quantity of care provided by families. It recommends that while there may be continuing support for reciprocal filial piety, a stronger government welfare system is needed in order to support older people and their families in contemporary Chinese society.

Abstract

Background: Dementia has become one of the leading causes of death across the world. Aims: The aim of this study was to investigate the incidence, prevalence, and mortality of dementia in China between 2010 and 2020, and to investigate any geographical, age, and sex differences in the prevalence and incidence of dementia. Methods: Five databases were searched. The Joanna Briggs Institute (JBI) critical appraisal tool was used to assess the quality of the included studies. A random-effects meta-analysis was performed to estimate the pooled prevalence of dementia. Subgroup analysis was based on the type of dementia. The incidence and mortality of dementia were synthesized qualitatively. Results: A total of 19 studies were included. The meta-analysis showed that the prevalence of dementia was 6% (95%CI 5%, 8%), the prevalence of Alzheimer's disease (AD) was 5% (95%CI 4%, 6%), and the prevalence of vascular dementia (VaD) was 1% (95%CI 0%, 2%). The subgroup analysis showed that the prevalence rates of dementia in rural (6%, 95%CI 4%, 8%) and urban areas were similar (6%, 95%CI 4%, 8%). Deaths due to dementia increased over time. Conclusion: The prevalence, incidence, and mortality of dementia increased with age and over time. Applying consistent criteria to the diagnosis of cognitive impairment and dementia is necessary to help with disease monitoring. 48. Online Survey to Investigate Asthma Medication Prescription and Adherence from the Perspective of Patients and Healthcare Practitioners in England

This study explored patients’ and healthcare practitioners’ (HCPs’) perspectives to contextualize asthma treatment patterns observed in real-world studies.Methods: Data were collected using online surveys from HCPs and people with asthma (≥ 18 years old with a confirmed asthma diagnosis of any severity) who had consented to research participation through the Clinical Practice Research Datalink. Data were analysed using descriptive statistics.Results: In total, 76 HCPs and 63 patients were invited to take part. Of 48 valid HCP responders, 54.2% (n=26) reported scheduling an annual asthma treatment review with their patients and 83.3% of general practitioners (n=40) had prescribed repeated inhalers at the patient’s request. Of 47 valid patient responders, 57.4% (n=27) reported using their reliever (SABA) inhaler daily and 55.3% of patients (n=26) reported being prescribed a preventer inhaler. Of the total patient responders, 31.9% (n=15) reported that they never used their preventer inhaler. Consistent annual adherence with preventer inhalers was reported by 44.7% of all valid responders (n=21), while other patients admitted to using preventers intermittently.Conclusion: SABA and ICS prescription patterns are driven by a combination of HCP and patient factors. However, many studies have found that patient adherence to maintenance medication is poor.Aim: We undertook a meta-synthesis of qualitative studies, to investigate asthma patient and healthcare professionals’ perspectives of medication adherence.Methods: This systematic review was reported by following the PRISMA guidelines. The Joanna Briggs Institute (JBI) meta-aggregative approach was used for the qualitative synthesis. The protocol was registered in PROSPERO (CRD42022346831).Results: In total, 12 articles were included in the review. These articles reported findings from 433 participants in total, which included 315 patients and 118 healthcare professionals. Four synthesised findings with sub-themes were identified from the reviewed studies. These synthesised findings were described as: 1) The role of relationship and communication with/between Healthcare Professionals in medication adherence; 2) Insufficient information from Healthcare Professionals acting as a barrier for adherence; 3) How patient’s attitude/beliefs effect their adherence to medication; and 4) Patients’ personal behaviour and other relevant barriers.Conclusion: The synthesized findings provide a strong evidenc
