- Abstract
[Summary] The rise and fall of the Roman Empire was a socio-political process with enormous ramifications for human history. The Middle Danube was a crucial frontier and a crossroads for population and cultural movement. Here, we present genome-wide data from 136 Balkan individuals dated to the 1st millennium CE. Despite extensive militarization and cultural influence, we find little ancestry contribution from peoples of Italic descent. However, we trace a large-scale influx of people of Anatolian ancestry during the Imperial period. Between ∼250 and 550 CE, we detect migrants with ancestry from Central/Northern Europe and the Steppe, confirming that “barbarian” migrations were propelled by ethnically diverse confederations. Following the end of Roman control, we detect the large-scale arrival of individuals who were genetically similar to modern Eastern European Slavic-speaking populations, who contributed 30%–60% of the ancestry of Balkan people, representing one of the largest permanent demographic changes anywhere in Europe during the Migration Period.

- Abstract
We present the first ancient DNA data from the Pre-Pottery Neolithic of Mesopotamia (Southeastern Turkey and Northern Iraq), Cyprus, and the Northwestern Zagros, along with the first data from Neolithic Armenia. We show that these and neighboring populations were formed through admixture of pre-Neolithic sources related to Anatolian, Caucasus, and Levantine hunter-gatherers, forming a Neolithic continuum of ancestry mirroring the geography of West Asia. By analyzing Pre-Pottery and Pottery Neolithic populations of Anatolia, we show that the former were derived from admixture between Mesopotamian-related and local Epipaleolithic-related sources, but the latter experienced additional Levantine-related gene flow, thus documenting at least two pulses of migration from the Fertile Crescent heartland to the early farmers of Anatolia.

- Abstract
Literary and archaeological sources have preserved a rich history of Southern Europe and West Asia since the Bronze Age that can be complemented by genetics. Mycenaean period elites in Greece did not differ from the general population and included both people with some steppe ancestry and others, like the Griffin Warrior, without it. Similarly, people in the central area of the Urartian Kingdom around Lake Van lacked the steppe ancestry characteristic of the kingdom’s northern provinces. Anatolia exhibited extraordinary continuity down to the Roman and Byzantine periods, with its people serving as the demographic core of much of the Roman Empire, including the city of Rome itself. During medieval times, migrations associated with Slavic and Turkic speakers profoundly affected the region.

12. The potential role of polygenic risk scores on the development of antisocial and criminal behaviour

Dissoziales Verhalten beschreibt Charaktereigenschaften, welche im Zusammen- hang mit kriminellem Verhalten stehen. Ein großer Protzentsatz der Population zeigt im Laufe des Lebens dissoziales Verhalten, dieses ist aber situationsabhängig und zeitlich begrenzt. Nur bei einem kleinen Teil der Menschen ist dissoziales Verhalten anhaltend und stabil. Dissoziales Verhalten kann sich bereits bei Kindern zeigen, meist in Form von lügen, beißen oder schlagen. Später kann es sich zu Mobbing oder verbalem Missbrauch, Drogenmissbrauch, Diebstahl und anderen Formen von kriminellem Verhalten entwickeln. In dieser Studie wurde ein explorativer Ansatz ge- wählt, um die potenzielle Rolle von polygenetischen Risikofaktoren auf die Entwick- lung von dissozialem oder kriminellem Verhalten zu analysieren. Dieser Studie liegt Datenmaterial der Wisconsin Longitudinal Study (WLS) zugrunde. Regressionsana- lysen wurden durchgeführt, um den Einfluss von polygenetischen Risikofaktoren, so- wie einiger Kovariablen auf folgende Variablen zu testen: „Hatten Sie jemals ernst- hafte rechtliche Schwierigkeiten?“, „Waren Sie jemals im Gefängnis?“ und „Wie wü- tend/gereizt fühlen Sie sich in einer typischen Woche?“. Folgende polygenetischen Risikofaktoren zeigten eine positive Assoziation mit den Variablen: Abenteuerlust, Symptome von Depression, Menarche, Cannabiskonsum, Neurotizismus, Alkoholab- hängigkeit, COPD, Rauchen und das Gefühl von Einsamkeit. Im Gegensatz dazu zeigten selbst berichtete Mathematikfähigkeit, subjektives Wohlbefinden, Zufrieden- heit mit Familie, Freunden, Finanzen und Arbeit, sich bei sozialen Aktivitäten ausge- schlossen fühlen, sowie selbst eingeschätzte Gesundheit eine negative Assoziation mit den Variablen. Diese Studie trägt zu dem Verständnis bei, dass es eine potenziel- le genetische Basis für Charaktereigenschaften geben kann, die zu dissozialem oder kriminellem Verhalten führen. Nichtsdestotrotz sollte man diese Ergebnisse mit Vor- sicht betrachten, da sie nur einen kleinen Teil der Varianz erklären., Antisocial behaviour describes behavioural traits related to crime or deviant behav- iour. A high percentage of people displays antisocial behaviour during life, but this behaviour is only situational and temporary. Only a small number of the population shows persistent and stable antisocial behaviour. In children, signs of antisocial be- haviour are lying, sneaking, hitting, or biting. This can progress to bullying and verbal abuse, substance abuse, theft and other forms of delinquent behavior. In this study, an exploratory approach was used to examine the potential role of a set of polygenic risk factors on the development of antisocial or criminal behaviour. The data was ob- tained via the Wisconsin Longitudinal Study (WLS). Regression analyses were used to calculate the influence of polygenic risk scores and a set of covariables on the fol- lowing variables: “have you ever had serious legal difficulties?”, “have you ever been in jail or prison?” and “in a typical week, how angry/irritated are you?”. The following polygenic risk scores showed a positive association on the variables: adventurous- ness, symptoms of depression, menarche, cannabis use, neuroticism, alcohol use disorder, COPD, smoking, and feelings of loneliness. Contrary, self-reported math ability, subjective well-being, life satisfaction with family, friends, finances, and work, feeling left-out of social activity and self-rated health presented a negative associa- tion. This study adds to the understanding that there is a potential genetic basis for behavioural characteristics that can lead to antisocial or criminal behaviour. Never- theless, it is important to look at these results carefully since they only explain a small amount of the variance.

BackgroundDespite a large number of clinical trials aiming at evaluating the digital self-management of chronic diseases, there is little discussion about users’ experiences with digital approaches. However, a good user experience is a critical factor for technology adoption. Understanding users’ experiences can inform the design of approaches toward increased motivation for digital self-management. ObjectiveThis study aimed to evaluate the self-management of cystic fibrosis (CF) with a focus on gastrointestinal concerns and the care of young patients. Following a user-centered design approach, we developed a self-management app for patients and parents and a web tool for health care professionals (HCPs). To evaluate the proposed solutions, a 6-month clinical trial was conducted in 6 European CF competence centers. This paper analyzes the user acceptance of the technology and the benefits and disadvantages perceived by the trial participants. MethodsA mixed methods approach was applied. Data were collected through 41 semistructured qualitative interviews of patients, parents, and HCPs involved in the clinical trial. In addition, data were collected through questionnaires embedded in the self-management app. ResultsSupport for enzyme dose calculation and nutrition management was found to be particularly useful. Patients and parents rapidly strengthened their knowledge about the treatment and increased their self-efficacy. Reported benefits include reduced occurrence of symptoms and enhanced quality of life. Patients and parents had different skills, requiring follow-up by HCPs in an introductory phase. HCPs valued obtaining precise information about the patients, allowing for more personalized advice. However, the tight follow-up of several patients led to an increased workload. Over time, as patient self-efficacy increased, patient motivation for using the app decreased and the quality of the reported data was reduced. ConclusionsSelf-management enfolds a collaboration between patients and HCPs. To be successful, a self-management approach should be accepted by both parties. Through understanding behaviors and experiences, this study defines recommendations for a complex case—the demanding treatment of CF. We identify target patient groups and situations for which the app is most beneficial and suggest focusing on these rather than motivating for regular app usage over a long time. We also advise the personalized supervision of patients during the introduction of the approach. Finally, we propose to develop guidance for HCPs to facilitate changes in practice. As personalization and technology literacy are factors found to influence the acceptance of digital self-management of other chronic diseases, it is relevant to consider the proposed recommendations beyond the case of CF.

BackgroundRemote care services and patient empowerment have boosted mobile health (mHealth). A study of user needs related to mHealth for pediatric cystic fibrosis (PCF) identified the set of preferred features mobile apps should support; however, the potential use of PCF apps and their suitability to fit into PCF clinical management remains unexplored. ObjectiveWe examine whether PCF holds potential for the implementation of mHealth care. MethodsThe study is based on a literature review and qualitative analysis of content and was conducted in two parts: (1) we reviewed scientific and gray literature to explore how European countries manage PCF and conducted a qualitative study of 6 PCF units and (2) we performed a systematic review of apps available in the myhealthapps.net repository searching for cystic fibrosis (CF) management and nutrition apps, which we analyzed for characteristics, business models, number of downloads, and usability. ResultsEuropean CF routine care guidelines are acknowledged in most European countries, and treatments are fully covered in almost all countries. The majority of teams in CF units are interdisciplinary. With respect to the systematic review of apps, we reviewed 12 apps for CF management and 9 for general nutrition management in the myhealthapps.net directory. All analyzed apps provided functionalities for recording aspects related to the disease and nutrition such as medication, meals, measurements, reminders, and educational material. BackgroundDigital self-management in cystic fibrosis (CF) is foreseen as a means toward better understanding of the disease and its treatment and better adherence to the treatment. Mobile apps hold the potential to provide access to information, motivate, and strengthen compliance. However, to deliver high-quality apps, the development should be based on thorough knowledge about user needs. Empirical research on the user-centered development of mobile apps for health care is, however, still limited. ObjectiveThe aim of this research is to develop and evaluate an app ecosystem for self-management in CF. It targets not only those directly affected by CF but also parents and health care professionals involved in the treatment. This paper covers the first step of the design process that aims to analyze the context and the user requirements. The primary research question is as follows: what digital support has the potential to usefully support persons with CF and their caregivers in the CF care? To answer this question, we address two preliminary questions: what important factors in everyday life affect the care of persons with CF? and how is the CF care delivered today and what are the limitations of CF care services? MethodsThe overall research adopts a user-centered design approach in which future users are involved in the development process from the very beginning to ensure that the apps developed best suit the potential users. The research presented in the paper follows an interpretative case study research strategy seeking to understand the concerns and needs of persons with CF and their caregivers. Data were collected through semistructured qualitative interviews involving 74 participants in seven European countries and from internet forums. ResultsThe results of the analysis phase show a strong need for individuality of the digital support, as well as for its adaptability to different contexts. The paper presents the concerns and needs of the participants in the study and extracts a set of relevant features for a self-management app ecosystem. Education, enzyme dosage calculation, nutrition management, treatment organization, health diary, treatment follow-up, practical guidelines for treatment, communication with doctors, and communication with peers are foreseen as useful features. ConclusionsThe results indicate the readiness for self-management in the CF care even in countries that provide well-functioning health care services for CF care. The large diversity of user requirements identified reflects the crucial role user integration plays in developing apps for a chronic condition such as CF. The need for personalization stemming from the individuality of the patients and the need for communication with health care professionals support the idea of an app ecosystem for the self-management of CF.
