3. Radiotherapy in Cancer Treatment During The Covid-19 Pandemic: One Year Results From Northeast Turkey

OBJECTIVE The goal of this article was to assess the effects and consequences of strict measures in radiation oncology centers in northeastern Turkey during the coronavirus disease-2019 pandemic and to ensure that the reorganization of programs occurred without interruption to radiotherapy (RT). METHODS We retrospectively reviewed the records of 1063 patients (median age 63 years; range 3-93 years) who were treated in RI centers from March 2020 to March 2021. The study cohort was divided into two groups: Group A consisted of patients who received RT in the first 6 months and Group B comprised patients who received RT in the second 6 months. RESULTS The age distribution was similar in both groups. The analysis of the intent of treatment showed that 585 (55%) patients were treated with a radical intent in Group A, whereas only 478 (45%) were treated with a radical intent in Group B. Lung, breast, and genitourinary cancers were the most common cancer types in both periods. There was a significant decline in the number of female patients in Group B. Test results of 31 (2.9%) patients, 8 (25.8%) women and 23 (74.2%) men, who had nasopharyngeal swabs, were positive. CONCLUSION This study shows that oncology patients in our region are more worried about disease progression than the pandemic and insist on going to RT, especially during the first 6-month period when the pandemic was thought to be temporary. As a result of careful precautions, treatments in the clinics continued without contamination.

OBJECTIVE The aim of this study is, therefore, to compare three different treatment modalities for symptomatic plantar fasciitis (PF), namely, effective in pain control. METHODS This study involved 205 patients with a diagnosis of painful PFs, treated during the years 2013 through 2017 at three institutions. Of these, 67 patients received a total dose of 6.0 Gy radiotherapy (RT in 3-weekly fractions of 1 Gy (RT arm); 65 patients received local steroid injection (SI) of 40 mg (1 ml) of methylprednisolone; and 0.5 ml of 1% lidocaine in the painful heel spur, using palpation (palpation guided) (PG SI arm) and 73 received extracorporeal shock wave therapy (ESWT) in five sessions with weekly intervals (ESWT arm). Patients recorded visual analog scale (VAS) score, a modified Pannewitz score and a 5-level function score. Patients with available ≥12 months of data were included in the present study. RESULTS Follow-up data were available for patients with a median of 15.5 (range, 6.5-37.4) months. There was a significant improvement in VAS scores and 5-level function scores in the RT arm at 3 and 6 month period arm (p<0.001). In univariate and multivariate analyses, only treatment scheme was associated with considerable pain control (p=0.006). CONCLUSION These findings suggest that all treatment modalities provide significant pain relief in patients with painful heel spur. This analysis also demonstrates RT is superior choice for chronic PF in cases of failure with conservative treatments when compared to extracorporeal shock wave therapy and PG SI. Introduction: The number of elderly patients with bladder cancer is expected to triple in the coming years in proportion to the increasing elderly population in the world. The aim of this study was to identify the prognostic factors affecting survival among elderly patients undergoing chemoradiotherapy for muscle-invasive bladder cancer. Materials and Method: We retrospectively reviewed medical records from 93 biopsy-proven nonmetastatic elderly patients with bladder cancer = 65 years who were treated with helical tomotherapy. The patients received 64.8 Gy to the bladder with concurrent intravenous cisplatin (40 mg/m2) chemotherapy weekly as a radiosensitizer. Results: The mean follow-up time was 34.1 months (range, 4.0-99.1 months). The two- and five-year overall survival, disease-free survival, and cancer-specific survival rates were 70.6%, and 36.9%, 50.6% and 28.5%, 89.1%, and 58.5%, respectively. Multivariate analysis indicated that urothelial obstruction was an independent prognostic factor affecting survival rates. No grade 4 adverse events and deaths attributable to treatment occurred during chemoradiotherapy Conclusion: Image guided radiotherapy with chemotherapy as a bladdersparing approach is a tolerable alternative therapeutic option without severe acute and late toxicities in elderly patients who are unfit for radical cystectomy or refuse surgery. Occult breast cancer (OBC) and rectum cancer in the same patient is a rare event. Multiple primary tumors are being frequently reported in recent years owing to better screening programs, diagnostic procedures, and prolonged life span. In the case of a new cancer diagnosis, staging examinations are performed to evaluate the extent of disease, and sometimes a primary tumor is localized in cases where the metastatic area is a sign of malignancy. Adequate investigations, such as immunohistochemistry, must be performed in order to rule out metastasis of each other. We report the case of one patient diagnosed with OBC and rectum cancer synchronously in this case report. [ABSTRACT FROM AUTHOR]

ÖZET Rüya hakikat alemine açılan pencerelerden, olmuş ve olacak hadiselerin aynen, veya bazı sembollerle çözümlenmesidir. İnsan, rüyanın geleceği bildirmede vasıta olmasını ister. Bu düşünceden hareketle hem İslam hem de batı toplumlarında bazı hadiseler,rüya yoluyla önceden tahmin edilmiştir.Bu bakımdan da rüya, insanlığın ilgisini çekmiştir.Gerek bilim dünyasında gerek tasavvufta rüyayla alakalı pek çok çalışma yapılmıştır. Müslüman ve batılı alimlerin rüya konusundaki müşterekleri pek çoktur. Ancak farklılıkları da vardır.Rüyalar yorumlanırken, yaşantılardan sembolize edilmiş hareketlerden yola çıkılmıştır. Tasavvuf! olmayan rüyayı yorumlamada benzerlikler vardır. Hak ve gerçek denilen Allah tarafından doğrudan doğruya veya bir melek aracılığıyla meydana gelen rüyayla, sadece İslam alimleri meşguldür. Rüya İslam alimlerince Allah'ın insana gönderdiği bir manevi işaret olarak kabul edilmiştir ve İslam-Türk Edebiyatı içerisinde `tabirname` adı verilen rüya motiflerinin neyi sembolize ettiğini ortaya koyan pek çok eser meydana getirilmiştir. Okuduğumuz Seyyid Süleyman'ın rüya tabirnamesi de bunlar içindedir. Rüya tabiri, İslam dünyasında nurani ilimlerden sayılmaktadır. Rüya tabirciliğinin İslam dünyasında gelişmesi, Kur'an ve peygamberle yakından alakalıdır. Peygamberin gördüğü rüyaları yorumlamak `rüya tabir ilmi` diye kabul edilmektedir. Tasavvufta Kur'an ve Hadislerle yapılan tabirler kabul görmektedir. Tasavvufta Kur'an-ı Kerim'den sonra rüya tabirinde baş vurulacak kaynakhadislerdir.Hz. Muhammed'in gördüğü rüyaların ta'birleri rüya hakkındaki söz ve davranışları rüya tabirine kaynaklık etmiştir. Mutasavvuflara göre tasavvufı şiirin kaynağı ilham-ı ilahidir. Tasavvufı semboller alem-i misaldendir. Rüya motiflerinin de kaynağı ilahi ve alem-i misal olduğuna göre tasavvufı şiirle rüya motifleri bir noktada örtüşmektedir. Yani şiirlerdeki bilinen yorumlara, rüya tabirnamelerdeki motiflerin yorumları metin şerhine yeni bir nitelik kazandırabilir Bunu yaptığımız örnek çalışmada gösterdik. ABSTRACT Dream is a solution of the events which happened or will happen by using the symbolls from the window that open the real world. Human wants the dream is being a mean to annonce the future. That is why, some events which happened in the Muslem World and Western have guessed before in the dream. Both the science world and mysticisim there are many studies about the dream. The Muslem and Western scientist have got the some thoughts abaut the dream. But there are difference too. While the dream comments, we use the real symbolls. It's the same the unmysticism dream comments. Only the muslem scientists are busy with the right and real dream which is happening by Allah or an angle. Dream is accepted a spiritual signal sending by Allah to the human and there are many interpretation books is called `tabirname` which are use the dream motifs. Seyyid Süleyman' s Tabirname which we read is the same book among these books. The interpretation is accepted the spiritual science in Muslem Worlds. The development of interpretation in Muslem Worlds nearly connected with Kur' an and the prophets. Interpretation of the prophet dreams is accepted `the science of dream interpretation` The interpretations in the Kur' an and Hadith are accepted in spiritual. In spiritual hadis is accepted main source after the Kur' an. The interpretations of Prophet's Dreams are the main source of the other interpretations. According has to the Sofies, the main soure of the spiritual poems was hymn inspiration. The spritual symbols were hymn examples. The interpretations source are there hymn inspiration and example so the spritual poems are the same with the dream symbols. So that the comments in the poemsand the comments of the interpretations motifs are caused gained a guality to the passage. We showed that with the example study. 276
