3. Genetic testing for inherited arrhythmia syndromes and cardiomyopathies: results of the European Heart Rhythm Association survey.

Abstract

Aims Indications and clinical impact of genetic testing for cardiac diseases have increased significantly over the past years. The aim of this physician-based European Heart Rhythm Association (EHRA) survey was to assess current clinical practice and access to genetic testing for cardiac diseases across European Society of Cardiology countries and to evaluate adherence to the 2022 EHRA/HRS/APHRS/LAHRS Expert Consensus Statement on genetic testing. Methods and results An online questionnaire composed of 28 questions was submitted to the EHRA Research Network and European Reference Network GUARD-Heart healthcare partners and promoted via dedicated social media channels. There were 357 respondents from 69 countries, 40% working in a hospital setting with a cardiac genetic service and/or a dedicated clinic focusing on inherited cardiac diseases and 27% with an onsite genetic laboratory. No genetic testing or low annual rate (<10/year) was declared by 39% of respondents. The majority of respondents (78%) declared issues or limitations to genetic testing access in their clinical practice. The main reasons for not providing or limited access to genetic testing were no availability of dedicated unit or genetic laboratory (35%) or reimbursement issues (25%). The most frequently reported indication for genetic testing was diagnostic purpose (55%). Most respondents (92%) declared offering genetic testing preceded by genetic counselling and 42% regular multidisciplinary evaluations for patients with cardiac genetic diseases. The perceived value of genetic testing in the diagnostic, prognostic, and therapeutic assessment was variable (67, 39, and 29%, respectively) and primarily based on the specific inherited disease. The majority of respondents recommended cascade genetic testing for the first-degree family members in case of pathogenic/likely pathogenic variant in the proband. Conclusion This survey highlights a significant heterogeneity of genetic testing access and provision and issues attributable to the availability of dedicated unit/genetic laboratory and reimbursement. Fascicular left ventricular tachycardia (VT) is the second most frequent idiopathic left VT in the setting of a structurally normal heart. Catheter ablation is curative in most patients with low complication rates. We report a case of ostial left anterior descending coronary artery (LAD) occlusion during fascicular ventricular tachycardia ablation.

22. Ba6Ge2Se12and Ba7Ge2Se17: Two Centrosymmetric Barium Seleno-Germanates with Polyatomic Anion Disorder

Abstract

Herein, the crystal structures and physical properties of two previously unreported barium seleno-germanates, Ba6Ge2Se12and Ba7Ge2Se17, are presented. Ba6Ge2Se12adopts the P21/cspace group with a= 10.0903(2) Å, b= 9.3640(2) Å, c= 25.7643(5) Å, and β = 90.303(1)°, whereas Ba7Ge2Se17crystallizes in the Pnmaspace group with a= 12.652(1) Å, b= 20.069(2) Å, c= 12.3067(9) Å. Both structures feature polyatomic anion disorder: [Se2]2–in the case of Ba6Ge2Se12and [GeSe5]4–in the case of Ba7Ge2Se17. The anion disorder is verified by comparing pair distribution functions of ordered and disordered models of the structures. These anions are split unevenly across two possible sets of atomic coordinates. The optical band gaps obtained from the powdered samples are found to be 1.75 and 1.51 eV for Ba6Ge2Se12and Ba7Ge2Se17, respectively. Differential scanning calorimetry experiments indicate that the compounds are stable under the exclusion of air up to at least 673 K. The thermal diffusivity measurements revealed thermal conductivities reaching values as low as 0.33 W m–1K–1in both compounds at 573 K.

Abstract

BACKGROUND Radiofrequency catheter ablation of idiopathic ventricular arrhythmias (VAs) is performed to eliminate symptoms and to prevent or reverse arrhythmia-induced cardiomyopathy. Preprocedural prediction of the chamber of VA origin is critical for patient counseling, procedure planning, and guidance of invasive mapping. OBJECTIVE We aimed to assess the performance of manual expert versus automated 12-lead electrocardiogram (ECG) analysis in the prediction of VA origin. METHODS Patients with ablation of idiopathic VA and sustained success were included. The VA origin was defined as the site where ablation caused arrhythmia suppression. Standard baseline 12-lead ECGs with documentation of the VA were analyzed manually in a blinded fashion by three electrophysiologists and three electrophysiology (EP) fellows. In addition, the same standard 12-lead ECG was analyzed by an automated computer algorithm using a vectorcardiographic approach. RESULTS Thirty-eight patients (median age, 47 [interquartile range, 37-58]; 68% female) were enrolled. The VA originated from the right ventricle in 24 (63%) and the left ventricle in 14 (37%) patients. The electrophysiologists and EP fellows identified the VA chamber of origin with a similar accuracy of 73% and 72% (P = .72). The automated algorithm showed a higher accuracy of 89% (P = .03 compared with electrophysiologists and EP fellows). This resulted in a sensitivity of 95% and specificity of 86%. CONCLUSION While the manual ECG analysis of the standard 12-lead ECG by both electrophysiologists and EP fellows correctly identified the chamber of VA origin in around 75% of cases, an automated vectorcardiographic computer algorithm achieved an accuracy of 89% with clinically acceptable diagnostic parameters.

Abstract

AIM: Between October 2015 and December 2016, 11,088 patients from 42 countries having cardiac resynchronisation therapy (CRT) devices implanted were included in the CRT II Survey. We compared the characteristics of Swiss CRT recipients with the overall European population. METHODS: Demographic and procedural data from seven Swiss centres recruiting all consecutive patients undergoing either de-novo CRT implantation or an upgrade to a CRT system were collected and compared with the European population. RESULTS: A total of 320 Swiss patients (24.4% female, mean age 71.0 ± 10.2 years, 47% ischaemic cardiomyopathy) were enrolled, which amounts to 38% of all CRT implantations in Switzerland during this period. Of the patients enrolled, 38% had atrial fibrillation, 27% second- or third-degree atrioventricular block, and 68% complete left bundle-branch block. Swiss patients had significantly less often the classical indication of heart failure with a wide QRS complex (40 vs 61%; odds ratio [OR] 0.44, 95% confidence interval [CI] 0.35-0.55; p <0.001). Compared with the European population, Swiss patients were significantly older (71 vs 68.5 years, p <0.001), less symptomatic from heart failure and had more chronic kidney disease. Swiss patients significantly more often received a CRT-pacemaker (37 vs 30%; OR 1.37; 95% CI 1.09-1.73; p = 0.007) and quadripolar left ventricular leads (69 vs 57%; OR 1.67, 95% CI 1.32-2.13; p <0.001). CONCLUSION: Compared with European CRT recipients, Swiss CRT patients are older, less symptomatic and suffer more often from comorbidities. Although two thirds of the implantations were CRT-defibrillator systems, Swiss patients more often received CRT-pacemaker systems than their European counterparts.

Abstract

Cilj rada je bila procjena razine tjelesne aktivnosti mjerene Baecke-ovim upitnikom i antropometrijskih parametara kod bolesnika s akutnim koronarnim sindromom (AKS), kao i usporedba razine tjelesne aktivnosti bolesnika s AKS-om s onom reprezentativnog uzorka opće populacije Hrvatske. U istraživanje je uključeno ukupno 196 bolesnika s akutnim koronarnim sindromom, koji su ispunili Baecke- ov upitnik kako bi se procijenila razina njihove tjelesne aktivnosti, te su utvrðeni antropometrijski parametri. Razina tjelesne aktivnosti ispitanih bolesnika s AKS-om, mjerena indeksima Baecke-ovog upitnika, usporeðena je s razinom tjelesne aktivnosti reprezentativnog uzorka opće populacije Hrvatske. Bolesnici s akutnim koronarnim sindromom imali su niži indeks aktivnosti u slobodno vrijeme (2.8 vs. 3.0, p= 0.0002) i indeks ukupne aktivnosti (2.6 vs. 2.8, p= 0.0005) u usporedbi s indeksima tjelesne aktivnosti reprezentativnog uzorka opæe populacije Hrvatske. Uz to su imali statistički značajno veći obujam struka (cm) (101.4 vs. 93.2, p= 0.000), kao i omjer obujma struka i kukova (0.985 vs. 0.912, p=0.000), ali nisu imali statistièki značajno veći indeks tjelesne mase (BMI) (kg/m2) (28.36 vs. 27.64, p= 0.07). U usporedbi s općom populacijom Hrvatske, bolesnici akutnim koronarnim sindromom su prekomjerno uhranjeni te su imali niže indekse aktivnosti u slobodno vrijeme, kao i niže indekse ukupne aktivnosti. Obujam struka i omjer obujma struka i kukova dokazali su ovdje veću značajnost u odnosu na BMI u procjeni rizika debljine.

Aim To assess the efficacy of propafenone in prevention of atrioventricular nodal reentrant tachycardia (AVNRT) and orthodromic atrioventricular tachycardia (AVRT) based on the clinical results of arrhythmia recurrence and find the electrophysiological predictor of propafenone effectiveness. Methods This retrospective study Included 44 participants in a 12-month period, who were divided In two groups: group A - In which propafenone caused complete ventriculoatrial block and group B - in which propafenone did not cause complete ventriculo-atrial block. Results Group A had significantly lower incidence of tachycardia than group B (95% vs 70.8%, P=0.038), and complete ventriculoatrial block predicted the efficacy of propafenone oral therapy In the prevention of tachycardia (sensitivity 87.5%, specificity 52.8%, positive predictive value 95%, negative predictive value 29.2%). Patients with AVNRT In group B who did not experience the recurrences of tachycardia had significantly shorter echo zone before Intravenous administration of propafenone than the patients who experienced episodes of sustained tachycardia (median 40 ms [range 15-60 ms] vs 79 ms [range 50-180 ms],P= 0.008). Conclusion In patients with non-lnducible tachycardia, complete ventriculoatrial block can be used as an electrophysiological predictor of the efficacy of propafenone oral therapy In the prevention of tachycardia. In patients with non-lnduclble AVNRT, but without complete ventriculoatrial block, propafenone was more effective In patients with shorter echo zone of tachycardia. [ABSTRACT FROM AUTHOR]
