Background: Major depressive disorder (MDD) is prevalent and disabling among older adults. Standing on its tolerability profile, vortioxetine might be a promising alternative to selective serotonin reuptake inhibitors (SSRIs) in such a vulnerable population. Methods: We conducted a randomised, assessor- and statistician-blinded, superiority trial including older adults with MDD. The study was conducted between 02/02/2019 and 02/22/2023 in 11 Italian Psychiatric Services. Participants were randomised to vortioxetine or one of the SSRIs, selected according to common practice. Treatment discontinuation due to adverse events after six months was the primary outcome, for which we aimed to detect a 12% difference in favour of vortioxetine. The study was registered in the online repository clinicaltrials.gov (NCT03779789). Findings: The intention-to-treat population included 179 individuals randomised to vortioxetine and 178 to SSRIs. Mean age was 73.7 years (standard deviation 6.1), and 264 participants (69%) were female. Of those on vortioxetine, 78 (44%) discontinued the treatment due to adverse events at six months, compared to 59 (33%) of those on SSRIs (odds ratio 1.56; 95% confidence interval 1.01–2.39). Adjusted and per-protocol analyses confirmed point estimates in favour of SSRIs, but without a significant difference. With the exception of the unadjusted survival analysis showing SSRIs to outperform vortioxetine, secondary outcomes provided results consistent with a lack of substantial safety and tolerability differences between the two arms. Overall, no significant differences emerged in terms of response rates, depressive symptoms and quality of life, while SSRIs outperformed vortioxetine in terms of cognitive performance. Interpretation: As opposed to what was previously hypothesised, vortioxetine did not show a better tolerability profile compared to SSRIs in older adults with MDD in this study. This article presents a cross-domain model that defines and sets the acoustic signal generation process due to the fast dose deposition induced by a proton beam penetrating an energy absorber. The proposed model reduces the complex iono-acoustic energy transformation process to a simple impulse-response of a linear-time-invariant (LTI) system and is validated by comparing LTI output simulation results (in both time and frequency domain) with experimental acoustic signals emitted by a physical proton beam at 20 MeV and 70-120-ns pulse time width. Thanks to the intrinsic simplicity of the system, it is possible to predict and estimate the effective acoustic power at the sensor and the resultant beam range measurement precision. More importantly, the information coming from LTI simulations provides well-defined methodologies that allow to increase acoustic signal amplitude from +3 up to +9 dB and to improve the measurement precision of the beam range localization (up to +/ - 0.3 mm versus +/ - 0.6 mm state of the art for 200-MeV energy and 75-mGy total dose).

Introduction: Depression is a highly prevalent condition in the elderly, with a vast impact on quality of life, life expectancy, and medical outcomes. Selective serotonin reuptake inhibitors (SSRIs) are the most commonly prescribed agents in this condition and, although generally safe, tolerability issues cannot be overlooked. Vortioxetine is an antidepressant with a novel mechanism of action. Based on studies to date, it may have a promising tolerability profile in the elderly, as it does not adversely affect psychomotor or cognitive performance and does not alter cardiovascular and endocrine parameters. The present study aims to assess the tolerability profile of vortioxetine in comparison with the SSRIs considered as a single group in elderly participants with depression. The rate of participants withdrawing from treatment due to adverse events after 6 months of follow up will be the primary outcome. Methods and analysis: This is a pragmatic, multicentre, open-label, parallel-group, superiority, randomized trial funded by the Italian Medicines Agency (AIFA - Agenzia Italiana del Farmaco). Thirteen Italian Community Psychiatric Services will consecutively enrol elderly participants suffering from an episode of major depression over a period of 12 months. Participants will be assessed at baseline and after 1, 3 and 6 months of follow up. At each time point, the following validated rating scales will be administered: Montgomery-Åsberg Depression Rating Scale (MADRS), Antidepressant Side-Effect Checklist (ASEC), EuroQual 5 Dimensions (EQ-5D), Short Blessed Test (SBT), and Charlson Age-Comorbidity Index (CACI). Outcome assessors and the statistician will be masked to treatment allocation. A total of 358 participants (179 in each group) will be enrolled. Ethics and dissemination: This study will fully adhere to the ICH E6 Guideline for Good Clinical Practice. Aims: As refugees and asylum seekers are at high risk of developing mental disorders, we assessed the effectiveness of Self-Help Plus (SH +), a psychological intervention developed by the World Health Organization, in reducing the risk of developing any mental disorders at 12-month follow-up in refugees and asylum seekers resettled in Western Europe. Methods: Refugees and asylum seekers with psychological distress (General Health Questionnaire-12 ≥ 3) but without a mental disorder according to the Mini International Neuropsychiatric Interview (M.I.N.I.) were randomised to either SH + or enhanced treatment as usual (ETAU). The frequency of mental disorders at 12 months was measured with the M.I.N.I., while secondary outcomes included self-identified problems, psychological symptoms and other outcomes. Results: Of 459 participants randomly assigned to SH + or ETAU, 246 accepted to be interviewed at 12 months. No difference in the frequency of any mental disorders was found (relative risk [RR] = 0.841; 95% confidence interval [CI] 0.389-1.819; p-value = 0.659). In the per protocol (PP) population, that is in participants attending at least three group-based sessions, SH + almost halved the frequency of mental disorders at 12 months compared to ETAU, however so few participants and events contributed to this analysis that it yielded a non-significant result (RR = 0.528; 95% CI 0.180-1.544; p-value = 0.230). SH + was associated with improvements at 12 months in psychological distress (p-value = 0.004), depressive symptoms (p-value = 0.011) and wellbeing (p-value = 0.001). Conclusions: The present study failed to show any long-term preventative effect of SH + in refugees and asylum seekers resettled in Western European countries. Analysis of the PP population and of secondary outcomes provided signals of a potential effect of SH + in the long-term, which would suggest the value of exploring the effects of booster sessions and strategies to increase SH + adherence.

We assessed the effectiveness of a self-help psychological intervention developed by the World Health Organization, called Self-Help Plus, in preventing the development of mental disorders among Syrian refugees experiencing psychological distress in Turkey. A two-arm, assessor-masked RCT was conducted in two Turkish areas. Eligible participants were adult Syrian refugees experiencing psychological distress (General Health Questionnaire ≥3), but without a diagnosis of mental disorder. They were randomly assigned either to the Self-Help Plus arm (consisting of Self-Help Plus combined with Enhanced Care as Usual, ECAU) or to ECAU only in a 1:1 ratio. Self-Help Plus was delivered in a group format by two facilitators over five sessions. The primary outcome measure was the presence of any mental disorder assessed by the Mini International Neuropsychiatric Interview at six-month follow-up. Secondary outcome measures were the presence of mental disorders at post-intervention, and psychological distress, symptoms of post-traumatic stress disorder and depression, personally identified psychological outcomes, functional impairment, subjective well-being, and quality of life at post-intervention and six-month follow-up. Between October 1, 2018 and November 30, 2019, 1,186 refugees were assessed for inclusion. Five hundred forty-four people were ineligible, and 642 participants were enrolled and randomly assigned to either Self-Help Plus (N=322) or ECAU (N=320). Methods of computational statistics allow efficient estimation of extreme ocean environments, and facilitate optimal operational decision making. We describe estimation of extreme quantiles of total water level and related quantities from a non-stationary hierarchical model for ocean storms. The model incorporates a directional–seasonal extreme value model for occurrences of storm peak significant wave height, a conditional directional model for within-storm evolution of sea states relative to storm peak, a conditional model for the maximum crest within a sea state, and models for total water level. Importance sampling is used for efficient computation of marginal total water level characteristics. We use the model to estimate an optimal un-manning procedure for a notional North Sea offshore structure in severe conditions.

The high spatial variability of soils is a relevant issue at local and global scales, and determines the complexity of soil ecosystem functions and services. This variability derives from strong dependencies of soil ecosystems on parent materials, climate, relief and biosphere, including human impact. Although present in all environments, the interactions of soils with these forming factors are particularly striking in mountain areas.

Purpose: Climate change is driving strong variations in mountain habitats, such as glacier retreat, which is releasing large surfaces soon colonized by vegetation and attacked by weathering and pedogenesis. Many proglacial soil chronosequences have been studied in different parts of the world, but no study is available on early soil development and pedogenesis on serpentinite. Materials and methods: We analysed the development of the main chemical (pH, organic matter, nutrients and exchangeable cations) and morphological properties in three soil chronosequences in the Verra Grande Glacier forefield (Italian side of the Monte Rosa Group, Western Alps), characterized by slightly different parent materials (pure serpentinite or serpentinite with small gneiss inclusions) and topography (steep lateral moraines or flat basal till). Results and discussion: Organic matter accumulation, acidification and base and metal leaching are the most important pedogenetic processes active during early stages of soil formation on serpentinite in the upper subalpine altitudinal belt. These processes are associated with minor changes in color and structure showing weak mineral weathering. Biocycling of nutrients is limited on pure serpentinite because of weak primary productivity of the plant community. Pedogenesis is quite slow throughout the forefield, and it is slowest on pure serpentinite. On flat surfaces, where slow erosion permits a fast colonization by Ericaceae, the podzolization process begins after few centuries since moraine deposition, while on steep slopes more time is required. Conclusions: Pedogenesis on serpentinite is extremely slow. The fast colonization by grassland species increases the speed of pedogenetic trends where serpentinitic till is enriched by small quantities of P-rich gneiss. The encroachment of forest-shrub species increases the speed of pedogenetic trends thanks to a strong nutrient biocycling.
