Abstract

Today the locution “looking glass” survives almost exclusively thanks to the extraordinary success of Lewis Carroll’s novel Through the Looking-Glass. This expression underlines the ambiguity between the glass surface intended as a device through which we can see the world or as an actual object to be “looked at.” Apparently, the early Renaissance perspective window, thanks to the mildness of the Mediterranean climate, did not need any panes. And certainly, even when glass panes are there, they are usually not reproduced in painting. The glass main virtue is its transparency, which makes it almost invisible. Something similar happens with other “glasses” specifically made to look through them: the drinking glass and the lens. Glass panes appear to sight only when different practical needs come into play, as in perspective drawing machines, or when its transparency is contradicted by a precise action that compromises or denies it: when panes are broken, as in this enigmatic portrait of early XIX c., or voluntarily covered, like for a blackout, as in Duchamp’s Fresh Widow. Looking through the glass, looking at the image reflected in the mirror and, finally, looking at the glass itself, as a device for presenting and representing spaces, are three recurring attitudes in the work of Italian artists of the late 20th century, like Lucio Fontana and Luciano Fabro.

Abstract

Na sociedade contemporânea, as discussões envolvendo a inclusão da Pessoa com Deficiência no mercado de trabalho passaram a ganhar notoriedade com a publicação dos dispositivos legais. Nesse sentido, o presente estudo teve como objetivo analisar e entender o que preconizam a Lei Brasileira de Inclusão da Pessoa com Deficiência nº 13.146/2015 e outras leis correlatas em relação à inclusão da pessoa com deficiência no mercado de trabalho. A problemática que investigamos pode ser colocada da seguinte maneira: o que dizem as normas jurídicas sobre a inclusão da Pessoa com Deficiência no mercado de trabalho? Caracterizada como bibliográfica e documental, a pesquisa articula reflexões a partir de estudos bibliográficos e de documentos. Ademais, destacamos que este estudo se justifica pela importância de profissionais da área da Educação e do Direito refletirem e debaterem sobre as limitações e possibilidades da inclusão no ambiente de trabalho. Como resultados, verificamos que os direitos das pessoas com deficiência, no Brasil, são amplamente tutelados por diversos veículos legais; contudo, mesmo com mudanças transcorrendo, ainda há discriminação e preconceito no mercado de trabalho.

Abstract

Quercetin, the most abundant dietary flavonol, has antioxidant effects in cardiovascular disease, but the evidence regarding its effects on blood pressure (BP) has not been conclusive. We assessed the impact of quercetin on BP through a systematic review and meta-analysis of available randomized controlled trials. We searched PUBMED, Cochrane Library, Scopus, and EMBASE up to January 31, 2015 to identify placebo-controlled randomized controlled trials investigating the effect of quercetin on BP. Meta-analysis was performed using either a fixed-effects or random-effect model according to I(2) statistic. Effect size was expressed as weighted mean difference (WMD) and 95% CI. Overall, the impact of quercetin on BP was reported in 7 trials comprising 9 treatment arms (587 patients). The results of the meta-analysis showed significant reductions both in systolic BP (WMD: -3.04 mm Hg, 95% CI: -5.75, -0.33, P=0.028) and diastolic BP (WMD: -2.63 mm Hg, 95% CI: -3.26, -2.01, P500 mg/day. Further studies are necessary to investigate the clinical relevance of these results and the possibility of quercetin application as an add-on to antihypertensive therapy.

Abstract

Como integrantes do público alvo da educação especial, os estudantes com Altas Habilidades/Superdotação têm direito ao atendimento educacional especializado. Nesse sentido, o presente artigo tem como objetivo analisar o estado do conhecimento sobre o trabalho desenvolvido nas Salas de Recursos Multifuncionais e nos Núcleos de Atividades de Altas Habilidades/Superdotação. Para tanto foi realizada uma pesquisa bibliográfica, buscando analisar as teses e dissertações publicadas na base de dados da Biblioteca Digital Brasileira de Teses e Dissertações do Instituto Brasileiro de Informação em Ciência e Tecnologia (BDTD/IBICT), no período entre 2011 a 2020. Os resultados revelam que o trabalho desenvolvido no AEE enfrenta muitos desafios, como: dificuldades na identificação desses discentes, o não cumprimento das políticas públicas, infraestrutura precária e formação deficitária dos profissionais que atuam com este público. Concluímos que apesar das dificuldades, este atendimento se mostra de suma importância a este alunado.

Abstract

The MADS-domain transcription factor SEEDSTICK (STK) controls several aspects of plant reproduction. STK is co-expressed with CESTA (CES), a basic Helix-Loop-Helix (bHLH) transcription factor-encoding gene. CES was reported to control redundantly with the brassinosteroid positive signaling factors BRASSINOSTEROID ENHANCED EXPRESSION1 (BEE1) and BEE3 the development of the transmitting tract. Combining the stk ces-4 mutants led to a reduction in ovule fertilization due to a defect in carpel fusion which, caused the formation of holes at the center of the septum where the transmitting tract differentiates. Combining the stk mutant with the bee1 bee3 ces-4 triple mutant showed an increased number of unfertilized ovules and septum defects. The transcriptome profile of this quadruple mutant revealed a small subset of differentially expressed genes which are mainly involved in cell death, extracellular matrix and cell wall development. Our data evidence a regulatory gene network controlling transmitting tract development regulated directly or indirectly by a STK-CES containing complex and reveal new insights in the regulation of transmitting tract development by bHLH and MADS-domain transcription factors.

Abstract

These practice guidelines on the management of arterial hypertension are a concise summary of the more extensive ones prepared by the Task Force jointly appointed by the European Society of Hypertension and the European Society of Cardiology. These guidelines have been prepared on the basis of the best available evidence on all issues deserving recommendations; their role must be educational and not prescriptive or coercive for the management of individual subjects who may differ widely in their personal, medical and cultural characteristics. The members of the Task Force have participated independently in the preparation of these guidelines, drawing on their academic and clinical experience and by objective examination and interpretation of all available literature. A disclosure of their potential conflict of interest is reported on the websites of the ESH and the ESC.

Il primo numero de «L’uo­mo nero» usciva nel 2003 e la rivista affronta ora un grosso cambiamento, passando dall’edizione cartacea a quella online, ad accesso aperto. Per celebrare questo passaggio viene presentato un numero antologico che raccoglie una scelta di contributi pubblicati negli anni dai membri del Comitato scientifico e da alcuni amici di vecchia data dell’«Uomo nero». La scelta di quattordici articoli tra gli oltre duecen­toquaranta pubblicati non potrà essere del tutto rappresentativa della fisionomia della rivista, ma potrà almeno servire a restituire per sommi capi il senso di un percorso., The first issue of «L’uomo nero» was published in 2003 and the journal is now facing a major change, passing from printed to online edition, in open access. To celebrate this passage, an anthological issue is presented which collects a selection of contributions published over the years by the members of the Scientific Committee and some old friends of «L’uomo nero». The choice of fourteen articles among the more than two hundred and forty published may not be fully representative of the physiognomy of the journal, but may at least serve to give a summary of the meaning of its project.

31. Blood pressure-lowering treatment strategies based on cardiovascular risk versus blood pressure: A meta-analysis of individual participant data.

Abstract

BACKGROUND:Clinical practice guidelines have traditionally recommended blood pressure treatment based primarily on blood pressure thresholds. In contrast, using predicted cardiovascular risk has been advocated as a more effective strategy to guide treatment decisions for cardiovascular disease (CVD) prevention. We aimed to compare outcomes from a blood pressure-lowering treatment strategy based on predicted cardiovascular risk with one based on systolic blood pressure (SBP) level. METHODS AND FINDINGS:We used individual participant data from the Blood Pressure Lowering Treatment Trialists' Collaboration (BPLTTC) from 1995 to 2013. Trials randomly assigned participants to either blood pressure-lowering drugs versus placebo or more intensive versus less intensive blood pressure-lowering regimens. We estimated 5-y risk of CVD events using a multivariable Weibull model previously developed in this dataset. We compared the two strategies at specific SBP thresholds and across the spectrum of risk and blood pressure levels studied in BPLTTC trials. The primary outcome was number of CVD events avoided per persons treated. We included data from 11 trials (47,872 participants). During a median of 4.0 y of follow-up, 3,566 participants (7.5%) experienced a major cardiovascular event. Areas under the curve comparing the two treatment strategies throughout the range of possible thresholds for CVD risk and SBP demonstrated that, on average, a greater number of CVD events would be avoided for a given number of persons treated with the CVD risk strategy compared with the SBP strategy (area under the curve 0.71 [95% confidence interval (CI) 0.70-0.72] for the CVD risk strategy versus 0.54 [95% CI 0.53-0.55] for the SBP strategy). Compared with treating everyone with SBP ≥ 150 mmHg, a CVD risk strategy would require treatment of 29% (95% CI 26%-31%) fewer persons to prevent the same number of events or would prevent 16% (95% CI 14%-18%) more events for the same number of persons treated. Compared with treating everyone with SBP ≥ 140 mmHg, a CVD risk strategy would require treatment of 3.8% (95% CI 12.5% fewer to 7.2% more) fewer persons to prevent the same number of events or would prevent 3.1% (95% CI 1.5%-5.0%) more events for the same number of persons treated, although the former estimate was not statistically significant. In subgroup analyses, the CVD risk strategy did not appear to be more beneficial than the SBP strategy in patients with diabetes mellitus or established CVD. CONCLUSIONS:A blood pressure-lowering treatment strategy based on predicted cardiovascular risk is more effective than one based on blood pressure levels alone across a range of thresholds. These results support using cardiovascular risk assessment to guide blood pressure treatment decision-making in moderate- to high-risk individuals, particularly for primary prevention.

Abstract

Objectives: To establish the positive predictive values (PPV) of cfDNA testing based on data from a nationwide survey of independent clinical cytogenetics laboratories. Methods: Prenatal diagnostic test results obtained by Italian laboratories between 2013 and March 2020 were compiled for women with positive non-invasive prenatal tests (NIPT), without an NIPT result, and cases where there was sex discordancy between the NIPT and ultrasound. PPV and other summary data were reviewed. Results: Diagnostic test results were collected for 1327 women with a positive NIPT. The highest PPVs were for Trisomy (T) 21 (624/671, 93%) and XYY (26/27, 96.3%), while rare autosomal trisomies (9/47, 19.1%) and recurrent microdeletions (8/55, 14.5%) had the lowest PPVs. PPVs for T21, T18, and T13 were significantly higher when diagnostic confirmation was carried out on chorionic villi (97.5%) compared to amniotic fluid (89.5%) (p < 0.001). In 19/139 (13.9%), of no result cases, a cytogenetic abnormality was detected. Follow-up genetic testing provided explanations for 3/6 cases with a fetal sex discordancy between NIPT and ultrasound. Conclusions: NIPT PPVs differ across the conditions screened and the tissues studied in diagnostic testing. This variability, issues associated with fetal sex discordancy, and no results, illustrate the importance of pre- and post-test counselling.

Abstract

Background: The effects of pharmacological blood-pressure-lowering on cardiovascular outcomes in individuals aged 70 years and older, particularly when blood pressure is not substantially increased, is uncertain. We compared the effects of blood-pressure-lowering treatment on the risk of major cardiovascular events in groups of patients stratified by age and blood pressure at baseline. Methods: We did a meta-analysis using individual participant-level data from randomised controlled trials of pharmacological blood-pressure-lowering versus placebo or other classes of blood-pressure-lowering medications, or between more versus less intensive treatment strategies, which had at least 1000 persons-years of follow-up in each treatment group. Participants with previous history of heart failure were excluded. Data were obtained from the Blood Pressure Lowering Treatment Triallists' Collaboration. We pooled the data and categorised participants into baseline age groups (<55 years, 55–64 years, 65–74 years, 75–84 years, and ≥85 years) and blood pressure categories (in 10 mm Hg increments from <120 mm Hg to ≥170 mm Hg systolic blood pressure and from <70 mm Hg to ≥110 mm Hg diastolic). We used a fixed effects one-stage approach and applied Cox proportional hazard models, stratified by trial, to analyse the data. The primary outcome was defined as either a composite of fatal or non-fatal stroke, fatal or non-fatal myocardial infarction or ischaemic heart disease, or heart failure causing death or requiring hospital admission. Findings: We included data from 358 707 participants from 51 randomised clinical trials. The age of participants at randomisation ranged from 21 years to 105 years (median 65 years [IQR 59–75]), with 42 960 (12·0%) participants younger than 55 years, 128 437 (35·8%) aged 55–64 years, 128 506 (35·8%) 65–74 years, 54 016 (15·1%) 75–84 years, and 4788 (1·3%) 85 years and older. Abstract

Background: The effects of pharmacological blood pressure lowering at normal or high-normal blood pressure ranges in people with or without pre-existing cardiovascular disease remains uncertain. We analysed individual participant data from randomised trials to investigate the effects of blood pressure lowering treatment on the risk of major cardiovascular events by baseline levels of systolic blood pressure. Methods: We did a meta-analysis of individual participant-level data from 48 randomised trials of pharmacological blood pressure lowering medications versus placebo or other classes of blood pressure-lowering medications, or between more versus less intensive treatment regimens, which had at least 1000 persons-years of follow-up in each group. Trials exclusively done with participants with heart failure or short-term interventions in participants with acute myocardial infarction or other acute settings were excluded. Data from 51 studies published between 1972 and 2013 were obtained by the Blood Pressure Lowering Treatment Trialists' Collaboration (Oxford University, Oxford, UK). We pooled the data to investigate the stratified effects of blood pressure-lowering treatment in participants with and without prevalent cardiovascular disease (ie, any reports of stroke, myocardial infarction, or ischaemic heart disease before randomisation), overall and across seven systolic blood pressure categories (ranging from <120 to ≥170 mm Hg). The primary outcome was a major cardiovascular event (defined as a composite of fatal and non-fatal stroke, fatal or non-fatal myocardial infarction or ischaemic heart disease, or heart failure causing death or requiring admission to hospital), analysed as per intention to treat. Findings: Data for 344 716 participants from 48 randomised clinical trials were available for this analysis.
