Abstract

The prevalence of diverticular disease (DD) increases with age, being reported in less than 5% of patients younger than 40 years and in 65-70% of patients older than 80 years. Diverticula represents the most frequent finding at colonoscopy. The pathogenesis of divericula is multifactorial, depending on diet and structural alterations of the colonic wall. Up to 75% of patients remain completely asymptomatic throughout their life while the remaining 25% of patients develops diverticular disease (DD) which includes symptomatic uncomplicated DD (20%) and diverticulitis (5%). The latter may be further complicated by perforation and bleeding (1-2% of the cases). In spite of the large amount of literature data, many unmet needs regarding risk factors, comorbidities, timing of surgery, neoplastic risk, and quality of life, still affect the management of patients with DD.

Abstract

Irritable bowel syndrome (IBS) is the commonest functional gastrointestinal disorder, affecting between 10% and 20% of the adult population worldwide. Recently, IBS research has focused on organic components of causative factors such as gut microbiota that in turn may trigger at mucosal level low-grade inflammation and immune activation. To date, four main lines of reasoning support the role of gut microbiota in the pathogenesis of IBS: 1) IBS develops in up to 30% of individuals recovering from acute gastroenteritis "post-infective IBS"; 2) small intestinal bacterial overgrowth (SIBO) is characterized by symptoms like IBS; 3) quantitative and qualitative alterations in gut microbiota "dysbiosis "have been reported in subjects with IBS; 4) IBS symptoms can be improved by treatments (antibiotics, probiotics, prebiotics) that target the microbiota. These arguments open the way to new therapeutic options to treat patients with IBS by the administration of antibiotics, probiotics and perhaps prebiotics. Antibiotic treatment results in significant improvement rates in IBS symptoms; however, side or systemic effects, drug resistance and cost limit the chronic use. Many studies report a positive effect of probiotics, however, at present randomized placebo-controlled trials taking into account well-defined and measurable endpoints and a long follow-up should be planned.

Abstract

Background & Aims: Determining risk for recurrence or survival after curative resection or ablation in patients with hepatitis C virus (HCV)-related hepatocellular carcinoma (HCC) is important for stratifying patients according to expected outcomes in future studies of adjuvant therapy in the era of direct-acting antivirals (DAAs). The aims of this meta-analysis were to estimate the recurrence and survival probabilities of HCV-related early HCC following complete response after potentially curative treatment and to identify predictors of recurrence and survival. Methods: Studies reporting time-dependent outcomes (HCC recurrence or death) after potentially curative treatment of HCV-related early HCC were identified in MEDLINE through May 2016. Data on patient populations and outcomes were extracted from each study by three independent observers and combined using a distribution-free summary survival curve. Primary outcomes were actuarial probabilities of recurrence and survival. Results: Eleven studies met the inclusion criteria. Pooled estimates of actuarial recurrence rates were 7.4% at 6months and 47.0% at 2years. Pooled estimates of actuarial survival rates were 79.8% at 3years and 58.6% at 5years. Heterogeneity among studies was highly significant for all outcomes. By univariate meta-regression analyses, lower serum albumin, randomized controlled trial study design and follow-up were independently associated with higher recurrence risk, whereas tumour size and alpha-foetoprotein levels were associated with higher mortality. Conclusions: This meta-analysis showed that recurrence risk and survival are extremely variable in patients with successfully treated HCV-related HCC, providing a useful benchmark for indirect comparisons of the benefits of DAAs and for a correct design of randomized controlled trials in the adjuvant setting.

Abstract

none 48 no Background: In HCV-infected cirrhotic patients with successfully treated early hepatocellular carcinoma (HCC), the time to HCC recurrence and the effects of sustained viral eradication (SVR) by interferon (IFN)-based or IFN-free regimens on HCC recurrence remain unclear. Aim: To perform an indirect comparison of time to recurrence (TTR) in patients with successfully treated early HCC and active HCV infection with those of patients with SVR by IFN-based and by IFN-free regimens. Methods: We evaluated 443 patients with HCV-related cirrhosis and Barcelona Clinic Liver Cancer Stage A/0 HCC who had a complete radiological response after curative resection or ablation. Active HCV infection was present in 328, selected from the Italian Liver Cancer group cohort; 58 patients had SVR achieved by IFN-free regimens after HCC cure, and 57 patients had SVR achieved by IFN-based regimens after HCC cure. Individual data of patients in the last two groups were extracted from available publications. Results: TTR by Kaplan–Meier curve was significantly lower in patients with active HCV infection compared with those with SVR both by IFN-free (P = 0.02) and by IFN-based (P < 0.001) treatments. TTR was similar in patients with SVR by IFN-free or by IFN-based (P = 0.49) strategies. Conclusion: In HCV-infected, successfully treated patients with early HCC, SVR obtained by IFN-based or IFN-free regimens significantly reduce tumour recurrence without differences related to the anti-viral strategy used. Abstract

Background & Aims: Information on safety and efficacy of systemic treatment in patients with hepatocellular carcinoma (HCC) under dialysis are limited due to patient exclusion from clinical trials. Thus, we aimed to evaluate the rate, prevalence, tolerability, and outcome of sorafenib in this population. Methods: We report a multicenter study comprising patients from Latin America and Europe. Patients treated with sorafenib were enrolled; demographics, dose modifications, adverse events (AEs), treatment duration, and outcome of patients undergoing dialysis were recorded. Results: As of March 2018, 6156 HCC patients were treated in 44 centres and 22 patients were concomitantly under dialysis (0.36%). The median age was 65.5years, 40.9% had hepatitis C, 75% had Child-Pugh A, and 85% were Barcelona Clinic Liver Cancer-C. The median time to first dose modification, treatment duration and overall survival rate were 2.4months (interquartile ranges [IQR], 0.8-3.8), 10.8months (IQR, 4.5-16.9), and 17.5months (95% CI, 7.2-24.5), respectively. Seventeen patients required at least 1 dose modification. The main causes of first dose modification were asthenia/worsening of Eastern Cooperative Oncology Group-Performance Status and diarrhoea. At the time of death or last follow-up, four patients were still on treatment and 18 had discontinued sorafenib: 14 were due to tumour progression, 2 were sorafenib-related, and 2 were non-sorafenib-related AE. Conclusions: The outcomes observed in this cohort seem comparable to those in the non-dialysis population. Thus, to the best of our knowledge, this is the largest and most informative dataset regarding systemic treatment outcomes in HCC patients undergoing dialysis.

Abstract

[Background & Aims] The coronavirus disease 2019 (COVID-19) pandemic has posed unprecedented challenges to healthcare systems and it may have heavily impacted patients with liver cancer (LC). Herein, we evaluated whether the schedule of LC screening or procedures has been interrupted or delayed because of the COVID-19 pandemic., [Methods] An international survey evaluated the impact of the COVID-19 pandemic on clinical practice and clinical trials from March 2020 to June 2020, as the first phase of a multicentre, international, and observational project. The focus was on patients with hepatocellular carcinoma or intrahepatic cholangiocarcinoma, cared for around the world during the first COVID-19 pandemic wave., [Results] Ninety-one centres expressed interest to participate and 76 were included in the analysis, from Europe, South America, North America, Asia, and Africa (73.7%, 17.1%, 5.3%, 2.6%, and 1.3% per continent, respectively). Eighty-seven percent of the centres modified their clinical practice: 40.8% the diagnostic procedures, 80.9% the screening programme, 50% cancelled curative and/or palliative treatments for LC, and 41.7% modified the liver transplantation programme. Forty-five out of 69 (65.2%) centres in which clinical trials were running modified their treatments in that setting, but 58.1% were able to recruit new patients. The phone call service was modified in 51.4% of centres which had this service before the COVID-19 pandemic (n = 19/37)., [Conclusions] The first wave of the COVID-19 pandemic had a tremendous impact on the routine care of patients with liver cancer. Modifications in screening, diagnostic, and treatment algorithms may have significantly impaired the outcome of patients. Ongoing data collection and future analyses will report the benefits and disadvantages of the strategies implemented, aiding future decision-making., [Lay summary] The coronavirus disease 2019 (COVID-19) pandemic has posed unprecedented challenges to healthcare systems globally. Abstract

The coronavirus infection is constantly diffusing worldwide and the incidence of death is dramatically increasing, representing one of the greatest disasters in human history. Nowadays, no effective therapeutic approaches have been licensed, despite the rising interest of the scientific research in this specific field, and the daily growing number of publications, while the need to find novel strategies is urgent. Evidence in the literature reported the antiviral activity of polyphenols, the largest class of bioactive compounds in nature. Interestingly, a limited number of studies investigated the efficacy of polyphenols from different raw materials, directly against coronaviruses. The present manuscript aimed to report this evidence and provide a viewpoint on the possibility to use it as a start point for the development of novel natural approaches against this viral infection, eventually designing further appropriate researches.

Abstract

Irritable bowel syndrome (IBS) is the commonest functional gastrointestinal disorder, affecting between 10% and 20% of the adult population worldwide. Recently, IBS research has focused on organic components of causative factors such as gut microbiota that in turn may trigger at mucosal level low-grade inflammation and immune activation. To date, four main lines of reasoning support the role of gut microbiota in the pathogenesis of IBS: 1) IBS develops in up to 30% of individuals recovering from acute gastroenteritis "post-infective IBS"; 2) small intestinal bacterial overgrowth (SIBO) is characterized by symptoms like IBS; 3) quantitative and qualitative alterations in gut microbiota "dysbiosis "have been reported in subjects with IBS; 4) IBS symptoms can be improved by treatments (antibiotics, probiotics, prebiotics) that target the microbiota. These arguments open the way to new therapeutic options to treat patients with IBS by the administration of antibiotics, probiotics and perhaps prebiotics. Antibiotic treatment results in significant improvement rates in IBS symptoms; however, side or systemic effects, drug resistance and cost Limit the chronic use. Many studies report a positive effect of probiotics, however, at present randomized placebo-controlled trials taking into account well-defined and measurable endpoints and a long follow-up should be planned.
