Abstract

D-dimer assay is used to stratify patients with unprovoked venous thromboembolism (VTE) for the risk of recurrence. However, this approach was never evaluated since direct oral anticoagulants are available. With this multicenter, prospective cohort study, we aimed to assess the value of an algorithm incorporating serial D-dimer testing and administration of reduced-dose apixaban (2.5 mg twice daily) only to patients with a positive test. A total of 732 outpatients aged 18 to 74 years, anticoagulated for ≥12 months after a first unprovoked VTE, were included. Patients underwent D-dimer testing with commercial assays and preestablished cutoffs. If the baseline D-dimer during anticoagulation was negative, anticoagulation was stopped and testing repeated after 15, 30, and 60 days. Patients with serially negative results (286 [39.1%]) were left without anticoagulation. At the first positive result, the remaining 446 patients (60.9%) were given apixaban for 18 months. All patients underwent follow-up planned for 18 months. The study was interrupted after a planned interim analysis for the high rate of primary outcomes (7.3%; 95% confidence interval [CI], 4.5-11.2), including symptomatic proximal deep vein thrombosis (DVT) or pulmonary embolism (PE) recurrence, death for VTE, and major bleeding occurring in patients off anticoagulation vs that in those receiving apixaban (1.1%; 95% CI, 0.4-2.6; adjusted hazard ratio [HR], 8.2; 95% CI, 3.2-25.3). In conclusion, in patients anticoagulated for ≥1 year after a first unprovoked VTE, the decision to further extend anticoagulation should not be based on D-dimer testing. The results confirmed the high efficacy and safety of reduced-dose apixaban against recurrences. This trial was registered at www.clinicaltrials.gov as #NCT03678506.

Abstract

Objective: Little is known about the process of becoming a mother in women who experienced a breast cancer diagnosis (BC). In this qualitative study, we investigated maternal representations in pregnant women with experience of BC and those with no oncological history. Methods: A total of 38 women were recruited, 19 women who experienced a BC diagnosis and 19 who had not. To explore maternal representations, semi-structured interviews were conducted and analysed through thematic analysis. Results: Four main themes were identified: fears and worries, meaning of motherhood, mother–foetus relationship and partner support. Across themes, differences between primiparous and multiparous are reported. Women with gestational breast cancer (GBC) described fear for their own and their child's survival. Women with previous BC recall contrasting emotions. All women with experience of BC perceived breastfeeding as fundamental and inability to do so provoked worry. Relationship with the partner was considered central, while healthy women were projected towards the future triadic relationship. Conclusions: Finding a mental space during pregnancy for the representation of the future child could be hard for women with GBC. Dissimilarities in the experience of motherhood in cancer patients provide insight into psychological aspects that should be taken into account in clinical practice.

Abstract

Objectives A continuous 13-h infusion of gabexate starting 30–90 min before endoscopic cholangiopancreatography (ERCP) can reduce postprocedural pancreatitis, the onset of which is generally observed within the first 6 h after ERCP. This study was designed to verify whether a 6.5-h infusion of gabexate was as effective as a 13-h infusion, at the same concentration, for reducing the incidence of post-ERCP pancreatitis (primary endpoint) and pancreatic hyperenzymemia and pain (secondary endpoints). Methods A total of 434 patients (201 male and 233 female; mean age 63.9 yr, range 18–96 yr) scheduled for ERCP were prospectively recruited in 25 Italian centers. Patients were randomized double-blind to two treatment groups. All subjects enrolled were first treated with a 500-mg continuous intravenous infusion of gabexate, starting 30 min before the endoscopic maneuvers and continuing up to 6.5 h after it. Over the next 6.5 h, 214 patients (group I) continued the infusion of gabexate (for a total of 1 g over 13 h) and 220 patients (group II) were given placebo (saline solution). Results The overall incidence of acute pancreatitis was 1.8% (eight patients), which included 1.4% in group I (three of 214 patients) and 2.2% in group II (five of 220 patients). Serum amylase and lipase values over time, peak levels of the two enzymes, pancreatic pain, and need for analgesics did not significantly differ in the two groups. Conclusions These results suggest that a 6.5-h infusion of gabexate (for a total of 500 mg) is not less effective than a 13 h infusion, with evident savings.

Abstract

Premessa Dalla letteratura emerge la descrizione della condizione di salute del paziente con lesione al midollo spinale (SCI) secondo il modello biopsicosociale dell’ICF, mancanza di strumenti di valutazione che prendano in considerazione gli aspetti dell’attività e partecipazione, dell’ambiente, sociali, necessari per un programma riabilitativo multidisciplinare per il paziente con SCI. Obiettivo Individuare le categorie ICF all’interno del core set che sono più efficaci nella descrizione della condizione del paziente ed utili nell’impostazione del pri. Valutare e monitorare mediante l’uso del core set ICF for SCI il percorso riabilitativo del paziente sottoposto ad intervento di chirurgia funzionale e restituito all’attività lavorativa. Correlazione tra modificazione icf, in particolare attività e partecipazione e carico assistenziale caregiver Risultati Paziente maschio di 38 anni, lesione vertebro-midollare C6-C7 traumatica (2009) , tetraplegico completo (ASIA A), livello neurologico C6, con CIRS 23 e presenza di retrazioni. Il core set ICF mostra discontinuità (5) del midollo cervicale, completa (4), bilaterale (3) e cambiamenti (7) delle strutture dell’arto superiore, gravi (3), bilaterali (3). Il paziente ha effettuato interventi di chirurgia funzionale in più tempi consistenti in trasferimenti tendinei multipli all’arto superiore sia sinistro che destro in data 14.3.2013 e 13.6.2013, presso Unità Spinale di Firenze. Prima degli interventi di chirurgia funzionale: FIM=52 (FIM motoria 17), SCIM=17 (SCIM cura persona 0, respirazione e controllo sfinteri 14,mobilità 3), livello neurologico=6 (livello sensitivo 6 dx e sin, livello motorio 6dx e sin). Al termine del percorso riabilitativo: FIM=72 (FIM motoria 37, cognitiva 35), SCIM 28/100 (SCIM mobilità 3), livello neurologico=6 (livello sensitivo 6 dx e sin, livello motori 7 dx e 6 sin) Il core set ICF mostra miglioramento delle funzioni dell’arto superiore, in particolare nelle performance e attività dell’attività manuali (raccogliere, afferrare, manipolare). Conclusioni Le scale funzionali (FIM e SCIM) e il core set ICF mostrano che il paziente , in seguito all’intervento di chirurgia funzionale e dopo un adeguato percorso riabilitativo è migliorato in relazione alle funzioni motorie. La FIM mostra in particolare miglioramenti relativi alla cura della persona e alla mobilità, la SCIM, l’ICF alle funzioni dell’arto superiore e nelle attività manuali.

Abstract

The Erectile Dysfunction Observational Study (EDOS) is a 6-months observational prospective multicentric study enrolling men with erectile dysfunction (ED) who asked, to be started on a treatment or to change a previous treatment. Aims of the study were to analyse the pattern of treatment and compare the efficacy of treatments used. Patients were enrolled during a normal hospital visit and were prescribed a treatment for ED. They were asked at baseline and after 3 and 6 months, to answer a set of questions from the International Index of Erectile Function, Erectile Dysfunction Inventory of Treatment Satisfaction (EDITS) and Short Form of the Psychological and Interpersonal Relationships Scale questionnaires (SF-PAIRS). Clinicians were free to prescribe any therapy for ED available in the market, and to change therapy at any time during the study. Out of 1 338 patients, available for analysis at 6 months, 624 (47%) changed their treatment during the study and 714 (53%) continued with the drug prescribed at baseline. Patients assuming tadalafil had a significantly higher probability of maintaining the same treatment compared to sildenafil or vardenafil. There was no clinically significant difference in terms of efficacy, patient satisfaction, self-confidence and spontaneity between the different inhibitors of PDE5. The 'time concerns' domain score of SF-PAIRS, was statistically better in patients assuming tadalafil. In conclusion sildenafil, vardenafil and tadalafil show similar efficacy in the clinical practice. However, patients receiving tadalafil display a lower risk to discontinue or change the treatment.

29. Organizational Capabilities and Industry Dynamics: a Computational Model

Abstract

This chapter aims to reconcile the logic behind stochastic models of firm growth and the notion of organizational capabilities as drivers of economic performance. In the proposed behavioral model of bounded rational firms, two mechanisms drive growth: independent stochastic growth of individual opportunities and the process by which firms capture new opportunities. To extend the stochastic framework, this research incorporates behavioral assumptions about the interactions between the firm and the business environment and the mechanism by which firms sense and seize business opportunities. The model generates statistical regularities in firm size, growth rates, and profit differentials between firms that are consistent with observed patterns in real-world settings. The greater the selective power of organizational capabilities, the more the steady-state distribution appears to deviate from log normal, which provides a potential explanation of various observed departures from pure proportional law. With regard to firm diversity, the distribution of opportunities per firm is skewed; just a few entities account for most of the business opportunities that arise during the simulation period. Moreover, the interaction between the external environment and the internal structure of firms influences heterogeneity in the value of the opportunities that they capture, as well as the persistence of long-run profits.
