- Abstract
High blood cholesterol is typically considered a feature of wealthy western countries1,2. However, dietary and behavioural determinants of blood cholesterol are changing rapidly throughout the world³ and countries are using lipid-lowering medications at varying rates. These changes can have distinct effects on the levels of high-density lipoprotein (HDL) cholesterol and non-HDL cholesterol, which have different effects on human health4,5. However, the trends of HDL and non-HDL cholesterol levels over time have not been previously reported in a global analysis. Here we pooled 1,127 population-based studies that measured blood lipids in 102.6 million individuals aged 18 years and older to estimate trends from 1980 to 2018 in mean total, non-HDL and HDL cholesterol levels for 200 countries. Globally, there was little change in total or non-HDL cholesterol from 1980 to 2018. This was a net effect of increases in low- and middle-income countries, especially in east and southeast Asia, and decreases in high-income western countries, especially those in northwestern Europe, and in central and eastern Europe. As a result, countries with the highest level of non-HDL cholesterol—which is a marker of cardiovascular risk—changed from those in western Europe such as Belgium, Finland, Greenland, Iceland, Norway, Sweden, Switzerland and Malta in 1980 to those in Asia and the Pacific, such as Tokelau, Malaysia, The Philippines and Thailand. In 2017, high non-HDL cholesterol was responsible for an estimated 3.9 million (95% credible interval 3.7 million–4.2 million) worldwide deaths, half of which occurred in east, southeast and south Asia. Aim: To perform a simple, rapid and sensitive Real-time PCR based SYBR Green method to determine the human leukocyte antigen (HLA)-DQ 2/8 alleles in celiac disease (CD) patients. Background: Many molecular techniques are available to determine the HLA-DQ2 and DQ8 alleles, but they are too expensive and have many steps that make them difficult to use. Methods: To determine the HLA-DQ 2/8 alleles we have developed a new real-time PCR assay, using SYBR Green technique with melting curve analysis on genomic DNA isolated from 75 CD patients and 94 healthy controls. The specific primers to examine HLA-DQA1*05, HLA-DQB1*02 and HLA-DQB1*0302 alleles were used and results were compared with commercially available kits. Results: Using this method, the presence of HLA-DQ2 and HLA-DQ8 alleles were determined with sensitivity and specificity 80% and 100% respectively and compared to low resolution commercially available kits, the results of this method were more efficient. The frequency of DQ2 and DQ8 in patients was 76% and 29%, respectively and overall 96% of patients were carries DQ2 and/or DQ8 alleles. Conclusion: The result of this study showed that Real-time PCR using SYBR Green method with melting curve analysis has good efficiency to identify the HLA-DQ2/8 risk alleles.

Background and Objective: One of the most important effects of nanomaterials on cells is the induction of programmed cell death (apoptosis). The aim of this study is to investigate the toxicity of graphene oxide-gold nanoparticles and to evaluate the expression of caspase 3 gene in HepG2 liver cancer cell line. Methods: In this experimental study, hepatocellular carcinoma cell line (HepG2) was prepared from Pasteur Institute of Iran. Cells were treated with concentrations of 10 to 500 µg/ml Gold-rGO and untreated cells were used as control. The level of caspase 3 gene expression and cell viability were investigated and compared within 24 and 48 hours in three groups of A, B and control (C). The effect of concentration was investigated by XTT method and acridine orange ethidium bromide staining. Caspase 3 gene expression was measured by relative quantification using real time PCR test. Findings: Based on XTT results, EC20, EC50, EC80 values were calculated as 386.420, 90.680, 24.151 µg/ml in 24 hours and 358.146, 89.536, 22.384 µg/ml in 48 hours. Examining the level of caspase 3 gene expression change in these concentrations compared to untreated cells showed mean fold changes (mean±SEM) of 3.436±1.022 in 24 hours and 4.054±0.02 in 48 hours, according to which the level of gene expression changes significantly. The viability of control cells (C) compared to cells treated with concentrations higher than 50 µg/ml (B) has a relative increase, which was statistically significant (p<0.0001). Examination with AO/EB staining to determine the level of apoptosis showed a higher level of apoptosis with increasing dose. Conclusion: The results of the study showed that shorter times and lower concentrations should be used in the application of this nanoparticle. [ABSTRACT FROM AUTHOR]

Micro-organisms contain 90% of cells in human body and trillions foreign genes versus less than 30 thousand of their own. The human colon host various species of microorganisms, appraised at more than 1014 microbiota and contained of over a thousand species. Although each one’s profile is separable, the relative abundance and distribution of bacterial species is the same between healthy ones, causing conservation of each person’s overall health. Germline DNA mutations have been attributed to the less than 5% of CRC occurrence while more than 90% is associated with the epigenetic regulation. The most ubiquitous environmental factor in epigenetic modification is gut microbiota. Disruptive changes in the gut microbiome strongly contributed to the improvement of colorectal cancer. Gut microbiota may play critical role in progression of CRC via their metabolite or their structural component interacting with host intestinal epithelial cell (IEC). Herein we discuss the mechanism of epigenetic modification and its implication in CRC development, progression even metastasis by gut microbiota induction.

BACKGROUND AND OBJECTIVE: Colorectal cancer is one of the growing cancers in Iran and one of the few preventable cancers in the World Health Organization. One of the most important ways to early diagnose this malignancy and its underlying lesions (polyps) and subsequently to prevent it is screening. Colorectal cancer is one of the growing cancers in Iran and one of the few preventable cancers in the World Health Organization. One of the most important ways of early detection of this malignancy and its underlying lesions (polyps) and consequently its prevention is screening. Different protocols have been developed for the diagnosis of first degree relatives of known hereditary cases such as HNPCC and FAP, but the diagnosis plan for first degree relatives has not been defined as a group of familial colorectal cancers without known mutations (type X colorectal cancer). The purpose of this study was to evaluate the results of colonoscopy screening in first-degree relatives of these patients. METHODS: This cross-sectional study was performed on the families of patients with suspected Lynch syndrome who had no deficiency in the expression of repair proteins. Data were collected from the medical records system of colorectal cancer patients who had been enrolled in the system for prevention and early detection of colorectal cancer at Shahid Beheshti University of Medical Sciences Research Center from 1387 to 1397. FINDINGS: The results of colonoscopy revealed that 18 out of 77 subjects had 23% polyps or tumors in their colon, out of which 55% were polyps and the rest were adenomas. About 60% of polyps were in the rectum and sigmoid and the rest in the colon and cecum. Also, 4 patients in this screening showed tumors in their colon, all of which were newly diagnosed, and 2 patients in this screening were diagnosed with inflammatory bowel disease. CONCLUSION: According to the results of this study, in the case of colon polyps, the families of these patients appear to be at high risk like those of Lynch syndrome families and need regular colonoscopy care. Background: Cryptosporidium is a worldwide protozoan parasite and one of the most common causes of infection and diarrhea in humans and cattle. The aim of the present study was determination of subtypes of Cryptosporidium among children with diarrhea in Tehran by sequence analysis of the highly polymorphic 60-kDa glycoprotein (GP60) gene.Methods: Fecal samples were collected from 794 diarrheic children. Initial identification of Cryptosporidium was carried out on stool samples by Ziehl-Neelsen acid-fast staining method. DNA was extracted from positive microscopically samples and Cryptosporidium genotypes and subtypes were determined, accordingly."nResults: Out of 794 collected samples, 19 (2.40 %) were positive for Cryptosporidium oocysts. Sequences analysis of GP60 gene showed that 17 (89.47 %) of the positive isolates were Cryptosporidium parvum and 2 (10.52 %) were C. hominis. All subtypes of C. parvum isolates belonged to allele families IIa (6/17) and IId (11/17). The most common allele in all 17 isolates belonged to IId A20G1a (41.18%). A22G1 (IF) subtype was detected in two C. hominis isolates of the children."nConclusion: The predominancy of C. parvum species (specially, IId A20G1a subtype) in current study underlines the importance of zoonotic Cryptosporidium transmission in Iran.

BACKGROUND AND OBJECTIVE: Transforming growth factor beta (TGF-β) has an inhibitory role in homeostasis of T-cell response and regulatory T cells and regulation of immune response against viral infections. Codon 10 of protein is located in the signal peptide and involved in secretion of cytokine. The aim of this study was to investigate the association of leu-pro polymorphism of codon 10 and hepatitis C susceptibility in patients.METHODS: This case-control study was performed on 112 chronic hepatitis C patients and 122 healthy control subjects. TGF-β1 gene was amplified with PCR method and genotypes were determined using Restriction Fragment Length Polymorphism (RFLP) with MspA1I restriction enzyme.FINDINGS: The frequency of CT genotype in both groups was higher than CC and TT. Genotyping results for CC, CT and TT states in patients was 50%, 19.6% and 30.4% and in healthy controls was 59%, 13.9% and 27.1% respectively. We found no significant difference between patients and healthy controls according to codon 10 polymorphism. CONCLUSION: According to the results of this study, no relationship was found between this protein’s genetic variations and chronic hepatitis C infection susceptibility.

Introduction: Cryptosporidium is an Apicomplexa parasite that infects humans and a wide range of domestic and wild animals. However, the sub-genotypes of the species infecting animals in Iran are unclear. The aim of the present study isto identify DNA fingerprinting of bovine Cryptosporidium in Qazvin province using sequences of GP60 gene. Materials and Methods : In this study we investigated 25 C. parvum isolated form bovine in Qazvin animal husbandries. Subgenotypes were determined by DNA sequencing of 60-kDa glycoprotein gene. Results: Using DNA sequencing of GP60 gene, two subtype families within the C. parvum included IId (15/22) and IIa (7/22) were recognized. Also three subtypes in these two subtype families included IIa A15G2R1 (22/25), IIa A16G3R (1/25), IId A15G1 (2/25) were determined. Conclusion: Today, new zoonose strains are identified which based on severity of infection compared with human strains are more sever and the source and transmission of their infection are unclear. Therefore, determination of C .parvum strains, genotypes and sub-genotypes for epidemiological studies are quite necessary specially to identify the animal sources and to improve the control and prevention programs due to the lake of effective drugs for this infection.

In endemic regions, visceral leishmaniasis is one of the most common opportunistic infections in HIV positive patients. Simultaneous infection with Leishmania and HIV has been reported in some countries but this is the first report of such a case in Iran. Our patient was a 27 years old man with intermittent night fever, abdominal pain, loss of appetite, vomiting, watery diarrhea and severe weight loss for 6 months. He had low socio-economic status with an imprisonment history. The patient was quite cachectic and had low grade fever. Physical exam and upper GI endoscopy revealed oropharyngeal candidiasis. Microscopic evaluation of duodenal biopsy material showed Leishmania amastigotes in macrophages of lamina propria. Leishman bodies were also observed in bone marrow aspiration specimen. Serologic tests were positive for Leishmania infantum. HIV antibody was also positive with a CD4+cell count of 80/μl. The diagnosis was acquired immunodeficiency syndrome with simultaneous visceral leishmaniasis involving intestinal mucosa.

Background: we assessed the prevalence, the related symptoms, and the endoscopic and histologic gastric features of celiac disease (CD) in patients with Helicobacter pylori (Hp). Methods: 450 dyspeptic patients were studied. Biopsies of gastric antrum and duodenum, CD serology, and total IgA were obtained. Histological findings were scored with the Marsh-Rostami criteria. Results: 411 (91.3%) patients were Hp positive. Duodenal histology was normal in 385 (85.6%) patients, 124 (27.5%) had duodenitis and 28 (6.2%) showed duodenal abnormalities (Marsh I-IIIc). Twenty three/28 (82.1%) patients with malabsorption pattern were also Hp positive. Serological analysis: 12 of 31 (38.7%) positive patients had abnormal histology (Marsh I,-IIIc). Nine out 450 patients were IgA deficient; none of them was serologically positive for CD. Conclusion: although a high prevalence of Hp infection was found in this study, the relationship between Hp infection and CD was similar to that reported in other geographic areas.

Background and Objective: some of predisposing factors for enterococci colonization are hospitalization in ICU, prolonged use of antibiotics and continued bed rest in hospital. In this study antibiotic resistance of enterococcus in hospitalized patients of four hospitals in Tehran were studied. Material and Methods:the Clinical samples were taken from patients admitted to the ICU, from September 2011 to April 2012. Enterococci isolates were confirmed by biochemical tests, and Enterococcus faecalis and Enterococcus species by species-specific ddl genes. The disk diffusion and micro agar dilution susceptibility tests were performed according to Clinical and Laboratory Standards Institute (CLSI). Results: of 41 isolates in ICUs, 22 (5.52%) were E. faecium and 19 (5.47%) were E. faecalis. Most of E. faecium was isolated from urine and E. faecalis from trachea specimens. The rate of resistance to vancomycin, ampicillin, gentamicin, chloramphenicol and nitrofurantoin in E. faecium isolates was more than that of E. faecalis and the rate of resistance to tetracycline, ciprofloxacin and erythromycin was the same in both of them. MIC50 in vancomycin and ampicillin resistant E. faecium isolates was greater than 256 microgram and the MIC50 in gentamicin resistant isolates was more than 1024 microgram. Conclusion: The presence of multi-resistant E. faecium strains in ICUs can be a serious warning for physicians and patients. [ABSTRACT FROM AUTHOR]

Background and Objective: The aim of this study was to detect bacteria associated with biliary tract diseases and relationship between their class 1, 2 and 3 integrons and antibiotic resistance patterns. Materials and Methods: Bile samples of biliary patients undergoing cholangiopancreatography were examined for the presence of bacteria and antibiotic resistance patterns using biochemical tests and disk diffusion method. PCR was used for detection of the presence of integrase genes intI1, intI2, and intI3 in total DNA and plasmid extracts of these bacterial isolates. Results: Out of 102 bile samples, 41.2% were positive by culture. The isolated bacteria belonged to E. coli (35.5%), Enterococcus spp. (19.4%), Klebsiella pneumoniae (17.74%), Pseudomonas aeruginosa (17.74%), Acinetobacter spp. (6.45%) and Staphylococcus epidermidis (1.6%). All isolates were resistant to amoxiclav and 87.5% of the isolates showed multidrug resistance (MDR) pattern. intI1and intI2 were found in E. coli (59.09%, 50%), P. aeruginosa (81.81%, 45.45%), K. pneumonia (63.6%, 27.27%) and Acinetobacter spp. (100%, 100 %), respectively. These results showed the presence of class 1 and 2 integrons on the extracted plasmids and indicated a significant association between class 1 integron and resistance to gentamicin and ciprofloxacin. Conclusion: High frequency of class 1 and 2 integrons among the bile isolates and their association with increased antimicrobial resistance phenotypes suggests bile components as selective agents for bacterial strains encoding these elements. CONTEXT: Familial adenomatous polyposis (FAP) is one type of hereditary colon cancer with a large number of precancerous polyps that initiation to growth in childhood and adolescent. Mutation in adenomatous polyposis coli (APC) gene is the cause of FAP. AIMS: The aim of the current study was to standardize multiplex ligation probe amplification (MLPA) method in screening of APC large deletions for the first time in Iranian patients with FAP. SUBJECTS AND METHODS: Deoxyribonucleic acid was extracted from 34 FAP patients by saluting out method. All patients were screened for APC large deletions whit MLPA and for the positive results, respective region was investigated by polymerase chain reaction sequencing. All genetic alterations were doubled checked in two separated rounds of MLPA. RESULTS: The detection rate of large fragment deletions in APC was 5.8% (2/34). Both of the Iranian patients had deletion in the middle and the end of exon 15, however, comparing of clinical features between patient with the large deletion and patients without deletion did not show any significant difference in each variable including, age at diagnosis, signs of disease and poly type. CONCLUSIONS: It seems that exon 15 of APC gene is probably the hotspot region in Iranian FAP patients. Association of genotype/phenotype is well known in FAP patients, but in this study statistical analyses did not show a significant difference in each considerable factor between patients with and without large deletions. Background: Food contact surfaces are a major concern for food services facilities in controlling the spread of food borne pathogens. This study was aimed to investigate roles of food utensils in transmission of the most important bacteria associated with nosocomial infections and hospital food borne diseases in one hospital in Tehran. Materials and methods: During the three independent sampling processes, samples of common used utensils were analyzed for bacterial contamination. Through a sterile swab the samples were collected in sterile tubes and after transferring, were immediately subjected to culture on gram negative specific and general media for bacteria. Total bacterial counts of each sample were determined and their characterizations were performed by biochemical diagnostic tests according to standards methods. Results: Among the studied cooking utensils' and food samples, the most contaminated samples were determined as samples from board and blender as 38% and 35%, respectively. The most bacteria isolated from these samples were related to species of Enterobacteriaceae, Staphylococcus aureus, Klebsiella pneumoniae and Bacillus species that were similar to the isolates from studied food samples. Conclusion: Results of this study, in addition to high contamination rates of medical foods and utensils in their contact showed that there are significant weaknesses in proper cooking and utensils hygiene conditions in preparation and distribution of medical food in the studied hospital's samples. Background and Objective: Vancomycin-resistant enterococci (VRE) are the major causative agents of urinary tract infections (UTIs) in hospitalized patients. The aim of this study was to determine the prevalence of urinary tract infections caused by enterococcus faecium and the level of resistance to vancomycine and other antibiotics which are effective in enterococcal infection therapy of hospitalized patients in four university teaching hospitals of Tehran. Materials and Methods: This study was carried out between September 2011 and May 2012. Urine samples were collected from hospitalized patients in Taleghani, Loghman , Mofid and Labaffi nejad Hospitals in Tehran. Enterococcus species were detected by biochemical and molecular tests. Antimicrobial susceptibility and minimum inhibitory concentration (MIC) of vancomycine were determined by disk diffusion and micro agar dilution methods. The presence of vanA and vanB genes were investigated in VRE strains by PCR. Results: 86 enterococci were isolated from urine samples of which 45(52%) were E.faecium. 42.2% of E. faecium isolates were resistant to vancomycin (VRE) and showed vanA genotype. All VRE isolates were resistant to ampicillin, gentamicin, ciprofloxacin and erythromycin, and 78% were resistant to nitrofurantoin. Furthermore, all VRE isolates were sensitive to linezolide and quiopristin-dalfopristin. MIC50 ≥ 128 and MIC90 ≥ 256 were detected in the VRE strains. Conclusion: The increase in the prevalence of vancomycin resistant E. faecium with high risk resistance profiles is a serious threat for some Iranian hospitals and limits the therapeutical options for patients infected with E. faecium. Background: A number of methods for detecting diversity in Entamoeba have been described over the years. In the present study the genetic polymorphism of noncoding locus A-L was analyzed using PCR and sequencing in order to clarify the genotypic differences among E. dispar isolates. Methods: A total of 28 E. dispar from patients with gastrointestinal symptoms were determined and the genomic DNA was extracted directly from stool. For genotype analysis; Locus A-L was amplified by PCR and PCR products were sequenced .The sequences obtained were edited manually and aligned using Gene Runner software. Results: With sequencing of PCR products a reliable genetic diversity in size, number and position of the repeat units were observed among the Iranian E. dispar isolates in locus A-L gene. Sequences showed variation in length from 448bp to 507bp and seven distinct types were identified. Conclusion: The genetic diversity of loci like A-L shows them to be suitable for epidemiological studies such as the characterization of the routes of transmission of these parasites in Iran. Background & Aims: Hepatitis E virus (HEV) is the second prevalent causative agent of acute hepatitis in adults after hepatitis B. Transmission routes of this infection is fecal-oral. There are limited studies on epidemiology of this virus in Iranian general population. In this study, we evaluated the prevalence of hepatitis E infection in general population of some towns of Tehran province and Eastern and Northern section of Tehran city. Materials & Methods: In this cross-sectional study conducted in 2006 and 2007 a number of 501 subjects selected by random cluster sampling from Tehran province were studied. Demographic data (through a questionnaire) and blood samples were collected and anti-HEV antibodies were measured by ELISA method. Chi-square test, logistic regression, and T-test were used for statistical analysis. Result: O f the total 493 subjects with mean age of 40.98± 17.10, 180 were men and the rest (313 subjects) were women. Of these numbers, 48 subjects (9.7%) were positive for anti-HEV antibody. There were no significant differences in HEV seropositivity and different sex or age groups. Conclusion: The prevalence in Tehran province was high and this indicates the endemic state of this infection in the studied population. Results are similar to those in eastern section of Golestan province and are in contradiction with results from Isfehan Province. [ABSTRACT FROM AUTHOR]

Background: Although, family history of cancer is an important risk factor for upper gastrointestinal cancers development, but limited information is available on the upper gastrointestinal cancers associated with family history in Iran. The purpose of this study was to define upper gastrointestinal cancers risk associated with family history of cancer. Methods: This study was conducted as a case control study. A total number of 1,010 cases of upper gastrointestinal cancer and 1,010 healthy controls were recruited. For family history of cancer, questions were asked about any malignant tumor in first and second degree relatives. Adjusted odds ratio estimates for the association family history and upper gastrointestinal cancers risk and corresponding 95% confidence intervals were obtained. Results: A family history of any malignant tumor in relatives was associated with 1.3 fold increased risks of upper gastrointestinal cancers. A first-degree family history of esophageal and gastric cancer was significantly associated with upper gastrointestinal cancers development, with an adjusted OR of 4.7(CI 95%: 2.6-8.4). Conclusion: Our findings suggested that risk for upper gastrointestinal cancers increases among individuals with family history of cancer. Therefore, appropriate screening strategies especially in relatives of patients should be considered to prevent and control of disease. [ABSTRACT FROM AUTHOR]

Background and Objective: Several strains of the Echinococcus granulosus have been described based on morphological characters, intermediate host specificity and/or genetic analysis of mitochondrial and nuclear DNA. The aim of this study was to characterize different E.granulosus isolates by using sequences of mitochondrial atp6 gene. Materials and Methods: In this study, Sixty infected liver and lungs of cattle, sheep and goats were collected from the abattoir of Varamin city-Iran during 2008. Protoscoleces were removed from each fertile cyst and DNA extracted. New and specific primers were designed for two existing genotypes (G1 and G6) of E. granulosus known to occur in Iran and applied in PCR reactions. Results: The new primers selectively amplified the G1 and G6 genotypes of E. granulosus with specific bands of 708 and 705 bp respectively. The G1 genotype was identified in all fertile cyst samples. Conclusion: This study showed that the new primer pairs which specifically amplify portions of the mitochondrial atp6 gene of the G1 and G6 strains of Echinococcus granulosus are proper molecular marker for investigating genetic variation in a number of isolates of E. granulosus from a range of hosts (sheep, goats, cattle) in Iran.
