Abstract

PurposeThere is an unmet need for identifying novel biomarkers in Barrett's esophagus that could stratify patients with regards to neoplastic progression. We investigate the expression patterns of extracellular matrix (ECM) molecules in Barrett's esophagus and Barrett's esophagus-related neoplasia, and assess their value as biomarkers for the diagnosis of Barrett's esophagus-related neoplasia and to predict neoplastic progression.Experimental designGene-expression analyses of ECM matrisome gene sets were performed using publicly available data on human Barrett's esophagus, Barrett's esophagus-related dysplasia, esophageal adenocarcinoma (ADCA) and normal esophagus. Immunohistochemical expression of basement membrane (BM) marker agrin (AGRN) and p53 was analyzed in biopsies of Barrett's esophagus-related neoplasia from 321 patients in three independent cohorts.ResultsDifferential gene-expression analysis revealed significant enrichment of ECM matrisome gene sets in dysplastic Barrett's esophagus and ADCA compared with controls. Loss of BM AGRN expression was observed in both Barrett's esophagus-related dysplasia and ADCA. The mean AGRN loss in Barrett's esophagus glands was significantly higher in Barrett's esophagus-related dysplasia and ADCA compared with non-dysplastic Barrett's esophagus (NDBE; P < 0.001; specificity = 82.2% and sensitivity = 96.4%). Loss of AGRN was significantly higher in NDBE samples from progressors compared with non-progressors (P < 0.001) and identified patients who progressed to advanced neoplasia with a specificity of 80.2% and sensitivity of 54.8%. Moreover, the combination of AGRN loss and abnormal p53 staining identified progression to Barrett's esophagus-related advanced neoplasia with a specificity and sensitivity of 86.5% and 58.7%.ConclusionsWe highlight ECM changes during Barrett's esophagus progression to neoplasia. BM AGRN loss is a novel diagnostic biomarker that can identify patients with NDBE at increased risk of developing advanced neoplasia.

Abstract

Colorectal cancer (CRC) is a biologically heterogeneous disease. To characterize its mutational profile, we conduct targeted sequencing of 205 genes for 2,105 CRC cases with survival data. Our data shows several findings in addition to enhancing the existing knowledge of CRC. We identify PRKCI, SPZ1, MUTYH, MAP2K4, FETUB, and TGFBR2 as additional genes significantly mutated in CRC. We find that among hypermutated tumors, an increased mutation burden is associated with improved CRC-specific survival (HR = 0.42, 95% CI: 0.21-0.82). Mutations in TP53 are associated with poorer CRC-specific survival, which is most pronounced in cases carrying TP53 mutations with predicted 0% transcriptional activity (HR = 1.53, 95% CI: 1.21-1.94). Furthermore, we observe differences in mutational frequency of several genes and pathways by tumor location, stage, and sex. Overall, this large study provides deep insights into somatic mutations in CRC, and their potential relationships with survival and tumor features.

22. Curriculum Q-Learning for Visual Vocabulary Acquisition

Abstract

The structure of curriculum plays a vital role in our learning process, both as children and adults. Presenting material in ascending order of difficulty that also exploits prior knowledge can have a significant impact on the rate of learning. However, the notion of difficulty and prior knowledge differs from person to person. Motivated by the need for a personalised curriculum, we present a novel method of curriculum learning for vocabulary words in the form of visual prompts. We employ a reinforcement learning model grounded in pedagogical theories that emulates the actions of a tutor. We simulate three students with different levels of vocabulary knowledge in order to evaluate the how well our model adapts to the environment. The results of the simulation reveal that through interaction, the model is able to identify the areas of weakness, as well as push students to the edge of their ZPD. We hypothesise that these methods can also be effective in training agents to learn language representations in a simulated environment where it has previously been shown that order of words and prior knowledge play an important role in the efficacy of language learning., Comment: Accepted at Visually Grounded Interaction and Language Workshop (NIPS 2017)

Abstract

To understand the genetic drivers of immune recognition and evasion in colorectal cancer, we analyzed 1,211 colorectal cancer primary tumor samples, including 179 classified as microsatellite instability-high (MSI-high). This set includes The Cancer Genome Atlas colorectal cancer cohort of 592 samples, completed and analyzed here. MSI-high, a hypermutated, immunogenic subtype of colorectal cancer, had a high rate of significantly mutated genes in important immune-modulating pathways and in the antigen presentation machinery, including biallelic losses of B2M and HLA genes due to copy-number alterations and copy-neutral loss of heterozygosity. WNT/β-catenin signaling genes were significantly mutated in all colorectal cancer subtypes, and activated WNT/β-catenin signaling was correlated with the absence of T-cell infiltration. This large-scale genomic analysis of colorectal cancer demonstrates that MSI-high cases frequently undergo an immunoediting process that provides them with genetic events allowing immune escape despite high mutational load and frequent lymphocytic infiltration and, furthermore, that colorectal cancer tumors have genetic and methylation events associated with activated WNT signaling and T-cell exclusion.Significance: This multi-omic analysis of 1,211 colorectal cancer primary tumors reveals that it should be possible to better monitor resistance in the 15% of cases that respond to immune blockade therapy and also to use WNT signaling inhibitors to reverse immune exclusion in the 85% of cases that currently do not. 41. Abdominal Wall Hernias in Patients with Hiatal Hernias: Predictors of Association in a Large Cohort of Patients

Abstract

Introduction: Hiatal hernia and abdominal wall hernia (AWH) have a common theme of pathogenesis and etiology. This suggests a potential relationship between these 2 types of hernia. However, no large studies have examined such an association. We designed the current study to characterize this association and identify factors associated with this coexistence.Methods: A database of patients seen at a large multi-center health network serving 3 states in the mid-Atlantic region of the United States was queried for patients with a documented diagnosis of hiatal hernia (HH) or paraesophageal hernia (PEH). This population was then divided into groups based on whether or not they had a history of another type of AWH (ventral, inguinal/ femoral, umbilical, or incisional hernia). Demographic and clinical data were compared between groups using univariate, followed by multivariable logistic analysis.Results: The final study population consisted of 28 114 patients (66.5% female) with HH/PEH. The prevalence of AWH in this population was 8.2% (n = 2299). Of these 69.1% had only one AWH with the remaining 30.9% having two or more hernias. Multivariable analysis showed male patients with HH/PEH were more likely to have AWH (OR: 1.97; 95% CI: 1.810-2.162, P<0 .0001). The odds of AWH were higher in patients aged 50 to 80 years old compared to younger patients (OR: 1.527; 95% CI: 1.348-1.730, P<0 .0001). Patients with any comorbidity were also more likely to develop AWH (OR: 1.764; 95% CI: 1.566-1.986, P< 0.0001), with obstructive sleep apnea (OSA) being the comorbidity with the highest risk (OR: 1.295; 95% CI: 1.161-1.444, P<0 .0001), followed by obesity (OR: 1.267; 95% CI: 1.156-1.388, P<0 .0001).Conclusion: The prevalence of an AWH in patients with a HH/PEH was 8.2%. Although HH/PEH are more prevalent in females, males with HH/PEH are at higher risk for AWH. Obesity, OSA, tobacco use, hypertension, diabetes, and hypothyroidism are independent risk factors for presence of an AWH in patients with HH/PEH.

46. Synthesis and characterization of novel thin films derived from pyrazole-3-one and its metal complex with bivalent nickel ion to improve solar cell efficiency

Abstract

In this work, an organic ligand and its metal complex with Ni (II) ion was synthesized and used to improve solar cell efficiency. The organic Schiff base ligand, 4-(1, 3-benzodioxol-5-ylmethylideneamino)-1, 5-dimethyl-2-phenylpyrazole-3-one [L], was produced by condensation reaction of 4-aminoantipyrine with piperonaldehyde in absolute ethanol. The synthesized organic ligand [L] was characterized using 1H and 13C NMR, FT-IR, UV–Vis, elemental analysis (C.H.N), X-ray diffraction (XRD), mass spectrum, and atomic force microscope (AFM). The Ni (II) complex [M] was obtained in a molar ratio (2:1) (L:M). The prepared complex was characterized using X-ray diffraction (XRD), magnetic susceptibility measurement, elemental analysis (C.H.N), electrical molar conductivity, Fourier-transform infrared FT-IR, UV–Vis spectroscopy, atomic force microscope (AFM) and flame atomic absorption technique. Results suggested an octahedral geometry around the bivalent Ni (II) ion. Drop casting method was employed to prepare thin films of nanoparticles of the synthesized compounds. The optical, morphological, and structural properties for those particles were also studied and they were used to improve the silicon solar cell after the depositing on the silicon slides. The efficiency of the resulted inorganic solar cell was higher than the organic solar cell.
