Abstract

The journal of research on history of medicine (RHM) starts to be published from Feb. 2012. It passed its first decade of life by publishing current issue. By analyzing these 10 volumes, it shows that there are 284 articles that are written by 342 authors from 18 countries. These authors are affiliated to 90 institutions, that Shiraz University of medical sciences has the most contribution among them. In addition, the share of Iranian authors is more than other countries significantly. The journal is indexed in 14 indexing and abstracting databases. It needs to set a program for improving the quality of international cooperation, indexing of the journal and published papers for next decade.

2. Clinical Signs, Maternal and Neonatal Outcomes Associated with Cerebral Vein Thrombosis during Pregnancy and Postpartum period in Mashhad Ghaem Hospital, 2011-2016

Abstract

Introduction: Cerebral vein thrombosis is a rare cerebrovascular disorder. Pregnancy and postpartum are associated with an increased risk of cerebral vein thrombosis due to hypercoagolopathy state. With regard to serious morbidity and mortality and the importance of pregnancy and postpartum period, special attention should be given to this disorder in this period. This study was performed with aim to determine the clinical signs and maternal and neonatal outcomes of cerebral vein thrombosis during pregnancy and postpartum period. Methods: This retrospective study was performed on 30 women with cerebral vein thrombosis during pregnancy and postpartum period in Mashhad Ghaem Hospital from 2011-2016. Data collection tool was a researcher-made questionnaire. Demographic data, initial clinical symptoms, affected sinus, infarct and hemorrhage, and maternal and neonatal mortality were collected. Data were analyzed by SPSS software (version 16) and paired t-test. P < 0.5 was considered statistically significant. Results: Cerebral vein thrombosis was diagnosed in 13 women (43.3%) during pregnancy and in 17 (56.7%) during postpartum period. Primary manifestation in more than half of patients was headache (57.7%). The upper sagittal sinus was the most common (70%) and the right sigmoid sinus was the rarest involved sinus (16.6%). Among patients with cerebral vein thrombosis, 4 patients and one fetus died that all 4 patients had hemorrhagic infraction. Conclusion: Considering the importance of pregnancy and postpartum period, rapid diagnosis and timely treatment of cerebral vein thrombosis, especially pay attention to the symptom of headache, is important to improve the prognosis.

Abstract

The study assessed the HIV treatment cascade of people living with HIV (PLWH) in Iran and the reasons for gaps in HIV services in 2014. We did the cascade analysis using the WHO HIV test–treat–retain cascade analysis tool (2014). To measure the gaps, we checked the records for all PLWH reported to the national HIV surveillance till September 2014 to define how many were alive, linked to HIV care, assessed for CD4 cell count, started antiretroviral therapy (ART), and reported to be alive up to five years after ART. To assess the reasons for gaps in the cascade, we ran desk review, four focus group discussions and 51 in-depth interviews with key informants including PLWH. Spectrum model estimated 75,700 PLWH in Iran, of whom only 22,647 people (%30) were diagnosed. Of those who were diagnosed with HIV, 16,266 people (%72) had enrolled in HIV care. About 13% of all PLWH were retained in HIV care and only 9% (6745 people) had ever been tested for CD4 cell count. Only 8% (6018 people) of all PLWH had received ART by September 2014. We found the biggest gap in the HIV treatment cascade to be HIV diagnosis as only 30% of PLWH have been diagnosed. Innovative strategies to improve HIV testing, particularly those that work best for key populations, are critical to reach the 90–90-90 targets.

Abstract

The cause-specific annual death rate due to traffic injuries is around 30 in 100,000 in Iran. On the other hand, this country has the highest proportion of opiate users in the world. Little is known about the transient effect of opium on traffic injuries. The objective of this study was to explore the effect of opium consumption on traffic injuries in drivers who use opium.Seventy-five regular opium users who suffered traffic injuries were studied in a case-crossover investigation. The study subjects had been admitted to the single trauma emergency department in Kerman, a city in southeast Iran. The relative risk (RR) of short-term opium effect was estimated by considering frequency of driving after opium consumption during 6 hours before the accident in comparison to the usual frequency of driving after opium consumption by the same persons. Stratified data analysis was performed by the Mantel-Haenszel method.The opium consumption of drivers up to 6 hours before the accident was associated with an increased RR = 3.2, 95 percent confidence interval (CI): 1.9, 5.4. The third hour after consumption had the greatest magnitude of effect considering RR = 4.29, 95 percent CI:2.65, 6.95.These results suggest a heightened risk of traffic injuries after opium consumption in regular users. The RR in the third hour after consumption could be explained by considering the greater probability of driving compared to the immediate hours after use, rather than peak effect time of opiates. The results indicate necessity of regular assessment of all common drivers, especially truck and bus drivers, regarding use of opium.
