4. The association between healthcare staff personal branding and patients’ perceived service quality: An evidence based research of the healthcare sector in Pakistan

Personal branding has become an important notion in health care management literature in the current era. The healthcare staff should be recognized as members of the team who provide quality care to patients. The present research examines the impact of personal branding of radiologists on perceived service quality placing particular emphasis on the health care sector in Pakistan. A convenience sampling method was used. The variables of personal branding are communication, behaviour and appearance and the variables of Perceived Service quality are responsiveness and reliability. The results of the study indicate that Communication is significantly correlated to responsiveness and reliability. Behaviour was significantly correlated to responsiveness while appearance significantly correlated to reliability. The finding of the study affirms the significance of personal branding as one of the crucial components in improving Perceived Service quality by health care units.

One of the main issue to extensive concept analysis from the perspective of healthcare sector is the domain's rapid-changing panorama. The concept of transformational leadership in terms of health managers is a new insight that has been highlighted in the study. In this current study, a sample of 261 individuals employed in managerial roles within the hospitals was included. Results show that there is a positive correlation between the sub variables of transformational leadership with organizational commitment and its dimensions. The results also indicate that the emotional Intelligence have a significant moderator effect on the association between the dimensions of transformational leadership and organizational commitment in the context of Pakistani health sector. Moreover, the results also indicate that the study also found that perceived benefits of training has a significant mediating role in the relationship between the dimensions of transformational leadership and organizational commitment. This shows that the training programs may produce more positive results when health managers understand the training benefits, which is likely to boost their commitment to the hospitals. Hence, it is suggested that hospitals should arrange training sessions timely to improve transformational leadership competencies of health managers. The research is the effective contribution to the contemporary literature on the role of transformational leadership in healthcare from the developing country prospect. [ABSTRACT FROM AUTHOR]

9. The Effect on Histo-morphology of Lungs in Fluconazole and Vitamin E Treated Sprague Dawley Albino Rats with Septic Shock

Fluconazole is a subclass of triazole antifungal agents, is available as oral tablets, powder or for oral suspension, and as a sterile solution for intravenous administration. Fluconazole is designated chemically as 2,4-difluoro-α,α1 -bis benzyl alcohol in the form of white crystalline solid which is slightly soluble in water and saline. Fluconazole oral suspension contains 350 mg or 1400 mg contains sucrose, citric acid anhydrous, sodium benzoate, sodium citrate, colloidal, silicon dioxide, titanium dioxide, xanthan gum, and natural orange flavor. After reconstitution with 24 ml of distilled water or purified water each ml of reconstituted suspension contains 10 mg or 40 mg of fluconazole. Animals are divided into 4 groups. The rats of group A were taken as control group. In group B rats are kept nil per orally at 12 hours prior to induction of septic shock produce by Candida albicans s. Group C animals are administrated with fluconazole at doses of 3mg/kg, 10mg/kg and 30mg/kg after the septic shock produce by Candida albicans s. Since fluconazole at a higher dose had clearly shown to decrease tissue injury. We decided to keep fluconazole dose as constant and vitamin E would be administered in dose of 10mg/100gm of body weight in group D. Both the drugs are administrated after inducing septic shock. After the study, animals sacrificed and lungs obtained within 8 hours of septic shock. Lungs are taken out from sacrificed animals and then fixed with paraffin and then sectioned and stained by eosin and haemotoxylin. With the help of cardiac puncture blood samples were taken to estimate the infectious markers. Study of CBC and CRP was done. The levels were estimated and compared with control and correlated with histologic findings. The lung histology is determined by mucosal thickening and inflammatory cells. All these results were in favor of group D fluconazole along with vitamin E that shows significant results in all parameters. There was significant corre- lation between control and fluconazole along with vitamin E treated group because vitamin E reduces the histopathological changes in lungs parenchyma. It is concluded from the study that vitamin E when treated with combination of fluconazole it decreases the lung tissue injury and oxidative stress.

A Transcranial Doppler (TCD) is an inexpensive noninvasive ultrasonography technique that helps provide a rapid real time measure of blood flow from the basal intracerebral vessels, which may be used for the diagnosis and follow-up of cerebrovascular disease. By placing the ultrasound probe on the scalp; it utilizes low frequency soundwaves to record cerebral blood flow velocity, and its change in multiple conditions. Technology offers several diagnostic tests available in the evaluation and treatment of cerebrovascular diseases (CVD). Transcranial ultrasonography may represent a valuable tool for patients with CVD in neurocritical care unit. However due to geographic, financial or patient tolerance of procedures, physicians may be limited to the tools they can utilize.

12. EFFICACY OF METHOTREXATE VERSUS AZATHIOPRINE IN THE TREATMENT OF CHRONIC ACTINIC DERMATITIS: A RANDOMIZED CONTROL TRIAL.

Background: Azathioprine is first line immunosuppressive agent in treatment of chronic actinic dermatitis. The role of methotrexate has been effective in different dermatosis and it seems reasonable to use it in the treatment of chronic actinic dermatitis. Aims: We sought to compare the efficacy of methotrexate versus azathioprine in treatment of chronic actinic dermatitis. Methods: Patients with chronic actinic dermatitis were randomized to receive methotrexate in group A and azathioprine in group B. The response to treatment in terms of percentage PASI reduction and side effects of medications were assessed 12 weeks follow-up. Results: In group A, the percentage PASI reduction was <25% in 2 (1.19%) patients, 25-49% in 47 (27.9%) patients, 50-74% was achieved by 35 (20.8%) patients while in group B, the percentage PASI reduction of 25% was achieved by 2 (1.19%) patients, 25-49% in 45 (26.7%) patients, 50-74% in 37 (22.0%) patients. More than or equal to 75 percentage PASI reduction was not achieved by any patient in the study. Both drugs were found efficacious in treatment of CAD. A total of 23 (27.38%) patients in group A and 22 (26.19%) patients in group B showed derangement in laboratory investigations during 12 weeks treatment. The limitation of study was inability to do photo-patch test, so patients were diagnosed clinically and biopsy was done in clinically challenging cases. Conclusion: This study shows that methotrexate is equally effective as azathioprine in the treatment of chronic actinic dermatitis with its added benefits of being cost effective and better safety profile. The Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) first broke out in Wuhan (China) and subsequently spread worldwide. Coronaviruses (CoVs) primarily cause zoonotic infections in birds and mammals however, in the last few decades have shown to be capable of infecting humans as well. The outbreak of severe acute respiratory syndrome (SARS) in 2003 and more recently, Middle-East respiratory syndrome, (MERS) has demonstrated the lethality of CoVs when they cross the species barrier and infect humans. Coronavirus (CoV) is a large family of viruses that cause afflictions ranging from the common cold to more severe pathologies such as Middle East Respiratory Syndrome (MERS-CoV) and Severe Acute Respiratory Syndrome (SARS-CoV). A novel coronavirus (nCoV) is a new strain that has now been identified in humans. The recognition of a new coronavirus identified in December 2019, named CoVID-19 are common for coronavirus researchers. Detailed investigations found that SARS Coronavirus-2 was initially transmitted from civets to humans and MERS-CoV from dromedary camels to humans. Advances in biology have resulted in a greater understanding of coronavirus, including them to adapt to new environments, trans-species infection and the emergence of new subtypes. New tools of cell and molecular biology have led to an increased understanding of intracellular replication and viral cell biology. Along with the advent of reverse genetic approaches in the past five years; it is now possible to begin to define the determinants of viral replication, trans-species adaptation, and human disease. The most progress has been made on SARS-CoV 2, highlighting specific structural requirements for its functions in the CoV life cycle as well as mechanisms behind its pathogenesis. In this review, we will provide a through insight to the life cycle of CoV, its genetics, replication process and reverse genetic applications to SCoV along with advances in its research. This review aims to establish the current knowledge on CoV-2 by highlighting the recent progress that has been made and comparing it to previous knowledge. We also conclude with a brief discussion on practices to decrease risk factors for transmission and treatment options., İlk olarak Wuhan'da (Çin) patlak veren Şiddetli Akut Solunum Sendromu Koronavirüs 2 (SARS-CoV-2), sonrasında dünyaya yayılmıştır. - Abstract
OBJECTIVES: To determine the frequency of Xerosis in children presenting with chronic pruritus to Hayatabad Medical Complex Peshawar, Khyber Pakhtunkhwa, Pakistan and compare it with national and international data. METHODOLOGY: This was a cross sectional study, carried out in the Department of Dermatology, Hayat Abad MedicalComplex Peshawar from 24th May 2020 till 24th November 2020. All patients of either gender with age range up to 13 years, presenting with chronic pruritus were enrolled for this study.All the findings were recorded on a preformed proforma and results were compiled and tabulated. RESULTS: Out of 136 patients presenting with chronic pruritus, 75 were male (55.1%) and 61 (44.9%) were female. 85 (62.5%) patients were having dry skin (Xerosis) while 37.5% were having normal skin. CONCLUSION: Xerosis is a common and under diagnosed skin condition commonly associated with chronic pruritus and screening all children for Xerosis is simple measures to improve quality of life of affected children.

Introduction: University academicians face stress and their profession is burdensome and this job stress has catastrophic effect on their performance. High level of Emotional Intelligence maintains positive emotions which enhances confidence to accept and face difficult situations. Emotional intelligence works as a buffer in stress and individual can easily make out the situation. Thus the present study aims to explore the soak up effect of emotional intelligence in job stress in terms of self appraisal and others, regulation and utilization of emotions in the academicians of Pakistan and Turkey.Method: The data was collected from 18 universities of Pakistan and Turkey through online distribution of Google survey form via email correspondence. The Quantitative approach was adapted with the cross sectional study design. The data was gathered through self-administered questionnaire. The first section of questionnaire consisted of demographic while the remaining part of questionnaire was adopted from two valid and reliable questionnaires (Schutte et al., EI questionnaire and Job stress questionnaire), being analyzed by Smart PLS software using construct reliability, construct validity i.e. convergent and Discriminate validity and Structural model assessment Results & Conclusions: 261 academicians filled out the questionnaires. Results showed an inverse correlation among the factors of EI: AP, RE and Utilization scores with the level of Job Stress(r=-0.235, p=0.005).Appraisal has negative relation with job stress AP (p < 0.01 and t >2.67 at significant level of 0.05 ).RE ( P value 2.67) and Utilization( P value 1.96) supported the hypotheses that all facets of emotional intelligence reduce the job stress and have significant relation to absorb job stress .So, it is recommended that EI short-term training courses should be designed and scheduled in the universities.

SCARS ,ACNE ,TREATMENT effectiveness ,WOMEN patients ,INFORMED consent (Medical law) - Abstract
Objectives: To compare the effectiveness and safety of Dermaroller with TCA CROSS technique for the management of post acne scars. Material and methods: This comparative study was conducted in Dermatology unit, Lady Reading Hospital Peshawar, Pakistan from April 2019 to December 2019. A total of 98 patients with post acne scars fulfilling the inclusion criteria were included in the study and were divided into two groups with 49 patients in each. Informed consent was taken. Patients of group 1 underwent four session of Dermaroller therapy while patients of group 2 were treated with 100% CROSS TCA with each session four weeks apart. Results: A total of 98 patients were included in the study. Mean age was 29.55 year ±5.0 SD. There were 30 (30.6%) male and 68 (69.4%) female patients. Forty two (42.9%) patients had ice pick scars, 38(38.8%) had box scar and 18(18.4%) patients had rolling type of acne scars. Efficacy of treatment in Group 1 was excellent in 15(30.6%) patients, 18 (36.7%) patients showed good response, 9(18.3%) patients showed fair and 7 (14.28%) patients showed poor response. While in group 2, 15(30.6%) patients showed excellent response, 16(32.65%) patients showed good, 13(26.5%) patients showed fair and 5(10.2%) patients showed poor response to treatment. There was no statistically significant difference found in the efficacy of treatment in both groups (p-value of 0.758). Conclusion: Both the techniques i.e CROSS TCA and Dermaroller are effective and comparable in the treatment of post acne scars. [ABSTRACT FROM AUTHOR]

Objective: To determine the relationship of serum ferritin level with telogen effluvium (TE) in female patients. Material and Methods: This case control study was conducted at Dermatology unit, Khyber Teaching Hospital, Peshawar- Pakistan, from June 2017 to May 2018. A total of 100 female individuals who visited outpatient department (OPD) were included in the study. They were divided in two groups, 50 were patients and 50 were healthy individuals. After taking informed consent and fulfilling the inclusion criteria, individuals were enrolled in study and serum ferritin level was checked once at first visit. Results: One hundred female individuals were studied. They were divided in two equal groups, 50 were in patients group (cases) and 50 were healthy individuals (controls). The mean age was 26.26±7.61years in patients group while 31.88±6.62 years in healthy controls. The mean serum ferritin level was 47.54±19.85 ng/ml among patients with telogen effluvium as in comparison to72.13±5.40 ng/ml in healthy controls which shows statistically remarkable difference among the two groups (p=0.00). Mean hemoglobin level was 10.53 g/dl among patient group while 12.55 g/dl in healthy individuals. Again statistically result was very significant with (p=0.0001). MCV in patient group was 47.36±10.67 and healthy control was 83.38±5.80. Conclusion: This study clearly highlights that low serum ferritin level is associated with telogen effluvium. - Abstract
Environmental quality indicators are crucial for responsive and cost-effective policies.The objective of the study is to examine the relationship between environmental quality indicators and financial development in Malaysia. For this purpose, the number of environmental quality indicators has been used, i.e., air pollution measured by carbon dioxide emissions, population density per square kilometer of land area, agricultural production measured by cereal production and livestock production, and energy resources considered by energy use and fossil fuel energy consumption, which placed an impact on the financial development of the country.The study used four main financial indicators, i.e., broad money supply (M2), domestic credit provided by the financial sector (DCFS), domestic credit to the private sector (DCPC), and inflation (CPI), which each financial indicator separately estimated with the environmental quality indicators, over a period of 1975–2013. The study used the generalized method of moments (GMM) technique to minimize the simultaneity from the model. The results show that carbon dioxide emissions exert the positive correlation with the M2, DCFC, and DCPC, while there is a negative correlation with the CPI.However, these results have been evaporated from the GMM estimates, where carbon emissions have no significant relationship with any of the four financial indicators in Malaysia. The GMM results show that population density has a negative relationship with the all four financial indicators; however, in case of M2, this relationship is insignificant to explain their result. Cereal production has a positive relationship with the DCPC, while there is a negative relationship with the CPI.Livestock production exerts the positive relationship with the all four financial indicators; however, this relationship with the CPI has a more elastic relationship, while the remaining relationship is less elastic with the three financial indicators in a country.Energy

Objective: To assess the efficacy and safety of 0.1% tazarotene gel in treatment of mild to moderate acne vulgaris. Methodology: This randomized controlled trial was conducted in Dermatology unit, MTI Lady Reading Hospital (LRH), Peshawar from June to December 2019. Total 90 patients with mild to moderate acne vulgaris were included in the study. Patients were divided into three groups by lottery method. Group A patients received topical 0.1% tazarotene gel in morning and evening application. Group B patients received topical vehicle (plain gel) in morning and 0.1% tazarotene gel in evening while group C received vehicle (plain gel) in both morning and evening. Data was recorded in pre-designed proforma. The one-way analysis of variance (ANOVA) was used to determine statistical significance. Results: In group A, 19 patients showed 61-95 % decrease while 11 patients showed complete clearance of lesions after treatment with tazarotene gel for 12 weeks. In group B, 17 patients showed 61-95% decrease, 8 patients showed 41-60% decrease while 5 patients showed complete clearance of acne lesions. In group C, 22 patients showed less than 10% while 8 patients showed 10-20% decrease in the number of lesions. The difference was found significant with p-value of 0.000 between three groups in terms of reduction in the number of lesions at 12 weeks of treatment. Erythema, burning, peeling and itching were the main side effects Conclusion: Short term topical application of 0.1% tazarotene gel is safe and effective for the treatment of acne vulgaris. - Abstract
Objective: To compare the effectiveness of calcipotriol versus betamethasone dipropionate in the treatment of mild to moderate chronic plaque psoriasis. Methodology: A total of 112 patients with mild to moderate psoriasis were enrolled in this randomized controlled trial. These patients were randomly allocated into two groups (A and B) by lottery method. Betamethasone dipropionate was given to group A, while calcipotriol was given to group B. Response to treatment was assessed on the basis of 50% reduction in psoriasis area and severity index (PASI) score after 4 weeks of topical treatment. SPSS version 21 for Windows was used for data entry and analysis. Results: In group A, Betamethasosne was effective in 44 patients (78.57%), while it was not effective in 12 patients (21.43%). In group B, calcipotriol was effective in 41 patients (73.21%). while it was not effective in 15 (26.79%). The P value was 0.659 which was statistically not significant. Conclusion: Both betamethasone dipropionate and calcipotriol were found effective in mild to moderate forms of chronic plaque psoriasis. Background: Covid-19 emerged as a global pandemic and its catastrophic effects have been felt throughout the world. The pandemic has caused a serious impact on mental health and loneliness of the people. The aim of the present research was to analyze the impact of the Covid-19 on the psychological health of residents of Karachi during the second lockdown. Method: It was a cross-sectional study conducted during the second wave of corona virus via an online survey, in which 380 persons participated. The survey consists of questions for the assessment of symptoms of depression, anxiety, stress by using DAAS-21 scale and for the questions related to loneliness, the UCLA-3 Item Loneliness Scale was used. Participants were selected by convenient and snow ball sampling method. Results: The results revealed that the psychological health was affected by the lockdown. It is also highlighted that women experienced more stress, anxiety, and depression than men and there is also significant difference between men and women in terms of loneliness. Conclusion: During the second lock down in Karachi the residents suffered from moderate level of depression, severe level of stress and mild level of anxiety. It is also evident that men have experienced less psychological impact of COVID 19 pandemic and with low level of loneliness than women. The outbreak of this pandemic indicates the need to pay greater attention to this issue to prevent and reduce the psychological outcomes of the pandemic.

Objective: To compare the efficacy of combination of Intense Pulse Light and topical eflornithine therapy versus Intense Pulse Light therapy alone in the treatment of idiopathic facial hirsutism., Methodology: A total of 78 patients were included in the study, and were divided into two groups, having 39 patients each. Patients in group A were subjected to combined treatment i.e., topical eflornithine to be applied twice a day and IPL session to be received by patients once a month. Patients in group B were subjected to receive IPL monthly sessions alone. Treatment was continued for 6 months in both groups. After completion of treatment, average number of terminal hair and percentage hair reduction were calculated to see the efficacy., Results: Out of 78 patients, 39 patients were included in group A, and similar number of patients were included in group B. The age of patients ranged from 22-42 years with mean age of 29.59±5.29. Maximum number of patients were in their third decade of life. Mean age in group A was 29.23±5.44, while in group B it was 29.95±5.19. Efficacy in group A was 39(100%) with mean percentage hair reduction of 90.44%, while efficacy in group B was 19 (48.7%) with mean percentage hair reduction of 59.23%, with statistically significant p-value of 0.00., Conclusions: The combination of intense pulse light and topical eflornithine therapy is more efficacious than intense pulse light therapy alone in the treatment of idiopathic facial hirsutism.
