Abstract

In this work we present a deduction of the Saint-Venant-Exner model through an asymptotic analysis of the Navier-Stokes equations. A multi-scale analysis is performed in order to take into account that the velocity of the sediment layer is smaller than the one of the fluid layer. This leads us to consider a shallow water type system for the fluid layer and a lubrication Reynolds equation for the sediment one. This deduction provides some improvements with respect to the classical Saint-Venant-Exner model: (i) the deduced model has an associated energy. Moreover, it allows us to explain why classical models do not have an associated energy and how to modify them in order to recover a model with this property. (ii) The model incorporates naturally a necessary modification that must be taken into account in order to be applied to arbitrarily sloping beds. Furthermore, we show that this modification is different of the ones considered classically, and that it coincides with a classical one only if the solution has a constant free surface. (iii) The deduced solid transport discharge naturally depends on the thickness of the moving sediment layer, what allows to ensure sediment mass conservation. Moreover, we include a simplified version of the model for the case of quasi-stationary regimes. Some of these simplified models correspond to the generalization of classical ones such as Meyer-Peter$\&$M\"uller and Ashida-Michiue models. Three numerical tests are presented to study the evolution of a dune for several definition of the repose angle, to see the influence of the proposed definition of the effective shear stress in comparison with the classical one, and by comparing with experimental data., Comment: 44 pages, sumbitted to Advances in Water Resources 17 july 2014

Abstract

The aim of this work is to prove the existence of mild solutions for some nondensely nonautonomous partial functional differential equations with state-dependent delay in Banach spaces. We assume that the linear part is not necessarily densely defined and generates an evolution family. Our approach is based on a nonlinear alternative of Leray–Schauder type and nonautonomous evolution family with nondensely domain. We propose a reaction diffusion equation with state-dependent delay to illustrate the obtained result.

Abstract

l’infarctus du myocarde (idM) constitue une urgence cardiologique absolue. Peu d’études ont été réalisées sur les complications précoces de cette affection au Burkina Faso. le but de ce travail était de décrire les aspects épidémiologiques, et évolutifs des complications aiguës de l’infarctus du myocarde dans le service de cardiologie du Chu Yo et d’entrevoir les implications en santé publique. il s’est agi d’une étude rétrospective sur une période de cinq (05) ans allant du 1er Janvier 2010 au 31 décembre 2014, portant sur les dossiers médicaux des patients hospitalisés dans le service pour syndrome coronarien aigu confirmé par l’eCg de surface et le dosage de la troponine sérique (SCa ST+ Troponine+). durant cette période, 2 469 patients ont été hospitalisés dans le service dont 102 pour infarctus du myocarde ST+, soit une prévalence hospitalière de 4,1 %. Quatre et vingt (80) patients étaient de sexe masculin contre 22 de sexe féminin, soit un sexe ratio homme/Femme de 3,6. l’âge moyen de nos patients était de 57 ans avec des extrêmes de 21 et 90 ans. l’hTa et l’obésité étaient les facteurs de risque modifiables les plus rencontrés avec respectivement 57,7 % (n= 58) et 29,7 % (n = 21). la grande majorité de nos patients (74,8 % (n = 76)) résidait en zone urbaine, et 84,3 % (n = 90) de nos patients avaient été hospitalisés dans le service après la 12e heure du début de la douleur. a l’électrocardiogramme de surface (eCg), les atteintes du territoire antérieur étaient le plus retrouvées, suivi de celles des territoires inférieur et antéro-septal avec respectivement 25,2 % ; 23,4 %. Soixante de nos patients (58,8 % des cas) avaient présenté des complications précoces, dominées par les complications hémodynamiques (37,2 %), rythmiques (25,5 %) et thromboemboliques (14,7 %). la thrombolyse à la streptokinase n’a pu être réalisée que chez 08 patients (7,8 %). nous avons enregistré 11,7 % (12) cas de décès. l’infarctus du myocarde est une urgence cardiovasculaire absolue avec la possibilité de survenue de complications surtout à la phase aiguë de la maladie. il s’agit complications en lien avec le retard de consultation des patients. Pour éviter les conséquences socio-économiques suite à l’idM, il importe de réduire les facteurs de risque cardiovasculaire modifiables et à réorganiser les services de santé pour y faire face avec une offre de soins et services de santé conséquente à tous les niveaux du système de santé.Mots-clés : infarctus du myocarde, complications aiguës, Chu Yo.

Abstract

Introduction-Objectifs : La présence d’une tachycardie ventriculaire (TV) doit nécessiter une prise en charge rapide. L’objectif de cette étude était d’étudier les aspects épidémiologiques, cliniques, paracliniques, étiologiques, thérapeutiques et évolutifs de la TV en hospitalisation de cardiologie au CHU-YO.Matériel et méthodes : il s’est agi d’une étude rétrospective descriptive allant du 1er Janvier 2012 au 31 décembre 2014.Ont été inclus les patients âgés de plus de 18 ans, hospitalisés dans le service de cardiologie du CHU Yalgado Ouédraogo, ayant présenté une TV soutenue ou non soutenue à l’ECG de surface douze dérivations ou à l’enregistrement continu Holter ECG.Résultats : Trente-six (36) cas de TV ont été retenus. La prévalence hospitalière de la TV était de 2,34 %. L’âge moyen des patients était de 61,44 ± 14,55 ans (extrêmes 34-85 ans). Le sex-ratio était de 2,6. Un antécédent d’hypertension artérielle était retrouvé dans 47,22 %. A l’admission un syndrome d’insuffisance cardiaque était présent dans 88,88 %. Le collapsus, l’état de choc et les palpitations ont été les manifestations cliniques les plus retrouvées au moment de la TV. La TV était soutenue dans 36,11% et non soutenue dans 63,88 %. Les principales étiologies étaient représentées par les cardiopathies hypertensives (30,30%) et les cardiopathies ischémiques (30,30%). La réduction de la TV soutenue a été effectuée par choc électrique externe dans 77 %. L’amiodarone a été utilisée dans 87% en cas de TVNS. La réduction et le maintien en rythme sinusal a été obtenu globalement dans 63,88 % des cas. La durée moyenne d’hospitalisation était de 14,30 ± 9,99 jours (extrêmes 2-36 jours). La mortalité globale était de 44,44 %.Conclusion : Le pronostic de la TV suggère une surveillance rigoureuse des patients présentant des cardiopathies sous-jacentes à risque.Mots clés : tachycardie ventriculaire, trouble du rythme, cardiopathie

Abstract

Functional Capacity Assessment in Patients with Chronic Heart Failure through the Six-minute Walk Test in the Cardiology Department of Yalgado Ouedraogo University Hospital, Burkina Faso Objectives: We aim to evaluate functional capacity in patients with chronic heart failure, using the six-minute walk test in the cardiology department of Yalgado Ouedraogo University Hospital. Patients and methods: We conducted a prospective study from December 1st 2013 to March 31st 2014. Hospitalized patients with chronic heart failure who gave their informed consent were included. Functional exercise capacity was assessed through the six-minute walk test and quality of life appreciated through the SF- 36 questionnaire which was filled at the time of discharge and at six weeks post discharge. Results: We included sixty one patients in our study and 32 of them were female (52%). The mean age was 46.9 ± 14.1 years old. Seventy percent of patients had NYHA stage II dyspnea. The mean ejection fraction was 32.4 ± 8.2% (extremes: 13 and 45%). The mean distance walked during the six-minute walk test was 336.3 ± 65 m at the time of discharge and 367.9 ± 68.7 m six weeks later. The mean gain between these measurements was 23.7 ± 31.5 m. Abstract

Method – The authors retrospectively analyzed 38 files of patients hospitalized in the cardiology and pediatric units of the Bobo-Dioulasso hospital from January 1, 1994 to December 31, 1997.Results – Male to female sex-ratio was 1.9. The mean age (± standard deviation) was 35.5 (± 8.5) years (8–68). HIV seroprevalence was 60.5%. Pericardiocentesis was performed in 38 cases and the effusion was serofibrinous in 15 cases and haemorrhagic in 23 cases. 28 cases of pericarditis were of tuberculous origin; seven were associated with HIV infection, and the aetiology remained undetermined in three cases. The outcome after one year of follow-up was 12 deaths (9 cases by adiastolia), ten cases of constrictive pericarditis, ten cases of full recovery, and six patients were lost to follow-up.Comments – Tuberculous pericardial effusion is strongly associated with HIV infection.
