L’épreuve d’effort est un moyen diagnostic utile pour les cas de suspicion d’angor avec une faible sensibilité mais une bonne spécificité. Elle est également très utile dans l’évaluation du risque, de l’efficacité du traitement et la guidance des prescriptions médicales après contrôle des symptômes d’ischémie. L’objectif de notre travail était d’analyser l’apport de l’épreuve d’effort à la prise en charge des cardiopathies ischémiques dans le Service de Cardiologie du CHU YO. Il s’est agi d’une étude rétrospective sur 60 patients ayant bénéficié d’une épreuve d’effort de janvier 2012 à décembre 2013. L’épreuve d’effort a été réalisée sur tapis roulant selon le protocole de Bruce modifié. Soixante patients ont bénéficié d’une épreuve d’effort durant notre période d’étude. L’âge moyen des patients était de 49 ± 10,8 ans. Le sex-ratio était de 1,2. Tous les patients avaient effectué l’épreuve d’effort sur tapis roulant. Un antécédent de coronaropathie était noté chez 22 patients. La recherche d’une insuffisance coronarienne était l’indication de l’épreuve d’effort dans 83% des cas. Elle était démaquillée dans 78% des cas. L’épreuve d’effort était arrêtée pour effort maximal dans 46 cas (soit 77%). La durée moyenne de l’effort était de 11,7 mn ± 3,2. Dix pour cent des patients avaient eu une épreuve d’effort positive et 10% une épreuve d’effort litigieuse. Notre étude va en outre contribuer à vulgariser davantage cet examen qui reste peu prescrit dans notre environnement et même parmi les médecins cardiologues. Cependant, des efforts restent à faire afin d’améliorer la qualité de la pratique de cet important examen dans la prise en charge des coronaropathies dans un contexte de pays à ressources limitées.

Etudier le profil des patients hospitalisés pour hémorragie sous traitement anticoagulant. Il s’est agi d’une étude rétrospective à visée descriptive sur une période de 02 ans allant du 1er Janvier 2007 au 31 Décembre 2008, dans le Service de Cardiologie du Centre Hospitalier Universitaire- Yalgado Ouédraogo. Tous les patients hospitalisés présentant une hémorragie sous anticoagulant ont été inclus dans l'étude. L’âge moyen des patients était de 49,31 ± 17,68 ans, le sex-ratio était de 2,17. L’infarctus du myocarde était la première indication du traitement anticoagulant avec une fréquence de 21,05%. L’anti Vitamine K (AVK) a été pourvoyeur d’hémorragies dans 63,16% (n = 12) des cas contre 36,84% (n = 7) pour les Héparines de Bas Poids Moléculaire (HBPM); Il s’agissait de 10 cas d’hémorragie majeure et de neuf cas d’hémorragie mineure. La durée moyenne du traitement AVK était de 16 semaines ± 58 semaines. Le saignement par le tractus digestif était le plus fréquent (31,58%) et dans 89,47% des cas, le traitement était associé à un antiagrégant plaquettaire. L’arrêt définitif de l’anticoagulant constituait la prise en charge de l’accident hémorragique dans 73,68%. Il y avait quatre (21,05%) décès. L’inaccessibilité des antidotes comme le sulfate de protamine et les PPSD (Prothrombine Proconvertine Stuart B) constituent un véritable frein à la prise en charge adéquate des complications; mais une meilleure éducation de patients utilisateur de ces drogues constituerait la plus importante des préventions, car plus de 50% de ces accidents sont évitables.

Les troubles de conduction auriculo-ventriculaire sont relativement fréquents en Afrique grâce à l’élévation de l’expérience de vie dans nos sociétés en pleine transition épidémiologique. Ces troubles sont le plus souvent dus à une dégénérescence du tissu de conduction. La prise en charge des troubles conductifs (BAV3) consiste en l’implantation d’un stimulateur cardiaque définitif. Mais cette thérapeutique doit être précédée par une enquête étiologique pour détecter les autres causes de BAV transitoire. Nous rapportons l’observation d’une patiente de 75 ans, admise pour bloc auriculo-ventriculaire 2/1 et un goitre multi-nodulaire hyperthyroïdique ; le traitement avec des antithyroïdiens de synthèse a vu la disparition du trouble conductif. Les BAV au cours de l'hyperthyroïdie sont rares, mais bien décrits. L’évolution est favorable sous traitement par antithyroïdiens de synthèse. Il ne faudrait donc pas implanter des pacemakers définitifs en première intention.Key words: Atrio-ventriculaire, goitre multinodulaire, hyperthyroïdique

Objectives: We aim to describe the epidemiological and prognostic characteristics of non valvular atrial fibrillation related stroke. Patients and methods: We retrospectively analyzed data of hospitalized patients from January 1 st 2010 to June 30th 2012 in the two contiguous departments of cardiology and neurology, at the teaching hospital of Yalgado Ouedraogo, Burkina Faso. Results: We recorded 391 cases of ischaemic stroke. Cardioembolic stroke was observed in 159 patients out of who, non valvular atrial fibrillation accounted for 43.5%. Atrial fibrillation was persistent/permanent in 60 cases (87%). The mean age ± SD was 63.3 ± 14.2 years (extremes: 26-91 years). Hypertension was observed in 85.5% of the cases. The mean CHA 2 DS 2 -VASc score was 4.72 ± 1.16. The mean HAS-BLED score was 2.35 ± 0.92 with a low to intermediate bleeding risk score (≤ 2) in 60.9 % of patients. The rate of vitamin K antagonists use was 52%. They were introduced at least a week after stroke onset in 34 cases. A mean follow up of two weeks in hospital noticed haemorrhagic transformation and ischaemic recurrence in respectively 8.7% and 2.9% of the cases. Introduction: Hypertension in pregnancy remains one of the leading causes of maternal and fetal morbidity and mortality worldwide. We aim to assess the prognostic pattern of hypertension (HPT)during pregnancy in the cardiology and obstetrics/gynaecology departments of Yalgado Ouedraogo University Hospital. Patients and methods: An observational cohort study was conducted from July 1st 2012 to March 31st 2013 in pregnant women with HPTwho consent to participate. Follow-up visits were monthly performed. Results: Overall 3247 pregnant women attended the Obstetrics/Gynaecology department and 312 were hypertensive. The prevalence of HPT during pregnancy was 9.6%. Study population comprised 126 patients who consented to participate in the study. Chronic HTP and preeclampsia accounted for 19% and 49.2 % of all cases respectively. The mean age was 29 ± 6.7 years. Antihypertensive drugs were used in 108 cases (85.7%). At least a bitherapy was found in 46 (36.5%). Cesarean section was the mode of delivery in 79 cases (62.7%). Blood pressure control was significantly higher in patients with new cases (gravidic HPT) compared to those with chronic hypertension over time. Eclampsia was observed in 19.8% and no maternal death occurred. Intrauterine fetal death and prematurity were found in 14.5% and 28.3% of fetuses. Proteinuria and hyperuricemia were associated with fetal complications. Conclusion: Hypertension in pregnancy remains a public health concern in developing countries. Efforts should be made to strengthen and promote maternal and perinatal heath care.

Cardio embolic stroke: Data from 145 cases at the Teaching Hospital of YalgadoOuedraogo, Ouagadougou, Burkina Faso Introduction: Cardioembolic strokes represent a major public health concern worldwide due to associated high morbidity and mortality. Cardiac sources of embolism are leading cause of stroke after atherosclerosis. We aim to describe the epidemiological profile and outcome of cardioembolic stroke. Patients and methods: We retrospectively analyzed medical records of hospitalized patients who were admitted from January 1rst 2010 to May the 31th 2012 in the departments of Cardiology and Neurology at the Teaching University Hospital of YalgadoOuedraogo, Burkina Faso, West Africa. All patients diagnosed with ischaemic stroke on the basis of CT-scan and known to have a heart disease were included in the study. Results: Overall 582 cases of stroke were reported. Ischaemic stroke was observed in 370 patients (63.6%). Cardioembolic disease was reported in 145 patients (39.2%) among whom 73 female. Mean age was 61.7 ± 15 years (extremes: 21 - 90 years). Hypertension and active smoking were respectively observed in 65.5% and 25.5% of cases. Etiologic factors were atrial fibrillation (42.8%) and intra-cardiac blood clot (13.8%). Vitamin K antagonists were prescribed in 41.4% of cases. A two-week in-hospital follow-up reported hemorrhagic transformation in 8.3% of cases. In-hospital mortality rate was 15.2% and was significantly associated with hemorrhagic transformation (n = 10, RR = 9.24, CI95% = [5.1-16.8], p < 0.001) and congestive heart failure (n = n= 10, RR = 4, CI95%= [1.9-8.2], p < 0.001) and altered consciousness on admission (n= 8, RR = 2.7, CI95% = [1.3-5.8], p =0.009). Conclusion: Cardioembolic strokes are of frequent occurrence and associated with high in-hospital mortality. Therefore there is a need for early management of their etiologic factors.
