1. Early Efficacy Assessment of Targeted Therapy for Crohn's Disease by Ultrasound

Abstract

Crohn's disease (CD) is a chronic inflammatory bowel disease with complicated pathogenesis and prolonged condition. In recent years, with the development of targeted therapy, biological agents have been widely used in the treatment of CD, which has significantly improved the prognosis of CD patients. If the efficacy evaluation can be carried out at the early stage of targeted therapy, it may help to figure out the patients who can benefit from the treatment and optimize the treatment plan for the ineffective patients in time. In the evaluation of CD activity and efficacy, ultrasound has the advantages of high accuracy, no radiation and high patient acceptance, and therefore has potential clinical application value in the early efficacy evaluation of CD targeted therapy. This article reviews the research progress to provides a reference for optimizing the treatment plan for CD patients.

2. Progress in Imaging Study of Creeping Fat in Crohn's Disease

Abstract

Crohn's disease(CD)is a kind of inflammatory bowel disease, and its incidence rate is increasing year by year. Mesenteric creeping fat, a special sign of CD closely related to its intestinal inflammation and stenosis, has become a research hotspot in recent years. The formation mechanism of mesenteric creeping fat may be related to the migration of adipocytes and the translocation of intestinal microbiota. MRI, CT and US are commonly used imaging evaluation methods for CD, which can show the sign of creeping fat and evaluate the inflammation of intestinal wall and intestinal stenosis. However, the relationship between mesenteric creeping fat and inflammatory activity, stenosis and prognosis of CD needs to be further studied. The purpose of this review is to introduce the pathogenesis and clinical significance of mesenteric creeping fat, as well as the evaluation indicators of different imaging methods, and to further explore the relationship between mesenteric creeping fat and CD inflammatory activity and stenosis.

Abstract

Breast cancer, as the most common malignant tumor in women worldwide, has become the focus of global attention. Axillary lymph node tumor burden is an important prognostic indicator of it. Ultrasound is the most commonly used imaging method, but its sensitivity is not high, especially for the diagnosis of small and micro lymph node metastasis. In recent years, the emerging Radiomics in machine learning field has been widely used in the field of medical imaging. Because it can extract high-level information of images that is difficult to be recognized by human eyes, it has been used to establish clinical prediction models. This paper introduced the value of preoperative sonography, sketched Radiomics, and summarized the research progress of this method in predicting lymph node metastasis of breast cancer. This new method is expected to provide a reliable basis for individualized and accurate diagnosis and treatment of breast cancer.

Abstract

Breast cancer is the most common malignant tumor in Chinese women. Early diagnosis, treatment, and prognosis assessment are important clinical problems to be solved. Radiomics is a non-invasive method for high-throughput extraction and analysis of lesion features of images to provide more potential information of tumors, and guide precise diagnosis and treatment. Recently, it has been widely concerned and studied in breast cancer, covering every stage in the care of patients with breast cancer. In terms of diagnosis, the research has reached the level of maturity and gradually proceeded to the clinical setting. Regarding efficacy evaluation and prognostic prediction, although in its infancy, radiomics shows promising potential. This paper mainly reviews the application of radiomics in the diagnosis, response evaluation, and prognosis prediction of breast cancer.

Abstract

Data-driven deep learning models have shown great capabilities to assist radiologists in breast ultrasound (US) diagnoses. However, their effectiveness is limited by the long-tail distribution of training data, which leads to inaccuracies in rare cases. In this study, we address a long-standing challenge of improving the diagnostic model performance on rare cases using long-tailed data. Specifically, we introduce a pipeline, TAILOR, that builds a knowledge-driven generative model to produce tailored synthetic data. The generative model, using 3,749 lesions as source data, can generate millions of breast-US images, especially for error-prone rare cases. The generated data can be further used to build a diagnostic model for accurate and interpretable diagnoses. In the prospective external evaluation, our diagnostic model outperforms the average performance of nine radiologists by 33.5% in specificity with the same sensitivity, improving their performance by providing predictions with an interpretable decision-making process. Moreover, on ductal carcinoma in situ (DCIS), our diagnostic model outperforms all radiologists by a large margin, with only 34 DCIS lesions in the source data. We believe that TAILOR can potentially be extended to various diseases and imaging modalities.

45. Impact of new undercrossing tunnel excavation on the stability of the existing tunnel

Abstract

The rapidly expanding transportation hub system in China has driven a large number of track and highway engineering projects nationwide, resulting in increasing interchanges of highway and railway tunnels year on year. The mutual impact between new and existing tunnels is one of the most important topics in geotechnical engineering. Constructed undercrossing, an existing highway tunnel is commonly seen in new railway tunnel projects. The new railway tunnel excavation will affect the stability and safety of the existing highway tunnel. Our study takes the newly built Huilongwan Railway Tunnel section undercrossing the existing Longzhouwan Highway Tunnel, one of the East Ring Line Projects of Chongqing Railway Terminal, as the object. This article simulated the displacement changes and structural deformation pattern of the existing highway tunnel caused by the excavation of the new railway tunnel with finite element analysis software and compared them with on-site measurements one by one to clarify the impact of new railway tunnel excavation on the stability of the existing highway tunnel. The result proved that the non-blasting excavation with a three-step temporary invert method could ensure the safety of the existing highway tunnel.
