The relationship between end-tidal partial pressure of carbon dioxide (PETCO2), arterial partial pressure of carbon dioxide (PaCO2), and blood pH in isoflurane-anesthetized raptors was evaluated. PaCO2 and pH were determined in serial arterial samples from isoflurane anesthetized birds and compared with concurrent end-tidal partial pressure of carbon dioxide measured with a Microstream sidestream capnograph. Forty-eight paired samples, taken from 11 birds of prey (weighing 416-2,062 g), were used to determine correlations coefficients between PaCO2 and PETCO2, and between PETCO2 and pH. Limits of agreement between PaCO2 and PETCO2 also were calculated. Strong correlations were observed between PaCO2 and PETCO2 (r = 0.94; P0.0001) as well as between PETCO2 and pH (r = -0.90; P0.0001). However, the level of agreement between PaCO2 and PETCO2 varied considerably. Low values of PETCO2, ranging from 18 to 29 mm Hg, exceeded the concomitantly measured values of PaCO2 by an average of 6.0 mm Hg (6.0 +/- 1.9 mm Hg; mean +/- SD). Conversely, high values of PETCO2, ranging from 50 to 63 mm Hg, were on average 7.6 mm Hg (7.6 +/- 9.8 mm Hg) lower than values of PaCO2. In the 30 to 49 mm Hg range for PETCO2, the difference between PETCO2 and PaCO2 was on average 1.0 mm Hg (1.0 +/- 8.5 mm Hg). These results suggest that the capnograph used provided a sufficiently accurate estimation of arterial partial pressure of carbon dioxide for birds weighing400 g and receiving manual positive ventilation with a Bain system. In our study, the linear relationship observed between the pH and the end-tidal partial pressure of carbon dioxide suggested that the monitoring of end-tidal partial pressure of carbon dioxide also can be useful to prevent respiratory acidosis.

Objective To assess the suitability of lingual venous blood (LBG) as an alternative to arterial blood (ABG) samples in determining acid–base balance and blood–gas status in dogs anesthetized for elective procedures and with medetomidine and isoflurane administration under experimental conditions. Study design Prospective, randomized clinical and experimental study. Animals Clinical population of 18 ASA I/II dogs for elective surgery and five healthy Beagles (3 females and 2 males) for the experimental study. Methods Blood sampling was simultaneously performed at dorsal pedal arterial and lingual venous sites, generating paired data. Two paired samples were collected from each dog in the clinical part and four from each dog in the experimental part (two during isoflurane anesthesia and two during isoflurane plus medetomidine). A modified Bland and Altman method was used to examine data from the clinical part and the experimental data were subjected to a paired sign's test following transformation where appropriate. Results The pH of LBG overestimated ABG, with limits of agreement of (−0.01, 0.02). The partial pressure of carbon dioxide (PCO2) of LBG overestimated ABG by 0.6 mmHg [0.1 kPa], with limits of agreement of (−3.5, 4.6) mmHg [−0.5, 0.6 kPa]. The partial pressure of oxygen (PO2) of LBG underestimated ABG by 86.3 mmHg [−11.5 kPa], with limits of agreement of (−199.8, 27.3) mmHg [−26.6, 3.6 kPa]. During medetomidine administration values for PO2 (p = 0.03) and lactate (p = 0.03) were lower for LBG when compared with ABG. The LBG value of PO2 was lower (p = 0.03) during medetomidine and isoflurane administration versus isoflurane alone. Conclusions and clinical relevance The pH and PCO2 of LBG samples provide clinically acceptable substitutes of ABG samples in the dog population studied. The wider limits of agreement for PO2 render it less reliable as a substitute for ABG. The difference in PO2 identified between LBG and ABG during medetomidine administration may not preclude the use of LBG as substitutes for ABG samples.

The purpose of this study was to provide safe anesthesia for bronchoalveolar lavage and assess the utility of premedication with i.m. midazolam for short-duration anesthesia with isoflurane in harp seals (Phoca groenlandica). Fourteen yearling harp seal pups were anesthetized three times each as part of a prospective, cross-over, blinded study. Each animal received i.m. premedication with saline, low-dose, or high-dose midazolam (respectively 0.1 and 0.2 mg/ kg). Following premedication, anesthesia was induced with 4% isoflurane in oxygen delivered through a mask and connected to a Bain non-rebreathing system. A significantly longer time was taken from the end of general anesthesia to head movement in the high-dose group compared with the saline group (P = 0.002). A significantly longer time was taken from the end of general anesthesia to ambulation in the high-dose group compared with the saline group (P = 0.006). There were no significant differences between groups in the subjective assessment of anesthetic quality or ease of intubation. Premedication with i.m. midazolam at the dosages used did prolong recovery from anesthesia, although to a degree unlikely to be significant clinically.

A 6-year-old, 3.0 kg, neutered female, Yorkshire terrier was referred for orthopedic surgery. Cardiac arrest followed unsuccessful treatment of bradycardia and systemic arterial hypotension under general anesthesia. Postmortem examination revealed hypertrophic cardiomyopathy. A possible relationship between treatment of bradycardia, systemic arterial hypotension, and sudden cardiac death is described.

Objective To characterize the acute cardiopulmonary effects of severe hemorrhage in anesthetized horses. Study design Prospective experimental study. Animals Three geldings and six mares, aged 14.4 ± 2.7 years, weighing 486 ± 41 kg (range: 425-550 kg). Methods Horses were anesthetized using xylazine, guaifenesin, ketamine and halothane or isoflurane. Cardiovascular variables, hematocrit, total solids, capillary refill time (CRT) and color of mucous membranes were measured as blood was collected from the carotid artery into sterile plastic bags. Arterial blood gas analysis was also performed. Results The average amount of blood collected from these horses was (mean ± SD) 53 ± 4.8 mL kg−1 bodyweight (range: 23-32 kg) over 39 ± 4 minutes. Hematocrit decreased from 38 ± 3 to 32 ± 2% after induction of anesthesia and did not change significantly over the period of blood loss. Total solids decreased significantly after induction of anesthesia, and over the period of blood loss. Systolic, mean, diastolic and pulse pressures decreased as blood was lost. Heart rate did not change significantly. Capillary refill time increased from 1.6 ± 0.4 seconds to 4.8 ± 1.3 seconds as blood loss increased. Mucous membrane color deteriorated progressively. Arterial PO2 decreased significantly over the period of blood loss. Conclusions Hematocrit and heart rate do not change significantly during acute severe hemorrhage in the anesthetized horse. Arterial blood pressure, pulse pressure and PaO2 decrease as blood loss increases. Changes in mucous membrane color and CRT also occur as blood loss increases. Clinical relevance During severe hemorrhage in the inhalant-anesthetized horse, both heart rate and hematocrit remain unchanged. Blood pressure decreases and changes in arterial PO2 correlate most strongly with volume of blood lost.
