2. Effects of palm oil mill effluent (POME) anaerobic sludge from 500 m3 of closed anaerobic methane digested tank on pressed-shredded empty fruit bunch (EFB) composting process

In this study, co-composting of pressed-shredded empty fruit bunches (EFB) and palm oil mill effluent (POME) anaerobic sludge from 500 m3 closed anaerobic methane digested tank was carried out. High nitrogen and nutrients content were observed in the POME anaerobic sludge. The sludge was subjected to the pressed-shredded EFB to accelerate the co-composting treatment. In the present study, changes in the physicochemical characteristics of co-composting process were recorded and evaluated. The cocomposting treatment was completed in a short time within 40 days with a final C/N ratio of 12.4. The co-composting process exhibited a higher temperature (60 - 67°C) in the thermophilic phase followed by curing phase after four weeks of treatment. Meanwhile, pH of the composting pile (8.1 - 8.6) was almost constant during the process and moisture content was reduced from 64.5% (initial treatment) to52.0% (final matured compost). The use of pressed-shredded EFB as a main carbon source and bulking agent contributed to the optimum oxygen level in the composting piles (10 - 15%). The biodegradation of composting materials is shown by the reduction of cellulose (34.0%) and hemicellulose (27.0%) content towards the end of treatment. In addition, considerable amount of nutrients and low level of heavy metals were detected in the final matured compost. It can be concluded that the addition of POME anaerobic sludge into the pressed-shredded EFB composting process could produce acceptable and consistent quality of compost product in a short time.

- Abstract
This study aimed to estimate workers' occupational lifetime exposure to chrysotile and examine the respiratory symptoms and lung cancer risk. A total of 112 workers were interviewed about their occupational histories. Exposure modeling using information on the determinants of exposure was used to estimate chrysotile emissions. The cumulative lifetime exposure was then assessed for each worker. Respiratory symptoms were obtained using a validated questionnaire. Lung cancer mortality rate was also predicted using a model. Almost all the workers were male and young (mean age = 30 years, SD = 7). The estimated lifetime occupational chrysotile inhalation exposure ranged from 0.0001 to 0.0486 f/mL.years (median = 0.0018 f/mL.years, IQR = 0.486). A high prevalence of cough symptom (11.7%), and low estimated cancer risk (<1%) were reported. - Abstract
Introduction: Since the emergence of COVID-19, the Malaysian government has made wearing a face mask in public mandatory since August 1, 2020, as an effort by the government to control the transmission of COVID-19. However, Malaysians' willingness to wear face masks in public is unknown., Objective: Thus, this study aimed to evaluate their perception of face mask wearing during COVID-19 and its contributing factors., Methodology: A total of 1024 respondents, aged ≥ 18 years, participated in this online cross-sectional survey from October 2021 to December 2021. The Face Mask Perception Scale (FMPS) was used to measure their perceptions., Results: Most of the respondents perceived wearing a face mask as uncomfortable. Our findings also revealed statistically significant differences and a small effect (f2 = 0.04) in which respondents who were concerned about being infected by the virus perceived face mask wearing appearance positively (B = - 0.09 units of log-transformed, 95% CI = - 0.15, - 0.04), whereas married respondents perceived it negatively (B = 0.07 units of log-transformed, 95% CI = 0.03, 0.09). There were no statistically significant differences in other domains of FMPS., Conclusion: In conclusion, discomfort was a major complaint. Marital status and fear of COVID-19 infection affected their perceptions. The public health implications of these findings highlight the importance of addressing discomfort and societal perceptions, particularly those influenced by factors such as marital status and COVID-19 experience, to promote widespread acceptance and consistent usage of face masks, which is crucial in mitigating the spread of COVID-19., Competing Interests: The authors have declared that no competing interests exist., (Copyright: © 2024 Yusof et al. 6. A Qualitative Systematic Review of Healthcare Practitioners' Experience of Workplace Violence.

- Abstract
Healthcare practitioners face significant risks of workplace violence due to various reasons such as hospital congestion, miscommunication, and aggressive behaviours of patients and relatives. Exposure to workplace violence may disrupt the workflow process and compromise patient care in healthcare facilities, ultimately affecting job performance, reducing job satisfaction, and negatively affecting the physical and mental health of healthcare practitioners. This study aimed to review all the published studies conducted on the experiences of workplace violence among healthcare practitioners. This study is a systematic review of qualitative studies. Data were collected through online databases including ScienceDirect, PubMed, MEDLINE and JSTOR were searched from the year 2015-2021. The inclusion criteria were: qualitative methods and mixed methods of data collection and analysis; studies that were carried out among healthcare practitioners who have been experience on workplace violence; scope of the primary studies included experience of workplace violence; and published in English/Malay in academic journal between 2015 and 2021. A total of 15 papers were included in the final analysis. The overall quality of the included papers was high. Of the 15 papers, 12 studies fully met the CASP criteria. The results of the 15 included studies were organised into the thematic groups of: i) verbal violence as the common workplace violence; ii) perceived causes of workplace violence and iii) seeking help. Across different countries, verbal violence was the most common type of workplace violence reported by healthcare practitioners. 7. Patient's satisfaction towards healthcare services and its associated factors at the highest patient loads government primary care clinic in Pahang.

- Abstract
Introduction: Patient satisfaction is widely used to monitor the quality of health care services. A significant patient load may influence health care services and patient satisfaction. Klinik Kesihatan Bandar Kuantan, Pahang (KKBK) has the highest patient loads in Pahang state, followed by Klinik Kesihatan Beserah and Klinik Kesihatan Kurnia. There are up to 700 attendees at KKBK per day, representing a population of 209679. KKBK had receives several complaints and patient unhappiness with its health care services, despite the administration's efforts to improve the clinics healthcare delivery. Thus, this study aimed to measure patients' satisfaction towards health care services at Pahang's highest patient loads primary care clinic., Materials and Methods: A cross-sectional study was conducted at Klinik Kesihatan Bandar Kuantan, Kuantan, Pahang. Patients were selected using stratified random sampling, and 201 participants were selected. The selected participants were asked to fill up the self-administered validated questionnaires consisting of background characteristics and Patient Satisfaction Questionnaire 18 (PSQ-18). Data collection period was from March 2022 to August 2022. Descriptive analysis was used to describe the background characteristics of respondents and the score of patient satisfaction. Multiple linear regression was used to determine the factors associated with patient satisfaction while adjusting for cofounders., Results: A total of 201 eligible data points were analysed in the study. The respondent mean age was 47.1 ± 16.9. Most respondents were Malay (68.7%), having secondary education (54.2%) and predominantly from the B40 income class (88.1%). The overall mean patient satisfaction score was 3.83 ± 0.31. There were significant associations between overall satisfaction with patient education level (B = -0.144; 95% CI -0.246, -0.042; p = 0.006), waiting time (B = -0.371; 95% CI -0.534, -0.209; p = 0.001) and consultation duration (B = -0.154; 95% CI -0.253, -0.055; p = 0.0020). It was found that patients with secondary education were less satisfied compared to patients with primary education level on health care services they received. Meanwhile, those who were not happy with the waiting time and consultation duration showed less satisfaction with overall healthcare services., Conclusion: Despite serving the most significant number of patients in Pahang state, most of the patient were satisfied by the health care services at Klinik Kesihatan Bandar Kuantan. However, it is recommended to improve the waiting time and the consultation time in this clinic.

8. Mosquito Control Workers in Malaysia: Is Lifetime Occupational Pesticide Exposure Associated With Poorer Neurobehavioral Performance?

Background: Use of pesticides has been linked to neurobehavioral deficits among exposed workers. In Malaysia, organophosphate and pyrethroid pesticides are commonly used to control mosquito-borne diseases., Objectives: This study aims to assess workers' lifetime occupational pesticide exposure and examine the relationship with neurobehavioral health., Methods: A cross-sectional study was conducted on 158 pesticide-exposed and 176 non-exposed workers. To collect historical exposure and job tasks, a questionnaire and an occupational history interview were used. Pesticide exposure was measured in a subgroup of workers via inhalation and skin contact. The total pesticide intake of each worker was assessed using inhalation and dermal exposure models. CANTAB® computerised neurobehavioral performance assessments were used., Results: The participants' mean age was 31 (8) years. Pirimiphos-methyl (median = 0.569 mg/m3, Interquartile range [IQR] = 0.151, 0.574) and permethrin (median = 0.136 mg/m3, IQR = 0.116, 0.157) had the highest measured personal inhalation concentrations during thermal spraying. The estimated total lifetime pesticide intake for exposed workers ranged from 0.006 g to 12800 g (median = 379 g and IQR = 131, 794 g). Dermal exposure was the predominant route of pesticide intake for all workers. Compared to controls, workers with high lifetime pesticide intake had lower Match to Sample Visual (adjusted B = -1.4, 95% Confidence Interval (CI) = -2.6, 0.1), Spatial Recognition Memory (adjusted B = -3.3, 95% CI = -5.8, 0.8), Spatial Span (SSP) (adjusted B = -0.6, 95% CI = -0.9, 0.3) scores. Workers with low pesticide intake performed worse than controls (adjusted B = -0.5, 95% CI = -0.8, -0.2) in the SSP test, but scored higher in the Motor Screening test (adjusted B = 0.9, 95% CI = 0.1, 1.6). Higher Paired Associates Learning test scores were observed among higher (adjusted B = 7.4, 95% CI = 2.3, 12.4) and lower (adjusted B = 8.1, 95% CI = 3, 13.2) pesticide intake groups. There was no significant difference between the Reaction Time and Pattern Recognition Memory tests with lifetime pesticide intake after adjusting for confounders., Conclusion: Pesticide exposure has been linked to poorer neurobehavioral performance. As dermal exposure accounts for a major fraction of total intake, pesticide prevention should focus on limiting dermal exposure., (© The Author(s) 2022. 9. Prevalence of smartphone addiction and its associated factors among pre-clinical medical and dental students in a public university in Malaysia.

Introduction: Smartphone addiction is becoming a global concern affecting every part of society, including healthcare professionals. This study aimed to identify the prevalence of risk of smartphone addiction and its associated factors among medical and dental students in a public university in Malaysia., Method: This cross-sectional study was conducted among pre-clinical medical and dental students using convenience sampling. Questions regarding sociodemographic profile and responses to the Smartphone Addiction Scale Short Version (SAS-SV) and Depression, Anxiety and Stress Score questionnaire (DASS-21) were collected. Multiple logistic regression testing was used to analyse the factors associated with smartphone addiction., Results: We invited 409 pre-clinical medical and dental students to participate voluntarily, resulting in a response rate of 80.2%. The prevalence of high-risk smartphone addiction among the participants was 47.9%. Male participants, participants who used smartphones mainly for social media, and participants with depressive symptoms were more likely to have a high risk of smartphone addiction. Medical students, participants who spent less than 3 hours per day on a smartphone, and participants who used smartphones for education-related activities were less likely to have a high risk of smartphone addiction., Conclusion: Smartphone addiction prevalence among pre-clinical medical and dental students was high. 10. A preliminary studies to assess public knowledge of beach safety in east coast Malaysia.

- Abstract
Rip currents are one of the coastal hazards that put Malaysian beachgoers in a risky position. Most of the drowning accidents that occur at beaches worldwide are closely associated with this phenomenon. Research on rip currents is needed to build an effective measuring tool to overcome these issues. However, to date, research on rip currents is mainly focused on its physical aspects, commonly concentrating on the processes that influence and relate to the rips' generation. As an effort to minimize the negative consequences exerted by the rips, there is an urgent need to enhance the rip-related research in the social sciences field. Comprehensive research that includes all fields might produce more beneficial and reliable information. Therefore, this study intends to examine the level of public understanding of rip currents and beach safety knowledge of the Teluk Cempedak Beach. A questionnaire comprising 5 sections and 31 questions was developed as the primary tool in this study. A total of 60 beachgoers have been surveyed for this preliminary study through a questionnaire to investigate their demographic profile, frequency of visiting the beach, swimming ability, and their knowledge of rip currents and beach safety. The results show that the beachgoers have poor knowledge of rip currents. Conversely, they are observed to have higher beach safety knowledge. Also, the findings help in filling the research gaps of this study in terms of the instrument used for the data collection procedure. 11. The Proposed Future Infrastructure Model for Basic Occupational Health Services in Malaysia.

- Abstract
The objectives of occupational health services (OHS) are to create a healthy and safe working environment, prevent work-related diseases, optimise employees' functional capacity and promote health. According to the literature, global accessibility to OHS has not shown much improvement and even worsened in certain countries. The main challenges come from the small and medium enterprises (SMEs). To respond to these global challenges, the basic occupational health services (BOHS) guideline was published under the purview of the World Health Organization and the International Labour Organization. The guideline describes BOHS as part of the infrastructure called the occupational safety and health system, an essential element that ensures the high service coverage and sustainability of the programme. The BOHS guideline was introduced in Malaysia by the Department of Occupational Safety and Health with a focus on SMEs, but its accessibility is low. A gap analysis was conducted between the current BOHS in Malaysia and the published international guideline. The important challenges identified that contributes to the low BOHS accessibility in Malaysia is the weakness in the BOHS infrastructure and OHS system provision. The proposed BOHS infrastructure model is meant to increase accessibility and to provide fair and equitable health services for Malaysians., Competing Interests: Conflict of Interest None.

- Abstract
A study on the radiation dose associated with cerebral CT angiography (CTA) and CT perfusion (CTP) was conducted on an anthropomorphic phantom with the aim of estimating the effective dose (E) and entrance skin dose (ESD) in the eyes and thyroid gland during different CTA and CTP protocols. The E was calculated to be 0.61 and 0.28 mSv in CTA with 100 and 80 kV(p), respectively. In contrast, CTP resulted in an estimated E of 2.74 and 2.07 mSv corresponding to 40 and 30 s protocols, respectively. The eyes received a higher ESD than the thyroid gland in all of these protocols. The results of this study indicate that combining both CTA and CTP procedures are not recommended in the stroke evaluation due to high radiation dose. Application of modified techniques in CTA (80 kV(p)) and CTP (30 s) is highly recommended in clinical practice for further radiation dose reduction., (© The Author 2013. 13. "Is there an Association Between Self-Reported Sleep Duration, Body Mass Index and Waist-Hip Ratio in Young Adults? A Cross-Sectional Pilot Study".

Introduction: Sleep is vital for mental and physical health of an individual. Duration of sleep influences the metabolism and regulates body weight., Objective: To assess the cross-sectional association of sleep duration with body mass index (BMI) and waist-hip ratio in Malaysian students., Methods: Eighty-nine Malaysian students of both genders, and with a mean (standard deviation) age of 21.2 (0.9) years were included. Institutional Ethics Committee clearance was obtained prior to the start of study. The subjects were interviewed regarding the average hours of sleep/day, their self-reported sleep duration was categorized as < 6hour/day (short sleep duration), 6-7hour/day and > 7hour/day. Their height (in meters), weight (in kilograms), waist and hip circumference (in centimetre) were measured. BMI and waist-hip ratio were calculated using appropriate formulas and expressed as mean (standard deviation). The duration of sleep was compared with BMI and waist-hip ratio using one way ANOVA., Results: No statistical significance was observed when sleep duration was associated with BMI (p=0.65) and waist-hip ratio (p=0.95). Duration of sleep did not affect BMI and waist hip ratio in the Malaysian students in our study. The age and healthy lifestyle of the subjects in this study may have been a reason for no significant influence of short sleep duration on the BMI and waist-hip ratio., Conclusion: No association was found between sleep duration with BMI and waist hip ratio in the Malaysian students.

14. Development of Welding Fumes Health Index (WFHI) for Welding Workplace's Safety and Health Assessment.

- Abstract
Background: This study aimed to develop an index that can rank welding workplace that associate well with possible health risk of welders., Methods: Welding Fumes Health Index (WFHI) were developed based on data from case studies conducted in Plant 1 and Plant 2. Personal sampling of welding fumes to assess the concentration of metal constituents along with series of lung function tests was conducted. Fifteen metal constituents were investigated in each case study. Index values were derived from aggregation analysis of metal constituent concentration while significant lung functions were recognized through statistical analysis in each plant., Results: The results showed none of the metal constituent concentration was exceeding the permissible exposure limit (PEL) for all plants. However, statistical analysis showed significant mean differences of lung functions between welders and non-welders. The index was then applied to one of the welding industry (Plant 3) for verification purpose. The developed index showed its promising ability to rank welding workplace, according to the multiple constituent concentrations of welding fumes that associates well with lung functions of the investigated welders., Conclusion: There was possibility that some of the metal constituents were below the detection limit leading to '0' value of sub index, thus the multiplicative form of aggregation model was not suitable for analysis. On the other hand, maximum or minimum operator forms suffer from compensation issues and were not considered in this study.
