Falls and fear of falling (FoF) inhibit healthy longevity and have been suggested to be associated with cognitive function. However, the domains of cognitive function that are associated with them remain controversial. It is speculated that clarifying this will help in the assessment of health status and interventions in the community.To analyse the associations between fall history and FoF and multidimensional cognitive function in independent community-dwelling older adults.The data from 9759 (73.3 ± 5.4 years, 59.9% women) older individuals enrolled in the cross-sectional ORANGE study were analysed. Simple questions were used to assess fall history in the past year and current FoF. Assessments of multidimensional cognitive function were performed using the National Center for Geriatrics and Gerontology-Functional Assessment Tool (NCGG-FAT) to evaluate memory, attention, executive function, and processing speed. The independent associations of fall history and FoF with multidimensional cognitive function were assessed using multivariate linear regressions adjusted for potential confounding variables.A total of 18.3% and 35.4% of participants presented with fall history and FoF, respectively. Fall history (p = 0.008) and FoF (p = 0.002) were significantly associated with memory. FoF, but not fall history was associated with attention (p = 0.004), executive function (p 0.01), and processing speed (p 0.01).In independent community-dwelling older adults, fall history was associated only with the memory domain; in contrast, fear of falling was associated with multidimensional cognitive function. This study provides weak evidence suggesting the need to assess falls and FoF in all situations involving independent community-dwelling older adults.

Background Poor sleep quality has a negative effect on pain among older adults. During the coronavirus disease 2019 (COVID-19) state of emergency, lifestyle changes can cause psychologic stressors and lead to poor sleep quality. Aim This study examined whether sleep quality status was associated with low back or knee pain changes during the COVID-19 state of emergency among community-dwelling Japanese old-old adults. Design Cross-sectional investigation. Methods In July 2020, during the COVID-19 epidemic, we conducted a postal survey for old-old adults aged ≥77 years and collected data on 597 participants. For those who had low back or knee pain at the time of the survey (in July), characteristics such as low back pain, knee pain, changes in pain status, and sleep quality status during the COVID-19 state of emergency (in March) were assessed. Results Data from 597 participants showed the prevalence of low back pain (50.6%) and knee pain (40.7%) in July. Of those with low back or knee pain, 374 had pain changes during the state of emergency, with 12.3% worsening. Of these, 23.9% had poor sleep quality in March compared to non-change (p = .008). In a multivariate logistic regression model adjusted for potential confounders, poor sleep quality was significantly associated with pain worsening (odds ratio 2.80, 95% confidence interval 1.26-6.22). Conclusions During the COVID-19 state of emergency, poor sleep quality was associated with worsening low back or knee pain. This may indicate the need to pay attention to poor sleep quality to prevent the exacerbation of pain among old-old adults.

Bridging exercise is commonly used to increase the strength of the hip extensor and trunk muscles in physical therapy practice. However, the effect of lower limb positioning on the joint and muscle forces during the bridging exercise has not been analyzed. The purpose of this study was to use a musculoskeletal model simulation to examine joint and muscle forces during bridging at three different knee joint angle positions. Fifteen healthy young males (average age: 23.5 ± 2.2 years) participated in this study. Muscle and joint forces of the lumbar spine and hip joint during the bridging exercise were estimated at knee flexion angles of 60°, 90°, and 120° utilizing motion capture data. The lumbar joint force and erector spinae muscle force decreased significantly as the angle of the knee joint increased. The resultant joint forces were 200.0 ± 23.2% of body weight (%BW), 174.6 ± 18.6% BW, and 150.5 ± 15.8% BW at 60°, 90°, and 120° knee flexion angles, respectively. On the other hand, the hip joint force, muscle force of the gluteus maxims, and adductor magnus tended to increase as the angle of the knee joint increased. The resultant joint forces were 274.4 ± 63.7% BW, 303.9 ± 85.8% BW, and 341.1 ± 85.7% BW at a knee flexion angle of 60°, 90°, and 120°, respectively. The muscle force of the biceps femoris decreased significantly with increased knee flexion during the bridging exercise. In conclusion, the knee flexion position during bridging exercise has different effects on the joint and muscle forces around the hip joint and lumbar spine. These findings would help clinicians prescribe an effective bridging exercise that includes optimal lower limb positioning for patients who require training of back and hip extensor muscles.

The contents and distribution of radionuclides in activated metallic wastes are major concerns while designing a leaching model in a safety case. This study investigated 60Co as a typical activation product in the lower tie plate (i.e., stainless steel endpiece of BWR fuel assembly) irradiated for 35.0 GWd/tHM burnup, using the imaging plate (IP) method. The activation calculations coupled with the Monte Carlo burnup and neutron transport calculation code (MCNP-BURN2) were also performed based on the burnup and operation conditions. The Co impurity content in the irradiated tie plate was obtained and used as an input to MCNP-BURN2. The IP method results showed that the intensity of photostimulated luminescence decreased with increasing distance from the top (near fuels) to bottom. This trend was consistent with that exhibited by MCNP-BURN2.Therefore, the simulations allowed us to model the three-dimensional radionuclide distribution within the tie plate. Glioblastoma multiforme (GBM) is the most prevalent malignant primary brain tumor with a high recurrence rate. Despite multimodal therapy including surgical resection, chemotherapy, and radiotherapy, the median survival time after the initial diagnosis of GBM is approximately 14 months. Since cancer stem cells (CSCs) are considered the leading cause of cancer recurrence, glioblastoma stem cell-targeted therapy is a promising strategy for the treatment of GBM. However, because CSC heterogeneity has been implicated in the difficulties of CSC-target therapy, more in-depth knowledge of CSC biology is still required to develop novel therapies. In this study, we established single cell-derived tumorspheres from human glioblastoma U87MG cells. One of these tumorspheres, P4E8 clone, showed CSC-like phenotypes, such as self-renewal capacity, expression of CSC markers, resistance to anti-cancer agents, and in vivo tumorigenicity. Therefore, we used P4E8 cells as a cell-based model of glioblastoma stem cells (GSCs). Gene expression analysis using microarray indicated that the most highly expressed genes in P4E8 cells compared to the parental U87MG were PC3-secreted microprotein (MSMP). Furthermore, MSMP was expressed in patient-derived GSCs and human glioma tissues at the protein level, implying that MSMP might contribute to glioma development and progression.

[Purpose] Lateral knee instability is frequently observed in patients with knee injury or risk factors associated with knee osteoarthritis. Physical exercises can strengthen muscles that stabilize the knee joint. The purpose of this study was to define the contribution of the knee and hip muscles to lateral knee stability by comparing the muscle forces, as assessed by musculoskeletal simulation using one or two degrees-of-freedom (1-DOF and 2-DOF) knee models. [Participants and Methods] We evaluated the normal gait of 15 healthy subjects. We conducted a three-dimensional gait analysis using a motion analysis system and a force plate. We considered a muscle as a lateral knee stabilizer when the calculated muscle force was greater with the 2-DOF model than with the 1-DOF model. [Results] During early and late stance, the muscle forces of the lateral knee and hip joint increased in the 2-DOF model as opposed to in the 1-DOF model. In contrast, the forces of the medial knee muscles decreased. Furthermore, hip muscle forces increased during the late stance. [Conclusion] Our results show that the lateral knee and hip muscles contribute to lateral knee stability. Thus, exercises to strengthen these muscles could improve lateral knee stability.

This study aimed to clarify the effect of total knee arthroplasty (TKA) on trunk fluctuation and regularity of gait in patients with knee osteoarthritis by an accelerometer. The participants included 18 patients with knee osteoarthritis undergoing TKA. The gait at a comfortable velocity was assessed pre- and post-TKA by a triaxial accelerometer attached to the neck and lumbar regions. Measurement post-TKA was performed 4 weeks after surgery. Trunk fluctuation was estimated by the root mean square (RMS) of acceleration and RMS ratio (the ratio of RMS in each direction to the total RMS). Regularity of gait was estimated using the autocorrelation function. The results showed that TKA significantly decreased the RMS ratio in mediolateral acceleration of the neck and lumbar regions and reduced gait regularity. TKA appears to reduce compensatory trunk motion through the improvement of knee function. An assessment of trunk fluctuation using an accelerometer is useful for the clinical assessment of patients with knee osteoarthritis pre- and post-TKA.

To investigate whether the type and frequency of social interaction during the state of emergency due to coronavirus disease were associated with self-rated health (SRH) after the state of emergency.Data from a cross-sectional study were collected for 889 oldest-old adults in Bibai City, Hokkaido, Japan. In total, 612 participants (mean age: 83.0 ± 4.3 years; women: 51.8%) were included in the analysis, taking biological sex into account. The self-reported questionnaire included questions about demographic variables, SRH (July 2020, after the emergency), and the type and frequency of social interaction (March 2020, during the state of emergency).There was no significant association between social interaction and SRH in men (P 0.05). Women who had social interactions (both face-to-face and non-face-to-face) more than once a week during the state of emergency reported higher SRH after the emergency than those who did not (odds ratio 2.17, 95% confidence interval 1.07-4.41).Having both types of interaction more than once a week during the state of emergency was related to higher SRH after the emergency among oldest-old women. It is suggested that having opportunities for both types of interaction at least once a week would potentially be beneficial for high SRH in women, even in situations where the declaration of a state of emergency restricts face-to-face interaction. The aim of this cross-sectional study was to examine the correlations between gait regularity, cognitive functions including cognitive domains, and the mild cognitive impairment (MCI) in community-dwelling older people. This study included 463 older adults (63.4% women, mean age: 74.1), and their step and stride regularity along the three-axis components was estimated from trunk acceleration, which was measured by inertial measurement units during a comfortable gait. Four aspects of cognitive function were assessed using a tablet computer: attention, executive function, processing speed, and memory, and participants were classified into those with or without MCI. The vertical component of stride and step regularity was associated with attention and executive function (r = −0.176–−0.109, p ≤ 0.019), and processing speed (r = 0.152, p &lt, 0.001), after it was adjusted for age and gait speed. The low vertical component of step regularity was related to the MCI after it was adjusted for covariates (OR 0.019, p = 0.016). The results revealed that cognitive function could affect gait regularity, and the vertical component of gait regularity, as measured by a wearable sensor, could play an important role in investigating cognitive decline in older people.

Objectives The purpose of this cross-sectional study was to examine the association between cognitive frailty (CF) and higher-level competence among community-dwelling older adults. Methods Data from 871 older adults (aged ≥ 65 years, mean age 74.3 years, 61.5% women) who had participated in a community-based health check survey (Tarumizu Study 2018-2019) were analyzed. CF was defined as comorbid physical frailty and mild cognitive impairment (MCI). We defined physical frailty as either slow walking speed or poor grip strength. MCI was defined as values below the age- and education-adjusted reference threshold in several tests. Participants were categorized into four groups: robust, physical frailty, MCI, and CF. Higher-level competence was assessed using the Japan Science and Technology Agency Index of Competence (JST-IC). The JST-IC consists of 16 items and four subscales (technology usage, information practice, life management and social engagement). The bottom 20–30% score of the JST-IC and each subscale corresponded to poor higher-level competence. Results The prevalence of CF was 14.4%. Multivariate logistic regression analysis showed that, CF was significantly associated with poor higher-level competence (odds ratio 1.92, 95% confidence interval 1.18–3.13) after adjusting for covariates. Using a similar analysis to assess the JST-IC subscales, CF was associated with limitations on technology usage (odds ratio 2.29, 95% confidence interval 1.36–3.85) and low social engagement (odds ratio 1.62, 95% confidence interval 1.00–2.61). Conclusions This study suggests that CF is associated with poor higher-level competence, especially limitations on technology usage and low social engagement.

Understanding the relationship between pain and physical activity (PA) levels is beneficial for maintaining good health status. However, the impact of pain on changes in PA during the coronavirus disease 2019 (COVID-19) pandemic is unknown. The purpose of this study was to examine whether PA levels pre-, during, and post-COVID-19 state of emergency differ between Japanese adults who had pain after the COVID-19 state of emergency and those who did not.Data were collected from a cross-sectional online survey conducted between October 19 and 28, 2020. The analytic sample consisted of 1967 Japanese adults aged ≥40 years who completed the online survey. Participants completed questionnaires on the presence of pain and duration of PA, defined as the total PA time per week based on activity frequency and time. Participants were asked to report their PA at 3 time points: October 2019 (before the COVID-19 pandemic), April 2020 (during the COVID-19 state of emergency), and October 2020 (after the COVID-19 state of emergency).Among participants aged ≥60 years who reported pain in October 2020, the total PA time was significantly lower than participants who did not report having pain. Furthermore, the total PA time in April 2020 was significantly lower than that in October 2019; however, no significant difference in total PA time was observed between April and October 2020. Among participants aged 40 to 59 years, no significant differences were observed in total PA times at the 3 time points between those with and without pain. In addition, the total PA time in October 2020 significantly increased compared to that in April 2020, although it significantly decreased in April 2020 compared to October 2019.This study suggests that older adults with pain have lower PA levels after the COVID-19 state of emergency.

The COVID-19 pandemic has caused an abrupt change in lifestyle for many people with restrictions, often leading to a decrease in physical activity (PA), and thus contributing to a negative perception of health status. The purpose of this study was to examine the effects of the COVID-19 epidemic on physical activity and perceived physical fitness in Japanese adults aged 40 to 69 years. Data were collected from an online survey conducted between October 19 and 28, 2020. The analytic sample consisted of 1989 Japanese adults (mean age, 50.1 ± 6.9 years, women, 38.9%) who were aged between 40 and 69 years and completed the online survey. Overall, the PA time per week decreased by 32.4% between October 2019 and April 2020. A decrease in PA time was recorded in October 2020, however, a decline of 15.5% was observed. Compared to individuals who did not perceive a decline in physical fitness, individuals who perceived declining physical fitness during the COVID-19 state of emergency demonstrated a greater decrease in PA time in April 2020 (–50.5%), and this trend continued into October 2020 (–25.0%). These findings may indicate that Japanese adults aged 40 to 69 years who perceived declining physical fitness experienced a greater decrease in physical activity.

Aim Perceived health status, including physical and cognitive fitness, will be negatively associated with future health conditions among old-old adults. The coronavirus disease 2019 (COVID-19) pandemic has caused sudden changes in lifestyle. Thus, this study examined the associations of participation in an exercise class before the COVID-19 pandemic and the exercise habits and disruption to the rhythms of daily life during the COVID-19 state of emergency with perceived declining physical and cognitive fitness among community-dwelling old-old adults. Methods In July 2020, we carried out a mail survey of community-dwelling old-old adults aged between 77 and 99 years in Bibai, Hokkaido, Japan, to determine their perceived declining physical and cognitive fitness during the COVID-19 state of emergency. Results Of the 774 responders, 339 (43.8%) participants reported a decline in physical fitness, whereas 259 (33.5%) perceived declining cognitive fitness during the COVID-19 state of emergency. In a multivariate logistic regression model adjusted for potential confounders, exercise habits during the COVID-19 state of emergency were significantly associated with a lower perception of declining physical fitness. Disruption to the rhythms of daily life during the COVID-19 state of emergency was significantly associated with a higher perception of declining physical and cognitive fitness. Conclusions Approximately half and more than one-third of community-dwelling old-old adults perceived declining physical and cognitive fitness, respectively, during the COVID-19 state of emergency. During this period, exercise habits were positively correlated with perceived health status among old-old adults, whereas disruption to the rhythms of daily life was negatively correlated. The importance of an interaction between trunk stability muscles and hip muscle function has been suggested. However, reported exercises rarely act on the trunk and hip muscles simultaneously. Here, we devised an abdominal oblique and hip muscle exercise, the Self-oblique exercise (SOE). We examined whether SOE activated abdominal and hip muscles in the supine and half-kneeling positions, compared with abdominal crunch (AC) and plank exercises; and whether participants could modulate the exercise load. Participants were 20 healthy males with some sports experience such as football and baseball on average 10.5 ± 4.0 years. Participants applied self-pressure to their right thighs using the contralateral upper limb with 40% or 70% of the maximum force in Supine SOE and Half- kneeling SOE. The following abdominal and hip muscles were measured using surface electromyography: bilateral external obliques (EO), bilateral internal obliques (IO), right rectus abdominis, right gluteus medius (GMed), and right adductor longus (ADD). All evaluated muscle groups showed significant differences between exercises (p < 0.001). Supine SOE-70% showed 80.4% maximal voluntary contraction (MVC) for left EO (p < 0.017), 61.4% MVC for right IO (p < 0.027), 24.3% MVC for GMed (p < 0.002), and 42.4% MVC for ADD (p < 0.004); these were significantly greatest among all exercises. Muscle activity during Supine SOE-70% was greater than that during Supine SOE-40%. Similarly, Half-kneeling SOE-40% promoted abdominal and hip muscle exertion, and showed more significant activity in GMed (p < 0.006) and ADD (p < 0.001) than AC and plank. SOE could activate abdominal and hip muscles depends on the pressure applied by upper limb. Also, SOE allows participants to modulate the exercise load in a self-controlled step by step manner. Modulation of the exercise load is difficult in AC or plank compared to SOE, and AC or plank cannot obtain simultaneous oblique and hip muscle activity. SOE could be practiced anywhere, in various positions, without any tools.

Cosmic-ray muon scattering imaging is sensitive to high-atomic-number materials, and has potential applications in the nuclear industry. Deep learning technique was tested to estimate amount of nuclear materials of Fukushima Daiichi debris.

The Technical Design for the COMET Phase-I experiment is presented in this paper. COMET is an experiment at J-PARC, Japan, which will search for neutrinoless conversion of muons into electrons in the field of an aluminium nucleus ($\mu-e$ conversion, $\mu^- N \to e^- N$); a lepton flavor violating process. The experimental sensitivity goal for this process in the Phase-I experiment is $3.1\times10^{-15}$, or 90 % upper limit of branching ratio of $7\times 10^{-15}$, which is a factor of 100 improvement over the existing limit. The expected number of background events is 0.032. To achieve the target sensitivity and background level, the 3.2 kW 8 GeV proton beam from J-PARC will be used. Two types of detectors, CyDet and StrECAL, will be used for detecting the \mue conversion events, and for measuring the beam-related background events in view of the Phase-II experiment, respectively. Many stroke patients rely on cane or ankle-foot orthosis during gait rehabilitation. The purpose of this study was to investigate the immediate effect of functional electrical stimulation (FES) to the gluteus medius (GMed) and tibialis anterior (TA) on gait performance in stroke patients, including those who needed assistive devices. Fourteen stroke patients were enrolled in this study (mean poststroke duration: 194.9 ± 189.6 d ; mean age: 72.8 ± 10.7 y ). Participants walked 14 m at a comfortable velocity with and without FES to the GMed and TA. After an adaptation period, lower-limb motion was measured using magnetic inertial measurement units attached to the pelvis and the lower limb of the affected side. Motion range of angle of the affected thigh and shank segments in the sagittal plane, motion range of the affected hip and knee extension-flexion angle, step time, and stride time were calculated from inertial measurement units during the middle ten walking strides. Gait velocity, cadence, and stride length were also calculated. These gait indicators, both with and without FES, were compared. Gait velocity was significantly faster with FES ( p = 0.035 ). Similarly, stride length and motion range of the shank of the affected side were significantly greater with FES (stride length: p = 0.018 ; motion range of the shank: p = 0.02 6). Meanwhile, cadence showed no significant difference ( p = 0.238 ) in gait with or without FES. Similarly, range of motion of the affected hip joint, knee joint, and thigh did not differ significantly depending on FES condition ( p = 0.115 ‐ 0.529 ). FES to the GMed and TA during gait produced an improvement in gait velocity, stride length, and motion range of the shank. Our results will allow therapists to use FES on stroke patients with varying conditions.

To visualize alpha contamination, an alpha-particle detection system has been developed that is capable of detecting alpha-induced UV lights from a few meters distance in dark environments. It detects UV lights from ionized nitrogen with a photomultiplier tube. To identify the contaminated spot quickly, large diameter lenses were designed and tested.
