1. Comparison between MRI FLAIR vascular hyperintensity-DWI mismatch and perfusion based triage for thrombectomy in the late time window

Abstract

BackgroundThe clinical impact of patient selection using FLAIR vascular hyperintensity (FVH)–diffusion-weighted imaging (DWI) mismatch for endovascular thrombectomy (EVT) in patients who have been symptomatic for over 6 h remains unclear. Herein, a retrospective study was conducted to compare the inter-rater reliability and clinical outcomes of patients selected for thrombectomy based on FVH-DWI mismatch with perfusion.MethodsPatients with anterior-circulation large-vessel occlusion selected simultaneously with MRI and perfusion imaging in the late time window from a single-center retrospective study were categorized into EVT-applicable (FVH-DWI mismatch on MRI or perfusion imaging meeting the DEFUSE3 standards) and EVT-inapplicable groups based on MRI and perfusion imaging. The primary outcome was the 90-day functional independence rate. Safety outcomes encompassed symptomatic intracranial hemorrhage and mortality in 90 days. We assessed the consistency of the two profiles and compared the differences in functional independence rates of EVT patients among the EVT-applicable groups determined by MRI and perfusion.ResultsA total of 130 patients were enrolled, of which 114 were classified into the EVT-applicable group after triaging using MRI images. In this group, 96 patients underwent EVT, with 53 of them (55.2%) achieving functional independence. A total of 110 patients were divided into EVT-applicable group based on perfusion, among which 92 underwent EVT, with 49 of them (53.2%) achieving functional independence. The consistency of identifying EVT indication was moderate between two groups (κ = 0.42, 95% CI, 0.17–0.67). The functional independence rate was comparable between patients in the two EVT-applicable groups based on the two methods (55.2% vs. 53.2%, p = 0.789).ConclusionMRI triaging based on FVH-DWI mismatch showed moderate inter-rater reliability compared with perfusion-based triage and comparable efficacy in predicting clinical outcomes after EVT.

Abstract

Introduction: Numerous countries went into lockdown to contain the COVID-19 outbreak, which has impeded follow-up of chronic diseases, such as cognitive impairment (CI). Cognitive and neuropsychiatric changes during the COVID-19 pandemic are neglected in China, which is the world's whistleblower. To investigate the cognitive and neuropsychologic changes in CI, as well as the proportions of rapid cognitive decline (RCD) before and during the COVID-19 pandemic to provide clinical evidence for CI intervention during a public health emergency.Methods: We performed a descriptive and retrospective study based on medical records from the memory clinic of Tianjin Dementia Institute collected through face-to-face evaluations. Information of 205 patients with CI, including patients with mild cognitive impairment and dementia, of whom 131 with Alzheimer's disease (AD) were analyzed and compared to a control group before the COVID-19 pandemic.Results: Among the 205 CI patients, the scores on the Chinese Mini Mental State Examination (C-MMSE), the Montreal Cognitive Assessment (MoCA), activities of daily living (ADLs), and the global Neuropsychiatric Inventory (NPI) were significantly different at the baseline and follow-up evaluations (p < 0.05) after 14.07 (±2.87) months. The same findings were recorded among AD patients, and they exhibited more sleep disturbances at the follow-up than at baseline (32.8 vs. 20.6%, p = 0.035). When compared to the control group, slightly worse performance of cognitive, −1.00 (−4.00, 1.00) from the C-MMSE, −1.00 (−2.00, 0.00) on the MoCA, 1.00 (0.00, 9.00) on ADLs and neuropsychological 0.00 (−1.00, 3.50) on the global NPI profile, at the follow-up were presented, particularly for delusion, agitation, irritability, and appetite disturbances (p < 0.05). Twenty-five (19.1%) AD patients and 48 (36.6%) controls suffered RCD during the COVID-19 pandemic. Moreover, AD patients during the COVID-19 pandemic were 0.408 times (95% confidence interval: 0.232–0.716) less likely to suffer RCD than the control.Conclusion: Confinement might ease the cognitive and neuropsychiatric deterioration of AD patients compared to those not in crisis and help prevent RCD in AD patients.

Abstract

Background: While the lockdown strategies taken by many countries effectively limited the spread of COVID-19, those were thought to have a negative impact on older people. This study aimed to investigate the impact of lockdown on cognitive function and neuropsychiatric symptoms over a 1-year follow-up period in patients with mild cognitive impairment (MCI), Alzheimer's disease (AD) and dementia with Lewy bodies (DLB).Methods: We enrolled consecutive patients with MCI, probable AD or DLB who were receiving outpatient memory care before the COVID-19 pandemic and followed-up with them after 1 year by face-to-face during the COVID-19 pandemic to assess changes in physical activity, social contact, cognitive function and neuropsychiatric symptoms (NPS).Results: Total 105 probable AD, 50 MCI and 22 probable DLB patients were included and completed the 1-year follow-up between October 31 and November 30, 2020. Among the respondents, 42% of MCI, 54.3% of AD and 72.7% of DLB patients had a decline in MMSE scores and 54.4% of DLB patients had worsening Neuropsychiatric inventory (NPI) scores. Patients with DLB showed a more rapid decline of MMSE than those with AD. Diminished physical activity and social contact might have hastened the deterioration of cognition and the worsening of NPS.Conclusion: Social isolation and physical inactivity even after strict lockdown for at least 6 months were correlated with accelerated decline of cognitive function and NPS in patients with AD and DLB.

Objectives Our study aims to examine the value of endovascular therapy (EVT) and its comparison to medical management (MM) in ischaemic stroke patients accompanied by large artery atherosclerosis (LAA) and non-LAAMethods modified Rankin scale score (mRS) was evaluated at 90 days post the stroke attack and was considered as the primary outcome. Other outcomes measured in this study included score changes of 0–2 and 0–3 on the mRS. The occurrence of symptomatic intracranial haemorrhage at 24 hours after EVT was also measured as a safety endpoint. Logistic regression analysis was used to determine the associations.Results In the LAA group, no significant difference in mRS at 90-day (median IQR 3 (2–5) vs 4 (3–4), 95% CI 0.53 to 2.00, p=0.924), mRS 0–2 and mRS 0–3 was observed between EVT and MM groups. Background: The relationship between cholesterol level and the risk of developing Alzheimer’s disease has been well established, but the relationship between cholesterol level and Lewy body dementia (LBD) is still not well known. Objective: The aim of this case-control study was to explore the association between blood cholesterol levels and LBD in Chinese older adults. Methods: A total of 65 patients with LBD and 110 older adult controls were enrolled during the study period. The levels of triglyceride, total cholesterol, low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and fasting glucose were measured separately. The associations between LBD, blood cholesterol levels, and fasting glucose levels were assessed using multiple binary logistic regression analyses adjusted for multiple covariates. Results: Increased plasma LDL-C levels and lower HDL-C levels were independently associated with the risk of LBD in models adjusted for age, sex, education, alcohol use status, smoking status, and vascular disorders. Higher fasting glucose levels may be associated with the risk of LBD. Conclusion: The results of this study suggest that elevated levels of LDL-C and reduced levels of HDL-C were associated with LBD development and therefore are potential nutritional risk factors for LBD. Adjusting diet and individualized and effective cholesterol-lowering therapy in high-risk adults may aid in the prevention or management of LBD.

Abstract

Parallel mechanism has been widely used in large-scale and heavy-duty attitude adjustment equipment. In order to improve the work reliability of the subreflector parallel adjusting mechanism, a fault-tolerant strategy based on the redundant degree-of-freedom and a workspace boundary identification method are proposed in this paper, which can realize the proper function of the subreflector parallel adjusting mechanism when it has a driven fault. The configuration and parameters of the parallel adjusting mechanism are introduced firstly, then the degrees-of-freedom of the parallel adjusting mechanism is calculated when it has a driven fault, and the principle of the fault-tolerant strategy based on the redundant degree-of-freedom is deduced in detail. Next, the method to solve the workspace boundary identification problem for the parallel adjusting mechanism in fault tolerance conditions is proposed, the maximum and minimum workspaces of the parallel adjusting mechanism at fault tolerance conditions in different frequency bands are analyzed. The results showed that the workspace calculated by the fault-tolerant strategy in the fault condition can completely meet the needs of the subreflector, where this method can also be applied to other parallel mechanisms. Lastly, an experiment is conducted to verify the correctness and effectiveness of the fault-tolerant strategy, in which the results showed that the fault-tolerant strategy can effectively improve the work reliability of the parallel adjusting mechanism. The fault-tolerant strategy and workspace boundary identification method can make the subreflector parallel adjusting mechanism work normally when it has a driven fault, which can significantly improve the work reliability and work efficiency and the maintenance cost can also be reduced. The fault-tolerant strategy and workspace boundary identification method can also be well applied to the research and development for this kind of parallel mechanical equipment.

Abstract

The transcription factor forkhead box P3 (FOXP3) has been demonstrated to play important roles in the development and function of regulatory T cells (Tregs). In addition, studies had recently demonstrated that FOXP3 also expressed in some tumor cells. However, the exact role and molecular mechanism of FOXP3 function in glioma’s cells are still unclear. This study aims to elucidate the functions of FOXP3 in glioma’s cells. Expression of FOXP3 in glioma cell U87 and LN229 was up-regulated and down-regulated by pCMV6-FOXP3-GFP and pRFP-C-RS shFOXP3 respectively. Then, CCK-8 assay, flow cytometry, migration and invasion assay, and western blot were used to detect cell proliferation, cell cycle, cell migration and invasion and related protein expression. All detection methods demonstrated that over-expression of FOXP3 in glioma cell U87 and LN229 inhibited cell proliferation, reduced cell migration, decreased cell invasion compared with control. Moreover, up-regulation of FOXP3 increased the protein levels of pro-apoptotic molecules caspases-3 and caspases-7, resulting in the promotion of cell apoptosis. Conversely down-regulation of the FOXP3 promoted cell growth and inhibited cell apoptosis and reduced the expression of caspases-3 and caspases-7. Our findings suggest that FOXP3 maybe act as a suppressor in glioma cells proliferation, migration and invasion and endogenous FOXP3 transfusion could be a novel approach for inhibiting glioma progression.

Abstract

At present the naked eye 3D display has only one appropriate viewpoint, and the slight movement of the viewer position will cause crosstalk and affect the viewing experience. To solve this problem, this paper presents an FPGA-based multi-view projection naked eye 3D video processing method. The video sources will be divided into nine equal parts, and each of them will be enlarged as the original resolution. The zoomed ones will be sent to nine projectors, then eight appropriate naked eye 3D display views will be achieved after the following optical processing. Taking into account the actual amplification effects, hardware limitations, the complexity of algorithm and other factors, the image magnification will adopt the bilinear interpolation algorithm.

Therefore, the main content of the article is to train a classifier and test the classifier. before training a classifier, it is necessary to get a lot of samples and them handle them. after training, there will be some experiments to study the factor which will affect on the detection time. Then, go to the source code of OpenCV, and find the relationship finally. at last, a good classifier is proposed.

Abstract

The subreflector's adjusting mechanism of the 65 meters radio telescope in Shanghai observatory is a Stewart parallel robot.In order to find out the problems of this robot's precision decline in time,caused by mechanical wear or error accumulation,inclinometers are used to measure the orientation of the mobile robot platform.Comparing the RMS (root mean square) error of the measured platform orientation with the expected orientation precision,thereby the user can judge whether maintenance or homing is required.For improving the easy maintenance of the parallel robot,two ways of homing are set up,by using photoelectric sensor and magnetic linearity displacement transducer(MLDT) respectively.The principles of the two ways of homing are analyzed,particularly that of calibrating the precision of the photoelectric sensor and the MLDT by using the homing operation.What's more,different conditions under which the parallel robot needs homing operation are summarized,and the corresponding homing control strategies are proposed.Test shows that the homing strategy proposed is an effective tool to solve homing problem of the parallel robot.
