Background: Ankylosing spondylitis (AS) is a chronic inflammatory disease that mainly affects the axial skeleton. Meanwhile, copper is a mineral nutrient involved in cell proliferation and death pathways and has been observed in various diseases. Methods: The purpose of this study was to identify potential new biomarkers of ankylosing spondylitis through biomarker analysis and explore its immune cell infiltration analysis. Gene expression profiles of GSE73754, GSE25101 and GSE18781 datasets were retrieved from the GEO database. Three machine learning algorithms, i.e., minimum absolute shrinkage and selection operator ( LASSO ), random forest ( RF ) and support vector machine recursive feature elimination ( SVM-RFE ), were used to screen cuprotosis-related genes ( CRGs ) of ankylosing spondylitis. Besides, ankylosing spondylitis specimens were classified using the consensus clustering method. The three machine learning results were intersected with the CRGs obtained by difference analysis and the differential CRGs obtained by clustering to obtain key genes. Additionally, the infiltration of immune cells was detected by ssGSEA, and the cuprotosis-related genes related to immune cells and immune function were detected by spearman correlation analysis. The nomogram model was established, and the accuracy of cuprotosis-related genes in predicting the development of AS disease was verified by external data sets. Result: The two AS libraries in the GEO database were merged after removing the batch effect. Through the difference analysis, 6 CRGs were obtained, 9 CRGs were obtained by LASSO algorithm, 4 CRGs were obtained by SVM-RFE, and 13 CRGs were obtained by RF. It was also found by clustering the consistency of AS samples that they could be divided into 2 subtypes, and 5 CRGs were obtained as well. Finally, two key genes of SLC31A1 and PDHB were obtained by VENN intersection. The immune infiltration and GSEA analysis showed that PDHB had the strongest positive correlation with T cells CD8, followed by T cells CD4 memory resting, and had the strongest negative correlation with Neutrophils. Besides, SLC31A1 had the strongest positive correlation with NK cells resting and the strongest negative correlation with T cells CD8. By establishing a nomogram model, it was found that the two genes could better predict the development of AS, and the verification of the external data set GSE18781 indicated the stronger predictive ability of PDHB. Conclusion: In summary, the present study revealed several newly discovered cuprotosis-related genes that interfered with the progression of ankylosing spondylitis through immune infiltration, and found the large research space of PDHB.

BACKGROUND AND OBJECTIVES Cafe-au-lait spots, also known as cafe-au-lait macules (CALMs), are a common pigmentary disorder. Although various laser modalities have been used to treat CALMs, the efficacy of laser treatment in children differs from that in adults. We investigated the efficacy, safety, and clinical factors of the treatment of CALMs using Q-switched alexandrite laser (755 nm) therapy in children. METHODS In total, 471 children with CALMs underwent Q-switched alexandrite laser therapy at a treatment interval of 3-12 months. The safety and efficacy of the laser treatment were evaluated by reviewing clinical records and photographs before and after treatments. RESULTS Of the 471 patients, 140 (29.72%) were cured completely, 124 (26.33%) showed substantial improvement, 110 (23.35%) showed improvement, and 97 (20.60%) showed no improvement after one to nine treatments. To explore the clinical efficacy and possible efficacy mechanisms of Governor Vessel moxibustion therapy in patients with carotid atherosclerosis (CAS).A total of 100 patients with CAS were randomly divided into Governor Vessel moxibustion therapy group (After the treatment, the constitution score and BMI were significantly reduced, and the IMT, TC, TG, LDL, Fbg and hs-CRP decreased significantly in the patients of the two groups as compared with those before treatment (The Governor Vessel moxibustion therapy can effectively treat CAS, and its mechanism may be related to the improvement of risk factors such as high level of TC, TG, LDL, Fbg, and hs-CRP.

A male patient with seventy-six years old, frequent urination at night, lassitude, aversion to cold, feeling tired easily on exertion, soreness and weakness in the lumbar region, dark and purplish tongue with white coating, deep, thread and rapid pulse. Diagnosis: nocturnal polyuria. Syndrome differentiation in traditional Chinese medicine: kidney yang deficiency. In treatment, the Du-moxibustion (moxibustion along the Governor vessel) was adopted in the lumbosacral region. A number of cross marks were marked along Zhōngshū (中枢 GV 7) to Yāoshu (腰俞 GV 2). Herbal powder for the Du-moxibustion was put evenly and straightly from GV 7 to GV 2 along the cross marks. The mulberry bark paper was put flatly over the treating region with the folded midline of the paper matching the powder line and the ginger paste was placed on the paper and flatted with the tongue depressor as a trapezium. A groove was pressed on the ginger paste, And moxa was put inside. The upper, middle and lower sites of the moxa were ignited for moxibustion,when the moxa was burnt out then new moxa was replaced. Totally, the moxa was changed 3 times in one treatment. The treatment was given once a week, and 4 treatments as one course. After 2-course Du-moxibustion, the frequency of nocturnal urination was 0–1 time, 0–350 mL a night. The patient had lassitude and fatigue occasionally and had no aversion to cold. The treatment was given once every two weeks on the lumbosacral region until the urine frequency was normal at night. The Du-moxibustion improves in kidney yang deficiency, kidney qi deficiency, post-operative body weakness and the deficiency cold of the lower Jiao. This therapy warms the meridian, promotes qi and blood circulation, shortens the recovery time of the normal circulation of the cerebrospinal fluid and nourishes cauda equina as well as alleviates the pressure to cauda equina due to edema. This therapy effectively treats nocturnal polyuria of kidney yang deficiency after lumbar disc herniation surgery.

Blue rubber bleb naevus syndrome (BRBNS) is an extremely rare venous malformation that often manifests as multiple haemangioma-like lesions in the skin and gastrointestinal tract. The drug sirolimus plays a key role in the signalling pathway of angiogenesis and subsequent development of BRBNS and its use has been described in several case reports. We present a case series of four patients with BRBNS who exhibited good treatment response to sirolimus. All four patients were administered oral sirolimus at doses of 1.0-1.5 mg/m2 /day with a target drug level of 5-10 ng/mL and median treatment duration of 20 months. All patients had a reduction in the size of the lesions and a normalization of coagulopathy with tolerable drug adverse reactions at follow-up. Sirolimus may be effective and safe in paediatric patients with BRBNS. Further prospective studies are suggested to evaluate the long-term effectiveness of this drug.

This meta-analysis compared outcomes between percutaneous pinning/wiring and open reduction internal fixation (ORIF) with locking plates for treatment of unstable distal radius fractures. Medline, Cochrane, EMBASE, and Google Scholar were searched through December 30, 2015. Twenty randomised controlled trials (RCTs) and non-randomised two-arm studies were included. Outcomes included scores of Disabilities of the Arms, Shoulders and Hands (DASH), visual analogue scale (VAS) pain, and patient rated wrist evaluation (PRWE) score, as well as range of motion (ROM) and complication incidence. ORIF/plating was associated with lower DASH scores but longer procedure time, while there was no difference between the two methods with respect to VAS pain score and PRWE score. The overall incidence of complications, including complex regional pain syndrome, was higher with pinning/wiring, though the incidence of carpal tunnel syndrome and nerve defects was not different. Supination and grip strength were better with ORIF. Radiographically, ulnar variation was greater with pinning/wiring. These results suggest that ORIF/plating is the preferred method of managing unstable distal radius fractures.Level of evidence: II

This paper introduces a new type of elastic gear coupling structure, and gives the calculating strength formula and related material of the parts, establishes the three-dimensional model, simulates the nonlinear elastic gear ring, gets the analysis of the distribution of strain and stress in the ANSYS software, verify the strength condition.

According to the control strategy of the supersonic speed target cruise missile and the demand of the actuator, the principle of choosing motor and the method of designing electric actuator were presented. The mathematic model of the electric actuator was built with the actual parameter and the checking method of the dynamic characteristic of the electric actuator was described. With the mathematic model and the actual electric actuator designed, the numerical simulation and the experiment on turret were made. The conclusion showed that the electric actuator designed with the designing and checking method described met the control demand of the target missile.

Thin-walled workpiece is easy to deform in machining. In order to predict the dimensional error of machined surface of thin-walled workpiece, computer simulation technology is studied, which is called virtual machining process. For the simulation of workpiece deformation, much numerical analysis should be done. Research indicates using FEA (Finite Element Analysis) in each step of simulation process needs too much time to meet the requirements of industrial application. Therefore it is important to decrease the simulation time. In this paper, a new method is proposed to realize rapid analysis of workpiece deflection in virtual machining process, which combines rapid analytical solution method with a few times of accurate FEA, thus greatly decreases the time required for whole simulation. For the simulation, several peripheral milling process models are presented with increasing order of sophistication and accuracy, which can be applied to simulate cutting process with the effect of workpiece deformation caused by cutting force. In the final section, the comparison between simulation results and experiments shows the proposed methods and models can closely predict dimensional error and texture of machined surface of flexible workpiece.

A multi-rope hoist steel rope tension on-line monitoring system is introduced in this paper, which consists of hydraulic sensors, data acquisition and sending devices, wireless communication devices and IPC. This system monitors the tension of multi-rope hoist steel ropes, in order to get the actual load of the hoist. When error happens the system can alarm to avoid accidents, such as serious imbalance of the steel ropes and hoist overload. Application of support vector machine (SVM) for large number of catalogs was studied, and the structure of optional SVM decision tree was introduced under the different background. Aiming at the defect recognition problem in X-ray inspection welding images and combining the basic theory of binary-tree, a binary-tree method of multi-class classification based on SVM was put forward. This method adopts `one-against-all' classification algorithm by which binary-tree multi-classifier for welding defects based on SVM was established. According to the characteristics of defects, six parameters were chosen as feature parameters, and familiar defects in the weld were classified into 6 classes. 84 defect samples were used to experiments, and the results show that the classifier possesses simple, intuitionistic and practical algorithm, and small number of repeated training samples.
