The aim of this study was to explore the optimal timing of holmium laser enucleation of the prostate (HoLEP) in patients presenting benign prostatic hyperplasia (BPH) with lower urinary tract symptoms (LUTS). A retrospective analysis was conducted based on the perioperative and postoperative outcome data of 1212 patients who underwent HoLEP in Shanghai Ninth People's Hospital (Shanghai, China) between January 2009 and December 2018. According to the preoperative International Prostate Symptom Score (IPSS), all patients whom we analyzed were divided into Group A (IPSS of 8–18) and Group B (IPSS of 19–35). Peri- and postoperative outcome data were obtained during the 1-year follow-up. IPSS changes were the main postoperative outcomes. The postoperative IPSS, quality of life, peak urinary flow rate, postvoid residual, and overactive bladder symptom score (OABSS) improved significantly. The IPSS improved further in the group with severe LUTS symptoms, but the postoperative IPSS was still higher than that in the moderate LUTS group. OABSSs showing moderate and severe cases after follow-up were more frequent in Group B (9.1%) than in Group A (5.2%) (P < 0.05). There were no significant intergroup differences in the intraoperative American Society of Anesthesiologists or hospitalization expense scores, and the medication costs, as well as the total costs, were significantly higher in Group B. In this retrospective study, HoLEP was an effective treatment for symptomatic BPH. For patients with LUTS, earlier surgery in patients with moderate severity may result in a marginally better 12-month IPSS than that in men with severe symptoms.

The purpose of this study was to promote the holmium laser enucleation of the prostate (HoLEP) technology for large-volume prostates (>80 ml) and compare it with the traditional method, we modified the technique and applied this technology to clinical practice. A retrospective study comprising 118 patients who underwent HoLEP surgery from February 2018 to December 2019 was conducted in our center. Group A consisted of 57 patients with large-volume prostate (>80 ml) who received modified “fenestration and tunnel method” from February 2019 till December 2019, while group B consisted of 61 patients who received the traditional trivalvular operation method from February 2018 to December 2018 for comparison. Control subjects are selected such that they match the cases concerning certain characteristics, and perioperative data, voiding outcomes, and complications were evaluated at 1- and 12-month follow-up. The international prostatic symptomatic score (IPSS), quality of life (QoL), and maximum flow rate (Qmax) at 1 month and 12 months were both significantly improved compared with the preoperative baseline, and no significant differences were found between Groups A and B. The mean enucleation time, operation time, catheter indwelling duration, and hemoglobin decrease during the operation of Group A showed superiority compared with Group B ( p 80 ml), modified HoLEP was suggested to be a better treatment option.

Stereocomplex (SC) crystallization has been an effective way to improve the physical performances of stereoregular polymers. However, the competition between homo and SC crystallizations can lead to more complicated crystallization kinetics and polymorphic crystalline structure in stereocomplexable polymers, which influences the physical properties of obtained materials. Herein, we select the medium-molecular-weight (MMW) poly(L-lactic acid)/poly(D-lactic acid) (PLLA/PDLA) asymmetric blends with different PDLA fractions (fD=0.01–0.5) as the model system and investigate the effects of fD and crystallization temperature (Tc) on the crystallization kinetics and polymorphic crystalline structure. We observe the fractionated (i.e., multistep) crystallization kinetics and the formation of peculiar β-form homocrystals (HCs) in the asymmetric blends under quiescent conditions, which are strongly influenced by both fD and Tc. Precisely, crystallization of β-form HCs is favorable in the MMW PLLA/PDLA blends with high fD (≥0.2) at a low Tc (80–100 °C). It is proposed that the formation of metastable β-form HCs is attributed to the conformational matching between β-form HCs and SCs, and the stronger constrain effects of precedingly-formed SCs in the early stage of crystallization. Such effects can also cause the multistep crystallization kinetics of MMW PLLA/PDLA asymmetric blends in the heating process. [ABSTRACT FROM AUTHOR]

The southern Jiangxi Province is an important part of the fluorite mineralization belt in South China. Fluorite ore bodies are primarily in the contact zone between the Devonian Huitong granitic complex and the Late Cretaceous Ganzhou Formation, controlled by the NE‐trending faults. Zircon U–Pb dating of the Huitong granitic complex yields emplacement ages of 410.7 ± 1.4 Ma and 400.7 ± 4.6 Ma, while the Sm–Nd dating of the fluorite yields an isochron age of 94 ± 2 Ma, suggesting that the Huitong granitic complex is the host rock. Fluid inclusions in the fluorite show low homogenization temperatures (136–207°C), salinities (1.23–3.87 wt% NaCl), and densities (0.87–0.95 g/cm3), suggesting that the ore‐forming fluid is an NaCl‐H2O system of low temperature, salinity, and density. Raman spectroscopy showed that the fluid phase is dominated by water. The δDVSMOW values of the fluid inclusions in the Tongda fluorite ranged between −59.5 and −55.2‰, while the δ18OVSMOW values of the fluorite ranged from −7.2 to −5.6‰. Collectively, the ore‐forming fluid is dominated by meteoric water, possibly with a minor contribution of hydrothermal fluid. Both the interaction with host rocks and the cooling of hydrothermal fluids are the likely mechanisms of underlaying fluorite precipitation at low temperatures. The mineralization occurred in extensional faults during the Late Cretaceous related to the subduction of the Palaeo‐Pacific Oceanic Plate. [ABSTRACT FROM AUTHOR]
