Chronic kidney disease (CKD) and Alzheimer’s disease (AD) are common chronic diseases in the elderly population. Although a relationship between CKD and the occurrence of AD has been proposed, previous research results have been disputed, and further investigation is necessary to confirm this relationship. In this longitudinal follow-up study, we examined data from the Korean National Health Insurance Service-Health Screening Cohort, consisting of 15,756 individuals with CKD and 63,024 matched controls aged ≥40 years who received health check-ups between 2002 and 2019. Overlap-weighted Cox proportional hazard regression models were exploited to calculate hazard ratios (HRs) for the association between CKD and AD. During the monitoring period, individuals with CKD had a greater incidence of AD than those without CKD (15.80 versus 12.40 per 1000 person years). After accounting for various factors, CKD was significantly associated with a 1.14-fold increased likelihood of developing AD, with a 95% confidence interval ranging from 1.08 to 1.20. Epidemiological studies have suggested the role of multiple genetic and environmental factors in the development of non-neoplastic gastrointestinal (GI) diseases; however, little information is available on these factors in the Korean population. Therefore, this cross-sectional study explored the effect of these factors by analyzing the concordance of several benign GI disorders in 525 monozygotic twins compared to that in 122 dizygotic twins aged >20 years from the Healthy Twin Study data of the Korean Genome and Epidemiology Study (2005–2014). Chi-square test, Wilcoxon rank-sum, and binomial and multinomial logistic regression models were used for statistical analysis. There was lack of concordance of gastric/duodenal ulcers and cholelithiasis/cholangitis between monozygotic twins compared to that in dizygotic twins, suggesting that environmental factors may mediate those concordant disease expressions in monozygotic twins. The concordance of intestinal polyps in monozygotic twins was 32% lower than that in dizygotic twins (p = 0.028), indicating that the effect of genetic factors on the risk for intestinal polyp development may be low. In conclusion, the lack or low concordance of several benign GI diseases between monozygotic and dizygotic twin groups suggests the relative importance of environmental factors, indicating that these are preventable diseases.

Objective The present study examined the contribution of panic appraisal (PA) dimensions to the prediction of panic and agoraphobic symptoms, above and beyond anxiety sensitivity (AS), among patients with panic disorder. PA dimensions consist of anticipated panic, perceived panic consequences, and panic coping efficacy. Method Panic disorder patients (N = 84, 60.7% female, M = 38 years) completed self-report measures of panic and agoraphobic symptoms, three dimensions of PA, AS, anxiety, and depression symptoms. Results PA dimensions significantly contributed to the prediction of overall and all facets of panic and agoraphobic symptoms, as well as anxiety symptoms, but not depressive symptoms, above and beyond AS. Of the three PA dimensions, both anticipated panic and panic coping efficacy uniquely contributed to agoraphobic avoidance after controlling for AS and the other dimensions of PA. Conclusion PA dimensions may emerge as an important predictor of panic and agoraphobic or anxiety symptoms.

We examined the factor structure and other psychometric properties of the Social Phobia Inventory (SPIN) in Korean nonclinical (N1 = 457, N2 = 444) and clinical undergraduate samples (N3 = 12). Results support the revised 3-factor structure and applicability of the SPIN as a measure yielding valid and reliable scores of social anxiety to Koreans.

We successfully fabricated a short-length active fiber based on fluorophosphate (FP) laser glass by the rod-in-tube technique and fiber drawing process. The base FP laser glass composition, 20Al(PO3)3-40BaF2-40SrF2 was used for making glass fiber with appropriate core and clad profiles. The physical, thermal, optical, and spectroscopic properties of core and clad materials were optimized in order to implement FP laser glass for active glass fiber fabrication. The difference of refractive index and the glass transition temperature between core and clad glass ingots was obtained to be close to 0.004 and 6 °C, respectively. The spectroscopic properties of FP laser glasses were also derived. The gain and gain coefficient of the indigenously fabricated glass fiber were measured to be 10.29 dB and 0.60 dB/cm at 1550 nm from a 17 cm bare fiber, respectively. The results direct that the short length active fiber based on FP laser glass could be useful for compact all-optical fiber amplifier at 1.5 μm.

An inline power tap has been demonstrated and realized by inscribing an optical track in a standard 62.5/125 μm multimode fiber. The fiber-optic tap may resort to a single track or a pair of tracks, which can be created by incorporating a single instance or double instances of femtosecond laser scanning, respectively. For the single-track configuration, the built-in tap is formed to traverse the center of the core, while a pair of parallel taps is designed to vertically flank the core for the double-track configuration. The tap ratio can be practically tuned from 1.0% to 3.4% by tailoring the laser-writing power in accordance with the configuration of the tap. The hybrid double-track structure helped reduce the polarization sensitivity to as low as 0.8 dB. The excess loss was consistently kept below 0.2 dB.

Positional distortion is a defocusing phenomenon in ultrafast laser inscription of fiber optic circuits induced by the cylindrical geometry of an optical fiber. In this Letter, a study on the positional distortion of ultrafast laser processing assisted by tightly focusing optics has been conducted. Attention has been paid to the effect of numerical aperture (NA) of the focusing optics and location of the laser-writing plane. The occurrence of convex positional distortion that decreased with the NA was observed in an array of laser-inscribed optical tracks when scanning across the fiber. It exhibited a maximum distortion of 28.9 and 23.8 μm in the center plane of the fiber for the 0.42-NA and 0.85-NA dry objective lenses, respectively, but only a negligible positional distortion in the track array written in an off-center plane.

We propose and demonstrate an inline fiber optic power sensor (IFPS) resorting to an embedded waveguide tap, which is formed to traverse across the cladding and core of a standard single-mode fiber. The tap was produced via a single-step inscription based on the femtosecond laser direct-writing method. A tightly focused pulsed laser beam has been particularly exploited to suppress the elongation along the laser propagation direction, thereby improving the cross-sectional symmetry of the created tap waveguide. The fabricated fiber optic tap has been stably combined with a photodiode via a compact package. The achieved tap ratio could be tuned from 1.0% to 5.9% at the wavelength of 1550 nm by adjusting the applied laser power, while the induced excess loss was kept below 0.6 dB. The proposed IFPS will be highly suitable for real-time power monitoring in a variety of applications, including optical communication networks and systems.

11-Deoxydaunomycinone 15 and 10-fluoroanthracyclinone derivatives 9, 10 were obtained. Naphthalenone 4 prepared from 2-(2,4-pentadienyl)-1,3-dioxane 2 with methyl vinyl ketone and hydrolysis with HClO4 was condensed with phthalidesulfone 5 through Michael type reaction, and was converted to 7 by epoxidation. Epoxide 7 was transformed to trione 12 using reduction-oxidation or hydrofluorination process, and then to 15 by introducing several functional groups. Compound 8 obtained in the course of the reaction of epoxide 7 and HF/ Pyr was used for the synthesis of compounds 9, 10.

This study was designed to examine the relation of trait anger and anger expression to blood pressure, cholesterol, and depression in middle-aged Korean women.This descriptive correlational design was conducted using a convenient sample taken from the health center of K University Hospital located in Kyungki province, Korea. The subjects were 252 women aged 40 to 64 years. Spielberger's state trait anger expression inventory--Korean version and Beck's depression inventory were used for measuring trait anger, state anger, anger expression and depression. Data was analyzed by descriptive statistics, t-test, Pearson correlation, two-way ANOVA, and cluster analysis using a pc-SAS program.The anger expression types by cluster analysis were Anger out/in type, Low anger expression type, and Anger control type. The level of cholesterol and depression were significantly higher in women with high anger in and high trait anger. In addition, the level of depression was significantly higher in women with a high anger temperament.Trait anger and anger in might be related to cholesterol and depression in women. However, this study does not reveal the relation between blood pressure and trait anger and anger expression.

Objectives: It is suspected that there is a large number of patients suffering from hand–arm vibration syndrome (HAVS) in Korea. However, no cases have been reported since 1992. This study was conducted to identify HAVS cases and determine the characteristics of the syndrome. Methods: In April 2001, the Busan, Ulsan, and Gyeong-Nam Province Occupational Disease Surveillance System (BUGODSS) was established to identify work-related HAVS and other occupational diseases. In the 2 years of this project, occupational physicians from five hospitals in these provinces collected information by way of interviews and questionnaires during mandatory occupational medical examinations. Among the initial 189 suspected HAVS cases, 58 volunteers were given cold-water provocation tests in order to diagnose the vascular component of the disorder. Results: One hundred fifty-four approximately occupational HAVS cases were identified from ca. 21,000 workers. One hundred fifty about of these cases were male. The cases were most often found in workers from the shipbuilding industry, and the grinder was the most common source of vibration exposure. Cases of sensorineural disorder (SD) were more common than cases of vascular disorder (VD). The mean values of the finger skin temperature and its recovery rate at 5 min and 10 min after cold-water provocation were significantly lower in the group with the VD than in the group with the SD. Conclusions: We identified 154 occupational HAVS cases, although no cases have been reported during the occupational medical examinations mandated by the state. The majority of the cases were in workers that used grinders in the shipbuilding industry. We determined that peripheral VD and peripheral SD can progress independently of each other. We conclude that exposure to hand-transmitted vibration (HTV) and HAVS cases are common in shipbuilding industry in Korea. The recovery rate of finger skin temperature after cold-water provocation is one of the useful methods for diagnosing the vascular component of HAVS.

A 25-year-old man, a field operator in a petroleum refinery was found unconscious. He was exposed to hydrogen sulfide and presented with Glasgow Coma Score of 5, severe hypoxemia on arterial blood gas analysis, normal chest radiography, and normal blood pressure. On hospital day 7, his mental state became clear, and neurologic examination showed quadriparesis, profound spasticity, increased tendon reflexes, abnormal Babinski response, and bradykinesia. He was also found to have decreased memory, attention deficits and blunted affect, which suggested general cognitive dysfunction, but which improved soon. MRI scan showed abnormal signals in both basal ganglia and motor cortex, compatible with clinical findings of motor dysfunction.

We examined whole blood (MnB), plasma (MnP) and urinary Mn (MnU) concentrations in 33 cirrhotics and 11 healthy controls to clarify: (1) whether, in chronic liver diseases, MnB or MnP reflects pallidal signal intensities in magnetic resonance imaging (MRI); and (2) which factors in chronic liver diseases correlate with pallidal signal intensities in T1-weighted MRI. Increased signal intensity in the pallidum was observed in 27 (81.8%) of 33 patients with liver cirrhosis in T1-weighted MRI. There was a significant correlation between MnB and pallidal index (PI) (gamma = 0.559, P0.01) in the patients. However, no significant correlation was observed between MnP and PI (gamma = 0.353, P0.05). According to a multiple linear regression, MnB reflected the signal intensities of T1-weighted MRI better than MnP or MnU. Child/Pugh score and total bilirubin level also correlated with PI. However, the hemoglobin level did not correlate with PI significantly.

We have employed an estrogen receptor dependent transcriptional expression assay and E-Screen assay systems to evaluate the estrogenicity of various heavy metals and their species. Using the former, the following estrogenicity ranking was measured: bis(tri-n-butyltin)>cadmium chloride>antimony chloride>barium chloride=chromium chloride>lithium hydroxide>sodium selenate=lead acetate>stannous chloride. Using the latter, the following estrogenicity ranking was measured: bis(tri-n-butyltin)>cadmium chloride>antimony chloride>lithium hydroxide>barium chloride>sodium selenate>chromium chloride. Especially, bis(tri-n-butyltin), cadmium chloride, antimony chloride, lithium hydroxide, barium chloride, and chromium chloride showed estrogenicity in both assay systems. Recent studies suggesting that bis(tri-n-butyltin), cadmium chloride, and lithium hydroxide have estrogenicities are compatible with the present findings. Furthermore, our studies are the first to suggest that antimony, barium, chromium may be estrogenic. A range of estrogenicity was observed for different species of the same heavy metal. The results demonstrate that an estrogen receptor dependent transcriptional expression assay and the E-Screen assay systems could serve as a useful method to assess the estrogenicity of heavy metals.
