Objective To study the effectiveness and safety of ultrasound-guided pectoral nerve block combined with no background patient controlled intravenous analgesia (PCIA) in postoperative pain control of partial mastectomy patients. Methods A total of 60 patients who underwent elective partial mastectomy at Pukou Traditional Chinese Medicine Hospital Affiliated to China Pharmaceutical University from May 2022 to May 2023 were selected and divided into group M and group N, with 30 cases in each growp. Both groups received no background PCIA. Patients in M group received 0.2% ropivacaine 30 mL for pectoral nerve block, while patients in group N did not receive any other treatment. The outcome measures were VAS scores during static and coughing at the time point of patient leaving resuscitation room (T0), 4 hours (T1), 12 hours (T2), 24 hours (T3), and 48 hours (T4) after surgery, as well as the total analgesia consumption and rescue analgesia dosage. Results Patients in group M showed a lower rest VAS at T0, T1, T2, and T3 time point (P＜0.05), and lower cough VAS at T0, T1, and T2 time point than patients in group N (P＜0.05). In group M, the total intravenous analgesia consumption and number of presses were significantly less than group N at 48 hours after surgery (P＜0.05). There was no statistically significant difference in the incidence of postoperative adverse reactions between the two groups (P＞0.05), and there was no complication related to nerve block in group M. Conclusion Ultrasound-guided pectoral nerve block combined with no background PCIA is able to provide potent postoperative pain control, reduce analgesia consumption in partial mastectomy patients.

Abstract Background Mesenchymal stem cells (MSCs) have been considered to be the attractive vehicles for delivering therapeutic agents toward various tumor diseases. This study was to explore the distribution pattern, kinetic delivery of adenovirus, and therapeutic efficacy of the MSC loading of E1A mutant conditionally replicative adenovirus Adv-Stat3(-) which selectively replicated and expressed high levels of anti-sense Stat3 complementary DNA in breast cancer and melanoma cells. Methods We assessed the release ability of conditionally replicative adenovirus (CRAd) from MSC using crystal violet staining, TCID50 assay, and quantitative PCR. In vitro killing competence of MSCs carrying Adv-Stat3(-) toward breast cancer and melanoma was performed using co-culture system of transwell plates. We examined tumor tropism of MSC by Prussian blue staining and immunofluorescence. In vivo killing competence of MSCs carrying Adv-Stat3(-) toward breast tumor was analyzed by comparison of tumor volumes and survival periods. Results Adv-Stat3(-) amplified in MSCs and were released 4 days after infection. MSCs carrying Adv-Stat3(-) caused viral amplification, depletion of Stat3 and its downstream proteins, and led to significant apoptosis in breast cancer and melanoma cell lines. In vivo experiments confirmed the preferential localization of MSCs in the tumor periphery 24 hours after tail vein injection, and this localization was mainly detected in the tumor parenchyma after 72 hours. Intravenous injection of MSCs carrying Adv-Stat3(-) suppressed the Stat3 pathway, down-regulated Ki67 expression, and recruited CD11b-positive cells in the local tumor, inhibiting tumor growth and increasing the survival of tumor-bearing mice. Conclusions These results indicate that MSCs migrate to the tumor site in a time-dependent manner and could be an effective platform for the targeted delivery of CRAd and the amplification of tumor killing effects.

Abstract Background Elevated Plexin-B1 expression has been found in diverse human cancers and in non-neoplastic tissues, and it mediates diverse biological and pathological activities. However, whether or not Plexin-B1 expression is involved in human ovarian tumors remains unclear. In the present study, Plexin-B1 expression was explored in benign and malignant human ovarian tumor tissues. In addition, the impact of Plexin-B1 expression on ovarian cancer cell proliferation, migration and invasion were investigated in vitro. Methods Plexin-B1 expression was analyzed in normal and benign ovarian tissues and serous ovarian tumors (both borderline and malignant) by immunohistochemical staining, as well as in four human ovarian cancer cell lines (A2780, C13*, SKOV3, and OV2008) by RT-PCR and western blot analyses. Furthermore, endogenous Plexin-B1 expression was suppressed by Plexin-B1 siRNA in SKOV3 cells, which overexpress Plexin-B1. Protein levels of Plexin-B1, AKT and AKTSer473 were examined by western blot analysis. Cell proliferation, migration and invasion were measured with MTT, wound healing and boyden chamber assays, respectively, and the cytoskeleton was monitored via F-actin staining. Results Expression levels of Plexin-B1 protein were significantly higher in serous ovarian carcinomas than in normal ovaries or benign ovarian neoplasms, and in the former, Plexin-B1 expression was positively correlated with lymphatic metastasis, and the membrane and cytoplasm of cancer cells stained positively. SKOV3 cells displayed the highest Plexin-B1 expression at both the mRNA and protein levels among the four tested human ovarian cancer cell lines and was selected as a cell model for further in vitro experiments. Plexin-B1 siRNA significantly suppressed phosphorylation of AKT at Ser473 in SKOV3 cells, but it did not alter total AKT expression. In addition, silencing of Plexin-B1 in SKOV3 cells inhibited cell migration and invasion and reorganized the cytoskeleton, whereas cell proliferation was not affected. Conclusion Plexin-B1 expression correlates with malignant phenotypes of serous ovarian tumors, probably via phosphorylation of AKT at Ser473, suggesting that Plexin-B1 might be a useful biomarker and/or a novel therapeutic target.

Objective To evaluate ultrasound guided cervical nerves pathway block (CNPB) for internal jugular vein cannulation in awake patients. Methods A total of 100 patients who underwent elective jugular vein puncture and catheterization at Nanjing Pukou District Hospital of Chinese Medicine from June to December 2021 were selected and divided into ultrasound guided CNPB group (group B, n=50) and local infiltration anesthesia group (group C, n=50). Both groups underwent internal jugular vein catheterization under ultrasound guidance. The outcome indicators were the Numeric Rating Scales (NRS), Bruggrmann Comfort Scale (BCS), hemodynamic parameters, sleeping quality, and the complications. The results were recorded in the morning of the cannulation operation (T0), during the operation (T1), 5 min after the cannulation (T2), that night of the cannulation (T3), postoperative day 1 (T4), postoperative day 2 (T5). Results The NRS of group B were significantly lower than that of group C at T1 and T3. The BCS of group B were higher than group C at T1, T2, T3, and T4. The heart rate and systolic blood pressure at T1 in group B were dramatically lower than group C. At T3 and T4, sleeping quality of group B was better than group C (P＜0.05). Conclusion Ultrasound guided CNPB, as the anesthesia for internal jugular vein cannulation in awake patients, can effectively reduce the NRS scaling and improve the BCS and sleeping quality without obvious adverse reactions.

Abnormal lymphangiogenesis is associated with several diseases such as tumor metastasis and lymphangioma. Human lymphangioma originated from the transformation of lymphatic endothelium is a benign malformation of lymphatic vessels and its pathogenesis has up to date not been illuminated and its cell model has also not been established. An optimized method was used to isolate lymphatic endothelial cells from human glossal lymphangioma (GL-LECs) and GL-LECs were further primarily cultured and expanded. GL-LECs were of typical cobblestone appearance when they reached confluence. The weible-palade body was observed in the GL-LECs cytoplasm. Almost all GL-LECs were strongly positive for specific lymphatic markers FLT-4, LYVE-1 and prox-1 by immunocytochemistry. Furthermore, three-dimension tube-like capillaries of GL-LECs resembled the lymphatic system in vivo, and the GL-LECs spheroids sprouted radically out to form three-dimensional buds when embedded in the cultured BME. These results indicated that high purity GL-LECs were successfully isolated and expanded. They had the abilities of tube formation and differentiation in vitro, which provide a favorable cell model for further uncovering the pathogenesis of human lymphangiomas.
