1. Single-use suvorexant for treating insomnia during overnight polysomnography in patients with suspected obstructive sleep apnea: a single-center experience

The aim of this study was to investigate the safety and efficacy of a new hypnotic agent, suvorexant, a dual orexin receptor antagonist, for insomnia in suspected OSA patients during in-laboratory PSG.Patients and methods: An observational study was conducted during PSG for 149 patients with suspected OSA who had no insomnia at home. Patients with difficulty in falling asleep during PSG were optionally permitted to take single-use suvorexant. Patients with residual severe insomnia (>1 hour) after taking suvorexant were permitted to take an add-on use zolpidem. Clinical data and sleep questionnaire results were analyzed between a no insomnia group (without hypnotics) and an insomnia group (treated with suvorexant).Results: Among 84 patients who experienced insomnia during PSG and required hypnotics (the insomnia group; treated with suvorexant), 44 (52.4%) achieved sufficient subjective sleep with single-use of suvorexant, while the other 40 (47.6%) required suvorexant plus zolpidem. An apnea hypopnea index (AHI) of ≥5 was observed in 144 out of 149 patients with predominantly obstructive respiratory events. Among those patients, 70.8% in the no insomnia group and 63.1% in the insomnia group had severe OSA. Regarding both subjective sleep time and morning mood, significant differences between the no insomnia group and the insomnia group were not observed. No patient taking suvorexant had an adverse event, such as delirium or falling.Conclusion: Single-use suvorexant seems to be a safe and effective (but mild) hypnotic agent for suspected OSA patients with insomnia during in-laboratory PSG. Abstract

River ecosystems receive and process vast quantities of terrestrial organic carbon, the fate of which depends strongly on microbial activity. Variation in and controls of processing rates, however, are poorly characterized at the global scale. In response, we used a peer-sourced research network and a highly standardized carbon processing assay to conduct a global-scale field experiment in greater than 1000 river and riparian sites. We found that Earth’s biomes have distinct carbon processing signatures. Slow processing is evident across latitudes, whereas rapid rates are restricted to lower latitudes. Both the mean rate and variability decline with latitude, suggesting temperature constraints toward the poles and greater roles for other environmental drivers (e.g., nutrient loading) toward the equator. These results and data set the stage for unprecedented “next-generation biomonitoring” by establishing baselines to help quantify environmental impacts to the functioning of ecosystems at a global scale.

15. Variation of iron redox kinetics and its relation with molecular composition of standard humic substances at circumneutral pH

Oxidation and reduction kinetics of iron (Fe) and proportion of steady-state Fe(II) concentration relative to total dissolved Fe (steady-state Fe(II) fraction) were investigated in the presence of various types of standard humic substances (HS) with particular emphasis on the photochemical and thermal reduction of Fe(III) and oxidation of Fe(II) by dissolved oxygen (O2) and hydrogen peroxide (H2O2) at circumneutral pH (pH 7-8). Rates of Fe(III) reduction were spectrophotometrically determined by a ferrozine method under the simulated sunlight and dark conditions, whereas rates of Fe(II) oxidation were examined in air-saturated solution using luminol chemiluminescence technique. The reduction and oxidation rate constants were determined to substantially vary depending on the type of HS. For example, the first-order rate constants varied by up to 10-fold for photochemical reduction and 7-fold for thermal reduction. The degree of variation in Fe(II) oxidation was larger for the H2O2-mediated reaction compared to the O2-mediated reaction (e.g., 15- and 3-fold changes for the former and latter reactions, respectively, at pH 8). The steady-state Fe(II) fraction under the simulated sunlight indicated that the Fe(II) fraction varies by up to 12-fold. The correlation analysis indicated that variation of Fe(II) oxidation is significantly associated with aliphatic content of HS, suggesting that Fe(II) complexation by aliphatic components accelerates Fe (II) oxidation. The reduction rate constant and steady-state Fe(II) fractions in the presence of sunlight had relatively strong positive relations with free radical content of HS, possibly due to the reductive property of radical semiquinone in HS. 35. Distributed model of hydrological and sediment transport processes in large river basins in Southeast Asia.

Abstract

Soil erosion and sediment transport have been modeled at several spatial and temporal scales, yet few models have been reported for large river basins (e.g., drainage areas > 100 000 km²). In this study, we propose a process-based distributed model for assessment of sediment transport at a large basin scale. A distributed hydrological model was coupled with a process-based distributed sediment transport model describing soil erosion and sedimentary processes at hillslope units and channels. The model was tested on two large river basins: the Chao Phraya River Basin (drainage area: 160 000km²) and the Mekong River Basin (795 000 km²). The simulation over 10 years showed good agreement with the observed suspended sediment load in both basins. The average Nash-Sutcliffe effciency (NSE) and average correlation coefifcient (r) between the simulated and observed suspended sediment loads were 0.62 and 0.61, respectively, in the Chao Phraya River Basin except the lowland section. In the Mekong River Basin, the overall average NSE and r were 0.60 and 0.78, respectively. Sensitivity analysis indicated that suspended sediment load is sensitive to detachability by raindrop (k) in the Chao Phraya River Basin and to soil detachability over land (Kf) in the Mekong River Basin. Abstract

Tonle Sap Lake (TSL) in Cambodia is the largest shallow lake in Southeast Asia. Influenced by flood pulse system of the Mekong River, TSL provides diverse benefits including ecosystem services, ecological functioning, and flood water storage in the floodplains. However, extreme events (e.g., flooding) due to rising water level caused by dam break and/or heavy rainfall in the Mekong River Basin could threaten the ecosystems of the lake, community health and economic growth in the region. Flood mapping under such extreme event could be informative in the flood risk and emergency management. In this study, we aim to develop a flood risk boundary map in TSL using an existing 2D hydrodynamic model (Caesar-Lisflood, CL) with rising water levels estimated by Gumbel distribution. As a result, the extreme water level of 1% chance (or 100-year flood return period) exceeding the annual maximum water level at Prek Kdam station was approximately 11.38 m resulting in the largest inundation area of 15193 km2. Overall, the employed method and flood risk mapping are useful for the decision makers to manage flood risks and emergency in the lake. This is to anticipate the consequences of a possible rising water level by an extreme event.

49. Cloning and characterization of a novel oxidoreductase KDRF from a human bone marrow-derived stromal cell line KM-102.

Abstract

A cDNA clone coding for a novel oxidoreductase was cloned from a human bone marrow-derived stromal cell line KM-102. We screened a cDNA library constructed from the mRNA of KM-102 cells stimulated with phorbol 12-myristate 13-acetate and calcium ionophore A23187 using a 32P-labeled 15-mer synthetic oligonucleotide (5'-TAAATAAATAAATAA-3') probe. This probe was designed as a complementary sequence to the three reiterated AUUUA sequences, which are contained in the 3'-untranslated regions of cytokine and some proto-oncogene mRNAs and correlate with rapid mRNA turnover. Then, we obtained one cDNA clone, and further sequence analysis revealed that it coded for a new protein exhibiting 30 to approximately 40% homology with glutathione reductase. By fusion protein analysis, this protein showed reducing activities on 2, 6-dichlorophenol-indophenol and 5,5'-dithio-bis(2-nitrobenzoic acid) but only a weak reducing activity on oxidized glutathione. Although it lacked a stretch of hydrophobic amino acids in its N terminus, it was secreted by monkey kidney-derived COS-1 cells when we introduced the expression plasmid into them and also secreted by a human lung carcinoma cell line A549. Northern blot analysis revealed that the mRNA turnover of this protein was regulated by inflammatory stimuli in KM-102 cells. These results show that this protein may have scavenging enzyme properties and has its mRNA expression regulated in a similar fashion to cytokine genes or proto-oncogenes. Thus, we named it KDRF (KM-102-derived reductase-like factor), and KDRF may play a role in scavenging reactive oxygen intermediates, which are possibly toxic to cells, in response to inflammatory stimuli.
