- Abstract
Aims: Treatment guidelines with respect to unspecified anxiety disorder have not been published. The aim of this study was to develop a consensus among field experts on the management of unspecified anxiety disorder. Methods: Experts were asked to evaluate treatment choices based on eight clinical questions concerning unspecified anxiety disorder using a nine-point Likert scale (1 = “disagree” to 9 = “agree”). According to the responses from 119 experts, the choices were categorized into first-, second-, and third-line recommendations.aResults: Benzodiazepine anxiolytic use was not categorized as a first-linearecommendationafor the primary treatment of unspecified anxiety disorder, whereas multipleanonpharmacological treatment strategies, including coping strategies (7.9 ± 1.4), psychoeducationafor anxiety (7.9 ± 1.4), lifestyle changes (7.8 ± 1.5), and relaxation techniquesa(7.4 ± 1.8), were categorized as first-linearecommendations. Various treatmentastrategies were categorized as first-linearecommendations when a benzodiazepineaanxiolytic drug did not improve anxiety symptoms, that is, differential diagnosisa(8.2 ± 1.4), psychoeducation for anxiety (8.0 ± 1.5), coping strategies (7.8 ± 1.5), lifestyleachanges (7.8 ± 1.5), relaxation techniques (7.2 ± 1.9), and switching to selectiveaserotonin reuptake inhibitors (SSRIs) (7.0 ± 1.8). These strategies were also highly endorsedawhen tapering the dosage of or discontinuing benzodiazepine anxiolytic drugs.aThere was no first-linearecommendation regarding excusable reasons for continuingabenzodiazepine anxiolytics. Conclusions: The field experts recommend that benzodiazepine anxiolytics should not be used as a first-line option for patients with unspecified anxiety disorder. Instead, several nonpharmacological interventions and switching to SSRIs were endorsed for the primary treatment of unspecified anxiety disorder and as alternatives to benzodiazepine anxiolytics.

本研究の目的は、A 氏の語るライフストーリーから、がんを持ちながら生きる長崎原爆被爆高齢者の「健 康観」を明らかにすることである。長崎で原爆被爆体験のある高齢者で、がんを持ちながら生活している A 氏を研究参加者とした。結果、A 氏のライフストーリーで語られた「健康観」のアウトラインは【感情 を凍結させることで正常を保つ】【がんに対する拭えない予期不安】【子や孫への健康への影響を心配する】 【原爆で死ななかった意味を問いながら生きる】の4 つで構成された。またA 氏のライフストーリーの中 で一貫して語られていた「健康観」のテーマは、『生かされながら生きる』であった。A 氏の「健康観」は、 被爆といったあまりにも衝撃的な出来事から、【感情を凍結させることで正常を保つ】といった防衛機能を 働かせる中で、放射線の影響による【がんに対する拭えない予期不安】【子や孫への健康への影響を心配す る】ことを体験しながら、【原爆で死ななかった意味を問いながら生きる】といった、生きる意味の問いに つながるものであったことが示唆された。そして、それらは一貫して『生かされながら生きる』というこ とを軸に意味づけられていたと考える。

本研究の目的は、緩和ケア実習における看護学生の学びを明らかにし、慢性期・終末期実習への教育の示唆を得ることである。成人看護学実習を履修した学生のうち研究参加に同意が得られた45名のレポートを分析対象とした。結果、【尊厳を支えるケアの必要性】【緩和ケアを受ける患者・家族の特徴をふまえた支援の必要性】【残された時間を有意義に過ごすためのケア】【死後のグリーフケアを見据えた支援の必要性】【緩和ケア看護に必要な知識・技術】【看護過程をふまえたケアの大切さ】【多職種連携の必要性と看護師の役割の認識】【看護の実際を観ることで既習知識が腑に落ちる】の8カテゴリが抽出された。学生は緩和ケア実習の中で、尊厳や根拠に基づいた看護実践という、看護の基本となるものを経験し学びに繋げていた。今後は、既習知識が自らの学びとして「腑に落ちる」経験を積み重ねていくために、臨床で行われる看護場面への参加の仕方や、臨床看護師も交えたリフレクションやクリティカルシンキングができる場を増やすなど、経験の中の学びを深める具体的な方法の検討が必要であると考える。

本研究の目的は、成人看護学の看護過程事例を用いたシナリオシミュレーション演習における学生の学習経験を明らかにし、教授方法への示唆を得ることである。シナリオシミュレーション演習実施後の看護学科3年次生15名に半構造化面接を行い、質的帰納的に分析した。結果、9つのカテゴリが形成された。学生は、紙面事例を患者シミュレータで観察することで、【看護過程事例と患者シミュレータがつながる現実感】や【看護過程展開とシミュレーションでんお観察項目のつながり】【変化を予測した観察や対処への備えの必要性】を実感し、訪室前の情報と観察した情報からアセスメントする重要性や確かな実践力の必要性の理解に至っていた。また【医療者の言動が患者の気持ちに影響を与えることの理解】という患者に配慮した対応の必要性、さらに、【グループでの発見や改善点の気づき】などの情報共有や連携の必要性を学習経験として見出していた。看護過程事例を用いたシナリオシミュレーション演習は、学生の臨地実習の前段階として有用な学習方法であることが示唆された。

Identifying the host genetic factors underlying severe COVID-19 is an emerging challenge. Here we conducted a genome-wide association study (GWAS) involving 2, 393 cases of COVID-19 in a cohort of Japanese individuals collected during the initial waves of the pandemic, with 3, 289 unaffected controls. We identified a variant on chromosome 5 at 5q35 (rs60200309-A), close to the dedicator of cytokinesis 2 gene (DOCK2), which was associated with severe COVID-19 in patients less than 65 years of age. This risk allele was prevalent in East Asian individuals but rare in Europeans, highlighting the value of genome-wide association studies in non-European populations. RNA-sequencing analysis of 473 bulk peripheral blood samples identified decreased expression of DOCK2 associated with the risk allele in these younger patients. DOCK2 expression was suppressed in patients with severe cases of COVID-19. Single-cell RNA-sequencing analysis (n = 61 individuals) identified cell-type-specific downregulation of DOCK2 and a COVID-19-specific decreasing effect of the risk allele on DOCK2 expression in non-classical monocytes. Immunohistochemistry of lung specimens from patients with severe COVID-19 pneumonia showed suppressed DOCK2 expression. Moreover, inhibition of DOCK2 function with CPYPP increased the severity of pneumonia in a Syrian hamster model of SARS-CoV-2 infection, characterized by weight loss, lung oedema, enhanced viral loads, impaired macrophage recruitment and dysregulated type I interferon responses. Background: Stuttering is a childhood‐onset fluency disorder. Part of the counseling for middle and high school students with persistent stuttering is related to school refusal. Anxiety disorders are known to contribute to school refusal. However, it is not known whether social anxiety disorder (SAD) is a factor in school refusal among adolescents who stutter. Methods: In our first study, we examined the relationship between school refusal and SAD in 84 middle and high school students who stutter; 26% of the 84 students were in the school refusal group and the remaining 74% were in the school attendance group. The second study examined whether SAD was associated with 10 factors related to speech and stuttering frequency using the Japanese version of the Liebowitz Social Anxiety Scale for Children and Adolescents to determine the presence of SAD. Of the 84 students in the first study, 40 participated in the second study. Results: The school refusal group of adolescents who stutter had significantly higher rates of SAD than the school attendance group. Fifty percent of adolescents who stutter met the criteria for SAD. Moreover, adolescents who stutter with SAD had significantly higher scores on the items "When speaking in public, do you experience tremors in your limbs?" and "After you stutter, do you have negative thoughts about yourself?" than the adolescents who stutter without SAD. Conclusions: When examining adolescents who stutter, checking for comorbid SAD may lead to better support. Moreover, noticing their repetitive negative thinking, nervousness, and trembling during speech may help to resolve SAD. [ABSTRACT FROM AUTHOR]

食の制限が必要である透析患者の旅行時の外食に対する困難と対処、長崎県における食のユニ バーサルツーリズムに対する提案を明らかにすることを目的として調査を実施した。 データの収集は半構造化面接を実施し内容分析を行った。研究対象は長崎への旅行経験のある透 析患者13名であった。結果、透析患者が抱く外食時の困難は、【味が濃く、量・塩分・リンやカル シウムの制限が難しい】【言っても無駄だと思うし面倒くさく言いにくい】などの9つのカテゴリが 抽出された。また外食時の対処は、【腎臓病食を基本にメニューや食べ方で工夫する】【経験から食 事制限の方法は身についている】などの8つのカテゴリが抽出された。さらに、患者自身が考える 提案は、【対応が可能なことがわかる表示の工夫】【塩分、リン、カリウムなどへの対応をしてほし い】【食への対応の提案を情報発信することが長崎の観光の魅力となる可能性】などの9つのカテゴ リが抽出された。これらの結果より、透析患者は様々な困難を感じながらも、旅行時の「食」が楽 しめる工夫をしていたが、個々人の対処だけでなく、食を含めた観光を楽しめる取り組みが長崎県 としても必要不可欠であることが示唆された。

『看護学実習の統合』科目を通して、臨地実習の中間期と終了期に学生が抱く課題を明らかにす ることを目的とした。対象は看護学科3 年次生62名であり、『看護学実習の統合』科目において、 臨地実習中間期と終了期にグループワークで抽出された課題をデータとし、時期ごとに内容分析を 行った。結果、臨地実習中間期と終了期において【知識・技術・経験の不足】、【看護展開能力の不 足】、【コミュニケーションの困難】、【充実したカンファレンスの運営困難】、【自己管理が困難】、 【実習環境への適応困難】、【自尊心の低下】の7 つの同じ課題が抽出された。サブカテゴリには変 化がみられ、臨地実習中間期に比べて終了期の方が具体的な課題内容を掲げていた。本科目におい て、学生が臨地実習やコミュニケーションに対する不安な気持ちを表出したり、体験した場面を振 り返り共有することにより、ストレス軽減や課題の明確化につながったと考える。しかし、個々の 学生が体験した現象を深め、意味づけを行うという点はやや不十分であった。今後は、学生の成長 を明らかにできるような授業展開方法を検討する必要がある。

本研究の目的は、学生が手術後の患者の観察を行う場面で、看護師の関わりをどのように認知している のかを明らかにし、学習環境調整に向けた示唆を得ることである。成人看護学実習において、看護師と共 に手術後の患者の観察を行った学生20名に、半構造化面接を行い、質的統合法(KJ法)を用いて分析した。 その結果、学生が認知する術後観察場面での看護師の関わりは、【看護師の観察技術が模範となる】、 【看護師の見守りと誘導で安心できる】、【看護師の質問や指導により学びが充実する】、【自分からの働き かけが看護師の指導を引き出す】、【看護師が求めるレベルと自分に差がある】、【看護師から十分な指導 を得られない】の6つが抽出された。 結果より、成人看護学実習における学習環境調整に向けた示唆が得られた。教員は看護師に、学生の レディネスと到達目標を伝え指導内容を調整する。また看護師が学生に質問や指導を行い、学生の学習 が促進されるよう、教員は調整を行う。そして教員は学生に対し、自らの意思や判断を臆せず看護師に 伝えるよう励ますことも必要である。

内部障害者が旅行先で安心して飲食をするためには、「食に関するバリアフリー化を目指した観 光地づくり」の推進が重要となる。そこで、外食・宿泊施設での食事提供に関する現状と課題を 明らかにすることを目的として、長崎市内の関連施設を対象に、腎疾患患者に対してより適切な 「食」を提供するためのアンケート調査と半構造化面接によるインタビューを実施した。食事提供 をする上で個別対応数が最も多かったのは、修学旅行などでの喫食者の1 割を占める食物アレル ギー対応だった。腎疾患患者は、直接施設側へ食事内容への要望が少なく、個別対応率は低かっ た。近年は健康的な食事を臨機応変に提供することへの理解が進み、予め要望を伝えればできる限 り対応したいという意見が多く、食事提供に対する心構え、使命感が高かった。その一方で、「食 のバリアフリー化」を推進するためには、疾病について知識を持った人の配置の検討が必要で、対 応する内容が複雑化すると、ヒューマンエラーや責任問題が生じる課題が示された。サービスをシ ステム化し、対応範囲を患者側と食事提供側で検討する必要がある。今後は、実現可能な食のバリ アフリー化を目指し、患者と食事提供施設の互いの立場を尊重し、対応可能な範囲を可視化して示 すことが重要と考えられた。

- Abstract
Identifying the host genetic factors underlying severe COVID-19 is an emerging challenge1–5. Here we conducted a genome-wide association study (GWAS) involving 2,393 cases of COVID-19 in a cohort of Japanese individuals collected during the initial waves of the pandemic, with 3,289 unaffected controls. We identified a variant on chromosome 5 at 5q35 (rs60200309-A), close to the dedicator of cytokinesis 2 gene (DOCK2), which was associated with severe COVID-19 in patients less than 65 years of age. This risk allele was prevalent in East Asian individuals but rare in Europeans, highlighting the value of genome-wide association studies in non-European populations. RNA-sequencing analysis of 473 bulk peripheral blood samples identified decreased expression of DOCK2associated with the risk allele in these younger patients. DOCK2expression was suppressed in patients with severe cases of COVID-19. Single-cell RNA-sequencing analysis (n= 61 individuals) identified cell-type-specific downregulation of DOCK2and a COVID-19-specific decreasing effect of the risk allele on DOCK2expression in non-classical monocytes. Immunohistochemistry of lung specimens from patients with severe COVID-19 pneumonia showed suppressed DOCK2 expression. Moreover, inhibition of DOCK2 function with CPYPP increased the severity of pneumonia in a Syrian hamster model of SARS-CoV-2 infection, characterized by weight loss, lung oedema, enhanced viral loads, impaired macrophage recruitment and dysregulated type I interferon responses. We conclude that DOCK2 has an important role in the host immune response to SARS-CoV-2 infection and the development of severe COVID-19, and could be further explored as a potential biomarker and/or therapeutic target.

長崎県立大学シーボルト校看護栄養学部看護学科では、「看護技術学習ノート」を作成し、平成 18年度に初版、平成20年度に第 2 版を発行した。今回、第 2 版を使用した学生が平成24年度に卒業 したことを受けて、第 2 版を用いて学生が自己評価した達成度をもとに、技術の習得状況を、平成 21年度・22年度入学生の 2 学年分について分析したので報告する。なお、本報告は、第 1 部は看護 技術学習ノートについての説明、第 2 部が、2 学年分の習得状況の分析の 2 部構成とした。 卒業時点での看護技術の習得状況は、261項目のうち、141項目(54%)が 3 点以上の「習得でき た」項目であった。到達水準のⅠ、Ⅱ、Ⅲ、Ⅳの順に到達度は低くなり、難易度の高い技術の達成 度が低かった。達成度の差は、臨地実習での技術項目の経験の差や学生の評価基準の理解不足によ るものが考えられた。評価時期による到達度の推移は、1 回目から 6 回目にかけて順調に達成度が 高まっていた。特に、臨地実習後に大きく高まり、水準Ⅳは国家試験の学習によって知識の習得が 進むことで卒業時まで高まっていた。すべての技術項目で十分なレベルまで達成するために、今以 上に教育環境を整えること、看護技術学習ノート改訂の必要性が示唆された。

本大学の看護基礎教育における看護師養成に特化したカリキュラム改正に伴い、看護技術学習 ノートの第 3 版改訂を行った。看護技術項目は、1 )保健師関連の科目削減に伴う技術項目の削除、 2 )新設科目に関連する技術項目の追加、さらに、3 )技術項目の卒業時の到達水準および教授領 域を見直した。本学科の看護技術に対する考え方および看護技術の 5 つの枠組み、【Ⅰ.看護実践 に共通する技術】、【Ⅱ.日常生活にかかわる援助技術】、【Ⅲ.診断・治療にかかわる援助技術】、 【Ⅳ.健康生活維持にかかわる援助技術】、【Ⅴ.看護システムにかかわる技術】と看護技術の到達 水準Ⅰ~Ⅳのレベルの内容は、看護技術学習ノート第 3 版においても変更はなく、大項目は28項 目、中項目は261項目で構成された。また、教授領域の一覧表に総合看護を追加し領域を超えた科 目にかかわる看護技術項目を明示した。この学習ノートは、学生が主体的に自己の学習習熟度を確 認する本大学の学士課程における看護実践能力の卒業時到達水準を示した学習支援ツールであり、 看護職として生涯にわたり成長していくためのポートフォリオの第一段階として自己学習力を育む 土台となるものである。

本研究の目的は、実習前準備教育としてのシミュレーション学習における学生の学びを明らかに し、教育への示唆を得ることである。成人看護学実習を履修する学生60名に対し、手術直後の紙面 事例とシミュレータを組み合わせた6つの演習項目を設定し、シミュレーション学習を実施した。 演習時の振り返り記録の質的帰納的分析から学生の学びを明らかにし、課題を検討した。シミュレー ション学習における学生の学びは、6つの演習項目から28のカテゴリーが抽出された。さらに28の カテゴリーの共通性を見出す中で7の大カテゴリー【目的と管理上の注意点の理解】【視覚的イメー ジ化による理解】【リスクと現象を関連づけたアセスメント】【患者の安楽への配慮】【感染予防 技術】【安全で確実な技術の習得】【計画的な行為】が導かれた。学生は視覚的イメージ化により 理解が促進され、既習の知識と技術を統合させて学んでいた。患者との実際の場面を想定したシミュ レーション学習は、実習前準備教育として意義があると考えられる。This study aimed to clarify the contents of students' learning in a simulation study as preparatory Education for the nursing Practicum and obtain educative suggestions. Six exercises of a combination of documented cases and simulators immediately after surgery were prepared for 60 students who were scheduled to undergo the adult nursing practicum to carry out a simulation study. Students’ learning was clarified by qualitative inductive analysis of records of the exercises and examined the problem. As students' learning in the simulation study, 28 categories were extracted from the six exercises. Furthermore, the following seven major categories were extracted by the similarity of 28 categories: "understanding of aims and reminders for management”, “understanding by visual imaging”, “assessment based on the relationship between risks and phenomena", “consideration of patients’ comfort", “Infection prevention skill", "acquisition of safe and reliable skills”, and “calculated behavior". Then, a structural drawing was constructed. Visual imaging aided understanding by the students and they learned by integrating the knowledge and skills acquired in previous learning. A simulation study with actual situations with patients in mind was considered meaningful as preparatory education for the nursing practicum.

看護技術学習ノート第2版を使用した2006年度入学生の看護技術の到達度の縦断的変化を明らかにし、長崎県立大学看護栄養学部看護学科における看護技術の学習支援のあり方を検討した。2006年度入学生55名を対象に、 3年次と4年次に看護技術学習ノート第2版262項目を4段階尺度で調査し、統計的解析はχ2検定を行い有意水準は5%未満とした。結果、4つの到達水準別、5つの枠組み構成別では3年次に比べ4年次で到達度は高くなった(p
