Abstract

In the near future, the traditional networks architecture will be difficult to be managed. Hence, Software Defined Network (SDN) will be an alternative in the future of programmable networks to replace the conventional network architecture. The main idea of SDN architecture is to separate the forwarding plane and control plane of network system, where network operators can program packet forwarding behaviour to improve the network performance. Congestion control is important mechanism for network traffic to improve network capability and achieve high end Quality of Service (QoS). In this paper, extensive simulation is conducted to analyse the performance of SDN by implementing Link Layer Discovery Protocol (LLDP) under congested network. The simulation was conducted on Mininet by creating four different fanout and the result was analysed based on differences of matrix performance. As a result, the packet loss and throughput reduction were observed when number of fanout in the topology was increased. By using LLDP protocol, huge reduction in packet loss rate has been achieved while maximizing percentage packet delivery ratio.

Abstract

Background:Chronic kidney disease (CKD) is a frequent, under-recognized condition and a risk factor for renal failure and cardiovascular disease. Increasing evidence connects non-alcoholic fatty liver disease (NAFLD) to CKD. We conducted a meta-analysis to determine whether the presence and severity of NAFLD are associated with the presence and severity of CKD.Methods and Findings:English and non-English articles from international online databases from 1980 through January 31, 2014 were searched. Observational studies assessing NAFLD by histology, imaging, or biochemistry and defining CKD as either estimated glomerular filtration rate (eGFR) andlt;60 ml/min/1.73 m2 or proteinuria were included. Two reviewers extracted studies independently and in duplicate. Individual participant data (IPD) were solicited from all selected studies. Studies providing IPD were combined with studies providing only aggregate data with the two-stage method. Main outcomes were pooled using random-effects models. Sensitivity and subgroup analyses were used to explore sources of heterogeneity and the effect of potential confounders. The influences of age, whole-body/abdominal obesity, homeostasis model of insulin resistance (HOMA-IR), and duration of follow-up on effect estimates were assessed by meta-regression. Thirty-three studies (63,902 participants, 16 population-based and 17 hospital-based, 20 cross-sectional, and 13 longitudinal) were included. For 20 studies (61% of included studies, 11 cross-sectional and nine longitudinal, 29,282 participants), we obtained IPD. NAFLD was associated with an increased risk of prevalent (odds ratio [OR] 2.12, 95% CI 1.69-2.66) and incident (hazard ratio [HR] 1.79, 95% CI 1.65-1.95) CKD. Non-alcoholic steatohepatitis (NASH) was associated with a higher prevalence (OR 2.53, 95% CI 1.58-4.05) and incidence (HR 2.12, 95% CI 1.42-3.17) of CKD than simple steatosis. Abstract

Objective: To review long-term safety data from the rollover study ETV-901, focusing on adverse events (AEs) with a potential nucleos(t)ide association. Method(s): The open-label study ETV-901 (AI463901) assessed the safety of entecavir in chronic hepatitis B patients who received entecavir, lamivudine or adefovir monotherapy in previous entecavir Phase II/III studies. Long-term cumulative safety results are based on reported AEs, regardless of causal relationship. Result(s): Median exposure to entecavir in study ETV-901 was 184 weeks. Commonly reported AEs (>= 10%) were upper respiratory tract infection, headache and nasopharyngitis. Most AEs were mild to moderate; 203 (19%) patients reported grade 3 4 AEs, with 45 (4%) considered related to entecavir. There were 14 (1%) discontinuations due to AEs. On-treatment alanine aminotransferase (ALT) flares were reported in 32 (3%) patients and were associated with a reduction in hepatitis B virus DNA of more than 2 log10 copies/ml in 25/32 patients. AEs potentially associated with nucleos(t)ide analogs were infrequent, the most common being myalgia (n = 54; 5%) and neuropathy-related AEs (hypoparesthesia and hyperparesthesia, polyneuropathy; n = 42; 4%). Conclusion(s): Long-term administration of entecavir was associated with low rates of serious AEs, discontinuations due to AEs and ALT flares. AEs potentially associated with nucleos(t)ide use occurred at low rates. © 2012 Informa UK, Ltd.

Abstract

Phase III studies of entecavir in chronic hepatitis B (CHB) infection have demonstrated superior efficacy and a comparable safety and tolerability profile compared to lamivudine. The long-term safety data from the ETV-901 (rollover) study are reviewed, with a focus on adverse events (AEs) with a potential nucleos(t)ide association. Method(s): Long-term cumulative safety and tolerability results are based on investigator-reported AEs, regardless of causal relationship. Result(s): In ETV-901 1051 CHB patients were treated with entecavir with a median exposure of 184 weeks. At the time of analysis, 488 (46%) patients had additional prior entecavir exposure from Phase II or III participation. Baseline characteristics were mean age 41 years; 810 (77%) male, 544 (52%) Asian, and 481 (46%) Caucasian. The most common AEs (>=10%) were upper respiratory tract infection, headache and nasopharyngitis. On-treatment alanine aminotransferase (ALT) flares (ALT >2 x baseline and >10 x ULN) were reported in 3% of patients. The cumulative rate of serious AEs was 16%. Discontinuations due to AEs were 1% (n = 14). Liver disease progression was reported in 3% of patients. Selected AEs with a potential nucleos(t)ide association were as follows (includes all grades, unrelated and related to ETV): myalgia (n = 54), neuropathy related adverse events (n = 42), elevated lipase (n = 24), serum creatinine increase (n = 8), lactate increase or bicarbonate decrease (n = 6), hypophosphataemia (n = 5), muscular weakness (n = 4), pancreatitis (n = 3), and creatinine phosphokinase increase (n = 2). Conclusions Long-term administration of entecavir was associated with low rates of serious AEs, discontinuations due to AEs and ALT flares. Spontaneous reports of AEs potentially associated with nucleos(t)ide use occurred at low rates.

Result(s): Median exposure to entecavir in ETV-901 was 168 weeks. Of the 1045 treated patients, 402 (38%) had received entecavir for =5 years at the time of analysis. Also, 488 (47%) patients had additional prior entecavir exposure from Phase II or III participation. Baseline characteristics were: mean age 41 years; 804 (77%) male, 539 (52%) Asian, and 480 (46%) Caucasian. The most common AEs (=10%) were upper respiratory tract infection, headache and nasopharyngitis. On-treatment alanine aminotransferase (ALT) flares were reported in 3% of patients. The cumulative rate of serious AEs was 15%. Discontinuations due to AEs were 1% (n=13), and generally (n=11) occurred during the first 2 years of ETV-901. Selected AEs with a potential nucleos(t)ide association are described below. Conclusion(s): Entecavir is a safe and well-tolerated treatment for patients with CHB and compensated liver disease. Long-term administration of entecavir was associated with low rates of serious AEs, discontinuations due to AEs and ALT flares. Spontaneous reports of AEs potentially associated with nucleos(t)ide use occurred at low rates.
