The horizontal distribution of radioactive cesium (Cs) derived from the Fukushima Dai-ichi Nuclear Power Plant (FNPP) in the North Pacific is still unclear due to the limitation of direct measurement of the seawater in the open ocean. We present the result of direct observation of radioactive Cs in surface seawater collected from broad area in the western and central North Pacific in July, October 2011 and July 2012. We also conducted a simple particle tracking experiment to estimate the qualitative spatial distribution of radioactive Cs in the North Pacific. 134Cs were detected at 94 stations out of 123 stations and 137Cs was detected at all stations. The high 134Cs and 137Cs concentrations more than 10mBq kg-1 were observed in the area where the northern part of Kuroshio extension at 144° E and 155° E in July 2011, in the area 147° E-175° E around 40° N in October 2011, and the northern part of Kuroshio extension at 155° E and 175° 30' E in July 2012. Combining the result of direct observations and particle tracking experiment, the radioactive Cs derived from FNPP had been dispersed eastward to the central North Pacific during 2011. It was considered from the horizontal distribution that radioactive Cs was dispersed not only eastward but also north- and southward in the central North Pacific. Pronounced dilution process of radioactive Cs from FNPP during study period is suggested from temporal change in the activity ratio of 134Cs/137Cs which was decay corrected at 6 April 2011, and relationships between radioactive Cs and temperature. The waters off Sanriku (located on the northeastern coast of Honshu Island, Japan) provide the southernmost habitats of northern fur seals (Callorhinus ursinus) during winter and spring in the western North Pacific. The southward flowing cold Oyashio current and northward-flowing warm Kuroshio extension mix there, making the area highly productive. Northern fur seals migrate into these waters from the breeding rookeries for feeding, and the locations of the southern margins of their habitats vary yearly. The key questions for understanding the seasonal migration patterns are "why" and "how" the species utilize these waters as the southernmost habitat. We estimated the density and abundance of northern fur seals using standard line-transect theory combined with habitat modeling. The spatial patterns of animal density were analyzed using generalized additive models with seven static and dynamic environmental covariates, and those covariates were selected based on Akaike's information criterion (AIC). The lowest AIC model included depth, sea surface temperature, slope, and gradient in sea surface temperature. This model estimated well the spatial patterns of the density of the species, in which fur seals were widely distributed in the study areas, but less frequently encountered between the isobaths 100 m and 200 m. These spatially separated habitats suggest that the shelf break and offshore front play an important role in creating the feeding grounds of fur seals. On the other hand, sea surface temperature positively correlated with fur seals' density up to 14°C. TAFRO syndrome is a systemic inflammatory disorder resembling multicentric Castleman disease; it is characterized by thrombocytopenia, anasarca, a fever, reticulin fibrosis, and organomegaly. Involvement of the adrenal glands, including adrenal infarction, hemorrhaging, and adrenomegaly, has recently been reported in several cases and been considered a characteristic early-stage symptom. We herein report a case of TAFRO syndrome initially presenting with bilateral adrenal infarctions and review the literature on TAFRO syndrome related to adrenal involvement. This case suggests that adrenal abnormalities as an early clinical feature of TAFRO syndrome may be useful for the early diagnosis.

Case Presentation: A 93-year-old man living in a nursing home presented to our emergency department with altered mental status. Examination revealed hypotension and severe hypoxia. Chest radiograph showed infiltrates in the right upper lobe, and computed tomography of the abdomen and pelvis demonstrated a left femoral neck fracture. A point-of-care transthoracic echocardiogram (TTE) revealed an enlarged right ventricle, severe tricuspid regurgitation, and numerous white floating dots moving toward the right atrium from the inferior vena cava (IVC), leading to the diagnosis of fat embolism syndrome (FES)., Discussion: Although imaging studies can facilitate diagnosis, the diagnosis of FES is typically made by clinical history and presentation, making a swift diagnosis often difficult in those who are critically ill. Recent case reports have described that TTE can detect fat emboli, seen as flowing hyperechoic particles in IVC. This image demonstrates the utility of TTE to diagnose FES.

Pancreatic panniculitis is a rare complication of pancreatic diseases. We aimed to evaluate a case of pancreatic panniculitis. A 58-year-old woman was referred to our hospital with complaints of painful cutaneous nodules on her limbs. Various diagnostic tests confirmed pancreatic panniculitis and pancreatic adenosquamous carcinoma. We diagnosed pancreatic panniculitis by a skin nodule biopsy that revealed fine basophilic material within anucleate cells and neutrophil infiltration. Abdominal imaging detected a tumor with necrosis on the pancreas and endoscopic ultrasound-guided fine-needle aspiration revealed it as an adenocarcinoma. The patient underwent pancreatoduodenectomy after neoadjuvant chemotherapy. The tumor was composed of differentiated adenocarcinoma and squamous cell carcinoma and diagnosed as adenosquamous carcinoma. This is the first report of pancreatic panniculitis in a patient with adenosquamous cell carcinoma of the pancreas.
