1. Successful salvage of allograft dysfunction triggered by transplant renal vein thrombosis immediately after kidney transplantation: a case report

Despite adequate interventions, including thrombolytic therapy or surgical thrombectomy, successful salvage of the allograft is often difficult. We observed a case of TRVT induced by compression of the renal vein immediately after intraoperative abdominal closure.Case presentation: A 41-year-old male underwent ABO-compatible living kidney transplantation. The donor was his 45-year-old sister, and her right kidney was donated. The allograft had a single artery and vein. One of the preoperative recipient problems was obesity (body mass index, 33.4 kg/m2). Intraoperative Doppler ultrasonography (US) revealed sufficient blood flow throughout the allograft, and urine output was also observed. After surgery, hematuria was observed; the urine output decreased and serum creatinine levels increased to 7.0 mg/dL. Doppler US showed a decrease in diastolic flow and an elevated resistive index, which were similar findings to those noted in acute rejection. Although steroid pulse therapy was initiated, allograft dysfunction was worsening. On postoperative day 4, surgical exploration revealed TRVT; consequently, thrombectomy was performed. The urine output increased, and serum creatinine levels decreased to 1.8 mg/dL. The cause of TRVT development may be that the transplant renal vein was relatively short, due to the right kidney being compressed by surrounding tissues after abdominal closure, and that TRVT was gradually developing due to stagnant blood flow.Conclusion: Although TRVT is induced by multiple factors, an accurate diagnosis is often difficult. Understanding these factors, including obesity, and considering TRVT as a cause of allograft dysfunction are important during the pre-, peri-, and postoperative periods. Knowledge of TRVT can lead to early and accurate diagnosis and intervention, resulting in better outcomes for the patients with allograft dysfunction. 12. Effect of Ni and Co additives on phase decomposition in TiB2-WB2 solid solutions formed by induction field activated combustion synthesis

Abstract

Solid solutions of TiB2-WB2 were densified and annealed simultaneously to cause the decomposition into the phases (Ti,W)B-2 and (W,Ti)B-2. Ni and Co were added to solid solutions formed by induction field activated combustion synthesis. The presence of these metals as additives markedly enhanced the kinetics of the subsequent decomposition process. With these additives, decomposition to the two phases occurred within minutes (360 s) in contrast to hours when the solutions did not include the additives. The phases resulting from decomposition, (Ti,W)B-2 and (W,Ti)B-2, were identified by X-ray to have the hexagonal AIB, and W2B5 structures, respectively. The precipitated phase, (W,Ti)B-2, occurred as elongated grains with aspect ratios of as high as about 10 in samples containing Ni as the additive.

Abstract

Near-infrared spectroscopy (NIRS) is a noninvasive neuroimaging tool used to measure activation-induced changes in cortical hemoglobin concentration. Using this technique, changes in the optical absorption of light are recorded to estimate changes in oxyhemoglobin and deoxyhemoglobin concentrations resulting from local vascular and metabolic effects during brain activity. NIRS has been successfully applied for early identification of neurodevelopmental and neurodegenerative disorders. Likewise, decreases in olfactory ability indicate similar disorders. High cortical involvement in the olfactory system makes NIRS ideal for in-depth investigation of the olfactory and cognitive systems. Due to growing popularity and potential for enhancing research in both techniques, we aimed to identify activation patterns during olfaction, summarize the locations of hemodynamic change during olfactory stimulation, highlight developmental differences, and outline functional lateralization of olfactory processes in studies using NIRS. We searched Web of Science, PsychInfo, CINAHL, PubMed, and Science Direct for olfactory related studies using NIRS methodology. The literature search generated 578 records and 124 full text articles were reviewed. Thirty-two articles comprising 588 participants from Japan and Europe met eligibility. Olfactory stimulation consistently induces hemodynamic changes in the fronto‐polar (inferior frontal), orbito-frontal, and temporal regions, while trigeminal scents also activate the somatosensory cortices. Healthy individuals showed consistent hemodynamic patterns, and these patterns differed from individuals with anosmia, developmental disorders, cognitive decline, and cognitive load. Our findings suggest that NIRS can be used to investigate olfaction across the lifespan and may have potential as a diagnostic tool. However, longitudinal, well-powered studies are needed to establish disease-specific sensitivity and specificity.

40. ACCESS TO COGNITIVE RESOURCES IN EARLY LIFE AND TRANSITIONS ACROSS COGNITIVE STATES IN MIDDLE AND OLDER ADULTHOOD

An expanding body of research highlights that a cognitively engaged lifestyle may protect against neurodegeneration in late life (e.g., thereby promoting cognitive reserve). The majority of research in this area uses education as a proxy to investigate the cognitive reserve hypothesis; however, more recent findings suggest that other factors from the lifespan contribute to cognitive reserve. The current study examined whether access to cognitive resources in childhood (e.g., number of books at home) influenced healthy and impaired life expectancies using coordinated analyses of data from eight countries. Multistate survival models examined transitions between healthy, mild, and severely impaired cognitive states and death, which were used to compute life expectancies for individuals with and without impairment. Controlling for age, sex, education, SES, and country, analyses revealed a pattern consistent with the cognitive reserve hypothesis. Specifically, having access to more cognitive resources in childhood was associated with compression of morbidity—longer life expectancy free of impairment and less time spent with cognitive impairment. Cross-country differences in this effect will be discussed. Overall, these findings underscore the importance of childhood cognitive stimulation for cognitive health in later life and emphasize the need for a lifespan-focused approach to studying cognitive reserve.
