Electron microscopy demonstrated the presence of glycocalyx on the cell surface of Chattonella antiqua, which was previously believed to be 'naked'. Histochemical analysis of this structure indicates that the glycocalyx consists of at least two different types of acidic complex carbohydrates, sulfated and nonsulfated, together with a neural carbohydrate-protein complex with 1,2-glycol groups and α-D-mannosyl and α-D-glucosyl residues. Many medications have established interactions with warfarin, potentially affecting international normalized ratio (INR) levels and increasing the risk of bleeding complications. We present the case of a 74-year-old female inpatient with late-onset schizophrenia who was initiated on brexpiprazole while concurrently receiving warfarin therapy for a ventricular thrombus. Despite INR levels being within a range of 1.8 to 2.7 prior to brexpiprazole initiation, the patient's INR increased sharply to 6 within five days, leading to a potential gastrointestinal bleed that required medical admission. While the manufacturer's in-vitro studies suggest that brexpiprazole does not displace warfarin from albumin, both drugs exhibit greater than 99% protein binding. This case highlights the critical need for close monitoring of INR levels when initiating brexpiprazole in patients on warfarin therapy, due to the potential for warfarin displacement from plasma proteins and subsequent bleeding complications. Proteins from hyperthermophiles often contain a large number of ionic interactions. Close examination of the previously determined crystal structure of the ATPase domain of MutL from a hyperthermophile, Aquifex aeolicus, revealed that the domain contains a continuous ion-pair/hydrogen-bond network consisting of 11 charged amino acid residues on a β-sheet. Mutations were introduced to disrupt the network, showing that the more extensively the network was disrupted, the greater the thermostability of the protein was decreased. Based on urea denaturation analysis, a thermodynamic parameter, energy for the conformational stability, was evaluated, which indicated that amino acid residues in the network contributed additively to the protein stability. A continuous network rather than a cluster of isolated interactions would pay less entropic penalty upon fixing the side chains to make the same number of ion pairs/hydrogen bonds, which might contribute more favorably to the structural formation of thermostable proteins., (© The Author(s) 2024. Signaling by classic analgesics, such as morphine, is governed primarily by the relative abundance of opioid receptors at the cell surface, and this is regulated by receptor delivery to, and retrieval from, the plasma membrane. Although retrieval mechanisms, such as receptor endocytosis, have been extensively investigated, fewer studies have explored mechanisms of receptor maturation and delivery to the plasma membrane. A previous study implicated receptor transporter proteins (RTPs) in the latter process. Since not much is known about regulation of RTP expression, we initiated studies examining the effect of chronic morphine administration on the levels of RTPs in the brain. Among the four RTPs, we detected selective and region-specific changes in RTP4 expression; RTP4 mRNA is significantly upregulated in the hypothalamus compared with other brain regions. We examined whether increased RTP4 expression impacted receptor protein levels and found a significant increase in the abundance of mu opioid receptors (MOPrs) but not other related G protein-coupled receptors (GPCRs, such as delta opioid, CB 1 cannabinoid, or D 2 dopamine receptors) in hypothalamic membranes from animals chronically treated with morphine. Next, we used a cell culture system to show that RTP4 expression is necessary and sufficient for regulating opioid receptor abundance at the cell surface. Interestingly, selective MOPr-mediated increase in RTP4 expression leads to increases in cell surface levels of MOPr-delta opioid receptor heteromers, and this increase is significantly attenuated by RTP4 small interfering RNA. The absorption of ursodeoxycholate and its tauro-conjugate by the jejunum and the terminal ileum of rat intestine was compared with that of other unconjugated bile acids and taurocholate. In the ligated jejunum, the efficacy of absorption of unconjugated bile acids was in the following order: ursodeoxycholate = deoxycholate > chenodeoxycholate = cholate > lithocholate. This order cannot be explained by the theory that the passive diffusion of bile acids is faster the less hydroxyl bonds in the molecule. These findings on the unconjugated bile acids in the ligated jejunum were further confirmed by perfusion experiments. In the ligated terminal ileum, ursodeoxycholate, cholate and deoxycholate were absorbed as fast as taurocholate or tauroursodeoxycholate, whereas absorption of chenodeoxycholate was significantly slower. The Na+-dependency of the absorption of ursodeoxycholate and cholate in the terminal ileum was confirmed by perfusion studies. In conclusion, intestinal absorption of ursodeoxycholate was efficient in both the jejunum and ileum and these results may contribute to the high availability of ursodeoxycholate in various hepatobiliary diseases.

The absorption of lithocholate and its sulfate and glucuronide in rat jejunum and terminal ileum was studied. Tracer amounts of radiolabelled bile acids were administered to the ligated intestinal segments, and their absorption was monitored by biliary excretion through a bile duct catheter. Absorption of lithocholate was faster in the terminal ileum than in the jejunum. Although the sulfation reduced lithocholate absorption in the jejunum, it did not affect lithocholate absorption in the terminal ileum. This was due to the Na+-dependency of ileal absorption of lithocholate-sulfate assessed by perfusion studies. In contrast, the glucuronidation markedly reduced lithocholate absorption both in the jejunum and the terminal ileum. These findings indicate that the glucuronidation is more effective than sulfation in detoxifying lithocholate as far as the prevention of its intestinal absorption is concerned.

Gallstones are often complicated with diseases such as liver cirrhosis, hemolytic anemia, post-valvular replacement, post-gastrectomy, biliary tract cancers, diabetes mellitus, and during clofibrate therapy. The frequency of gallstones, types of stones and their pathogenesis in these situations are discussed in this chapter. In liver cirrhosis, hemolytic anemia and post-valvular replacement, black stone formation is enhanced due to bilirubin over-production, caused mainly by hemolysis. Bilirubin stone formation is accelerated gastrectomy and by biliary tract cancer, because of the decrease in gallbladder contraction, cholestasis and bacterial infection, etc. The incidence of cholesterol gallstone is high in patients with diabetes mellitus and with clofibrate therapy.

We report a case of flomoxef-induced pneumonitis. A 22-year-old man was treated with flomoxef following liver biopsy. A few days later he developed a high fever and severe dyspnea, and his chest X-ray film revealed diffuse reticulo-nodular shadows in both lung fields. We suspected interstitial pneumonitis due to flomoxef, and pulse therapy with methylprednisolone was started. He showed rapid recovery of symptoms and marked regression of pulmonary infiltration in his chest X-ray. Lymphocyte stimulation test was positive to flomoxef, which was compatible with the diagnosis of drug-induced pneumonitis. To our knowledge, there has been no previous case of pulmonary hypersensitivity to flomoxef reported in Japan.
