Abstract

Introduction: The Synergy and Promus PREMIER stents have an identical stent platform and eluting drug but use different drug carrier systems: biodegradable polymer (abluminal coating) or durable polymer (conformal coating). The abluminal coating stent have generated early neointimal healing and less inflammatory reaction. The aim of this study was to evaluate potential impact of the different carrier systems on early vascular responses to the Everolimus-eluting stents by histologic analysis and scanning electron microscope in a porcine model. Methods: Eleven Synergy and 10 control Promus PREMIER were implanted in non-injured coronary arteries of 11 pigs. Histopathology of stented segments was performed at 2nd and 14th day after implantation. Morphometric analysis of the inflammation (graded as score 0 to 3) and the intimal fibrin content (graded as score 1 to 3) was also performed. Results: On the 2nd day, neointimal thickness and % neointimal area, defined as 100 х neointimal area / stent area, inflammatory and intimal fibrin content score were no significant differences between 2 groups. On 14th day, inflammatory and intimal fibrin content score were significantly lower in Synergy than in Promus PREMIER, although there were no significant differences in neointimal thickness and % neointimal area. Importantly, smooth muscle cells were found and neointimal layer were very smooth in Synergy (Figure). Conclusions: These results demonstrated that neointimal healing could occur early after abluminal coating stent implantation with less inflammatory reaction. Abstract

We report a case with pulmonary veno-occlusive disease (PVOD) associated with systemic sclerosis which exhibits strong resistance to pulmonary vasodilator. A 55-year-old female with severe pulmonary hypertension was admitted to our hospital to be introduced to epoprostenol infusion therapy. She was diagnosed as having pulmonary arterial hypertension (PAH) associated with systemic sclerosis at the age of 51. Several aggressive treatments with pulmonary vasodilators, including oral prostaglandin, endothelin receptor antagonists, and phosphodiesterase 5 inhibitors, failed to improve her symptoms. We introduced continuous intravenous epoprostenol therapy from 2 μg/kg/min for her. However, pulmonary edema appeared and worsened in a dose-dependent manner. We made a diagnosis of PVOD clinically at that time. Thereafter, pulmonary edema gradually disappeared consistent with the reduction of the dose of epoprostenol infusion. She died of renal failure and infection 4 months after the introduction of epoprostenol infusion therapy. A histological examination revealed severe stenosis and occlusions of pulmonary veins as well as pulmonary arteries over a wide area. We suggest that prevalence of veno-occlusive type of disease could be one of the major mechanisms of less responsive or even refractory to pulmonary vasodilator therapies in patients with PAH associated with connective tissue disease.
