Abstract

OBJECTIVES To characterize the location and frequency of the fracture lines and comminution zones of both-column fractures using fracture mapping. METHODS Both-column fractures were retrospectively reviewed in four Level-I trauma centers. Mimics software was used to reconstruct the both-column fractures and simulate the fracture reduction. Then, the fracture lines and the intra- and extra-articular comminution zones were drawn on a 3-D innominate bone template. The distribution of fracture lines and fracture fragments were also be mapped on the 2-D template of the acetabulum. All the included patients were divided into low- and high-variety group according to the anterior column fracture line above or below the anterior superior spine. The anatomical characteristics of the posterior wall fragment were also analyzed in these two groups. RESULTS Seventy-eight both-column fracture patients were included. The "T" type or inverted "Y" type main fracture lines divided the innominate bone into low-variety (n=11) and high-variety (n=67) both-column fractures. The extra-articular comminution zones are usually distributed between the posterior iliac fragment and anterior column fragment. High-variety-type both-column fractures are usually combined with "high" and "sharp" posterior wall fragments, while low-variety-type both-column fractures are usually accompanied by "low" and "flat" posterior wall fragments. The comminution zones in the acetabulum are mostly concentrated in the anterior and inferior parts of the acetabulum. CONCLUSIONS The innominate bone is divided into three major parts in the both-column fracture, and the fracture center is located at the proximal of the acetabular dome. The both-column fractures are divided into low- and high-variety patterns. The location of the anterior column fracture determined the characteristics of the posterior wall fragment.

45. Treatment of Unstable Intertrochanteric Fractures with Proximal Femoral Nailing Antirotation: Traction Table or Double Reverse Traction Repositor.

Abstract

Objective The objective of this retrospective study was to compare the efficacy of the double reverse traction repositor (DRTR) and traction table with proximal femoral nailing antirotation (PFNA) in the treatment of unstable intertrochanteric fractures. Patients and Methods: Data from 66 patients (36 patients treated with the traction table and 30 patients treated with DRTR) with unstable intertrochanteric fractures were reviewed from January 2017 to June 2017. The demographics, fracture characteristics, surgical data, and prognostic parameters were collected to compare the differences between the two groups. Results: The collodiaphyseal angle (CDA) was significantly lower in the DRTR group than in the traction table group (129.37° ± 7.47 and 135.67° ± 6.95, respectively, p < 0.001). The open reduction rate was significantly lower in the DRTR group than in the traction table group (3.3% and 13.9%, respectively, p < 0.001). No significant differences were found in the demographics, fracture characteristics, other surgical data, or prognostic parameters between the two groups. Conclusions: DRTR can facilitate safe, effective, and minimally invasive treatment of unstable intertrochanteric fractures with PFNA. [ABSTRACT FROM AUTHOR]

Supplemental Digital Content is available in the text, Background: Acute compartment syndrome (ACS) is defined as a clinical entity originated from trauma or other conditions, and remains challenging to diagnose and treat effectively. The review was aim to present the controversy in diagnosing, treating ACS. It was found that there was no criterion about the ACS, and result unnecessary osteotomy. The presence of clinical assessment (5P) always means the necrosis of muscles and was the most serious or irreversible stage of ACS. Besides pressure methods, the threshold of pressure identifying ACS was also controversial. Methods: Immediate surgical fasciotomy was important to prevent severe suquelae of the ACS. However, there was still controversy about the right time that fasciotomy should be done to avoid irreversible ischemic changes. The most important thing to treat ACS was comprehension to the true injury mechanism, but a systemic classification about traumatic mechanism in most literature was not clear. Results: After observations to fracture patients with blister, we recommended that surgeons dealing with such emergencies should be vigilant, and the indication for fasciotomy should be strictly controlled following with injury mechanism especially for patients without severe soft tissue injury. Conclusion: For those crushing and soft tissue injuries, the current evidence based strategies for managing patients was useful, but for those fracture related injury, more examination was necessary to avoid overtreatment especially for those patients with blister observed. In facing patients, medical history, injured mechanism should be paid special attention, and rigorous classification about traumatic etiology was the key for the treatment of these patients.

47. Open-Wedge HTO with Absorbable β-TCP/PLGA Spacer Implantation and Proximal Fibular Osteotomy for Medial Compartmental Knee Osteoarthritis: New Technique Presentation.

Abstract

The purpose of this study was to present a new absorbable spacer for medial compartmental knee osteoarthritis (OA). The functional and radiographic results of patients treated with the novel surgical technique were also evaluated to investigate its clinical feasibility. Patients with medial compartmental knee OA who underwent open-wedge high tibial osteotomy (HTO) with absorbable β-TCP/PLGA [30% beta-tricalcium phosphate and 70% poly (lactic-co-glycolic acid)] spacer implantation and proximal fibular osteotomy from January 2016 to February 2017 were retrospectively analyzed. The operation time, blood loss, and relevant complications were reviewed. The femorotibial angle (FTA), varus angle (VA), joint line convergence angle (JLCA), American Knee Society Score (KSS), and visual analog scale (VAS) score were recorded preoperatively and at the final follow-up, respectively. At the final follow-up, the mean FTA and JLCA were 179.85° ± 4.34° and 2.44° ± 1.26°, respectively, which were smaller than the preoperative values (182.94° ± 3.86° and 4.12° ± 1.65°, respectively; both p < 0.001). The final VA and VAS score were lower than those measured preoperatively (both p < 0.001). The clinical and functional KSSs at the final follow-up were higher than the preoperative scores (p < 0.001). This novel absorbable spacer could provide adequate stability for the tibial osteotomy gap, at a lower cost than a traditional plate. With the use of this spacer as an osteoinductive and biodegradable device, secondary surgery for fixation removal could be avoided.
