1. Significance of neutrophil extracellular traps in severe Mycoplasma pneumoniae pneumonia in children

Abstract

Objective To investigate the pathogenic role of neutrophil extracellular traps （NETs） in the pathogenesis and progression of severe Mycoplasma pneumoniae pneumonia （SMPP） in children and to assess their predictive value for SMPP. Methods Children with Mycoplasma pneumoniae pneumonia （MPP） admitted to the Second Hospital of Lanzhou University from October to December 2023 were selected as the study subjects. They were divided into the non-severe group （MPP group， n = 45） and severe group （SMPP group， n = 39）. The levels of NETs in the serum and bronchoalveolar lavage fluid （BALF） of all children were measured using ELISA. Additionally， BALF from both the affected and contralateral sides was collected from children in the SMPP group， and the content of NETs in BALF was determined using ELISA. Results A significant difference was observed in the serum concentration of NETs between MPP and SMPP patients （P < 0.01）. In the SMPP group， the level of NETs in the BALF of the affected lung was significantly higher than those of the contralateral side （P < 0.001）. Serum NETs levels were significantly higher than those in BALF in SMPP group （P < 0.001）. The area under the ROC curve （AUC） for predicting SMPP based on serum NETs levels was 0.892 （95%CI 0.810-0.963， P < 0.001）， with a sensitivity of 0.821 and specificity of 0.875， and a cut-off value of 17.24 ng/mL，respectively. The AUC of the combination of serum NETs with C-reactive protein （CRP） and lactate dehydrogenase （LDH） levels for predicting SMPP was 0.974 （95%CI 0.946-1.000， P < 0.001）， with a sensitivity of 0.923 and specificity of 0.950， respectively. Conclusions Excessive activation of NETs may contribute to the incidence of SMPP and localized lung injury in children with MPP. Monitoring the serum levels of NETs， CRP and LDH can effectively predict the incidence of SMPP. The combination of NETs， CRP， and LDH yields the best predictive performance.

Therefore, the aim of this study is to summarize and evaluate the evidence level and efficacy of treatment for TNBC.Materials and Methods: Retrospective and prospective studies on treatment of TNBC were searched in the PubMed, Embase, and Cochrane Library databases. The literature search deadline was June 30, 2021. Two investigators independently screened the literature and extracted the data. In addition, the joint World Health Organization–United Nations Food and Agriculture Organization expert consultation was used to evaluate the validity of the evidence.Results: A total of 28 meta-analyses were included in this study. The treatment interventions for TNBC mainly included surgery, chemotherapy (CT), radiotherapy, molecular targeted therapy, immunotherapy, zoledronic acid, and gonadotropin-releasing hormone (GnRH) analog. Platinum improves the pathological complete response (PCR) rate of patients treated with neoadjuvant chemotherapy (NACT), the objective remission rate (ORR) and overall survival (OS) in patients with metastatic triple-negative breast cancer. Capecitabine improves disease-free survival (DFS) and OS in patients treated with adjuvant CT. Bevacizumab was added to NACT to improve the PCR rate in patients. Immunotherapy improves the PCR rate in patients treated with NACT. The improvement in PCR rate in patients with high Ki67 expression treated with neoadjuvant therapy is highly suggestive. Other interventions had suggestive or weak evidence.Conclusion: Among the strategies for treating TNBC, platinum, bevacizumab, and immunotherapy can lead to better PCR rates as part of a NACT regimen. Capecitabine as adjuvant CT and platinum in the treatment of metastatic TNBC can benefit patients’ survival. However, the effectiveness of other interventions for TNBC is not yet clear. 16. Primary catastrophic antiphospholipid syndrome in children with midbrain infarction: a case report.

Abstract

Background: Catastrophic antiphospholipid syndrome (CAPS) is a multi-system autoimmune disease characterized by extensive thrombosis. Pediatric CAPS is extremely rare and associated with a high mortality rate, especially when midbrain infarction is involved. Hence, early diagnosis and prompt initiation of appropriate treatment for CAPS complicated by midbrain infarction are of utmost importance in achieving favorable outcomes., Case Presentation: In this report, we present the case of a 14-year-old girl who presented with neurological symptoms and digestive system infection and was initially diagnosed with an "intracranial infection". After a series of rigorous diagnostic procedures, the patient was ultimately diagnosed with primary CAPS and was immediately transferred to the intensive care unit where she was treated with anticoagulation, glucocorticoids, intravenous immunoglobulin (IVIG) therapy, and multiple plasma infusions. Twenty-seven days after admission, the patient's condition improved with standardized treatment, and she was discharged and followed up regularly., Conclusion: This case report provides a description of the clinical features observed in a pediatric patient with CAPS and concurrent midbrain infarction, highlighting the crucial role of early diagnosis and timely treatment in influencing patient prognosis., Competing Interests: The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest., (© 2024 Dong, Yin, Su and Ni.)
