Amaç: Meme kanserli hastalarda kemoterapi (KT) öncesi, sırası ve sonrasında beslenme durumu, oksidatif parametreler ve yaşam kalitesi arasındaki ilişkileri değerlendirilmesi amaçlandı.Gereç ve Yöntemler: KT planlanan meme kanserli hastaların klinik, antropometrik, demografik, yaşam kalitesi, kan oksidatif belirteç düzeyi ve diyet alım verileri kaydedildi. Toplanan tüm veriler, KT öncesi, KT ortası ve KT sonrası zaman noktaları arasında karşılaştırıldı. Bulgular: Çalışmaya meme kanseri tanısı konan ve KT tedavisine başlanacak 50 kadın dahil edildi. KT sonrası vücut ağırlığı ve beden kütle indeksi, KT öncesi değerlerden anlamlı derecede düşüktü (p˂0,001). Her ikisi de KT'nin sonunda toplam günlük kalori ve makro besin alımında bir düşüşle KT öncesi seviyelere döndüler. Belirli gıda gruplarının tüketimi, KT öncesi tüketim düzeylerine göre önemli ölçüde azaldı (p˂0,001). Kan oksidatif stres belirteç analizi, KT ile glutatyon peroksidaz düzeylerinde önemli bir azalma olduğunu ortaya koydu (p=0,007). Sonuç: Sağlıklı antropometrik ölçümler, oksidatif denge ve yaşam kalitesi sağlamak için bu hastalara KT sırasında diyet danışmanlığı yoluyla yeterli enerji alımı ve antioksidan alımı önerilmelidir., Aim: We aimed to assess the associations between nutritional status, oxidative parameters, and quality of life before, during, and after chemotherapy (CT) in breast cancer patients. Material and Method: Clinical, anthropometric, demographic, quality of life, blood oxidative marker level, and dietary intake data of breast cancer patients who planned to CT were recorded. All collected data were compared between pre-CT, mid-CT, and post-CT time points. Results: Fifty women who were diagnosed with breast cancer and would start CT treatment were included in the study. Post-CT body weight and body mass index were significantly lower than the pre-CT values (p˂0.001). They both turned back to pre-CT levels at the end of CT with a decrease in total daily caloric and macronutrient intake. Consumption of specific food groups significantly decreased compared to their pre-CT consumption levels (p˂0.001). Blood oxidative stress marker analysis revealed a significant reduction in glutathione peroxidase levels with CT (p=0.007). Analysis of the quality-of-life scores revealed that post-CT scores were significantly higher than the pre-CT scores while the life quality was lowest in the mid-CT period.Conclusion: Adequate energy intake and antioxidant intake should be recommended to these patients during CT via dietary counseling to maintain healthy anthropometric measures, oxidative hemostasis, and quality of life.

Kur’ân’ın nâzil olduğu çağda Arap yazısının yetersizliği yaygın olarak kabul edilen bir husustur. Nüshalarda noktalama ve harekelemenin bulunmayışı birçok problemi beraberinde getirmiştir. Hz. Osman döneminde istinsah edilerek çoğaltılmıştır. Zeyd b. Sabit, Kur’ân’ın cemi ve istinsahı konusunda önemli bir aktördür. Daha sonra lehçelerden ve daha başka sebeplerden dolayı farklı kıraatler meydana gelmiştir ki bu farklı kıraatleri hadis rivayetlerinden öğrenmekteyiz. Kıraat âlimleri senedleri bakımından kıraatleri mütevatir, meşhur, âhâd, şâz, mevdu’ ve müdreç (kıraat üzerine açıklama ve tefsir ile ziyade edilmesi) olmak üzere ayırmışlardır. Bunlardan mütevatir ve meşhur olanları makbul addedilirken, diğerlerine itibar edilmemektedir. Bu araştırmada, rivayet ilmindeki Hz. Osman nüshasını ve kıraat farklılıklarını ifade eden rivayetleri tespit etmek amaçlanmıştır. Bağlantıları tespit edebilmek için Hz. Peygamber, Hz. Ebu Bekir ve Hz. Osman dönemlerini izahın ardından Hz. Osman Mushafı’nın ortaya çıkışı ve rivayet farklılıklarının rivayet ilmine yansımaları tespit edilecektir. Araştırmada ilgili hadislerin tespiti ve tahrîcinin yapılması hedeflenmektedir., The aim of this research is to determine the Mushaf in terms of narration science and to determine and evaluate the narrations about the differences in recitation. In this context, brief information about the science and history of recitation is also presented. The main purpose is to reveal the role of the science of narration in the determination of recitations. As an introduction to the subject, some information has been given about the Qur’an’s revelation, its collection and its istinsah. The period of the Prophet, Abu Bakr and the developments that took place in the Othman period were discussed in stages, under headings. Relevant narrations in hadith books have been identified. The subject was supported by different sources and when necessary, the rumors were evaluated in terms of reliability. Obtained results are presented in the conclusion section. Although our study is limited to Kutub-i tis’a, we have pointed out the content evaluations of the narrations by looking at the hadith sources before and after. The works in the “Maktabah Shamilah - Islamic Library” program were used as a source in the study. For biographical information, “TDV Encyclopedia of Islam”, especially history and history books, was used. On recitation, “Ibnü’l-Cezerî’s work en-Neşr” and “Süyûtî’s el-İtkan work” have been benefited significantly. Recitation scholars have divided recitations (qira’ahs) into mutawatir, famous, ahad, shad, mawdu’ and mudrec (increasing recitation with explanation and commentary) in terms of their proofs. While the mutawatir and famous ones are considered acceptable, the others are not respected. In this context, it is debatable whether the recitations are taqqifi or tabbudi, or whether they have a quality that can change according to dialects. In this research, It is aimed to determine the narrations expressing the Othman copy and the differences in recitation. Therefore, Prophet, Abu Bakr and Othman times, The emergence of the Othman Mushaf and the reflections of the differences in narration on the science of narration will be determined. In the research, it is aimed to determine the related hadiths and to compile them. Under the title of the compilation of the Qur’an and the conditions of sound recitation, Prophet, Abu Bakr and The developments that took place in the Ottoman period are summarized. Sabit by establishing detailed system, he showed great sensitivity in terms of science and managed this event in a controlled manner with all its subtleties and used an unprecedented method.

Central nervous system (CNS) diseases have a very important place in terms of public health. Epilepsy is one of the most common CNS diseases. Epilepsy is a chronic disease and a cause of substantial morbidity and mortality. The causes of this disease, of which many people around the world suffer from epilepsy, are still not fully clarified. It is known that approximately 50 million people suffer from epilepsy. Antiepileptic drugs are frequently used in the treatment of epilepsy, but the difficulty with these drugs is the emergence of drug resistance additionally antiepileptic drugs can be administered in oral and intravenous ways. But these treatments are not every time effective. On the other hand, drugs used in the treatment of epilepsy must be delivered effectively and safely within the brain. Therefore, new delivery systems are needed to deliver drugs at concentrations determined for high therapeutic efficacy in epilepsy without side effects. Considering this information, there is a need to develop new treatment strategies. With the development of nanotechnology, nanoparticles as an effective drug delivery system have been shown to be significantly effective in the treatment of diseases. Nanocarrier systems can fulfill many functions such that they can cross the blood-brain barrier (BBB) pass a specific cell or signaling pathway, reply to endogenous stimulus, support nerve regeneration, and ensure cell survival. Thanks to these features, it is seen that nano-carrier systems are quite assertive in being the current treatment methods in epilepsy. Today, studies of the therapeutic efficacy of liposomes, micelles, solid lipid nanoparticles, dendrimers, and nanoemulsions as nano-carrier systems on central nervous system diseases are still ongoing. It holds promise in the concentration control of the drugs and in the delivery of the drug to the target tissue through the BBB. In this review, the role of nanocarrier systems in addition to current treatment methods in epilepsy disease was investigated.

Giriş: İlaç uygulama hataları bakımından riskli birimlerden biri olan acil servislerde, ilaç güvenliğinin ilk adımı olarak hemşirelerin ilaç uygulama hatalarını etkileyen faktörlerin belirlenmesi önem taşımaktadır. Amaç: Araştırma, acil hemşirelerinin ilaç uygulama hatalarını etkileyen faktörlerin incelenmesi amacıyla gerçekleştirildi.Yöntem: Kesitsel nitelikteki araştırma, İstanbul’da altı eğitim ve araştırma hastanesinde çalışan 126 acil hemşiresi ile gerçekleştirildi. Tanıtıcı Bilgi Formu, Hemşirelikte Tıbbi Hataya Eğilim Ölçeği İlaç ve Transfüzyon Uygulamaları Alt Boyutu, Pittsburgh Uyku Kalitesi İndeksi, Epworth Uykululuk Ölçeği ve İş Stresi Ölçeği ile veriler toplandı. Bulgular: Hemşirelerin %26,2’si son 12 ayda ilaç uygulama hatası yaptığını belirtti. Dönüşümlü vardiyada çalışan hemşirelerin, gündüz vardiyasında çalışanlara göre ilaç uygulama hatası yapma olasılığının yaklaşık 21 kat daha fazla olduğu belirlendi (p=0,024) (Güven Aralığı=1,492-299,922). İş Rolü Uyumsuzluğu alt boyutundan elde edilen bir puanlık artışın (p=0,003) (Güven Aralığı=0,507-0,875) ve İlaç ve Transfüzyon Uygulamaları Alt Boyutundan elde edilen bir puanlık azalışın (p=0,001) (Güven Aralığı=1,131-1,656) ilaç uygulama hatası yapma olasılıklarını sırasıyla 0,66 kat ve 1,36 kat arttırdığı saptandı. Sonuç: Araştırmanın sonuçları; ilaç uygulama hatalarını etkileyen faktörlerin dönüşümlü vardiya, iş rolü uyumsuzluğu ve ilaç ve transfüzyon uygulamalarına yönelik hata yapma eğilim ya da olasılığı olduğunu göstermektedir. Çalışma listelerinin düzenlenmesi, ilaç hatalarına yönelik farkındalığın arttırılması ve acil servislere özgü iyileştirmelerin gerçekleştirilmesi önerilmektedir., Introduction: In emergency departments, one of the risky units in terms of medication errors, it is important to determine the factors affecting medication errors of nurses as the first step of medication safety.Aim: The study was conducted to examine the factors affecting medication errors of emergency nurses.Methods: The cross-sectional study was carried out with 126 emergency nurses working at six training and research hospitals. Data were collected through Introductory Information Form, Medication and Transfusion Practices Subscale, Pittsburgh Sleep Quality Index, Epworth Sleepiness Scale and Job Stressors Scale. Results: 26.2% of nurses stated that they made a medication error in the last 12 months. It was determined that the nurses who worked rotational shifts had approximately 21 times greater odds of experiencing medication errors in the last 12 months than those working during day shift (p=0.024) (Confidence Interval=1.492-299.922). A one-point increase obtained from Work Role Conflict Subscale (p=0.003) (Confidence Interval=0.507-0.875) and a one-point decrease obtained from Medication and Transfusion Practices Subscale (p=0.001) (Confidence Interval=1.131-1.656) were found the odds of experiencing medication errors respectively 0.66 and 1.36 times in the last 12 months.Conclusion: The results of the study show that factors affecting medication errors are rotating shifts, work role conflict and tendency towards medication and transfusion errors. Organizing working lists, increasing awareness about medication errors and making improvements specific to emergency departments are recommended.

Bununla birlikte yatay dren kalınlığının etkisi ihmal edilebilir. Sonuçlar aynı zamanda düşey drenin kaba malzeme gradasyonunun çekirdekte daha dik bir sızma hattına neden olduğunu göstermektedir. Earth-fill dams are exposed to seepage throughout their lifetime. In many cases, seepage related safety precautions needed are to be taken to keep seepage rate and pore water pressures below certain limits. This is commonly handled by installation of drainage facilities which are blanket, chimney and toe drains. This study is aimed at finding suitability and the effectiveness of drainage facilities in earth-fill dams. For this purpose, various materials and geometries are considered for different drain types in separate cases. Steady-state seepage analyses are conducted using a finite element software. Results showed that increased length of blanket drain causes increased seepage flow and shorter path of phreatic line, whereas the effect of thickness of blanket drain can be neglected. The results also showed that coarser material gradation of chimney drain causes steeper phreatic line through the core. Increased height of the toe drain results in higher seepage rates through the dam. Material gradation of toe drain has not a distinct role on its performance. 132

Dünyada yaygın görülen fonksiyonel gastrointestinal bir bozukluk olan İBS, yaşam kalitesini olumsuz etkilemekte ve yüksek oranda sağlık harcamalarına neden olmaktadır. Patofizyolojisi tam olarak bilinmeyen bu hastalıkla ilgili çeşitli hipotezler öne sürülmektedir. Son zamanlarda yapılan birçok çalışmada düşük düzeydeki mukozal inflamasyon ve immun sistem disfonksiyonun İBS patofizyolojisindeki rolü vurgulanmaktadır. İBS hastalarında pro-inflamatuvar ve anti-inflamatuvar sitokinlerde dengesizlik olduğu gösterilmiştir. İnflamasyon ve enfeksiyona katılan çeşitli hücrelerden sekrete edilen YKL-40 proteininin immun regülasyonda rol oynadığı ileri sürülmektedir. Netrin-1' in inflamasyon aracılı doku hasarını azalttığı ve anti-inflamatuvar etkiye sahip olduğu gösterilmiştir. Çalışmamızda immun aktivasyon ve inflamasyon ile ilişkili olduğu düşünülen YKL-40 ve netrin-1 seviyelerinin İBS patofizyolojisindeki rolünü araştırmayı amaçladık. Gastroenteroloji polikliniğine başvuran ve Roma III kriterlerine göre İBS tanısı konulmuş 54 hasta ile 34 sağlıklı birey çalışmaya dahil edildi. Çalışmamızda YKL-40 ve netrin-1 seviyeleri ELISA (Enzime Bağlı İmmunosorbent Analizi) yöntemiyle ölçüldü. Verilerin istatistiksel analizi için bağımsız örneklem t-testi kullanıldı. Çalışmamızda kontrol grubu ile karşılaştırıldığında, İBS hastalarında YKL-40 seviyelerinin anlamlı olarak değişmediğini bulduk. Objective: Several metabolic pathways are affected by the reduction in the amount of iron iniron deficiency anemia (IDA). As a result, some hormonal control mechanisms, especiallycarbohydrates, proteins, nucleic acid metabolism, are influenced. In this study, we aimed toinvestigate the effect of IDA on serum lipid profile.Method: Files of 30 patients with IDA and 30 healthy people , whoconsulted to internal medicine and gastroenterology clinic in Namık Kemal University Facultyof Medicine, were scanned. The lipid and blood parameters of these people as well as theirdemographics were recorded.Results: While serum total cholesterol and low-density lipoprotein (LDL) levels were markedlylower in IDA cases than in controls, serum triglyceride, high-density lipoprotein (HDL) andvery low-density lipoprotein (VLDL) levels did not differ in patients with IDA compared withthe control group.Conclusion: It was observed that serum total cholesterol and LDL levels, whose main contentis cholesterol, were decreased in IDA. Çalışmamızda DEA’ninserum lipid profiline etkisini araştırmayı amaçladık.Gereç ve Yöntem: Namık Kemal Üniversitesi Araştırma ve Uygulama Hastanesi iç hastalıklarıve gastroenteroloji kliniğe başvurmuş ve DEA tanısı alan 18-60 yaş arası 30 hasta ve 30 sağlıklıkişiye ait dosya taraması yapıldı. Bu kişilerin demografik bilgileri ile birlikte lipid ve diğer kanparametreleri düzeyleri kayıt edildi.Bulgular: DEA hastalarının serum total kolesterol ve düşük yoğunluklu lipoprotein (LDL)düzeyleri kontrollere göre anlamlı olarak düşerken, serum trigliserid, yüksek yoğunluklulipoprotein (HDL) ve çok düşük yoğunluklu lipoprotein (VLDL) düzeylerinde ise anlamlı birdeğişiklik olmamıştır.Sonuç: DEA’nde serum kolestrol düzeyleri ile birlikte ana içeriği kolestrol olan LDLdüzeylerinin de azaldığı görülmüştür. Demir mikarındaki azalmanın kolesterol metabolizmasınıdeğiştirdiğini düşünmekteyiz
