Amaç: Allojenik kök hücre nakli (AKHN), uygun vericisi bulunan fit ağır aplastik anemili (SAA) ve çok ağır aplastik anemili (VSAA)hastalarda küratif tedavi yöntemi olarak uygulanmaktadır. Yaş, nakil sonrası sağkalımı belirleyen en önemli faktörlerden biridir ve ilerleyen yaş ile birlikte sağkalım oranları azalmaktadır, bu nedenle uygun hastaların geciktirilmeden nakle alınması büyük önem taşımaktadır. AKHN sonrasında miks kimerizm (MK) olması nüs ya da engraftman yetersizliğinin işareti olabilir. Biz bu çalışmada, erişkin SAA ve VSAA hastalarında AKHN sonrasındaki durumunu ve takipte kimerizmin önemini incelemeyi amaçladık.Yöntemler: Merkezimizde AKHN olan 16 AA hastası bu gözlemsel çalışmaya dahil edildi. HLA durumu, hazırlama rejimi, kimerizm ve remisyon durumu kaydedildi. Bulgular: Tüm hastalarda medyan PFS 70,4 ay ve medyan OS 89,7 ay olarak izlendi. Takip süresi boyunca, 9 hasta tam kimerik iken 6 hasta MK ve 1 hastada kimerizm, Aim: Allogeneic stem cell transplantation (allo-SCT) is performed as a curative treatment in young, fit, severe aplastic anemia (SAA) and very severe aplastic anemia (VSAA) patients. Age is one of the most significant factors that impact survival after transplantation. With advancing age, survival rates decrease, therefore it is crucial to perform allo-SCT without any delay in appropriate SAA and VSAA patients. Mixed chimerism (MC) in the post-allo-SCT period may be a sign of relapse or graft failure. In this study, we aimed to evaluate the outcome of allo-SCT in adult SAA and VSAA patients and the significance of chimerism monitoring in follow up.Methods: The data of 16 adult AA patients who underwent allo-SCT at our center were included in this observational study. HLA match, conditioning regimens, chimerism and remission status were recorded. Results: Median PFS was 70.4 months and median OS was 89.7 months in all patients. Meme kanseri, dünyadaki en yaygın ikinci kanserdir ve kadınlarda en sık görülen kanserdir. Daha az gelişmiş bölgelerde, kadınlarda en sık ölüm nedeni ve akciğer kanserinden sonra daha gelişmiş bölgelerde kanser ölümünün ikinci nedenidir. Türkiye'de ise kadınlarda en sık görülen kanser yine meme kanseridir. İmmünohistokimyasal biyobelirteçler östrojen reseptörü, progesteron reseptörü, insan epidermal büyüme faktörü reseptörü ve Ki-67, hastalığın durumu için tümör derecesi, boyutu ve nodal durumu kombinasyonu ile ilişkili olarak düşünülür. Günümüzde yirmiye yakın meme kanseri türü belirtilmiştir. Fakat bu çalışma için temel olarak, gen ekspresyon profillerine ve moleküler sınıflamaya dayalı olarak tanımlanmış dört alt grup seçilmiştir: Luminal A, luminal B, HER2 pozitif ve üçlü negatif meme kanser tipleri. Östrojen ve progesteron reseptörleri, hücre büyümesi ve proliferasyonunda rol alan steroid hormon reseptörleridir. PR, ligand ile aktifleşen transkripsiyon faktörüdür ve proliferatif sinyal yolağında, sinyal transdüksiyon yolunu aktive eder. Hücrede MAPK yolağının aktivasyonunu indükler. ER, PI3K / AKT sinyalini indükler ve meme kanseri hücrelerinde hücre büyümesini uyarır. Ayrıca dimerize haldeki aktifleşmiş HER2 reseptörü de RAS ve PI3K yollarını fosforile ederek alt yolaklarının aktiflenmesinde rol oynar. Çekirdekte, steroid hormon reseptörleri hormona duyarlı enhancer sekanslarını tanır ve bağlanarak hormone ilişkili transkripsiyonu artırır.Normal meme epitelyumuna kıyasla, ön-invaziv meme hastalıkları ve meme kanserinde Bcl-2 ile ilişkili anthanogen BAG-1 yaygın olarak artmaktadır. Ek olarak, Bag ailesi proteinleri, bir dizi insan kanserinde ve tümör hücre hatlarında, özellikle de lösemi, meme, prostat ve kolon kanserlerinde farklı olarak eksprese eder. BAG ailesi, çok çeşitli hedef moleküller ile etkileşime giren ve apoptozu, proliferasyonu, transkripsiyonu, metastazı, motiliteyi ve otofajiyi düzenleyen çok fonksiyonlu bir protein grubudur. BAG-1 ayrıca Bcl-2'den bağımsız anti-apoptotik etkilere sahiptir. Çok sayıda reseptör tirozin kinazına bağlanır ve apoptozun inhibisyonunda rol alır. Çalışmalar, BAG-1'in erken meme kanseri prognozunda klinik amaçlarla bir biyobelirteç olarak kabul edilebileceğini göstermektedir. Meme kanseri durumunda, BAG-1, AKT, BCL-2, B-RAF ve C-RAF, steroid hormon reseptörleri ve diğer proteinler ile etkileşime giren ısı şok protein HSP70 için bir eş-şaperondur. Öte yandan, ER fonksiyonunu düzenleyerek östrojen bağımlı transkripsiyonu arttırır. BAG-1 ve PR, ER arasındaki ilişkiler konusunda literatürdeki çalışmaların sonuçları değişkenlik göstermektedir. Bu konuyu ele almak için, BAG-1 ve BAG-1 ile ilişkili sağkalım faktörleri, laboratuvarımızda reseptör tipleri farklı meme kanseri hücre hatları ile çalışıdı. Ön çalışmalarımız bizi meme kanseri hücre sağ kalım yolağında BAG-1etkisi üzerine düşünmeye itmiştir.Alternatif translasyon mekanizması ile üretilen BAG-1'in üç izoformu, hücrede çeşitli bölgelerde lokalize haldedir. Hücre içinde farklı lokalize olabilen BAG-1 izoformları fonksiyonel olarak fark gösterir. Tek bir mRNA transkriptinden alternatif translasyon başlangıcı ile, 3 majör ve 1 minör izoform elde edilir. Bütün izoformlar BAG bölgesine sahiptir, fakat BAG-1L'de ve BAG-1M'de nükleer lokalizasyon sinyali bulunur. Böylece, BAG-1L nükleusta yer alır ve BAG-1M de çekirdeğe göç edebilir. BAG-1'in izoformları, lokalize oldukları doku ve hücrelere göre özelleştiği bilinmektedir. Hücre yolaklarınındaki rolü nedeniyle BAG-1, hücre sağkalımını düzenleyerek hastalıklar için önemli bir biyo-belirteç olarak kabul edilir.Apoptotik protein BAD'ın mitokondriyal membranda BCL-2 ve BCL-XL ile dimerize halde bulunmaktadır. Bu dimerizasyon, hücrede BAD aracılı apoptozu teşvik eder. BAD'ın, Ser112 ve Ser136 residülerinin BCL bağlama bölgesinde olduğu düşünülmektedir ve bu residülerinden fosforile olan BAD, mitokondriyal membrana BCL yoluyla bağlanamaz ve sitoplazmada kalır. Sitoplazmik BAD ise bu sayede hücre sağkalımında rolü bilinmektedir.Laboratuvarımızda yapılan önceki çalışmalarda, Bag-1 ve onun etkileşim partnerlerinin hücre sağkalımı ve apoptoz yollarında önemli rolleri olduğu gösterilmiştir. Çalışmalar, meme epitel MCF-10A, tümörijenik MCF-7 ve triple negatif MDA-MB-231 hücre hatları kullanılarak gerçekleştirildi ve p-AKT, BRAF, CRAF, BCL-2 ile BAG-1 etkileşimleri bu çalışmalarca doğrulandı. Ayrıca, etkileşim kompleksinin bir üyesi olan AKT, Ser 136 residüsünden BAD'ı fosforile ettiği, BAD'ın Ser 112 residüsünün ise p-CRAF vasıtasıyla fosforile edildiği de ekibimizin yaptığı çalışmalarca tekrarlandı. Bu iki lokasyondan birisinden fosforile olan BAD'ın, hücre sağkalımını artırmaktadır. Bu, bize meme kanseri hasta dokularında da bu yolakların doğrulamasının yapılabileceği hususunda fikir verdi. Başlangıçta, `Bu yolaklar meme kanserli hasta dokularında doğrulanabilir mi?'' ve `Hormon reseptörlerine bağlı meme kanserlerinde BAG-1 expresyonu ve ilişkili bu sağkalım yolağı reseptör bağımlı olarak değişkenlik gösterir mi?` soruları ile yola çıkıldı. Bu çalışmada, ER, PR, HER2 bağımlı meme kanserlerinde BAG-1'in ve sağ kalım yolağındaki BAD ilintili faktörlerin ekspresyon paternleri hastaşardan alınan tümörlü ve tümörsüz dokularda incelenmesi amacıyla yola çıkıldı. Bu amaca ulaşmak için BAG-1, BAD, AKT, BRAF, CRAF ve fosforile edilmiş formları ER, PR, HER2 bağımlı farklı tipteki kanser hastalarından alınan normal ve tümörlü meme dokularında araştırıldı. Eşzamanlı olarak, ortak tümör ilişkili genlerdeki mutasyon taraması için, genetic materyal eldesi aynı hastaların kan numunelerinden izole edildi. Bu çalışma, böylelikle hem protein temellli çalışma hem de genetic çalışma yapılarak iki iş akışından oluşmaktadır. Tümör doku örnekleri İstanbul Sağlık Bakanlığı Üniversitesi Ümraniye Eğitim ve Araştırma Hastanesi'nde gerçekleştirilen cerrahi işlem sonrası özel saklama solüsyonlarına alındı. Hem normal hem de tümör doku örneklerinin protein izolasyonu hemen yapıldı. Sonrasında immünoblotlama çalışmaları gerçekleştirilmek üzere protein izolasyonları soğuk zinciri bozulmadan kısa süre içerisinde gerçekleştirildi. Ayrıca immünhistokimya çalışmaları ile lokalizasyon çalışmaları yapıldı. Bu deneyleri yapmak için, dokular parafin bloklarına gömüldü. Formalin fiksasyonu yapılan dokular daha sonra ilgili antikorlarla boyanarak ışık mikroskobu altında incelendi. Öte yandan, aynı hastalardan alınan kan örneğinden izole edilen genetik materyal yoluyla major yaygın tümör baskılayıcı ve onkojenik genlerdeki varyasyonları saptamak için NGS çalışmaları gerçekleştirildi. Meme kanserinde yaygın sorumlu olan onkojen ve tümör baskılayıcı genler PCR bazlı çoğaltılarak, ve oligo dizileriyle işaretlenerek her örnek için ayrı ayrı kütüphaneler hazırlandı. Örnek bazlı kütüphaneler sonrasında birleştirilip örnek havuzu sekanslama teknolojisi kullanularak dizilendi. Dizileme cihazından alınan ham data variant çağırma uygulaması kullanılarak sıralandı ve sonrasında çeşitli databaseler ve varyant çağırma toolları kullanılarak potansiyel varyasyonu araştırmak için analiz edildi. Literatürdeki birçok küçük kohortlu çalışmalar, BAG-1'in meme kanserinde farklı sonuçları olan ekspresyonunu sergilemektedir. Araştırmacılar ve klinisyenler, nükleer BAG-1 ekspresyonu ile yüksek sitoplazmik Bag-1 arasındaki sağkalım veya ilişkinin azalması ve sağkalımın iyileştirilmesi gibi birçok farklı sonuca ulaşmışlardır. Ancak östrojen, progesteron, insan epidermal büyüme faktörü reseptörü ve Bag-1 interaksiyonları önemli bir konu olarak ortaya çıkmıştır. Ayrıca, Bag-1, ER ve PR arasındaki ilişkilendirmeler de ihtilaf göstermektedir. Bazı çalışmalar Bag-1 ekspresyonu ile ER veya PR ekspresyonu arasında bir ilişki olmadığını gösterirken, diğer çalışmalar Bag-1 ve ER'nün BAG-1 ve PR ile birlikte ifade eğiliminde artış gösterdiğini ifade etmiştir. Bununla ilgili çelişkiler henüz aydınlanmamıştır. Bu çalışmada protein deneylerini yürütebilmek için 48 farklı hastadan doku örnekleri toplandı. Çalışmadaki ana bulgu, tümörlerde, BAG-1 L aşırı ifadesinin, nükleustaki BAG-1 lokalizasyonununa eşlik etmesidir. Ayrıca bu durum ER ile olan ilişki ile paraleldir.Lumainal A tipinde, BAG-1L, p-AKT ve p-BAD Ser 136 nükleusta lokalizasyonları tekrarlı çalışmalarca tümör dokularında gözlemlenmiştir. Buna bağlı olarak, muhtemel BAG-1, p-AKT, BAD/p-BAD Ser 136 üçlü kompleksinin, ER varlığında nükleer lokalize olabileceği ihtimali üzerinde yoğunlaşılmıştır. Breast cancer is the second most common cancer in world, and by far the most frequent cancer among women. In less developed region, it is the most frequent reason of death in women, and second cause of cancer death in more developed regions after lung cancer. Besides, in Turkey the most common cancer in women is breast cancer. Immunohistochemical biomarkers oestrogen receptor, progesterone receptor, human epidermal growth factor receptor and Ki-67 are considered with the combination of tumor grade, size, and nodal status for the disease status. Presently, there are 20 major types of breast cancer. However, mainly four subgroups that has been defined clearly based on gene expression profiles and molecular classification were chosen for this study: Luminal A, luminal B, HER2 enriched and triple negative breast cancers. Estrogen and progesterone receptors are steroid hormone receptors that have roles in cell growth and proliferation. PR is ligand activated transcription factor and activate signal transduction pathway in pro-proliferative signalling. It induces the activation of MAPK signalling in cell. ER induces PI3K/AKT signalling and stimulates cell growth in breast cancer cells. Besides, activated HER2 receptor phosphorylates RAS and PI3K pathways also. In nucleus, steroid hormone receptors recognise and bind to hormone responsive enhancer sequences.Bcl-2 associated anthanogene BAG-1 is commonly increased in pre-invasive breast disease and breast cancer compared to normal breast epithelium. In addition, Bag family proteins express differentially in a range of human cancers and tumour cell lines, particularly leukaemia, breast, prostate, and colon cancers. Bag family, a multifunctional group of protein that interacts with a wide range of target molecules and regulate apoptosis, proliferation, transcription, metastasis, motility and autophagy. BAG-1 also has anti-apoptotic effects, which are independent of Bcl-2; it binds to numerous receptor tyrosine kinases and improves their ability to inhibit apoptosis. Studies show that BAG-1 can be accepted as a biomarker for clinical purposes in early breast cancer prognosis. In case of breast cancer, Bag-1 is a co-chaperone for the heat-shock protein HSP70 that interacts AKT, BCL-2, B-RAF and C-RAF, steroid hormone receptors and other proteins. In the other hand, it increases estrogen dependent transcription by regulating ER function. Reported associations between BAG-1 and PR, ER are variable. To address these issues we tried to assess the expression of BAG-1 and BAG-1 related survival factors with cell lines with breast cancer that differ from the receptor type in our laboratory.Our early studies encourage us to enlighten to find out exact role of BAG-1 in survival pathway.Three isoforms of BAG-1 that are generated by alternative translation mechanism are localized various location in the cell. Bag-1 isoforms, which can be differentially localized in the cell, are functionally distinct. Alternative translation initiation from a single mRNA transcript produces 3 major and 1 minor isoform. All isoforms have BAG domain, but nuclear localization signalling is found in BAG-1L and a part of NLS in BAG-1M. Thus, BAG-1L localizes on nucleus and BAG-1M can also be translocated to nucleus. The isoforms of BAG-1 are specialized according to the tissue and cell lines they localized. Because of the involvement in number of cellular pathways, BAG-1 is regarded as an important marker for diseases, regulating cell survival. Apoptotic protein BAD is known that reside in mitochondrial membrane with dimerized with BCL-2 and BCL-XL. This dimerization promotes BAD mediated apoptosis in cell. If BAD is phosphorylated on Ser112 and Ser136 that residues are considered to be in BCL-binding site, phosphorylated BAD unable to bind mitochondrial membrane via BCL and exist cytoplasm. Cytoplasmic BAD promotes cell survival.The previous studies in our lab demonstrated that Bag-1 and its interaction partners have impprtant roles in cell survival and apoptosis pathways. The studies were managed by using tumorigenic MCF-7 and triple-negative MDA-MB-231 and non-tumorigenic MCF-10A epithelial cell lines. In our laboratory, BAG-1 interactions with p-AKT, BRAF, CRAF, BCL-2 was verified. In further, the member of this interaction complex AKT, phosphorylates BAD at Ser 136 residue and Ser 112 residue of BAD is phosphorylated via p-CRAF. The phosphorylated BAD on these two residues promotes cell survival. This work exited us to find these pathways in breast carcinoma patients' tissue. To begin with, we think about that `Are these pathways also present in tissues from breast carcinoma patients?'' and `How is the situation in breast cancer linked to hormone receptors?'' In this study, it was intended to enlighten hormone receptor and HER2 dependent expression patterns of BAG-1 and survival factors affecting BAD in breast cancer tissues. To achieve this aim, BAG-1, BAD, AKT, BRAF, CRAF and phosphorylated forms were investigated in normal and tumor breast tissues across ER, PR, HER2 status. Simultaneously, proteomic effects if the mutation in common tumor related genes were searched to identify tumor progression for same patients' sample. This study consist of two workflow, one is conducted with tissue sample from patients. Tumor tissue samples were supplied from Univesity of Ministry of Health - Umraniye Training and Research Hospital of Istanbul. Tissues were taken from the lumpectomy, mastectomy surgeons. The fresh tissues were taken into RNA Later solution. Protein isolation of both normal and tumor tissue samples were done immediately. Immunobloting studies were done further. Besides, immunohistochemistry studies were conducted. To make this experiments, tumorous sections of tissues were embedded in paraffin blocks. Formaline fixed parafine embedded tissues then stained with interested antibodies. In the other hand, we conducted NGS study to detect major common tumor suppressor and oncogenic genes variations via the genetic material isolated from the blood sample from the same patients. We conducted to library preparation step via PCR based detection kit for our breast cancer panel. Sequencing was conducted further. Aligned fragments were then analysed to searched the potential variation by using genetic tools. Many small cohort studies in literature have measured expression of BAG-1 with different outcomes in breast cancer. Researchers and clinicians found that many different results such as association between nuclear BAG-1 expression and decreased survival or association between high cytoplasmic Bag-1 and improved survival. However the estrogen, progesterone, human epidermal growth factor receptor and Bag-1 interactions came up as an important issue. Also, described associations between Bag-1, ER and PR are different. Some studies have shown no association between Bag-1 expression and ER or PR expression while other studies have shown that Bag-1 and ER tend to co-express, as do BAG-1 and PR. There are many contradictions about it. Total forty eight tissue samples from different patients were used to make protein experiments in this study. The main finding in this study is that in tumors, BAG-1 L overexpression accompanied by BAG-1 localization in nucleus. Besides, ER positivity is parallel for this relationship. In lumainal A type, BAG-1L, p-AKT and p-BAD Ser 136 resided in nucleus. Depends on the finding, we suggested that possible BAG-1, p-AKT, BAD/p-BAD Ser 136 triple complex might be co-localized in nucleus in the condition of ER presence and BAG-1L could be the main target for the ER-related breast tumors. 76

Inherited pathogenic variants account for 5% to 10% of all breast cancer (BC) and colorectal cancer (CRC) cases. Here, we sought to profile the pathogenic variants in 25 cancer susceptibility genes in Turkish population. Germline pathogenic variants were screened in 732 BC patients, 189 CRC patients and 490 cancer‐free elderly controls, using next‐generation sequencing‐based multigene panel testing and multiplex ligation‐dependent probe amplification testing. Pathogenic variants were detected in 17.2% of high‐risk BC patients and 26.4% of high‐risk CRC patients. More than 95% of these variants were clinically actionable. BRCA1/2 and mismatch repair genes (MLH1, MSH2 and MSH6) accounted for two‐thirds of all pathogenic variants detected in high‐risk BC and CRC patients, respectively. Pathogenic variants in PALB2, CHEK2, ATM and TP53 were also prevalent in high‐risk BC patients (4.5%). BRCA1 exons 17‐18 deletion and CHEK2 c.592+3A>T were the most common variants predisposing to BC, and they are likely to be founder variants. Three frequent MUTYH pathogenic variants (c.884C>T, c.1437_1439delGGA and c.1187G>A) were responsible for all MUTYH biallelic cases (4.4% of high‐risk CRC patients). The total pathogenic variant frequency was very low in controls (2.4%) and in low‐risk BC (3.9%) and CRC (6.1%) patients. Our study depicts the pathogenic variant spectrum and prevalence in Turkish BC and CRC patients, guiding clinicians and health authorities for genetic testing applications and variant classification in Turkish population. What's new? Inherited cancer‐predisposing genetic variants vary in identity and frequency across populations, potentially impacting genetic testing for risk assessment and treatment. This study examined the spectrum and prevalence of pathogenic variants specifically among Turkish breast and colorectal cancer patients. A total of 25 cancer susceptibility genes were analyzed. Overall, pathogenic variants were mostly non‐recurrent or recurrent at low frequencies in the study population. The most notable exception, occurring at greater frequency, was deletion of exons 17‐18 in BRCA1, a potential founder variant that originated in northeastern Turkey. OBJECTIVE: Familial Mediterranean fever (FMF) is a monogenic inherited periodic fever syndrome presenting with episodes of self-limiting fever and inflammation of serosal membranes. Besides the findings in the diagnostic criteria, musculoskeletal findings can also be seen in FMF patients attacks. In this study, we aim to reveal the frequency and genotype association of musculoskeletal manifestations in children with FMF. METHODS: The patients diagnosed with FMF between January 1, 2017 and June 1, 2019, and followed for at least six months in our pediatric rheumatology clinic were included in this study. Musculoskeletal manifestations of patients were enrolled. The patients were grouped according to the "Mediterranean Fever" (MEFV) gene variants. Musculoskeletal manifestations of the patients were compared between the groups. RESULTS: The study group included 634 children with FMF (336 female and 298 male, F/M: 1.13/1). The clinical manifestations of patients in the attack period were as follows: 99% of the patients had a fever, 87.3% had abdominal pain, 20.7% had chest pain, 11.3% had vomiting, 10.7% had erysipelas like erythema, and 9.3% had a headache. The musculoskeletal symptoms were accompanied by 58.6% (n=372) of the patients during the attack period. The most common musculoskeletal manifestation was found as arthralgia (32.6%, n=206). Also, the other musculoskeletal manifestations were as follows during attacks: arthritis in 23.7% (n=150), myalgia in 20.5% (n=130), exertional leg pain in 6.5% (n=41), and protracted febrile myalgia in 1% (n=7) of the patients. It was observed that the musculoskeletal manifestations were significantly higher in patients with homozygous M694V variants in exon-10 (p=0.017). The musculoskeletal manifestations were more common in the attack periods of patients carrying the M694V variant in at least one allele (p=0.019). CONCLUSION: We found that the musculoskeletal manifestations were accompanied in more than half of patients with FMF. M694V variant was found as a risk factor for emerging musculoskeletal manifestations. [ABSTRACT FROM AUTHOR]

OBJECTIVE: Turkey is one of the latest countries that COVID-19 disease was reported, with the first case on March 11, 2020, and since then, Istanbul became the epicenter of the pandemic in Turkey. Here, we reveal sequences of the virus isolated from three different patients with various clinical presentations. METHODS: Nasopharyngeal swab specimens of the patients were tested positive for the COVID-19 by qRT-PCR. Viral RNA extraction was performed from the same swab samples. Amplicon based libraries were prepared and sequenced using the Illumina NextSeq platform. Raw sequencing data were processed for variant calling and generating near-complete genome sequences. All three genomes were evaluated and compared with other worldwide isolates. RESULTS: The patients showed various clinics (an asymptomatic patient, patient with mild disease, and with severe pulmonary infiltration). Amplicon-based next-generation sequencing approach successfully applied to generate near-complete genomes with an average depth of 2616. All three viral genomes carried the D614G variant (G clade according to GISAID classification) with implications for the origin of a spread first through China to Europe then to Istanbul. CONCLUSION: Here, we report the viral genomes circulating in Istanbul for the first time. Further sequencing of the virus isolates may enable us to understand variations in disease presentation and association with viral factors if there is any. In addition, the sequencing of more viral genomes will delineate the spread of disease and will guide and ease the necessary measures taken to stem the spread of the novel coronavirus. Amaç: Çocuklarda duyulan üfürümlerin büyük çoğunluğunun masum üfürüm olduğu gerçeğine rağmen, tüm dünyada çocuk kardiyoloji kliniklerine hasta gönderilmesinin en sık nedenini kardiyak üfürümler oluşturmaktadır. Çalışmada dinleme bulguları ile ekokardiyografi sonuçları karşılaştırılmıştır.Gereç ve Yöntemler: Aralık 2011 ile Haziran 2012 tarihleri arasında fizik muayenede üfürüm duyulması nedeniyle çocuk kardiyoloji kliniğine gönderilen 667 hasta duyulan üfürümler “masum” ve “patolojik” olarak ayrılarak ekokardiyografi ile değerlendirilmiştir.Bulgular: Yaş ortalaması 5.1 yıl olan hastaların %54.6’sı erkekti. Hastaların %67.3’ünde kardiyak üfürüme eşlik eden yakınma yoktu. Hastaların %7.9’unda fizik muayenede duyulan üfürümün patolojik olduğu düşünüldü. Ekokardiyografik incelemede hastaların %58.3’ünde normal kalp bulguları saptanırken, %41.7’sinde çeşitli bulgular tespit edildi. En sık tespit edilen bulgular patent foramen ovale (PFO) (%24.9), atriyal septal defekt (ASD) (%8.4) ve pulmoner darlık (%5.7)’di. Hastaların %38.4’ünde muayenede masum üfürüm duyulmasına rağmen ekokardiyografide küçük ASD ve minör kapak hastalıkları gibi bazı bulgular saptandı. Sanatın gelişim sürecinde resimsel biçimlendirmede etken olan doku, sanat yapıtlarında derinlik ve anlam kazandığı görülmektedir. Doku, Sanat Tarihi süreçlerinde, tamamlayıcı eleman olmaktan çıkmış, resmin kendisini oluşturan tek başına, belirleyici öge olmuştur. Yeni yaratım olanaklarına ulaşmada önemli bir unsur olan doku ayrıntıları, yaşamın ritmini oluşturmasının yanı sıra, geçmişten günümüze düşünce evrenimizi harekete geçirerek, evreni anlamlandırmamıza ve yorumlamamıza yardımcı olmuştur. Kompozisyon bütününde doku ayrıntısının sorgulanması ise evrende makro ve mikro kozmos ilişkiyi yorumlamamıza benzediği gözlenmektedir. Evrendeki her şey, makro ve mikro ilişki içerisinde olduğu bilim insanlarınca ortaya konulmakta ve bu alanda kanıtlar ileri sürülmektedir. Bu araştırmada Doku Ayrıntıları `mikrokozmos` olarak ele alınıp, `makrokozmos` olan kompozisyonda, yeni yaratım olanaklarına ulaşmada etkileri irdelenmektedir. Somut, soyutlama ve soyut kompozisyon çeşitlerinde doku, daima temel öge olarak değerlendirilmiştir. Araştırma dört bölümden oluşmaktadır. Birinci bölüm, Problem cümlesi, alt problemler, araştırmanın önemi, amacı ve sınırlılıklarından oluşmaktadır. İkinci bölümde, araştırmaya konu olan Doku ögesinin tanımı ve çeşitlerine değinildikten sonra Dokunun evren ile kompozisyon ayrıntılarındaki ilişkisine yer verilmiştir. Üçüncü bölümde ise; Dünya Sanat Tarihinin ve Türk Resim Sanatının soyut resimlerinde, doku ögesini kullanan sanatçılar ve eserlerinin incelenmesine yer verilerek, dokuyu ne tür yollarla elde ettiklerine değinilmiştir. Üçüncü bölümün son kısmında ise, araştırmacının soyut resimlerinde, doku ögesini ele alış şeklini, dokuya ulaşma serüvenini ve doku ögesinin resimlerinde nasıl soyut kompozisyona dönüştüğü gibi süreçlere ait incelemelere ve açıklamalara yer verilmiştir. Dördüncü bölümde, araştırmanın yöntemi, modeli, evren ve örneklemi ele alınarak veri toplama teknikleri ile sıralandırılmıştır. Beşinci ve son bölümde ise; Bulgular ve Yorumların ardından Sanat Eğitiminde soyut kompozisyon tasarımında doku ögesinin öğretimsel süreçlerinin önemine değinilerek, Sonuç ve Öneriler kısmına geçilmiştir. Sonuç ve önerilerde ise, neye ulaşıldığına ve daha bu konuda nelerin araştırma konusu edilmesi gerektiğine dair bir takım önerilere yer verilmiştir.Anahtar kavramlar: Doku, Soyut, Kozmos, Mikrokozmos, Makrokozmos In the process of art development, effective in shaping paintings, texture is seen to be become profound and meaningful in art structures. During the process of art history, texture becomes out of a complementary element, alone it becomes decisive, forming the painting itself. Besides forming life rhythm and giving momentum to our thought universe from past today, texture details, being an important factor in reaching new creation possibilities, helped in our understanding and interpretation of the universe. In a compositional integrity, questioning texture detail resembles to the interpretation of universe's micro and macro cosmos relationship. Everything in the universe is claimed by scientists to be in a micro and macro relationship and in this area evidence has been put forward. In this study, texture details, handled as `micro cosmos` are examined for the effect of reaching new creation possibilities in a `macro cosmos` composition. Texture always evaluated as fundamental element in perceptible, abstraction and abstract composition styles.This study comprises four sections. First section consists of problem phrase, sub problems, the importance of the study, objective and constrains. In the second section, the subject of the study, the texture element is defined and elaborated on its kinds. After, the texture's universe and composition details relationship was given. In section three, artists using texture elements and their work and in which way they produced the texture were interpreted in world art history and in Turkish painting arts' abstraction paintings. In the last part of the third section, examinations and explanations were given about texture element handling type in researcher's abstraction paintings, about the journey to reach the texture and in paintings how texture element transformed to abstraction composition process. In the fourth section, the study method, model, universe and its sampling are handled and sampling techniques were sorted. In the fifth and last section, after findings and interpretations, for art education and abstraction composition design, the importance of texture element in teaching processes was given then followed by the results and suggestions. Epidermolysis bullosa (EB) is a group of inherited bullous disorders characterized by blister formation in response to mechanical trauma. There are three types of EB; simplex, junctional (JEB) and dystrophic forms (DEB). JEB is a rare type with high mortality in the neonatal period. Gastrointestinal tract atresia and/or stenosis and genitourinary anomalies may also accompany to the disorder. We diagnosed EB by clinical and histopathologic findings in 3 neonates, and only with clinical findings in 2 neonates. One of them had cleft lip-palate, umbilical hernia, tracheoesophageal fistula and esophageal atresia. Herein, we present five neonates with EB diagnosed by clinical and pathological findings and associated congenital anomalies, Epidermolizis bülloza (EB), travmayı takiben oluşan büllerle karakterize, kalıtsal geçişli bir hastalıktır. EB simpleks, “junctional” (JEB) ve distrofik (DEB) tipleri vardır. JEB tipi yenidoğan döneminde yüksek mortaliteye sahipolup, nadir görülmektedir. Gastrointestinal sistemde atrezi ve/veya stenoz, genitoüriner anomaliler gibi patolojilere eşlik edebilmektedir. Epidermolizis bülloza tanısı alan olgularımız 3’üne klinik ve histopatolojik olarak, ikisine ise yalnızca klinik bulgularla tanı konulmuştur. Olgulardan birinde yarık dudak-damak, umbilikal herni, trakeoösefagiyal fistül ve ösefagus atrezisi mevcuttur. Burada EB tanısı alan beş yenidoğan klinik bulgular, patolojik incelemeler ve eşlik eden doğumsal anomalileriyle birlikte sunulmuştur.

Bu çalışmada aç bırakılan ve beslenen Oncorhynchus mykiss'in kas dokusu yağ asit bileşimleri araştırılmıştır. Yağ asit analizleri Gaz Kromatografisi (GC) ile yapılmıştır.Aç bırakılan ve beslenen balıkların yağ asit bileşimleri kalitatif açıdan fark göstermemiştir ancak, kantitatif farklılıkların olduğu gözlenmiştir. Kas dokusunda en fazla bulunan yağ asitlerinin, palmitik asit (C16:0), stearik asit (C18:0), miristoleik asit (C14:1), oleik asit (C18:1 ?-9), linoleik asit (C18:2 ?-3), linolenik asit (C18:3 ?-3) ve dokosaheksaenoik asit (C22:6 ?-3) olduğu bulunmuştur. Tek çift bağlı doymamış yağ asitleri ve bazı çoklu doymamış yağ asitlerinin (C18:2 ?-6, C18:3 ?-3, C22:6 ?-3) açlık periyodunda azaldığı belirlenmiştir. Çalışma sonuçları, balıkların kas dokusu yağ asidi bileşiminin besine bağlı olarak değiştiğini göstermiştir.İnsan sağlığı açısından çok önemli bir parametre olan ?-3/?-6 oranının beslenen balıkların kas dokusunda aç bırakılan balıklardan daha yüksek olduğu belirlenmiştir. Bu durum göz önüne alındığında insanlar tarafından, dengeli ve düzenli beslenen balıkları tüketmenin daha sağlıklı olabileceği sonucu çıkarılabilir.Anahtar Kelimeler: Oncorhynchus mykiss, kas, yağ, yağ asitleri, beslenme, açlık. In this study the fatty acid compositions of muscle tissue of Oncorhynchus mykiss's starved and fed were investigated. The fatty acid analyses were carried out by Gas Chromatography (GC).No qualitative variation occured in the fatty acid compositions of the fish which were fed and starved but there were quantitative differences between fish groups. The most abundant fatty acids in the muscle tissues were palmitic acid (C16:0), stearic acid (C18:0), miristoleic acid (14:1), oleic acid (C18:1 ?-9), linoleic acid (C18:2 ?-3), linolenic acid (C18:3 ?-3) and docosahexaenoic acid (C22:6 ?-3). It was found that the percentages of monounsaturated and some polyunsaturated fatty acids (C18:2 ?-6, C18:3 ?-3, C22:6 ?-3) were reduced in starved period. Results of this study show that fatty acid composition in muscle tissue of fish can considerably vary, depending on their diet.It was observed that ¬ ?-3/ ?-6 ratio which is a very important parameter in terms of human health was higher in muscles tissue of fed fishes than starved ones. The most common findings were patent foramen ovale (PFO) (24.9%), atrial septal defect (ASD) (8.4%) and pulmonary stenosis (5.7%). There were various echocardiographic findings such as small ASD or trace valvular regurgitations in 38.4% of patients, while their murmurs were considered innocent. Accordingly, the results of physical examination and echocardiography showed statistically significant differences (p <0.001). After elimination of physiological mitral regurgitation and PFO, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy rate of physical examination to discriminate between normal and abnormal echocardiographic findings was 31%, 99.2%, 92.5%, 82.2% and 83% respectively. Conclusion: This study showed that some minor cardiac defects can be missed by auscultation in patients referred to pediatric cardiology clinics because of cardiac murmurs.
