Diz osteoartriti nedeniyle trombositten zengin plazma uygulamasını takiben gelişen septik artrit olgusu, klinik ve laboratuvar bulguları eşliğinde sunuldu. 71 yaşında erkek hasta, sol dizine trombositten zengin plazma uygulaması sonrası gelişen ağrı, kısıtlılık ve şişlik şikayeti ile kliniğimize başvurdu. Fizik muayene ve laboratuvar bulguları ile septik artrit tanısı kondu. Antibiyotik ve fizik tedavi uygulamasıyla tedavi edildi. Diz osteoartriti önemli bir sağlık sorunudur. Pek çok farklı tedavi uygulanmasına rağmen, kesin tedavisi yoktur. Trombositten zengin plazma uygulaması son zamanlardaki en popüler tedavidir. Kolay uygulama, hastaneye yatmaya gerek olmaması ve yan etkilerinin düşük olması gibi avantajların yanında , antibakteriyel etkisi de vardır. Fakat diğer tüm intra artiküler uygulamalarda olduğu gibi septik artrit gibi önemli bir komplikasyonun ortaya çıkabileceği unutulmamalıdır., A case of septic arthritis following platelet-rich plasma treatment due to knee osteoarthrities is presented in the context of clinical and laboratory findings. A 71 years old male patient with osteoarthrities of the knee showed pain, limitation of movement and swelling in the left knee after platelet-rich plasma administration. Septic artist is made diagnosis as a result of physical examination and medical workup. The patient was mobilized by the help of suitable antibiotic and physical therapy application. Nowadays, musculoskeletal system injuries are significant health problems. Despite the fact that many different methods have been applied for the treatment, no definitive treatment method has been determined. The platelet-rich plasma method has become increasingly popular in recent years. Platelet-rich plasma has advantages such as easy application, no need to hospitalization and low side effects, it also has an antibacterial effect. However, it should be remembered that a significant complication may occur such as septic arthritis as in all intraarticular applications.

Background: The aim of this study was to investigate theassociation between rehabilitation outcomes and cost in patients withhemiplegia due to stroke.Methods: The files of stroke patients who were appliedto a rehabilitation program between April 2016 and February 2019 in HarranUniversity Research and Training Hospital, Physical Medicine and Rehabilitation(PMR) Clinic were retrospectively reviewed. The patients who had stroke for thefirst time with a sequelae of hemiplegia and who had been rehabilitated in ourinpatient clinic within four months after stroke were enrolled. Thedemographical and clinical characteristics of the patients as well as thehospital invoices were examined.Results: A total of 28 patients (13 females, 15 males)with a mean age of 60.8±11.3 years were included. The etiologies were ischemicand hemorrhagic in 82.1% and 17.9% of the patients, respectively. Hypertension(HT) was the most common comorbidity (82.1%) followed by diabetes mellitus(46.4%). While HT was present in all female patients, 61.5% of male patientshad HT (p=0.0013). Only 10.7% of the patients had no systemic disease. Allpatients (except one patient) were consulted to different departments, andInternal Medicine was the most frequent department with a percentage of 71.4%. Psychiatricconsultation was requested in 80% of patients aged 65 years and over, and thisrate was 16.7% in patients younger than 65 years (p=0.001). Shoulder pain wasthe most common complication (35.7%). While 50% of elderly patients hadshoulder pain, this rate was 10% under 65 years (p=0.034). The mean hospitalstay was 59.9 ± 31.4 days and the mean invoice was 10.660 ± 6130.8 TL. Afterthe rehabilitation program, functional ambulation category improved in 82.1% ofpatients.Conclusion: Stroke rehabilitation improves the functionalstatus of the patients, however rehabilitation programs inflict a financialburden. In order to reduce this cost, proper diagnosis and treatment ofsystemic diseases that may lead to stroke, particularly HT, may reduce the financialburden., Amaç: Bu çalışmada, inme nedeniyle hemipleji gelişen hastalarınrehabilitasyon sonuçları ile maliyet arasındaki ilişkinin değerlendirilmesiamaçlandı.Materyal veMetod: Harran ÜniversitesiAraştırma ve Uygulama Hastanesi Fizik Tedavi ve Rehabilitasyon (FTR)Kliniğinde, Nisan 2016 ila Şubat 2019 tarihleri arasında yatarak rehabilitasyontedavisi uygulanmış inme hastaların dosyaları retrospektif olarak tarandı. İlkdefa inme geçirip hemipleji sekeli olan, inme sonrası 4 ay içinde yatarakrehabilitasyon tedavisine başlanan hastalar dahil edildi. Hastaların demografikve klinik özellikleri yanı sıra hastane faturaları incelendi.Bulgular: Yaş ortalaması 60.8±11.3 yıl olan 28 (13 kadın, 15 erkek) hasta dahiledildi. Hastaların %82.1’inde etiyoloji iskemik nedenli iken, %17.9’undahemorajikti. Hipertansiyon (HT) en sık (%82.1) eşlik eden komorbidite olup,bunu diabetes mellitus (%46.4) takip etmekteydi. Kadın hastaların hepsinde HTvarken erkeklerde bu oran %61.5 idi (p=0.0013). Hastaların sadece %10.7’sinde ek hiçbir sistemik hastalık yoktu. Bir hasta dışında tüm hastalara farklıbranşlara konsültasyon istenirken, %71.4 ile Dahiliye bunlar arasında en sıktı.65 yaş ve üzeri hastaların %80’inde Psikiyatri konsültasyonu istenirken, buoran 65 yaş altındakilerde %16.7 idi (p=0.001). Omuz ağrısı en sıkkomplikasyondu (%35.7). 65 yaş ve üzeri hastaların %50’sinde omuz ağrısı varken65 yaş altında bu oran %10 idi (p=0.034). FTR kliniğinde ortalama yatış süresi59.9±31.4 gün ve fatura ortalaması 10.660±6130.8 TL idi. Rehabilitasyonprogramı sonunda hastaların %82.1’inde fonksiyonel ambulasyon skalasındailerleme kaydedildi. Sonuç: İnme rehabilitasyonu ile hastaların fonksiyonel durumlarında gelişmesağlanmakla beraber ciddi bir ekonomik yük meydana getirmektedir. Bu maliyetiazaltmak için başta HT olmak üzere inmeye yol açabilecek sistemik hastalıklarıntanı ve tedavisinin uygun şekilde yapılması bu mali yükün azalmasına neden olabilir.

Amaç: Romatoid artrite (RA) bağlı olarak el bileğinde oluşan kas iskelet sistemi hastalıklarını ultrasonografiile tespit etmek ve RA'lı hastalarda arteryel dopplerdeki kan akım patern değişikliklerini saptamakamaçlanmıştır.Materyal ve metod: Amerikan Romatizma Birliği (ACR) kriterlerine göre RAtanısı koyulmuş 35 hasta (70el bileği) ve kontrol grubu olarak 20 sağlıklı gönüllü (40 el bileği) çalışmaya alındı. Median sinir kesit alanı(MSA), anteroposterior (AP) ve transvers çaplar, karpal tünel girişi [proksimal (p)] ve bilek kıvrımı [distal(d)] düzeylerinde ölçüldü. Karpal tünel girişi düzeyinde fleksör retinakulum kalınlığı değerlendirildi. RenkliDoppler ultrason incelemesinde her iki el nötral pozisyonda iken radial ve ulnar arterler yüksek rezolüsyonlu12 MHz transduser (Logic 7) ile değerlendirildi.Bulgular: Gruplar arası karşılaştırmada RA grubunda, kontrol grubuna göre AP-p ve AP-d çaplar anlamlıolarak daha küçük bulundu (p0,05).Sonuç: KTS klinik bulgu ve semptomu olmayan orta hastalık aktivitesine sahip RA'lı hastalarda sağlıklıkontrollere göre MSA sonuçları benzerdir. Kronik inflamasyona bağlı olarak median sinirin vazomotorfonksiyonlarındaki bozukluk radial kan akımındaki azalmayı açıklayabilir., Objective: The aim of the study is to detect the musculoskeletal diseases of the wrist depending onrheumatoid arthritis (RA) by ultrasound and to determine changes in arterial Doppler blood flow patterns inpatients with RA.Materials and Methods: 35 patients (70 wrists) with a diagnosis of RA according to the American Collegeof Rheumatology criteria and a control group of 20 healthy volunteers (40 wrists) were enrolled in the study.The median nerve cross-sectional area (MSA), anteroposterior (AP), and transverse diameters at the levels ofcarpal tunnel entrance [proximal (p)] and wrist bend [distal (d)] were measured. The flexor retinaculumthickness was evaluated at the level of carpal tunnel entrance. In the color Doppler ultrasound examination,radial and ulnar arteries were evaluated with high resolution 12 MHz transducer (Logic 7) while both handswere in a neutral position.Results: In comparison between the groups, AP-p and AP-d diameters were significantly smaller in the RAgroup than in the control group (p0.05).Conclusion: RA patients with moderate disease activity and without clinical signs and symptoms of CTShave similar MSA results compared to healthy controls. The vasomotor function disorder depending on thechronic inflammation in median nerve may explain the reduction of radial blood flow.

The coexistence of rheumatoid arthritis (RA) and familial Mediterranean fever (FMF) has been rarely seen in case reports in the literature. Herein, we wanted to present a patient who had been followed up and treated as RA, but on investigation we concluded that he really had FMF and its joint complaints associated with sacroiliitis. Recovery was achieved by etanercept administered as if he was an RA patient.

Amaç: : İnme sonrası hemipleji gelişen hastalarda hareket güçlüğü ve fonksiyonel kısıtlılık önemli bir sorun teşkil etmekle birlikte bu hastalardaki motor sonuçların prognozu kortikospinal traktus (KST)'un hasarı ile yakından ilişkilidir. Difüzyon Tensör Görüntüleme (DTG), KST'nin durumunu ve beyaz cevherdeki hasarı değerlendirmede son zamanlarda kullanılan üstün bir görüntüleme yöntemidir. Konvansiyonel tedavilerin aksine düşük maliyetli sanal gerçeklik oyunları tedavisi (SGOT), tekrarlayıcı,yoğun eğitim,hedef ve görev odaklı,yüksek motivasyon ve aktif katılım sağlayan eğlenceli bir tedavi yöntemidir. Bu çalışmanın amacı hemiplejik hastalarda interaktif video oyunlarının motor ve fonksiyonel performans üzerine etkinliği ve radyolojik progresyonun DTG ile araştırılmasıdırGereç-Yöntem: Çalışmaya 18-80 yaş aralığında inmeye bağlı hemipleji gelişmiş 18 hasta alındı.. Hastalar randomize olarak iki gruba ayrıldı.Birinci grup (rehabilitasyon grubu), 9 hasta 4 hafta geleneksel fizyoterapi programına, ikinci grup (oyun grubu) 9 hasta ise bu programa ilave olarak Xbox 360 Kinect ile sanal gerçeklik tedavisine alındı. Tüm hastaların tedavi öncesi demografik verileri , tedavi öncesi ve sonrası üst ekstremite eklem hareket açıklığı (EHA) ve spastisite dereceleri, Fugl Mayer motor fonksiyon skalası (FM), Brunnstrom evreleri, Fonksiyonel Bağımsızlık Ölçütü (FBÖ) ve Motor Activity Log (MAL) skorları kaydedildi. Radyolojik progresyonu araştırmak için tüm hastalara tedavi öncesi ve sonrası DTG çekildi. DTG parametrelerinden Fraksiyone Anizotropi (FA) ve Mean Difüzivite (MD) değerlendirildi. Bulgular: Tedavi sonrasında her iki grupta EHA artmakla birlikte spastisite azalması oyun grubunda daha anlamlıydı. Rehabilitasyon gurubunda Brunnstrom üst ekstremite artışı anlamlı bulundu.Her 2 grupta tedavi sonrası FBÖ'de ve FM total skorlarında anlamlı artış bulundu.Her iki grubun tedavi sonrası MAL miktar ve nitelik sonuçları anlamlı bulunmadı. DTI verileri değerlendirildiğinde etkilenmiş hemisferde rehabilitasyon grubunda tedavi sonrası Korona radiata (KR) da ve oyun grubunda Posterior Limb of İnternal Kapsül (PLIC) de FA değerinde anlamlı azalma saptandı ,etkilenmiş hemisferde rehabilitasyon grubunda üst pons posterolateral (ÜPPL)'de MD değerinde anlamlı azalma saptandı. Sonuç: Geleneksel fizyoterapiye eklenen Kinect oyun teknolojisinin, spastisiteyi azaltmak ve EHA'nı arttırmak suretiyle inmeye bağlı hemipleji geçirmiş hastalarda motor ve fonksiyonel performansı arttırdığı gözlenmiştir.Bu teknolojinin KST'de etkilenmemiş hemisferde FA'yı arttırmak suretiyle kortikal reorganizasyon gelişiminde etkili olabileceği bulunmuştur. Sanal gerçeklik oyun tedavisi, hemiplejili hastaların rehabilitasyonunda ulaşımı kolay ve hastanın kendi evinde kullanabileceği alternatif bir tedavi yöntemi olarak kullanılabilir. Patients were randomly divided into two group.The first group(rehabilitation group) including 9 patients who received traditional rehabilitation programme and the second group (game group) also including 9 patients who received virtual reality theraphy with Xbox 360 kinect in addition to traditional rehabilitation for 4 weeks..All patients upper extremity range of motion(ROM) and degree of spasticity,Fugl Mayer Assesment Scores(FM), Brunnstorm Hemiplegia Classification, Functional İndependence Measure(FIM) and Motor Activity Log Score were recorded at beginning and after treatment and demographic data were recorded at begining. All patients underwent DTI at beginning and after treatment to investigate radiological progression.Fractional Anisotropy(FA) and mean diffusivity (MD) were analysed in DTI meter. Results: Upper extremity range of motion was increased in both two groups , spasticity reduction was significantly greater in game group.Brunnstorm Hemiplegia Classification was increased in rehabitation group.FIM and FM total scores were significantly increased in both two groups.MAL quantity and quality results were not significant in both two groups.In affected hemisphere FA values also were decreased in posterior limb of internal capsule(PLIC) in game group and corona radiata in rehabilitation group.MD values were decreased in posterolateral of upper pons(UPPL) in game group.Conclusion:It has been observed that Kinect game technology increased the motor and functional performance in patients with hemiplegia due to stroke by reducing spasticity and increasing upper ROM.This technology could be effective in cortical reoorganization development by increasing FA in unaffected hemisfer CST.Virtual reality gaming theraphy can be used as an alternative treatment method in rehabilitation of hemiplegic patients , because of its accessible and domestic treatment.Key Words:Stroke, hemiplegia,virtual reality theraphy, diffusion tensor imaging, neuronal plasticity 88

Ankilozan spondilit (AS) etyopatogenezinde adipo-sitokinlerin rolü olduğu birçok çalışmada öne sürülmüştür. Yapılan çalışmalarda AS hastalarında adipo-sitokinlerden bazıları yüksek, bazıları ise düşük bulunmuştur. Fakat bu adipo-sitokinlerin hangi mekanizma ile AS patogenezinde rol aldıkları henüz tam olarak anlaşılabilmiş değildir.Ankilozan spondilitte kas iskelet sistemi dışında en sık etkilenen organ göz olup, olası komplikasyonların erken tanı ve tedavisi çok önemlidir. Son zamanlarda kullanılmaya başlanan non-invaziv bir yöntem olan optik koherens tomografi (OKT) ile iyonizan radyasyona maruz kalmadan objektif ölçüm değerleri elde edilebilmektedir. Literatürde AS hastalarının OKT bulguları ile ilgili çok az sayıda çalışma mevcuttur. Bu çalışmanın amacı; AS'li hastalarda serum adipo-sitokin düzeylerini ve OKT bulgularını tespit etmekti. Literatür taramamızda AS hastalarında bu 2 bulgunun beraber incelendiği bir çalışmaya rastlamadık. Çalışmamızda modifiye New York kriterlerini karşılayan 47 AS hastası ve hastalarla yaş, cinsiyet, medeni durum, vücut kitle indeksi uyumlu 23 sağlıklı gönüllü incelendi. Hastalarda ve sağlıklı gönüllülerde adipo-sitokin seviyeleri ile OKT bulguları değerlendirildi.Çalışmanın sonucunda, AS hastalarında serum visfatin düzeylerinin yüksek, fetuin-A düzeylerinin ise istatiksel olarak anlamlı şekilde düşük olduğunu tespit ettik. Fakat bu farklılıklar hastalık aktivitesi veya akut faz reaktanları ile korele değildi.Rezistin ve adiponektin seviyelerinin ise kontrollere göre anlamlı bir farklılığı tespit edilememesine rağmen AS'li hastalarda rezistin seviyeleri ile CRP arasında negatif korelasyon olduğunu saptadık.Ayrıca AS hastalarının gözlerini OKT ile değerlendirdiğimizde, koroid kalınlıklarının arttığını tespit ettik. Dahası artan bu koroid kalınlığının adipo-sitokinlerden visfatin ve rezistin ile negatif korelasyonu da mevcuttu. Çalışmamızın sonuçları, AS hastalarında adipo-sitokinlerin de inflamasyona ve hastalığın etyopatogenezine katkısı olabileceği görüşünü desteklemektedir. Ayrıca OKT ile koroid kalınlığı değerlendirilerek bu hasta grubunda gelişebilecek olası göz komplikasyonlarının erken tanısına yardımcı olabileceğini düşünmekteyiz. Adipo-sitokinlerin etyopatogenezdeki rolü ve OKT'nin AS hastalarında sağlayabileceği faydalar için, daha büyük hasta ve kontrol gruplarıyla yapılacak çalışmalara ihtiyaç olduğu açıktır. In several studies, some of the adipocytokines in AS patients were found to be high and others were found low. But, the mechanism by which these adipocytokines are involved in the pathogenesis of AS is not yet fully understood.Ankylosing spondylitis is the most common organs affected eye beyond the musculoskeletal system, early diagnosis and treatment is very important for possible complications.Optical coherence tomography (OCT), a non-invasive method that has recently begun to be used, provides objective measurement values without exposure to ionizing radiation. There are very few studies in the literature regarding the OCT findings of AS patients. The aim of this study was to determine the levels of serum adipocytokines and OCT findings in AS patients. This study included 47 AS patients who met the modified New York criteria and 23 healthy volunteers with matched age, sex, marital status and boddy mass index. Adipocytokine levels and OCT findings were evaluated in patients and healthy volunteers. As a result of our study, we found that serum visfatin levels were significantly higher and fetuin-A levels were lower in AS patients. But, this difference did not correlate with disease activity or acute phase reactants.Although no significant difference was found between resistin and adiponectin levels compared to controls, it was found that there was a negative correlation between resistin levels and CRP in AS patients. Also we have found that the choroidal thickness increases in the eyes of AS patients. Moreover, we found that this increased choroidal thickness correlated negatively with visfatin and resistin from adipocytokines.The results of our study support the idea that adipocytokines in AS patients may also contribute to the inflammation and etiopathogenesis of the disease.We also believe that OCT may be helpful in the early diagnosis of possible eye complications in this patient group by evaluating the choroidal thickness.It is clear that, for the role of adipo-cytokines in etiopathogenesis and benefits of OCT for AS patients are a necessity for studies with larger patients and control groups.Key Words: Ankylosing spondylitis, adipocytokines, optical coherence tomography 88
