Resume Objectif La relation entre les inflammations chroniques et le declin cognitif est encore peu claire ; cependant, il existe de plus en plus d’elements indiquant une influence de l’inflammation systemique. L’objectif de cette etude est d’etudier si la rhinosinusite chronique (RSC) en presence de demence ou de deficience cognitive legere (DCL) est associee a l’aggravation du declin cognitif. Materiels et methodes Nous avons etudie retrospectivement les donnees de patients se plaignant d’un declin de la memoire ayant realise une imagerie par resonance magnetique (IRM) entre janvier 2006 et avril 2019. Selon leur score au Mini-Mental State Examination (MMSE), les patients (n = 661) ont ete divises en trois groupes : demence (≤ 17), DCL (18–23), et normal (≥ 24). La RSC a ete definie comme un score total egal ou superieur a 4 selon le systeme de score de Lund–Mackay a partir des IRM cerebrales. Les analyses de regressions logistiques multiples ont permis d’estimer le rapport des cotes ajustees (RCa) de l’association entre une RSC et la demence ou la DCL. Parmi les sujets ayant recu une MMSE de suivi (n = 286), une ANOVA a ete utilisee afin d’estimer les differences dans l’evolution des scores MMSE entre les groupes presentant une RSC ou non. Resultats D’apres le score MMSS initial, il y avait 221 sujets atteints de demence, 195 presentant une DCL et 245 avec des resultats normaux. La RSC n’a pas ete associee de facon significative avec la demence (RCa = 1,519, IC = [0,909 ; 2,538], p = 0,111) tandis qu’elle a ete associee de facon suggestive avec la presence d’une DCL (RCa = 1,740, IC = [1,041 ; 2,906], p = 0,034). Les scores MMSE de suivi ont diminue d’avantage dans le groupe des RSC que dans le groupe sans RSC (p = 0,009). Particulierement, dans le groupe demence initial, il existait une difference significative entre les groupes concernant l’evolution des scores MMSE initial et de suivi (13,6 ± 4,3 a 11,1 ± 6,3 dans le groupe de DCL contre 13,5 ± 3,3 a 14,4 ± 5,4 dans le groupe sans DCL, p = 0,002). Conclusion Les resultats de l’etude impliquent une association potentielle entre la presence d’une RSC et l’aggravation du declin cognitif. Les medecins doivent etre au fait de cette possibilite chez les patients diagnostiques cliniquement avec une RSC.

This paper describes the design of an integrated duplexer with electrical balance network using the phase locked loop (PLL) for impedance variation tracking. The hybrid transformer is devised for blocking the transmitted signal leaking to the receiver. We suggest a method of impedance tracking based on PLL system, which can maintain high isolation between the transmitter and the receiver. The PLL is employed to sense the transmitter leakage continuously and tune the impedance of the electrical balance network adaptively in real time. The isolation between the transmitter and the receiver is maintained almost constantly in a range of capacitances used in the antenna model, compared with that without the phase locked loop. The return loss at the transmitter port and the insertion loss from the transmitter to the antenna port are measured 30 dB and 6.9 dB at 1.95 GHz, respectively. The planar inverted-F antenna (PIFA) model is used for simulating and measuring antenna and electrical balance network. A capacitor value comprising the PIFA is varied and the proposed duplexer is investigated, leading to automatic impedance tracking in some range.

This study was conducted to develop calcium-fortified shank bone extract (SBE) and to determine the effect of adding calcium lactate on physico-chemical characteristics of SBE during cold storage. The following five experiment groups were used: Control (0%, no addition), T1 (0.05% calcium lactate), T2 (0.1% calcium lactate), T3 (0.5% calcium lactate), and T4 (1% calcium lactate). When the concentration of calcium lactate added to the SBE was increased, the pH, redness, and yellowness values were significantly reduced, whereas the salinity, sugar content, and turbidity of SBE were significantly increased. Aim The relationship between chronic inflammatory disease and cognitive decline is still unclear, but there is increasing evidence to support the role of systemic inflammation. The aim of this study was to investigate if chronic rhinosinusitis (CRS) in dementia or mild cognitive impairment (MCI) is associated with the progression of cognitive decline. Material and methods We retrospectively reviewed the data of patients who complained of memory impairment, and underwent brain magnetic resonance imaging (MRI) from January 2006 to April 2019. According to the Mini-Mental State Examination (MMSE) score, subjects (n = 661) were divided into three groups: dementia (≤ 17), MCI (18–23), and normal (≥ 24). CRS was defined as a total score of greater than or equal to 4 according to the Lund–Mackay scoring system using brain MRI. Multiple logistic regression analyses estimated adjusted odds ratio (aOR) for the association between CRS and dementia or MCI. Among the subjects with follow-up MMSE (n = 286), a repeated-measures ANOVA was used to assess the difference of changes in MMSE scores between the groups with and without CRS. Results According to the initial MMSE score, there were 221 subjects with dementia, 195 with MCI, and 245 with normal results. CRS was not significantly associated with dementia (aOR = 1.519, CI = 0.909–2.538, P = 0.111), while being suggestively associated with MCI (aOR = 1.740, CI = 1.041–2.906, P = 0.034). The MMSE scores at follow-up decreased further in subjects with CRS than in those without CRS (P = 0.009). Especially, in the initial dementia group, there was a significant between-group difference in the MMSE score from baseline to follow-up (13.6 ± 4.3 to 11.1 ± 6.3 in CRS group vs. 13.5 ± 3.3 to 14.4 ± 5.4 in no CRS group, P = 0.002). Conclusion The result of the present study implies a potential association between CRS and progression of cognitive decline. Physicians should be aware of this possibility in patients with clinically diagnosed CRS.

The isolation between transmitter and receiver for the radars is ultimately important since the transmitting radar signals can be penetrated the receiver directly in case a bad isolation is formed. This paper describes the isolation between the transceiver and the design of an integrated full duplexer using the phase locked loop (PLL). Although the antenna impedance varies arbitrarily, the PLL tracks the impedance variation in real time, leading to improvement of isolation between the transmitter and the receiver of radars. The full duplexer reduces the transmitter leakage up to 45 dB using the balance network along with the PLL in measurement.

Background The authors present surgical results of 64 Asian patients with deviated nose and introduce important key maneuvers used in the surgery. Methods Sixty-four consecutive patients who underwent corrective rhinoplasty because of a deviated nose were analyzed retrospectively. Approaches and techniques used for the surgery, surgical results, and complications were evaluated. A questionnaire was used to evaluate each patient's functional and esthetic satisfaction. Preoperative and postoperative standardized photographs of the face were evaluated to judge objectively the esthetic outcomes of the surgery. Results An endonasal approach was used in 44 patients and an open approach was used in 20 patients. Five important key maneuvers to correct the deviation were noted, in the order of frequency: septoplasty, osteotomy, spreader graft, turbinoplasty, and camouflage graft. Additional procedures included augmentation of the dorsum, tip surgery, and hump removal. On a 5-point box scale of 0–4, the patients scored their satisfaction on functional improvements as 3.2 ± 0.79 and their esthetic satisfaction as 3.0 ± 0.8. The objective evaluation of the appearance showed complete correction in 84.4%, a minimally visible deviation in 10.9%, and a residual deviation but less than before surgery in 4.7%. Aside from residual deviations, complications included a slight depression of the middorsum after spreader graft and a malpositioning of the dorsal cartilage graft. Conclusion Deviated nose in Asians can be managed successfully by combining key maneuvers that correct specific anatomic abnormalities. Many of these key maneuvers require modifications that take into account the race-specific characteristics of the Asian nose.
