Abstract Background A high-quality medical humanities (MH) education program is essential to developing a successful medical practitioner and can influence clinical performance. It is also vital to improve the evaluation of MH education to restore harmonious mutual relationships in medical care. However, studies have yet to discuss the correlation between the learning quality and quantity of medical humanities curriculums (MHC) and medical students' scores of clinical curriculums and clinical performance. The study aimed to assess the correlation between the learning quality and quantity of MHC and medical students' performance. Methods We conducted a retrospective cohort study by analyzing a dataset of students' learning records. After excluding students with missing demographic information (n = 1) and overseas Chinese students (n = 15), the study included six- and seven-year program medical school students (n = 354) at National Yang-Ming University who were admitted between 2012 and 2014. The correlation between learning quality and quantity in MHC and students' following performance was evaluated by multivariable-adjusted regression analyses. In Taiwan, breast cancer has the highest incidence among all cancers. Although adjunctive traditional Chinese medicine treatment (TCM) have been used to ameliorate the side effects or discomfort caused by cancer treatments, no study has focused on the assessment of the quality of life of patients undergoing adjunctive TCM treatments. This study compared the quality of life between breast cancer patients treated with and without adjunctive TCM. Questionnaires were collected from 7 hospitals with a Chinese medicine clinic in 2018 to 2019. Breast cancer patients who had cancer stages I, II, or III and also underwent resection surgery were included in the study. They were divided into 2 groups: patients receiving cancer treatments with adjunctive traditional Chinese medicine (TCM group) and those receiving cancer treatments without adjunctive traditional Chinese medicine (non-TCM group). A 1:1 matching was used to obtain the study participants. The EQ-5D questionnaire was used to assess the quality of life. Statistical analysis was performed using the t -test and ANOVA to compare the differences between variables. The conditional multiple regression model was applied to explore the factors associated with quality of life in breast cancer patients. A total of 543 participants were surveyed, and 450 participants were included in the study. Purpose Taiwan has implemented an integrated prospective payment program (IPP) for prolonged mechanical ventilation (PMV) patients that consists of four stages of care: intensive care unit (ICU), respiratory care center (RCC), respiratory care ward (RCW), and respiratory home care (RHC). We aimed to investigate the life impact on family caregivers of PMV patients opting for a payment program and compared different care units. Method A total of 610 questionnaires were recalled. Statistical analyses were conducted by using the chi-square test and multivariate logistic regression model. Results The results indicated no associations between caregivers’ stress levels and opting for a payment program. Participants in the non-IPP group spent less time with friends and family owing to caregiver responsibilities. The results of the family domain show that the RHC group (OR = 2.54) had worsened family relationships compared with the ICU group; however, there was less psychological stress in the RCC (OR = 0.54) and RCW (OR = 0.16) groups than in the ICU group. In the social domain, RHC interviewees experienced reduced friend and family interactivity (OR = 2.18) and community or religious activities (OR = 2.06) than the ICU group. The RCW group felt that leisure and work time had less effect (OR = 0.37 and 0.41) than the ICU group. Furthermore, RCW interviewees (OR = 0.43) were less influenced by the reduced family income than the ICU group in the economic domain. Conclusions RHC family caregivers had the highest level of stress, whereas family caregivers in the RCW group had the lowest level of stress.

Some patients with cancer use adjunctive Chinese medicine, which might improve the quality of life. This study aims to investigate the effects and relative factors of adjunctive Chinese medicine on survival of hepatocellular carcinoma patients at different stages. The study population was 23 581 newly diagnosed hepatocellular carcinoma patients and received surgery from 2004 to 2010 in Taiwan. After propensity score matching with a ratio of 1:10, this study included 1339 hepatocellular carcinoma patients who used adjunctive Chinese medicine and 13 390 hepatocellular carcinoma patients who used only Western medicine treatment. All patients were observed until the end of 2012. Kaplan-Meier method and Cox proportional hazards model was applied to find the relative risk of death between these 2 groups. The study results show that the relative risk of death was lower for patients with adjunctive Chinese medicine treatment than patients with only Western medicine treatment (hazard ratio = 0.68; 95% confidence interval = 0.62-0.74). The survival rates of patients with adjunctive Chinese medicine or Western medicine treatment were as follows: 1-year survival rate: 83% versus 72%; 3-year survival rate: 53% versus 44%; and 5-year survival rate: 40% versus 31%. The factors associated with survival of hepatocellular carcinoma patients included treatment, demographic characteristics, cancer stage, health status, physician characteristics, and characteristics of primary medical institution. Moreover, stage I and stage II hepatocellular carcinoma patients had better survival outcome than stage III patients by using adjunctive Chinese medicine therapy. The effect of adjunctive Chinese medicine was better on early-stage disease.

Background: In 1990, The National Institute of Health of Taiwan set up the Step-down care model for the prolonged mechanical ventilation (PMV) patients by four stages of care, Intensive Care unit for 21 days, Respiratory Care Center for 4 2 days and then the Respiratory Care Ward (institute care) or Home Care. Due to care quality improved, the number of patients were accumulated and the cost expenditure increased. The Government want to reduce the cost, so began to promote the concept of hospice palliative care (HPC) for these unconsciousness PMV patients since 2013. The purpose of this study was to determine the acceptance factors that affect the concept of HPC in the PMV patients' major caregivers in Taiwan. Method: The survey method and quota sampling by using a structured questionnaire to collect the information. After developing the questionnaire, the content was rated by seven experts, resulting in a mean content validity index (CVI) of 0.96. The data for this study were collected from four care stages by using a predesigned questionnaire, and 601 of the 687 valid questionnaires were returned. (The valid response rate was 87.5%). The collected data were analyzed using a descriptive statistics test, Fisher's exact test and multiple regression models with generalized estimating equations. This study was approved by the Institutional Review Board of the study organizations. Results: In terms of the basic characteristics of major caregivers, there are 49.25% male and 50.75% female, mostly families identity mainly adult children (54.74%) and spouse (18.47%). The key element of acceptance factors in the PMV patients' major caregivers that affect the concept of HPC, after controlling for patient characteristics, include the care stage of PMV patient used, daily ventilator used time, sex and identity of caregiver, ever heard the HPC from medical staff introduction during hospitalization, and the cognition toward the HPC. Conclusions: This study not to show positive relationship to the cognitive and acceptance of HPC. Facing the correlated problems of HPC, the frank attitude and communication among the patients and their family members might help the appropriate decision-making and afford the good HPC. This study can provide the government a reference when promoting the HPC. Background: Objective Structured Clinical Examination (OSCE) is an objective way to evaluate the clinical ability, include both skills and attitude, for the medical assessment, interviews and medical examinations. OSCE has become a part of doctor's national licensure examination. The purpose of this study is to explore the skills and attitudes change of respiratory therapy students by the OSCE test, was performed in the pre- and post-clinical practice. Method: There were 10 three years program students (8 female) who are in second grade enrolled pre-clinical practice evaluation, After one week entry level clinical practice in the medical center hospital, only 9 students (7 female) took the post-clinical practice evaluation. We recruit 4 standardized patients and 4 examiner in 4 OSCE stations, included how to use the small volume nebulizer (SVN), patient education of meter dose inhaler (MDI), peri-operative respiratory care (PORC) and nonpharmacological dyspnea management (Oxygen). The process of each station included reviewing the medical history, patient assessment and interviews, education for the standardized patient, clinical situation solving and feedback. The evaluation score divide to complete met (2 point), partial met (1 point), and unmet {0 point) three grade. The final total score for the student in each station was the partial score met plus complete met score as the numerator, and the total complete met items as the denominator. The statistic method was the paired t test to evaluate each station the difference between pre- and post-clinical practice. Results: The tour OSCE station score result was significantly improved after clinical practice except the SVN station (44.89±I7.77 vs 49.33±15.73, P=0.234). There were significant improve in MDI station (54.22±1 5.57 vs 77.78±12.19, P<0.05), PORC station (44.22±19.04 vs 75-33±11.03, P<0.05) and Oxygen station (34.67± 23.69 vs 66.22±12.19, P<0.05). Conclusions: This study demonstrates that the clinical practice can improve respirator)' therapy students' critical chinking and clinical skills. The result could be confirmed by the OSCE test.
