- Abstract
Background: Family medicine is a discipline that prioritizes the patient-physician relationship, particularly focusing on preventive and developmental health services. In family medicine, the comprehensive pediatric care includes monitoring growth and development from birth, assessing health status, and providing preventive healthcare elements such as vaccinations and health education. Objective: Our study aims to assess the knowledge levels, attitudes, and awareness of family physicians regarding newborn screening and monitoring. Method: For this study, a survey prepared by the researcher was utilized to ascertain the sociodemographic characteristics of family physicians and their knowledge, attitudes, and awareness regarding newborn period monitoring and screenings. Results: Out of the 318 family physicians included in our study, it was found that 99.4% performed weight measurement, 97.5% performed head circumference measurement, 97.5% assessed fontanels, 83.6% conducted height measurement, 83.3% evaluated newborn reflexes, 76.7% performed genital examination, 65.4% conducted eye examination, 48.4% performed hip examination, and 29.8% conducted hearing examination on newborns. Additionally, 58.2% of them performed red reflex testing in routine neonatal eye screening, while 8.5% evaluated for congenital critical heart disease using pulse oximetry in newborns. In the statistical analyses, no significant relationship was found between newborn reflexes, eye examination, genital examination, hip examination, hearing examination, and scoliosis examination with the variables of age and years of professional experience. Family medicine specialists perform genital examinations and hearing examinations at a statistically significantly higher rate compared to general practitioners and contracted family medicine residents (p<0.05). Conclusion: It is necessary to conduct educational interventions within the scope of professional development to enhance the knowledge levels and practices of family physicians regarding newborn screening and monitoring, as well as to increase their awareness on this matter. Laryngomalacia is the most common cause of stridor in newborns. Pediatric difficult airway is a major challenge for anesthetists and one of the main causes of perioperative respiratory complications. In order to prevent respiratory complications of general anesthesia, especially apnea, in newborns, regional anesthesia techniques such as caudal or spinal anesthesia may be preferred. Caudal anesthesia can be used as the sole anesthesia method, especially for subumbilical surgeries, or as an adjunct to general anesthesia and is an effective way to provide perioperative analgesia. Objective: The data on neonatal conjunctivitis are significantly limited in Türkiye. In this study, we aimed to investigate the epidemiology of neonatal bacterial conjunctivitis and the clinical and laboratory findings of patients in a tertiary neonatal referral hospital in İstanbul, Türkiye. Methods: This retrospective observational study was conducted over a five-year period between January 2015 and January 2020. Newborns 28 days of age who presented to our clinics with clinical features of conjunctivitis and had bacterial growth in the conjunctival culture were included. Results: Thirty-two newborns with culture-proven bacterial conjunctivitis were included. A total of 26 (81.3%) newborns required newborn intensive care unit (NICU) admission. Gram-positive bacteria growth was detected in 47% (n=15) of cases, and gram-negative growth was detected in 53% (n=17) of cases. Escherichia coli (n=12), Staphylococcus epidermidis (n=10) and Staphylococcus aureus (n=3) were the most frequently identified bacteria. The proportions of newborns with need for intravenous (IV) antibiotic treatment, bilateral eye involvement, and normal spontaneous delivery were significantly higher in the gram-negative conjunctivitis group. Conclusion: Our study provides important data regarding bacterial conjunctivitis in newborns in Türkiye, given that data in Türkiye are very limited. The high rates of NICUs admission, presence of clinical sepsis, and IV antibiotic administration show the importance of neonatal conjunctivitis as a clinical finding of systemic neonatal infections. Despite the lack of universal ocular prophylaxis in our high-volume neonatal referral center, the low number of culture-proven conjunctivitis cases challenges the current routine ocular prophylaxis suggestions. 14. Midwives’ Knowledge Level About Newborn Individualized Development Care and Assessments Program

Objective: The Newborn Individualized Developmental Care and Assessment Program (NIDCAP) has been developed to create a common point to enhance the parameters of neonatal health by Als et al. This research was planned to determine the knowledge level about NIDCAP of midwives working in the delivery room. Methods: The research is descriptive, which a sample consists of 385 midwives estimated with sampling method with the known universe. The ethical approval was obtained. Midwives’ consent was obtained. Data was collected with the questionnaire form that was prepared by researchers and five expert opinions. Numerical values and percent values were analyzed in the statistical evaluation and data were analysed with Chi-Square Test. Results: It was determined in our study that 59.2% of midwives didn’t know NIDCAP. The midwives answered correctly each of the questions about neonatal care in keeping with NIDCAP. Besides there was a significant relationship between “midwives who had foreknowledge” and “midwives who worked in Neonatal Intensive Care Unit (p=0.001), think that NIDCAP is implemented in their hospital (p=0.001) and think that their friends support NIDCAP (p=0.001)”. This result shows that getting information about NIDCAP has increased midwives’ awareness of NIDCAP. Even though the vast majority of midwives had no education, the care given by the midwives was found quite accordant with the physiology of the neonatal. Conclusion: It is important to provide training for NIDCAP and to enhance the education programs about NIDCAP for enhancing the quality of neonatal care and promoting neonatal health.

15. PERITONEAL DIALYSIS IN NEWBORNS AND INFANTS IN THE LAST 20 YEARS: SINGLE CENTER EXPERIENCE.

Objective: To evaluate patients who underwent acute and chronic peritoneal dialysis (PD) under the age of one one in terms of etiology, complications, and prognosis over 20 years and to compare the results between the first and last 10 years of acute and chronic peritoneal dialysis. Material and Method: Seventy-four peritoneal dialysis patients under the age of one in the Division of Pediatric Nephrology and Neonatal Intensive Care Unit of our hospital between January 2002 and December 2023 were evaluated retrospectively. The patients were divided into two groups: patients admitted in the 2002-2013 period (Group I) and patients admitted in the 2013-2023 period (Group II). Patients in Group I and Group II were compared in terms of acute and chronic peritoneal dialysis etiology, complications, and prognosis. Result: Forty-four of the patients (60%) were newborns, and the remaining 30 were infants (40%). There were 39 patients in Group I and 35 patients in Group II. There was no difference between Group I and Group II for acute dialysis in terms of neonatal and infant diagnoses, infectious and non-infectious complications, and prognosis (p>0.05). There was no difference between Group I and Group II in terms of neonatal and infant diagnoses, infectious and non-infectious complications, and infant prognosis (p>0.05). There was no death in the newborns in Group II and patient survival was higher than in the newborns in Group I (p=0.019). Conclusion: Peritoneal dialysis is the most commonly used method for renal replacement therapy in children facing both acute and chronic renal failure. In the last decade, despite high-quality care in the neonatal care unit, positive technological developments and effective management of PD, the most common infectious complication was still peritonitis, and the non-infectious complication was dialysate leakage, as in the previous 10 years. 16. Clinical and Laboratory Features of Culture-positive Neonatal Sepsis: A 5-year Single-center Experience at Tertiary Neonatal Intensive Care Unit in Turkey.

Objective: Neonatal sepsis is an important cause of neonatal mortality and morbidity, especially in low birthweight premature babies. This study aimed to examine the clinical and laboratory features of cases with culturepositive neonatal sepsis. Method: Medical records of 233 newborn infants with culture-positive sepsis among 4241 hospitalized patients between January 2013 and December 2017 were reviewed. Demographic and clinical data of these patients were retrospectively recorded. Results: The majority of patients was extremely and moderately preterm infants (39.1% vs. 11.6%). These patients had a history of invasive mechanical ventilation (74.2%) or central catheterization (26.9%). The mostly isolated pathogens (56.2%) were Gram-negative bacteria, especially Klebsiella pneumoniae in 67 (28.8%) cases. Post-hoc test showed a statistically significant difference in the incidence rates of leukopenia between patients infected with Gram-positive, Gram-negative bacteria and fungi (12.3%, 16.8% and 17.2%, respectively) (p=0.021). Patients who developed leukopenia (n=36, 15.5%) had a higher mortality rate compared to those with leukocytosis (n=50, 21.5%) (72.2% vs. 50%, p<0.001). The duration of total parenteral nutrition was found to be a significant risk factor in terms of mortality (p=0.015). Conclusion: Prolonged parenteral nutrition is an important risk factor for mortality in low-birth weight newborns and those with sepsis. It is noteworthy that the mortality rate is higher in newborns with sepsis who developed leukopenia and neutropenia. [ABSTRACT FROM AUTHOR]

22. Impact of transport outcomes of outborn newborns with critical congenital heart disease on surgery time and mortality rates.

Purpose: Transport of neonates with critical congenital heart disease (CCHD) necessitates professional and experienced staff and, well-equipped facilities for both the procedure and post-operative care. In this study, we aimed to evaluate the effect of transport on operation time and survival in neonates with CCHD and determine the relationship between transport characteristics and clinical status. Materials and Methods: A retrospective cross-sectional cohort study was conducted on all infants with CCHD who were transported to a university hospital between January 1, 2019 and December 31, 2019. Transport characteristics such as transport distance, accompanying healthcare provider, vital signs, oxygen saturation levels, prostaglandin E1 (PGE) use, and respiratory support were recorded. The effects of transport characteristics on surgery time, complications, and mortality were analyzed. The Society of Thoracic Surgeons-European Association for Cardio-Thoracic Surgery (STAT) Score was used to analyze mortality-related risks for congenital heart disease. They were compared with inborn newborns with CCHD. Results: A total of 55 patients with CCHD were evaluated. Thirty-one newborns (56.4%) were inborn (Inborn group), and 24 newborns (43.6%) were transferred from another center (Outborn group). There was no difference between the groups in terms of gender, gestational age, birth weight, pre-operative time, and hospital stay. Although the Outborn group had fewer STAT Score categories, there was no difference in the 30-day survival or mortality rates. In the Outborn group, five patients (20.8%) with duct-dependent CCHD were transported without PGE treatment and six ductdependent (25%) patients received oxygen during transport inappropriately. Conclusion: The administration of PGE and oxygen therapies to specific transported infants was inappropriate. Therefore, referral or transport team members should be familiar infants with CCHD. The impact of transport on surgical timing did not yield a significant effect. However, similar mortality rates were observed in both groups, despite the Outborn group demonstrating lower STAT scores. 23. Prenatal Substance Abuse: a 1-Year Single-Center Experience at a Tertiary Neonatal Intensive Care Unit.

Objective: Eight newborns diagnosed with prenatal substance abuse were evaluated for the clinical course of neonatal abstinence syndrome depending on the type of substances used by their mothers during pregnancy. Material and Methods: A retrospective study of neonates with prenatal substance abuse admitted to our tertiary care university hospital's Neonatal Intensive Care Unit (NICU) was conducted between February 2022 and March 2023. Demographic data, withdrawal symptoms, need for pharmacological treatment, and duration of hospitalization were collected. Newborns exposed to substances were divided into two groups: opioid and non-opioid (methamphetamine, Bonsai, marijuana). Results: Eight infants were included in the study. Four cases (50%) were in the group exposed to opioids. Pregnant users of opioids were older (28±6.73) than non-opioid substance users (21±4.83). The mean birth weight of newborns exposed to opioids (2541 g) was lower than that of the non-opioid group (3020 g). The average length of hospital stay was longer in the opioid group (34 days) compared to the non-opioid group (10 days). All newborns exposed to substances were born preterm (<37 gestational weeks). Withdrawal symptoms were observed in all cases in the opioid group and three cases in the non-opioid group to varying degrees. Medical treatment was required in three out of the seven cases with withdrawal symptoms. All patients requiring treatment were in the opioid group. All infants were discharged in good health. Conclusion: Substance exposure during the prenatal period is a significant cause of preterm birth in neonates. Many of these substances can cause varying degrees of withdrawal syndrome in newborns. Replacement therapies used during pregnancy containing heroin and naloxone can lead to a more severe, prolonged, and treatment-requiring neonatal withdrawal syndrome than other substances. In cases where morphine and methadone are unavailable, the symptoms of withdrawal syndrome can be managed with phenobarbital. It is crucial to monitor all newborns with prenatal exposure to substances early because it allows for appropriate intervention and treatment. [ABSTRACT FROM AUTHOR]

Background: It was aimed to investigate the clinical and demographic characteristics, risk factors and treatment methods of newborns hospitalised with a diagnosis of indirect hyperbilirubinemia. Materials and Methods: Demographic characteristics, laboratory data, and risk factors for neonatal jaundice were retrospectively examined in 615 newborn infants with jaundice admitted to the Neonatal Intensive Care Unit. Results: The infants comprised 340 (55.3%) females and 275 (44.7%) males; of which 532 (86.50%) were born at full-term, 80 (13%) premature, and 3 (0.50%) post-full-term. Birth weights; It was 3185±50 gr. The most common cause of jaundice was physiological jaundice/jaundice of unknown cause (25.7%) wheras the lowest rates were intracranial bleeding (0.3%) and congenital central nervous system anomalies (0.3%). In treatment, 406 (66.0%) of the babies received only phototherapy, and 35 (5.7%) of them also received exchange transfusion. To the remained 174 (28.3%) cases, antibiotics, sodium L-thyroxin, and/or intravenous fluid treatment were given according to the primary disease, together with phototherapy. Rebound jaundice developed in 13 (2.1%) infants, temporary hypocalcemia in 3 (0.5%), anemia in 1 (0.2%), and diarrhea in 1 (0.2%). Mortality associated with an underlying cause developed in 7 (1.1%) cases, kernicterus developed in 1 (0.2%), and 607 (98.7%) were discharged with medication. Conclusions: Hyperbilirubinemia is frequently seen in the neonatal period. Timely treatment of hyperbilirubinemia is extremely important for the prevention of morbidity and mortality. Objective: Neurological complications are among the main causes of mortality and morbidity in antenatal infections. Data on the long-term outcomes of infants exposed to severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) infection in the antenatal or neonatal period are limited. This study aimed to investigate potential neurological complications in children with antenatal or neonatal exposure to SARS-CoV-2. Method: In this prospective cross-sectional study, infants and toddlers with a history of antenatal or neonatal SARS-CoV-2 exposure underwent neurological evaluation by a pediatric neurologist. Results: Of 31 children (19 males, median age 9.3 months) included in the study, maternal coronavirus disease-2019 (COVID-19) diagnosis was made in the first trimester of pregnancy in 1, the second trimester in 3, and the third trimester in 25 children. Two children were diagnosed with COVID-19 in the neonatal period, and 3 children with maternal COVID-19 diagnosis during pregnancy were also diagnosed with COVID-19 neonatally. On neurological examination, hypotonia and motor/social delays were observed in 1, microcephaly in 2, and macrocephaly in 1 child. Of the 8 children evaluated with magnetic resonance imaging, 1 had findings consistent with Joubert syndrome and the others were normal. All infants passed the standard auditory brainstem response test. The only ocular abnormalities detected were retinopathy of prematurity (stage 3) in 1 infant and poor eye contact and object tracking in the child with Joubert syndrome. Conclusion: Our study suggests that neurological development is mostly favorable in infants and toddlers exposed to SARS-CoV-2 in the antenatal or neonatal period. [ABSTRACT FROM AUTHOR]

Introduction: Metabolic, congenital disorders, and complications seen in infants of diabetic mothers (IDM) are well defined in the literature. We aimed to compare perinatal problems in macrosomic IDM and infants of mothers without diabetes. Methods: We included all macrosomic infants admitted to neonatal intensive care unit (NICU) at two centers between 2017-2020. Birth history, anthropometric measurements, gestational age, metabolic and cardiac problems were compared between macrosomic IDMs and infants of non-DMs. The p-value less than 0.05 was considered statistically significant. Results: 156 (37 IDM, and 119 non-IDM) macrosomic newborns were included in the study. While the incidence of hypoglycemia, need for mechanical ventilation, respiratory distress syndrome, ventricular septal defect (VSD) and persistent pulmonary hypertension (PPH) were statistically similar, the incidence of cesarean section (p=0.002), myocardial hypertrophy (p=0.001), and polycythemia (p=0.019) was higher in the IDM group. While the incidence of respiratory problems and VSD was similar in both groups, myocardial hypertrophy was found in approximately in one fourth (22.2%) of the non-diabetic group. Conclusion: Macrosomic non-IDMs have a similar risk for perinatal-postnatal complications as macrosomic IDMs and should be evaluated accordingly. [ABSTRACT FROM AUTHOR]

Studies evaluating the safety and efficacy of currently available vaccines for SARS-CoV-2 do not include pregnant women and newborns. Safety data for vaccinating pregnant women with the COVID-19 vaccine is urgently needed since the vaccine is the only way to protect the vulnerable population. The 33 years-old pregnant women, a healthcare worker as an obstetrician and gynecologist, was vaccinated with inactivated SARS-CoV-2 vaccine with two doses within 28 days apart as the recommended protocol, on her 29th week and 33rd week of pregnancy. The rest of the pregnancy was uneventful and she gave birth on the 38th week to a healthy infant. The infant was a healthy boy upon first evaluation. Maternal blood was sampled immediately after birth, cord blood was obtained from a neonate for the immunoglobulin testing method. COVID-19 IgG and IgM antibodies for spike antigen was reported maternal as 2.04 (reactive) and the cord blood as 1.36 (reactive in a low titration). In this case we report an infant with SARS-CoV-2 IgG and IgM antibodies detectable in cord blood after maternal vaccination with inactivated vaccine. [ABSTRACT FROM AUTHOR]

39. Neonatal outcomes of patients with vaginal delivery after a Caesarean section.

Purpose: The rates of caesarean section (CS) have been increasing in recent years in Türkiye and throughout the world. A previous CS is known to not be an absolute indication for repeat CS, and vaginal delivery (VD) (trial of labor after caesarean (TOLAC)) can occur after a CS. The aim of this study was to investigate the neonatal outcomes of TOLAC patients in Konya City Hospital. Materials and Methods: A retrospective analysis was made of the records of all the births in Konya City Hospital between 10 August 2020 and 10 June 2022. The study included all patients with full or nearly complete cervical dilation-effacement and who had a previous CS. These patients were compared with patients who had elective CS and VD. A total of 119 patients were included in the study in 3 groups; TOLAC n:32, CS n:42, VD n:45. Results: TOLAC patients gave birth earlier and the infant birth weight (2991.25gr), length (50.66cm) and head circumference (34.09cm) measurements were found to be significantly lower than those of the other groups. The development of any problem (icterus, respiratory distress syndrome (RDS), oxygen requirement, hypoglycemia, hypothermia) was determined at a significantly higher rate in the TOLAC (21.9%). Conclusion: Patients with a history of CS do not have to have a repeat CS. TOLAC can be performed in appropriate cases, but care should be taken against uterine rupture, the need for emergency CS, and adverse neonatal outcomes. [ABSTRACT FROM AUTHOR]

Purpose: Information on systemic inflammatory indices in the field of neonatology is limited. How sex and mode of delivery affects systemic inflammatory indices is unknown. The aim of our study was to evaluate the effect of mode of delivery and sex on systemic inflammatory indices in preterm infants. Materials and Methods: Preterm infants <32 weeks of gestation were evaluated retrospectively. Complete blood count parameters, monocyte-to-lymphocyte ratio (MLR), neutrophil-to-lymphocyte ratio (NLR), pan immune inflammation value (PIV), platelet-to-lymphocyte ratio (PLR), systemic immune inflammation index (SII), systemic inflammation response index (SIRI), demographic and clinical characteristics were compared in terms of sex and mode of delivery. Results: In our study, 1322 preterm infants were evaluated. Leukocyte, platelet, neutrophil, monocyte, lymphocyte count, hemoglobin levels, NLR, MLR, PLR, PIV, SII, and SIRI values were found to be similar between genders. Leukocyte, neutrophil, monocyte, lymphocyte count, hemoglobin levels, MLR, PLR, and SIRI values did not differ significantly according to mode of delivery. The values of platelet count (225 103 µ/L), NLR (0.30), PIV (42.17) and SII (67.86) in the cesarean section group were higher than those in the vaginal delivery group (platelet count: 248 103 µ/L, NLR: 0.44, PIV: 80.88 and SII: 91.63). Conclusion: Gender had no effect on systemic inflammatory indices and NLR, PIV, and SII were lower in cesarean delivery in preterm infants born <32 weeks of gestation. [ABSTRACT FROM AUTHOR]

Purpose: The aim of this study was to examine the relationship between mechanical ventilation modes and stress levels by measuring salivary cortisol levels in preterm infants. Materials and Methods: The study group comprised 65 preterm infants on respiratory support and 43 control preterm infants. The Neonatal Infant Pain Scale (NIPS) was used to evaluate pain. To determine the stress level, salivary cortisol levels were studied in saliva samples taken in the morning and evening on the 4th postnatal day. Results: In the study group, morning and evening cortisol levels of the infants were measured 8.33±5.7 ng/ml and 8.05±5.6 ng/ml, respectively. In the control group, morning and evening cortisol values of the infants were measured 1.50±0.7 ng/ml and 1.48±0.7 ng/ml, respectively. The morning and evening salivary cortisol levels of the infants in the study group were significantly higher than those of the infants in the control group. In the invasive mechanical ventilation group infants, morning and evening cortisol levels of the infants were measured 12.46±5,3 ng/ml and 12.0±5,2 ng/ml, respectively. In the non-invasive mechanical ventilation group, morning and evening cortisol values were measured 4.57±2.7 ng/ml and 4.41±2.7 ng/ml, respectively. Both morning and evening salivary cortisol levels of infants on invasive mechanical ventilation were higher than those of infants on noninvasive mechanical ventilation. Conclusion: Increased salivary cortisol levels in invasive modes indicate that intubation causes pain and stress in newborns. [ABSTRACT FROM AUTHOR]

Purpose: Though there are gaps in the relationship between COVID-19 and newborns no bibliometric analysis was published until now. This study aims to perform a bibliometric overview of literature of the relationship between newborns and COVID-19 in order to guide future studies addressing the same problem. Materials and Methods: Web of science was used to scan the published literature on newborns & COVID-19 by searching studies with and bibliometric networks were visualized by VOSviewer software. COVID-19, SARS-CoV-2, 2019-nCoV with perinatal, neonatal, newborn and infant were used as keywords. Results: 912 keyword-coherent publications were found in the period 1970-2021, (491; 54.43% high quality studies) mostly about pediatrics and obstetrics gynecology with most frequent keywords; COVID-19, Sars-Cov-2, Pregnancy, Vertical Transmission and Newborn. United States, China, England and Italy have the highest number of studies as well as highest number of citations. COVID-19 on neonatal outcomes and pregnancy had highest citation. Conclusion: Scientific network monitoring via bibliometric analysis is crucial and proved highly beneficial during COVID-19 crisis. Despite the articles on COVID-19 & newborn have a decent number of citations, it vastly lower compared to the other studies about adults or clinical features and should be increased while it is still early. Objective: Although the role of thyroid hormones in functional and anatomical closure of patent ductus arteriosus (PDA) is well known, their effects on the medical or surgical closure of PDA in newborns remain unclear. This study aimed to assess the correlation between thyroid function tests and PDA closure through medical or surgical interventions in newborns. Methods: This retrospective study was conducted on 65 newborns diagnosed with hemodynamically significant PDA (hs-PDA), with a premature rate of 81.5% (n=53). The subjects were divided into two groups according to the nature of the ductal closure as medically responsive "MR-PDA" or surgically treated "ST-PDA". The groups were compared in terms of thyroid hormone levels and other clinical parameters. Results: Thirty-three (51%) of all 65 patients had PDA and responded to medical treatment. Gestational week, birth weight, and mode of delivery were similar between the medical and surgical treatment groups (p>0.05). Free thyroxine levels were significantly lower in the MR-PDA group than in the ST-PDA group (p=0.01). Conclusions: Because hs-PDA is associated with increased morbidity and mortality in the neonatal period, especially in premature infants, we hypothesize that thyroid hormone levels may play a role in the closure of hs-PDA. For this reason, following up-to-date information on microbiota, making efforts to ensure that the newborn has a healthy microbiota and preventing the negative factors affecting it, and providing support to women for the formation of healthy microbiota from the prenatal period are essential for the formation of healthy generations. Aim: To determine the effect of vitamin D use by women during pregnancy on the vitamin D level in the cord blood of their babies. Materials and Methods: The study, designed as a case-control type, was conducted at Karabük University Training and Research Hospital between 15 August 2020 and 15 January 2021. The data were collected in two st ges as data collection form and collection of blood samples. Results: The average age of pregnant women who took vitamin D supplements was 27.8±5.9 and that of pregnant women who did not take vitamin D supplements was 27.9±5.8. When the level in the cord blood was measured, the median vitamin D value of pregnant women who received vitamin D supplements was (19.0 ng/ml) and is statistically significantly higher than the median vitamin D value (10.5 ng/ml) of pregnant women who do not take vitamin D supplements (p<0,05). Conclusion: Cord blood vitamin D levels of babies of mothers who used vitamin D during pregnancy are higher than those of mothers who did not use vitamin D. Effective sustainability of the vitamin D support program for pregnant women and babies in the health system should be ensured. [ABSTRACT FROM AUTHOR]

47. Evaluation of Erythrocyte Suspension Usage in Neonates.

Objective: Erythrocyte suspension (ES) transfusion is frequently used in neonatal intensive care units (NICU). We evaluated the use of ES in hospitalized patients in the NICU of our hospital in terms of transfusion rate, indication, laboratory control, complications, and adherence to the guidelines. Material and Methods: Patients who were hospitalized at the University of Health Sciences of Turkey, Dr Sami Ulus Maternity and Children Research and Training Hospital, NICU, in 2016, and who received ES were included in this descriptive study.The demographic and clinical characteristics of the patients, the quantity of ES used, and the laboratory tests of the first three transfusions were recorded.The compliance of ES usage indications with the transfusion guidelines published in the Nelson Pediatric Textbook and the Turkish Neonatology (TND) Society Blood Products Transfusion Guidelines was assessed. Results: One hundred and ninety one of the 1538 admitted patients in the NICU received a total of 633 ES, for a 12.4% ES usage rate. Following an evaluation of the first three transfusions, it was determined that there was 66% compliance with the TND Blood Products Transfusion Guide and 64% compliance with the Nelson Pediatric Textbook transfusion protocol. It was significant that the frequency of retinopathy of prematurity (p=0.015) and intracranial hemorrhage (p=0.001) was high in premature infants who received more than one ES. Conclusion: Although transfusion is life-saving in crucial circumstances, there may be a cause-effect relationship between the detected morbidity and complications. Each newborn should be carefully evaluated individually and within the framework of the guidelines before having to decide on an erythrocyte transfusion. [ABSTRACT FROM AUTHOR]

Background: The causative agents and antibiotic resistances causing neonatal sepsis can vary according to the units. In this study, we aimed to analyze infants diagnosed with culture-confirmed late-onset neonatal sepsis in terms of frequency, etiologic factors, causative microorganisms and antibiotic resistance. Materials and Methods: The data of the patients monitored in the Neonatal Intensive Care Unit of Şanlıurfa Training and Research Hospital between 2018 and 2023 were obtained from infection control committee records, patient files, and the hospital data system. Among 1641 hospitalized cases during the specified period, 218 cases diagnosed with culture-confirmed late-onset neonatal sepsis were included in the study. Results: 218 cases that were detected late-onset neonatal sepsis out of 1641 blood cultures examined for sepsis suspicion were included during the study period. 76.1 % of the cases were receiving invasive mechanical ventilator support. 23.9 % had picc lines, 55.5% had umbilical catheters, and 68.8 % had total parenteral nutrition. Among the isolates obtained from blood cultures taken after the third day of the study, 67 % were gram-negative agents. Among the gram negative agents, 43.1% were Klebsiella pneumoniae. Mortality rates were significantly higher in female cases and those receiving central venous catheters and invasive mechanical ventilator support. When the causative agents of sepsis were analyzed, mortality rates were significantly higher in cases with gram-negative agents (p < 0.05). For Klebsiella pneumoniae and Escherichia coli, the resistance rates were found to be, respectively: 96.6% and 40% for Ampicillin, 83.1% and 55% for Gentamicin, 72.9% and 75% for Ceftazidime, 25.4% and 10% for Amikacin, 16.9% and 10% for Imipenem/Meropenem, 6.8% and 0% for Colistin, and 5.1% and 15.8% for Levofloxacin. Conclusions: The microorganisms detected in neonatal intensive care units and antibiotic resistance of these microorganisms may be variable. It is known that sepsis-related mortality rates can be reduced with appropriate empirical treatment, in early recognized late-onset neonatal sepsis cases. Therefore, each unit is recommended to formulate its treatment strategy as the sepsis agents and antibiotic resistances may differ among neonatal units. [ABSTRACT FROM AUTHOR]
