We conducted a cross-sectional research design among 1767 university students (71.2% were females and the remaining were males) who attended public universities in Sarawak. Questionnaires related to a history of lifetime trauma, posttraumatic stress disorder (PTSD), and depressive symptoms together with suicide behavior were surveyed. Results showed that at least 87% of students had lifetime trauma once, 9.6% exhibited PTSD symptoms, 15.5% exhibited depressive symptoms, and 12.9% of participants reported having suicidal behavior. There were significantly more females who reported having suicidal behaviors than males. Ethnicity was significantly associated with PTSD symptoms. PTSD and depressive symptoms were significant predictors of suicidal behavior. Other significant predictors for suicide behavior among those who were traumatized were near drowning, sexual assault, losing a family member, neglect, and bullying. This study provides a significant understanding of the link between PTSD, depressive symptoms, and suicidal behavior among Malaysian university students.

2. Effectiveness of acupuncture as adjunctive therapy in type 2 diabetic: study protocol for a randomized controlled trial

Abstract

Background: The incidence of type 2 diabetes mellitus is increasing worldwide. The literature suggests that acupuncture is a possible complementary therapy for type 2 diabetes mellitus. This study aims to determine the effect of acupuncture as an adjunctive therapy on homeostasis model assessment-insulin resistance (HOMA-IR), adiposity and health-related quality of life (HRQOL) in patients with type 2 diabetes mellitus. Method and design: This randomised, double-blind, placebo controlled and parallel design trial will be carried out in three public university teaching hospitals in Malaysia. Eligible type 2 diabetes mellitus subjects will be randomly assigned to receive either acupuncture (n=30) or a placebo (n=30). The intervention is carried out using acupuncture ‘press’ needle on abdomen area (10 sessions of treatment), twice a week over 6 weeks. Both groups will continue with their routine diabetes care. Primary outcome of HOMA-IR and secondary outcomes of body mass index and waist circumference will be measured at the time of recruitment (baseline) and after completion of 10 sessions (week 7) of the treatment. Additionally, secondary outcome of HRQOL will be measured at the time of recruitment (baseline), after completion of 5 sessions (week 3/4) and 10 sessions (week 7) of the treatment. Any adverse event will be recorded at every visit. Discussion: The ndings of this study will provide important clinical evidence for the effect of acupuncture as adjunctive therapy on HOMA-IR, adiposity and HRQOL of type 2 diabetes mellitus.

7. Shared decision-making in Malaysia – Legislation, patient involvement, implementation and the impact of COVID-19

Abstract

Shared decision making (SDM) activities in Malaysia began around 2010. The rise in the numbers of patients with chronic disease in Malaysia underscores a growing need for doctors to practice patient-centred care and SDM as more Malaysians come into regular contact with health decision-making scenarios. Recent guidelines for medical professionalism have emphasized that options and risks be discussed in consultations, especially for procedures with risk of adverse outcomes. Although SDM is not legally required, principles of SDM are applied in legal judgements on informed consent. Research on SDM has grown to include the adoption of patient and public involvement in research, an increased emphasis on incorporating local cultural values in SDM, and implementation of SDM in Malaysia’s health system and organizational culture. While COVID-19 hindered the progress of SDM research, one positive development was that vaccination choices heightened public consciousness about personal decisional autonomy and the need to discuss pros and cons with doctors before making a medical decision.
